Abstract:The clinical data of two hundred and four patients with juvenile xanthogranuloma (JXG) was analyzed. The ratio of male to female was 1. 65:1. 5. 88% of the patients presented with the lesions at birth. The lesions appeared before 1 year old in 28.43% of the patients and before 5 years old in 72.06% of the pa-tients. 73.53% of the patients had one single lesion. According the histological features of the lesions, JXG was classified into four subtypes in those patients: early JXG ( 24. 51%) , classical JXG ( 47. 06%) , transi-tional JXG (16.18%) and mixed JXG (12.25%). Immunohistochemistry showed positive of CD68, negative of CD1a and S-100. Surgical excision are the major strategy for treatment.

It affects children with a predilection for the head and neck region. A relatively rare, benign, histiocytic proliferative cutaneous disorder with a potential for malignancy. A prompt and wide resection is recommended.

Abstract:Objective To discuss the clinical and pathological features of the juvenile xanthogranuloma of testis.Methods A case of the juvenile xanthogranuloma of testis was reported.A 6-month-old boy presented with scrotal mass for 2 weeks before admission.Physical examination revealed that the left testis was hard with the size of 2.0 cm×2.0 cm and the serum alpha-fetoprotein was 37.60 μg/L.Chest X-ray and serum human chorionic gonadotropin investigations were normal.B-ultrasound examination showed a solid mass of uniform homogeneity in the left testis,with rich blood supply.Results The patient underwent operation after inguinal incision under general anaesthesia.During operation the left spermatic was augmented,and the mass was infiltrated into the whole testis and epididydis,and an inguinal orchiectomy was competed.Histopathological examination revealed that the lesion was intratesticular and involvement of the epididymis,predominantly occupied with an infiltrative pattern of by mononuclear cells,foamy cells and inflammatory cells (neutrophils,eosinophils,lymphocytes),without Touton giant cells.Immunohistochemical examination revealed positive for CD68 and CD163,and negative for CD1a.The pathological result was juvenile xanthogranuloma.During the follow-up period for 10 months,no recurrence was recorded.Conclusions Juvenile xanthogranuloma of the testis is an extremely rare benign disease lack of special feature in clinical manifestation.The diagnosis mainly depends on histological features and special markers in immunohistochemistry.Differential diagnosis should be made among leukemia,lymphoma and other distinctive neoplasms.

Abstract:Abstract Juvenile xanthogranuloma (JXG), a form of non-Langerhans cell histiocytosis, is a rare benign condition estimated to affect 1 per million children. In 80% it presents as a solitary papule or nodule. There are several described subtypes of JXG including mixed, giant, subcutaneous, eruptive, clustered and plaque-like. Symmetrical giant facial plaque-type JXG is a very rare subtype, first described by Gunson and Birchall in 2008 [Gunson TH, Birchall NM. Symmetrical giant facial plaque-type juvenile xanthogranuloma. J Am Acad Dermatol 2008; 59 (2 Suppl. 1): S56–7]. We present a 2-year-old girl with a slowly progressive, asymptomatic, symmetrical orange-coloured rash on both cheeks that started around age 1 year. She was otherwise well. Examination showed well demarcated, orange-coloured indurated plaques covering both cheeks in a mask-like distribution. There was no lymphadenopathy or hepatosplenomegaly. There were no signs of neurofibromatosis. Full blood count, renal and liver function tests and lipids were normal. Biopsy showed subepidermal sheets of histiocytic proliferation. Lesional cells were positive for CD68 and negative for SOX10, compatible with JXG. She is currently awaiting ophthalmology assessment and an ultrasound scan of her abdomen. Over the course of her follow-up, the plaques have slowly extended, spreading onto the temples and chin, the axillae and both upper arms. JXG belongs to the proliferative dendritic-cell-related disorders and is the commonest form of non-Langerhans histiocytosis (Itin K, Häusermann P, Itin P, Fosse N. Symmetrical facial giant plaque-type juvenile xanthogranuloma: case report and review of the literature. Case Rep Dermatol 2021; 13: 399–406). It is most often characterized by a solitary red–yellowish benign papule (< 0.5 cm) or nodule (< 2 cm) that usually involves the head and neck (42%) or trunk (20–40%), although involvement at any body site can occur, including extracutaneous sites (the eye being the most common). It may be congenital, but most cases develop in the first year of life. It more commonly affects male patients, in a ratio of up to 7 : 1. There is usually spontaneous resolution, which takes months to years, leaving an atrophic scar. The giant symmetric facial subtype of JXG is exceptionally rare, with only five reported cases worldwide. In these cases, there was no spontaneous resolution during a follow-up period of between 2 and 6 years. None of the reported cases had systemic or extracutaneous involvement. Treatment options are undefined – one child had a partial response to a single fractionated CO2 laser treatment. We present this case to highlight awareness of this unusual presentation.

Abstract:Background: Langerhans cell histiocytosis (LCH) represents a myeloid clonal proliferation that involves the skin and other organs. Occasionally, cases of LCH evolve into juvenile xanthogranuloma (JXG). Case presentation: A 7-month-old boy presented with an itchy, flaky rash resembling seborrheic dermatitis affecting the scalp and eyebrows. The lesions started at 2 months old. On physical examination, there were reddish/brown lesions on the trunk, denuded areas on the groin and neck, and a large lesion behind his bottom teeth. In addition, there were thick white plaques in his mouth and thick whitish material in both ears. A skin biopsy showed features of LCH. Radiologic examination demonstrated several osteolytic lesions. Chemotherapy produced marked improvement. A few months later, the patient developed lesions with clinical and histologic features of XG. Conclusion: A possible association between LCH and XG is explained by lineage maturation development. Chemotherapy may play a role in modifying the production of cytokines that enhance the transformation or 'maturation' of Langerhans cells into multinucleated macrophages (Touton cells) characteristic of a more favorable proliferative inflammatory condition.

Abstract:Background Juvenile xanthogranuloma is rare in children and there are limited data on its imaging features. Objective To analyze the computed tomography (CT) and magnetic resonance imaging (MRI) features of juvenile xanthogranuloma in children. Materials and methods A retrospective review was performed of clinical and radiographic data of histologically confirmed juvenile xanthogranuloma between January 2009 and June 2020. Results Fourteen children (4 girls, 10 boys; age range: 1 day to 13 years, mean age: 73 months) were included in the study: 4/14 had CT only, 5/14 had MRI only and 5/14 had CT and MRI. Sites of extracutaneous juvenile xanthogranuloma involvement included subcutaneous soft tissue (8/14), liver (2/14), lungs (2/14), kidney (2/14), nose (2/14), pancreas (1/14), central nervous system (1/14) and greater omentum (1/14), mainly manifested as single or multiple nodules or masses in different organs. On CT, the lesions mainly manifested as an iso-hypo density mass with mild or marked enhancement. On MRI, the lesions mainly manifested as slightly hyperintense on T1 and slightly hypointense on T2, with decreased diffusivity and homogeneous enhancement. Juvenile xanthogranuloma was not included in the imaging differential diagnosis in any case. Conclusion Juvenile xanthogranuloma mainly manifests as single or multiple nodules or masses in different organs. Slight hyperintensity on T1 and slight hypointensity on T2 with decreased diffusivity and homogeneous enhancement are relatively characteristic imaging findings of juvenile xanthogranuloma. Combined with its typical skin lesions and imaging features, radiologists should include juvenile xanthogranuloma in the differential diagnosis when confronted with similar cases.

Abstract:Objective To explore the relationship between the radiological characteristics and clinical and pathological manifestations of intracranial juvenile xanthogranuloma. Methods The radiological and clinical characteristics of two intracranial juvenile xanthogranuloma cases were analyzed respectively. In combination with reviewing literatures, the radiological characteristics and discipline of intracranial juvenile xanthogranuloma disease were also analyzed. Results Intracranial juvenile xanthogranuloma disease was accidently detected in one case. The other case was detected with headache and numbness in face. No abnormal sign was found in skin. On plain CT isodensity signals appeared and on contrast-enhanced CT homogeneous enhancement was found.On MR imaging,masses showed isointense signals on T1WI and iso-or hypo-intensity signals on T2WI, and enhanced homogeneously. Lesions were solitary or multiple in the external of brain. After operation intracranial juvenile xanthogranuloma was confirmed by pathology, and no recurrence occurred. Conclusion Intracranial juvenile xanthogranuloma is a kind of rarely encountered disease with typically radiological characteristics, which is difficult to detect before surgery and can be confirmed by post-operative pathological and immunohistochemical examinations.[Chinese Medical Equipment Journal,2018,39(5):71-74]

Approximately 0.4% of cases exhibit ocular manifestations, which can result in glaucoma and blindness. We present a case of a 7-month-old male with unilateral glaucoma associated with Juvenile xanthogranuloma, and emphasize the importance of an ocular screening in patients with Juvenile xanthogranuloma, especially those with periocular lesions.

Abstract:To Study Juvenile xanthogranuloma (JXG) in the nasal cavity, raise the level of diagnosis and treatment for this disease. We reported a case with JXG and reviewed the literatures. JXG in nasal vestibule is ex tremely rare. Only three cases were reported before. After surgical removal,no recurrence was found over a 10-month follow-up. Clinical manifestations and imaging examination is nonspecific for JXG, and the diagnosis of the disease relies on pathological examination. Surgical resection is an effective treatment method.

Abstract:Objective Tuberous sclerosis complex (TSC) is a rare autosomal dominant genetic disease with many manifestations, and it involves any organ. In this study, we report a TSC patient with new type skin lesions. Methods A 7-month-old TSC boy with multiple cutaneous nodules was admitted in our hospital. We collected the clinical data of the patient. We performed biopsy of cutaneous nodules and whole-exome sequencing in both paraffin block tissue and blood samples. Results The patient presented with a 2 month history of gradual growth multiple cutaneous nodules. He had cardiac rhabdomyoma, subependymal giant cell astrocytoma (SEGA) and hypomelanotic macules. The pathological finding of cutaneous nodules was consistent with juvenile xanthogranuloma (JXG). After 3 months of sirolimus treatment, the multiple nodules disappeared. The whole-exome sequencing identified TSC1 (c.2356C > T, p.R786*) mutation in both paraffin block tissue and blood samples. We overturned the original pathological diagnosis and finally identified JXG as a new type of skin lesions in TSC. Conclusion This is the first report on the occurrence of JXG skin lesions in TSC patient. Genetic testing is necessary in JXG. These findings expand the phenotype of skin in patients with TSC and contribute to the elucidation of JXG pathogenesis and treatment.

Abstract:Purpose. To report the clinical and histopathologic characteristics and prognoses of three ocular juvenile xanthogranuloma (JXG) cases. Case Reports. Three cases were included in this study. The first case involved a 5-year-old girl with an enlarging yellowish mass at the limbus with corneal involvement. Ultrasound biomicroscopy showed a poorly demarcated mass involving the underlying cornea and sclera. The mass was excised in combination with a lamellar keratoplasty procedure. No recurrence was seen at the 2-year follow-up. The second case involved a 2-year-old boy with an enlarging yellowish mass on the conjunctiva, without limbal involvement. The mass was excised with no recurrence noted 1 year later. The third case involved a 7-month-old girl with unilateral eye redness and photophobia combined with multiple orange-red, raised nodular lesions on the skin. Examination under general anesthesia revealed a gray-yellow mass in the inferior and temporal iridocorneal angles. The intraocular pressure and corneal diameter were normal. Examination using ultrasound biomicroscopy showed a high-level echo lump in the inferior and temporal angles. There was no treatment for this case. At the 1-year follow up, the eye symptoms had resolved and the skin lesions were flat. Histopathologic examinations were completed on all three cases. The presence of Touton giant cells in hematoxylin-eosin staining, positive CD68 staining, and negative S-100 and CD1a staining confirmed the diagnosis of JXG. Conclusions. We report three histopathologically confirmed ocular JXG cases involving the corneoscleral limbus, conjunctiva, and iris with angle involvement, respectively. Ultrasound biomicroscopy performed on two cases demonstrated no obvious division between the mass and the surrounding structures. The cases with ocular surface involvement were successfully treated by excision and the case with iris involvement spontaneously regressed without any treatment. Early excision may be the better choice for ocular surface lesions, especially when corneal involvement is a possibility.

Abstract:A 15-month-old boy was seen because of two distinct types of lesions, namely, yellowish papules on the scalp and face, and hemorrhagic macules and papules on the trunk. A biopsy specimen from one of the yellowish papules showed histopathologic and immunohistochemical changes of both juvenile xanthogranuloma and Langerhans cell histiocytosis. The section from the center of the biopsy specimen showed a proliferation of foamy histiocytes, among them Touton giant cells, which were positive for CD68, but negative for S-100 and CD1a. At the edges of the specimen was a predominantly histiocytic infiltration in the papillary dermis that was positive for S-100 and CD1a, but negative for CD68. The patient died 12 days after hospital admission consequent to disseminated intravascular coagulopathy. We did not biopsy the hemorrhagic lesions; however, this combination of findings suggests a possible relationship between juvenile xanthogranuloma and Langerhans cell histiocytosis, as previously reported.

Abstract:A case of xanthoma disseminatum is reported. A 6-year-old boy presented with one-year history of numerous asymptomatic yellow to brown papules on the face, neck, chest, abdomen, back, the proximal parts of the limbs, penis and scrotum. The routine urine test and blood lipid levels were normal. The histopathological features included numerous foamy histiocyte and multinucleated giant cell infiltration in the upper dermis, and many Touton-type giant cells with a few eosinophils. Immunohistochemical staining was negative for CD1a and S-100 proteins, but positive for CD68. The diagnosis of xanthoma disseminatum was made by a combination of histopathologic findings and clinical feature.

Abstract:Diseases under the non-Langerhans cell histiocytosis (LCH) group often share clinical and histological similarities, making proper delineation highly challenging. A two-year-old female child presented with multiple, small raised asymptomatic lesions all over the body for one year. Cutaneous examination showed multiple brownish flat-topped and yellowish-brown dome-shaped papules scattered all over the body with hyperpigmented macules over the face. A provisional diagnosis of generalized eruptive histiocytosis (GEH) was made. But the dermoscopy and histopathological examination of flat-topped and dome-shaped papules showed features suggestive of generalized eruptive histiocytosis and juvenile xanthogranuloma (JXG) respectively, with a few overlapping features. Systemic examination was found to be normal. Hence an overlap of GEH and JXG was considered, with a thought of ongoing xanthomatization of GEH lesions. The transformation of GEH into xanthoma disseminatum, multicentric reticulohistiocytosis, progressive nodular histiocytosis, and juvenile xanthogranuloma (JXG) has been reported. It has also been reported that GEH and JXG, the two entities that belong to a continuous spectrum of histiocytoses with xanthogranulomatous pathology, can rarely co-exist in the same individual. The distinction between GEH and JXG is crucial, as they may require different management strategies. Our case stresses the fact that relying only on clinical diagnosis can be deceiving owing to the overlapping clinical, dermoscopic, and histopathological features of histiocytoses.

Abstract:Xanthogranuloma is a rare and poorly understood disorder,manifested with the accumula-tion of cells containing lipid in the dermis or subcutaneous tissues,which can form yellow tumor-like lesions on the cutaneous surface. Xanthogranuloma can be classified into four clinical subtypes:adult-onset xanthogranu-loma,adult-onset asthma and periocular xanthogranuloma,necrotic xanthogranuloma and Erdheim-Chester dis-ease. These subtypes primarily differ in terms of clinical features,histopathology and systemic functions,al-though these conditions overlap occasionally. We reported one case of xanthogranuloma presenting with xantho-matous masses in bilateral eyelids and orbits.

Abstract:Objective: To investigate the clinical characteristics, treatment, and prognosis of children with systemic juvenile xanthogranuloma (JXG). Methods: Clinical data of children with JXG who were hospitalized in Beijing Children's Hospital, Capital Medical University, from January 2012 to December 2019 were retrospectively analyzed, including clinical manifestations, laboratory determinations, treatment, and prognosis of the children. Patients were treated with vindesine + prednisone as the first-line treatment and cytarabine + vindesine + dexamethasone ± cladribine as the second-line treatment. Results: Ten patients, including 8 males and 2 females, with a median of onset age of 1.95 (0.80–7.30) years, exhibited multi-system dysfunction. The median age of diagnosis was 2.45 (1.30–12.10) years. The most common location of extracutaneous lesions was the central nervous system (6 cases), followed by the lung (5 cases) and bone (4 cases). Nine patients underwent first-line chemotherapy, and 6 patients underwent second-line chemotherapy, including 5 patients with poorly controlled disease after first-line treatment. The median observation time was 29 (3–115) months. Nine patients survived, whereas one patient died of respiratory failure caused by pulmonary infection. At the end of follow-up, 7 patients were in active disease (AD)/regression state (AD-better), and 2 patients were in an AD/stable state (AD-stable). Three patients had permanent sequelae, mainly central diabetes insipidus. The rates of response to the first-line treatment and the second-line treatment were 40.0 and 66.7% respectively. Conclusion: The chemotherapy protocol for Langerhans cell histiocytosis (LCH) may be effective for patients with systemic JXG.
