Abstract:Abstract Scimitar syndrome is characterized by partial or total anomalous pulmonary venous return from the right lung along with pulmonary hypoplasia. We present a case of a 37 weeks' gestation male infant with antenatal ultrasound findings of suspected partial anomalous pulmonary venous return (PAPVR) and coarctation of the aorta. The newborn presented with respiratory distress, a chest X-ray and chest computed tomography (CT) angiogram confirmed the diagnosis of scimitar syndrome. The combination of scimitar syndrome with aortic coarctation is extremely rare with only a few cases previously reported.

Abstract:Scimitar syndrome (SS) is a rare entity with an incidence of approximately 1–3 in 200 000 people. It is typically characterized by complete or partial anomalous pulmonary venous drainage from the right lung into the systemic venous circulation, most commonly the inferior vena cava (IVC). For the first time, we report the diagnosis of SS in a fetus in utero using four‐dimensional (4D) spatiotemporal image correlation combined with high‐definition live flow rendering mode (STIC‐HD live flow).

Abstract:Scimitar syndrome is characterized by the anomalous connection of the right pulmonary venous return to the hepatic portion of the inferior vena cava. Its name is derived from the characteristic image observed in chest X-ray, CT scan, or during pulmonary angiography in cardiac catheterization. It is more common among females and rarely affects the left lung. The importance of knowing its symptoms and presentation allows for a high diagnostic suspicion, thus avoiding the underdiagnosis of the disease. The prognosis is generally good, and timely diagnosis can prevent the occurrence of complications such as pulmonary hypertension or portal hypertension. We present the case of an eight-year-old female patient, who was previously evaluated for episodes of lower respiratory tract infections at 18 months of age, detecting only dextroposition, without any diagnostic workup. She was then sent to our office at eight years of age, with the onset of exercise-induced dyspnea. A comprehensive workup was conducted, with a diagnosis of scimitar syndrome.

A Scimitar Syndrome Variant Associated with Critical Aortic Coarctation in a Newborn

Abstract:BACKGROUND Scimitar syndrome (SCS) is a rare congenital cardiopulmonary malformation, characterized by anomalous pulmonary venous drainage from the right lung associated with aortopulmonary collateral arteries and pulmonary hypoplasia. The variant described in the case presented here, with total anomalous right pulmonary venous drainage into the superior and inferior vena cava, can be expected in 2% of patients with scimitar syndrome. To the best of our knowledge, the association between the variant of SCS and coarctation of aorta described in our patient has never been reported before in the literature. CASE REPORT A female newborn with a gestational age of 35 weeks presented with a rare combination of scimitar syndrome and aortic coarctation. The patient had a variant of SCS that included anomalous drainage of the right upper and lower pulmonary vein into the superior and inferior vena cava, respectively; relative right lung hypoplasia; and right lower lobe sequestration supplied by aortopulmonary collateral arteries that originated from the truncus coeliacus. The diagnosis was confirmed with computed tomography angiography after resection of the aortic coarctation with extended end-to-end anastomosis. Subsequently, interventional closure of the collateral artery supplying the right lower lobe was performed with an AMPLATZER™ Vascular Plug 4. The patient's clinical course was complicated by suspicious acute endocarditis and chylous pleural effusion. After a prolonged hospitalization, she was discharged in clinically stable condition. CONCLUSIONS The diagnosis of SCS should be considered when pulmonary hypertension persists after coarctation repair in a child with dextroposition of the heart and right lung hypoplasia. Successful treatment of this rare combination of conditions calls for teamwork by highly experienced specialists.

Abstract:Objectives-To determine whether the use of 4-dimensional (4D) sonography with spatiotemporal image correlation (STIC) and tomographic ultrasound imaging (TUI) can provide additional information with respect to 2-dimensional (2D) echocardiography in the prenatal diagnosis of anomalous pulmonary venous connections.Methods-The study population consisted of 10 cases that were initially suspected to have total or partial anomalous pulmonary venous connections by prenatal 2D echocardiography between January 2008 and April 2011. All 10 cases were further examined and analyzed by 4D sonography with STIC-TUI. Detailed postnatal surgery or autopsy was performed on all 10 fetuses.Results-Total anomalous pulmonary venous connections were found in 5 cases, and a partial connection was diagnosed in 1 fetus postnatally. The remaining 4 cases were confirmed to have normal pulmonary venous connections. Four of the 5 fetuses with anomalous pulmonary venous connections had an additional major cardiac defect; 1 fetus had an isolated connection. Anomalous drainage was supracardiac to the superior vena cava in 2 cases, cardiac to the coronary sinus in 3, and partially infracardiac to the portal vein in remaining case. The pulmonary venous connections were completely and correctly visualized with 2D echocardiography in 2 of the 10 cases, partially identified in 4, and not distinguished completely in 4. Four-dimensional sonography imaging with STIC-TUI clearly visualized the connections in 9 of the 10 cases, and the remaining case was partially identified.Conclusions-Four-dimensional sonography with STIC-TUI facilitates visualization of pulmonary venous connections, thus supplying additional information with respect to 2D echocardiography in the prenatal diagnosis of anomalous pulmonary venous connections.
