Abstract:BACKGROUND:Since May 2022, outbreaks of monkeypox have occurred in many countries around the world, and several cases have been reported in China.CASE SUMMARY:A 38-year-old man presented with a small, painless, shallow ulcer on the coronary groove for 8 d. One day after the rash appeared, the patient developed inguinal lymphadenopathy with fever. The patient had a history of male-male sexual activity and denied a recent history of travel abroad. Monkeypox virus was detected by quantitative polymerase chain reaction from the rash site and throat swab. Based on the epidemiological history, clinical manifestations and nucleic acid test results, the patient was diagnosed with monkeypox.CONCLUSION:Monkeypox is an emerging infectious disease in China. Monkeypox presenting as a chancre-like rash is easily misdiagnosed. Diagnosis can be made based on exposure history, clinical manifestations and nucleic acid test results.

Abstract:Journal of International Medical Research, Volume 51, Issue 3, March 2023. The main symptom in primary syphilis is a small, painless, sore or ulcer called a chancre on the penis, vagina, or around the anus, although chancres can sometimes appear in the mouth or on the lips, fingers, or buttocks. We present the case of a man in his early 60 s with a chief complaint of a painful tongue ulcer. An ulcerated, indurated, and hemorrhagic lesion (23 × 14 mm) was found on the ventral tongue surface, near the oral floor. Palpation identified several swollen, mobile, elastic cervical lymph nodes, with no tenderness. We initially diagnosed tongue cancer; however, during a subsequent detailed examination for a malignant tumor, including biopsy and obtaining additional history, his disease was finally identified as primary syphilis with multiple swollen cervical lymph nodes. Oral amoxicillin and probenecid were started, and after 14 days, there was partial reduction in the size of the submandibular lymph nodes and the ulcer on the left tongue margin. The number of patients with syphilis in Japan increased by eight times from 2012 to 2018. We suggest that dentists consider primary syphilis as a differential diagnosis for oral refractory ulcer with induration and obtain a detailed patient history.

Abstract:A 39 year old woman presented with an asymptomatic ulcer on her left nipple. The ulcer had been present for five weeks. She also reported malaise. She denied any travel history in the past three months, but recalled a history of breast bites and breast-genital contact during sexual intercourse, which had occurred two weeks before the skin lesion appeared. Physical examination revealed demarcated ulceration with a border that was slightly elevated, crusted, and indurated (fig 1). Left axillary lymphadenopathy was noted. Abstract:A 42-year-old sexually active man with HIV on ART (antiretroviral therapy) who has a history of syphilis presented with fever and severe sore throat for which he could not eat or drink. He admitted to high-risk sexual intercourse with multiple partners 10 days prior. Physical examination revealed an injected throat and uvula ulcer. PCR for Neisseria gonorrhoeae, Chlamydia trachomatis, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma urealyticum and Ureaplasma parvum from pharynx and rapid group A streptococci test were negative. No significant bacteria were grown from the throat swab culture. The RPR (rapid plasma reagin) titer, which had previously been negative, increased to 1:2. From these results, uvula ulcer was thought to be caused by primary syphilis. He was treated with one shot of benzylpenicillin 2.4 million units intramuscularly, and his ulcer completely disappeared in seven days.

Abstract:Background The incidence of syphilis is increasing in many parts of the world. Clinicians may have limited experience in the diagnosis when the clinical appearance is unusual. If early diagnosis is not made and prompt treatment not given, then the disease may remain quiescent until more serious symptoms or systemic involvement develops. Case presentation We report the first case of a delayed diagnosis of syphilis with a ten-year history of persistent rupioid psoriasis-like lesions. Acute monoarthritis and high fever together with aggravation of skin lesions led to a careful clinical examination. Skin biopsies demonstrated syphilis spirochetes on immunohistochemical stain, and syphilis serological titers were positive. Standard treatment with benzathine penicillin brought a partial and transient improvement. A complete clinical and serological resolution of the disease was achieved by a prolonged and repeated penicillin treatment combined with methylprednisolone. A 7-year follow-up of the patient proved a full recovery. Conclusion Our case highlights the fact that clinical signs of syphilis can be diverse and complicated. Unusual clinical manifestations can happen in an immunocompetent individual. Treatment strategy may need to be adjusted in a difficult case.

Abstract:The clinical and histopathologic features of oral secondary syphilis are presented in two cases, one in an HIV-negative patient and the other in an HIV-positive patient. Case 1: A 22-year-old male, who was HIV-negative, presented with three painless, irregularly shaped whitish papules on the ventral surface of the tongue and two small pink confluent nodules on the hard palate. Case 2: A 20-year-old male, who was HIV-positive, had one painless yellow-whitish papule on the right lateral border of the tongue and four similar lesions on the left side.

Abstract:Background: Syphilis is a sexually transmitted disease caused by the spirochete Treponema pallidum. Recently, its incidence has been increasing worldwide. We encountered a young woman who presented with induration mimicking squamous cell carcinoma in the lower lip, without major medical conditions. Case presentation: A 25-year-old Japanese woman presented with a 1-month history of a painless induration in her lower lip. Because squamous cell carcinoma was suspected, a preoperation work up was performed, including laboratory tests, an ultrasonographic examination, and a biopsy. The ultrasonography findings showed an oval-shaped 17 × 11 × 12 mm tumor-like lesion with heterogeneous internal echo and an indistinct border. A pressure test and color Doppler sonography revealed that the lesion was soft with a very abundant blood flow. These findings suggested the possibility of underlying inflammatory causes rather than a neoplastic tumor. Serology tests for syphilis, including the anti-Treponema pallidum antibody and reactive rapid plasma reagin tests, were positive. The biopsy revealed no malignancy. Finally, she was diagnosed as having primary syphilis and treated with amoxicillin for 28 days. The rapid plasma reagin value gradually decreased and the initial induration in her lower lip disappeared. Conclusion: This case highlights the need for prompt examinations for possible underlying infective causes, such as syphilis, when seeing a painless induration with ulcer in the lip. Ultrasonography was helpful in the differential diagnosis of a tumor-like lesion and should be included in addition to syphilis serology tests, such as anti-Treponema pallidum antibody and rapid plasma reagin tests.

Abstract:Syphilis, a sexually transmitted disease caused by Treponema pallidum, manifests in distinct stages, each characterized by different oral lesions. A 38-year-old male presented with ulcerated lesions on the lateral border of the tongue and anterior vestibule fornix, evolving over 40 days. The patient reported additional symptoms such as weight loss, fever, and fatigue. An incisional biopsy of the anterior vestibule lesion revealed non-specific chronic ulceration. Following referral to the infectious and contagious diseases center, the patient received diagnoses of syphilis and HIV/AIDS, leading to a 6-month hospitalization.

Abstract:Abstract A 44-year-old man was referred for 2 months of erythematous ulcerative papules and plaques on the scalp, face and lower legs. The lesions started as painful ‘pimples’. He also reported fever and chills. He had a 5-year history of well-controlled HIV on antiretroviral therapy since diagnosis and a prior history of recurrent syphilis. Exam showed a large violaceous plaque on the right cheek with surrounding erythematous papules and an additional erythematous papule on the left ear. On the bilateral ankles and calves were four ulcers with necrotic centres and purple borders. The differential diagnoses included pyoderma gangrenosum and cutaneous lymphoma. A punch biopsy of the right zygomatic area showed dermatitis with a dense, diffuse plasma cell infiltrate suggestive of syphilis. The acid-fast bacteria (AFB), periodic acid–Schiff with diastase (PAS-D), and spirochete stains were all negative. Rapid plasma regain (RPR) was reactive at 1 : 4096. HIV was detected at < 30 copies per mL. Absolute CD4 T-cell count was 529 cells per μL. AFB and fungal cultures were negative, and bacterial culture showed rare Staphylococcus aureus. Although the spirochete stain was negative, it is only 70% sensitive and the positive RPR with histopathology supportive of syphilis in a patient who was positive for HIV confirmed the diagnosis of lues maligna. The patient received one dose of 2.4 million units of intramuscular penicillin G benzathine. He continued empiric doxycycline 200 mg daily, which had previously been initiated, and continued doxycycline postexposure prophylaxis after diagnosis. The patient reported healing with no new lesions 3 weeks after treatment. A new question brought on by this case is whether prior history of recurrent syphilis infection has a role in developing lues maligna. This patient in particular required syphilis treatment four times in the 3 years leading up to his presentation. Current literature describes a patient who was treated for secondary syphilis, then reinfected with malignant syphilis. However, further research is needed to elicit whether recurrent syphilis infections increase the chances of severe infection or malignant syphilis.

Upon clinical examination, a well-defined ulcerated lesion was identified, exhibiting indurated margins, situated on the left side of the lower lip, measuring approximately 1.5 cm. The lesion had been present for about a month and was associated with pain. The patient had a 30-year history of smoking, had substantial sun exposure due to his occupation, and was currently undergoing medical treatment for bladder infection and prostate-related conditions.

Abstract:DEAR EDITOR, A 23-year-old man presented with a 2-month history of recurrent fever and progressing skin ulcers with eschars on his face, trunk and extremities (a, b). Treponemal serology test was positive and immunohistochemistry of his skin biopsy showed numerous spirochetes (c, arrows). Additional results confirmed that he was infected with HIV, with a decreased CD4 T-cell count. A diagnosis of malignant syphilis associated with HIV infection was finally made. Malignant syphilis is an uncommon manifestation of secondary syphilis, characterized by generalized ulcerative skin lesions with prodromal symptoms. Generally, it occurs in immunosuppressed individuals, especially in HIV-infected patients. Malignant syphilis may be the initial presentation of underlying HIV infection. The patient was treated with doxycycline 100 mg twice daily for 14 days as he was allergic to penicillin. The therapy resulted in rapid regression of the skin lesions (d), and antiretroviral therapy was initiated.

Abstract:OBJECTIVE:To analyze the clinical and pathological features of 40 patients with secondary syphilis.METHODS:A total of 40 cases of secondary syphilis confirmed by serology were collected during 1994-2004 and were first diagnosed on presentation with oral lesions.RESULTS:The white patch in oral mucosa was found in 32 cases with painless or slight pain in most cases. The most common site of the lesion was the tongue. The histological examination on eight cases was initially misdiagnosed as oral candidosis or lichen planus, but confirmed as syphilis after serology revealed nonspecific inflammation with intraepithelial microabscess and dense perivascular infiltration of lymphocytes and plasma cells in connective tissue. The symptoms showed dramatic improvement in 16 cases after benzathine penicillin treatment.CONCLUSIONS:The oral manifestations of syphilis have specific clinical and pathological feature and attention should be paid to the suspicious oral lesions when patients are first presented in a dental office.

Abstract:A 31-year-old woman with a history of COVID-19 infection presented with a white plaque on the right border of her tongue, measuring 1 cm in diameter, and had been evolving for 2 months, causing mild discomfort. The initial clinical diagnosis considered lichen planus, oral leukoplakia, or possible trauma. To further investigate, an incisional biopsy was performed, revealing specific findings. Histopathological analysis indicated the presence of microabscesses on the epithelium and an intense mononuclear inflammatory infiltrate, primarily composed of plasmocytes in the subepithelial and perivascular regions.

Abstract:A 45-year-old man presented at our department with a 3-month history of persistent oral ulcer with mild pain in his oral mucosa. He denied any skin and genital lesions or other accompanying symptoms. He was initially diagnosed with a recurrent aphthous ulcer and was prescribed topical steroids with no improvement. Afterwards, he was referred to the otolaryngological department; the ENT doctor took the biopsy. It was reported as a non-specific inflammation pathologically, and he was given Cydiodine lozenge. The oral ulcer had not healed, and he came back to our department for further consultation. Physical examination revealed mucous patches with erosions in his soft palate and the abdomen of the tongue; the patches were irregular, slightly elevated, well-defined, and with a surrounding red halo, covered with a white or gray pseudomembrane (Figure 1). No genital or skin lesions were noted. Since the diagnosis remained indefinite, further investigations were ordered. Serological tests for rapid plasma reagin (RPR) [titer: 1:32] and treponema pallidum haemagglutination (TPHA) were positive, while the tests for HIV and hepatitis C were negative. Therefore, a definitive diagnosis of oral secondary syphilis was established. The patient was treated with standard intramuscular benzathine benzylpenicillin at 2 400 000 IU once a week, and the lesion resolved over a 3-week period. The global re-emergence of syphilis calls for more attention to recognize this entity. As the great imitator, oral secondary syphilis is commonly misdiagnosed or underdiagnosed because of its clinical presentations. It should be differentiated from an aphthous ulcer, oral candidiasis, lichen planus, erythema multiforme, hairy leukoplakia, pharyngolaryngitis, and even squamous cell carcinoma (Çakmak et al., 2019Çakmak S.K. This case report details an oral primary syphilis lesion in a 26-year-old white female patient who presented with a single ulcerated lesion on the left side of her lip, accompanied by spontaneous pain and a central black area. Serological tests, including Anti-HIV, complete blood count, and creatinine, were conducted. The patient tested negative for HIV but showed positive results for VDRL and FTA-ABS, confirming the diagnosis of syphilis.
