Abstract:Partial anomalous pulmonary venous connection (PAPVC) is a congenital heart defect in which one or more pulmonary veins drain abnormally into the systemic venous circulation, leading to the development of pulmonary arterial hypertension. It can be supracardiac type, draining into the superior vena cava or right atrium (also called cardiac type) and infracardiac type with drainage into the inferior vena cava (IVC). Abstract:To describe the anatomic variations of total anomalous pulmonary venous connection (TAPVC) and its corresponding surgical techniques.A total of 143 TAPVC subjects were hospitalized from April 1981 to July 2010. Those patients with other complex congenital heart diseases, such as transposition of great artery and single ventricle, were excluded. A pathological diagnosis was made by echocardiography, magnetic resonance imaging, computed tomography, catheterization and intra-operative findings. The specific types of TAPVC were as follows:supra-cardiac (49.7%, 71/143), cardiac (40.6%, 58/143), infra-cardiac (4.2%, 6/143) and mixed (5.6%, 8/143). The subtypes were classified by the pathway of common confluence, distribution of pulmonary vein and their orifice site. The techniques of surgical repairs included modified Warden procedure and pulmonary vein transplantation.The patients with supra-cardiac type were further divided into 4 subtypes according to the course of vertical veins and their orifice site: right and left veins forming a common confluence, then draining into vertical and innominate veins (n = 65); common confluence of pulmonary vein drainage into superior vena cava through a short vertical vein at the right pulmonary hilus (n = 3); right and left pulmonary veins separately draining into superior vena cava (n = 2); common confluence draining into innominate vein through a right path beside trachea (n = 1). Cardiac types were further divided into 3 subtypes: coronary sinus (n = 20), right atrium (n = 37) and right atrium & sinus (n = 1). Infra-cardiac type had no subtype. Mixed type was more complex and it was further divided into 3 subtypes: bilateral & symmetrical connection (right 2 + left 2, n = 5); bilateral & asymmetrical connection (3 + 1, n = 3). Surgical repairs were performed on 135 patients. The surgical mortality of TAPVC was 5.9% (8/135). And there was no late death. The major causes of death were pulmonary infection and low cardiac output syndrome.A detailed classification of TAPVC is of great importance for surgical approaches and methodological designs. And an individualized surgical plan yields excellent patient outcomes.

Abstract:Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital heart disease in which one or more pulmonary veins drain into the systemic venous circulation. The abnormal connection between the pulmonary vein and the right atrium can result in a right-sided volume overload due to a left-to-right shunt, followed by eventual right-sided pressure overload and right ventricular failure. PAPVC is usually associated with an atrial septal defect but can present as an isolated finding. We present a case of isolated PAPVC resulting in right heart failure and predominantly pre-capillary pulmonary hypertension. We discuss the challenges in the diagnosis and medical management of isolated PAPVC and highlight the clinical and hemodynamic indications for pulmonary vasodilators and diuretics.
Partial anomalous pulmonary venous connection associated with hemoptysis: A case report.

Abstract:Rationale: Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital anomaly characterized by the failure of fusion of embryologic pulmonary venous system with left atrium. Patient Concerns: A 45-year-old male patient with PAPVC who was hospitalized because of mild hemoptysis. Images showed the anomalous vein originated from the left upper pulmonary vein and flowed into the left brachiocephalic vein. No other underlying causes for hemoptysis were detected. Diagnosis: After multi-disciplinary discussion, the patient was diagnosed as PAPVC of left upper pulmonary vein draining into the left brachiocephalic vein with intact atrial septum. Interventions: Although surgical correction of PAPVC was feasible, left upper lobectomy was performed as the definitive treatment for both hemoptysis and PAPVC. Outcomes: The patient had an uneventful postoperative hospital course and was followed up for nearly 2 years without recurrence of hemoptysis. Lessons: PAPVC is associated with atrial septal defect in 80% to 90% of cases while isolated PAPVC with intact atrial septum is an extremely rare entity. We present a rare isolated PAPVC patient with hemoptysis. To our best knowledge, PAPVC associated with hemoptysis has never been reported before.

Abstract:Objective To investigate the anatomic features and surgical repair of cardiac total anomalous pulmonary venous connection(TAPVC).Methods Twenty-five patients underwent cardiac TAPVC repair between April 1981 and December 2006.Seventeen were males and 8 were females.The operative age ranged from 26 days to 14 years,mean(2.56±3.56)years.The weight ranged from 3.3 kg to 21.0 kg,mean(6.85±10.06)kg.According to Darling method,25 cases were divided into two subgroups(ⅡA and ⅡB).Diagnosis was made in all cases by echocardiography and cardiac catheterization preoperatively.All the patients underwent total correction under cardiopulmonary bypass.Resultb There was no hospital mortality.The main postoperative complications were low cardiac output syndrome and pulmonary edema in two cases.All patients were followed-up with a range of 3 months to 26 years.Heart function of all patients was grade L There was no postoperative pulmonary venous obstruction in these groups.Conclusions The surgical repair should be performed once the diagnosis is made.It is crueial to accurately understand the anatomic features and determine a suitable septum before operation.
Repair of Partial Anomalous Pulmonary Venous Connection: A Rare Case Evaluated by Multi-Slice Computed Tomographic Angiography

Abstract:Partial anomalous pulmonary venous connection (PAPVC) is an uncommon congenital heart disease, which may be difficult to identify and often remains undiagnosed. Accurate diagnosis of major aortopulmonary collaterals and partial anomalous pulmonary venous drainage in patients with congenital heart disease is important but problematic. The goal of this publication is to present the diagnosis and surgical repair of this rare pathology in an eight-year-old boy. Atrial septal defect was found by echocardiography, but no anomalous pulmonary vein was found. However, multi-slice computed tomographic angiography (MSCTA) revealed that the isolated right superior pulmonary vein was replaced by right superior pulmonary vein 1 (RSPV1), right superior pulmonary vein 2 (RSPV2) and right superior pulmonary vein 3 (RSPV3), which connected to the superior vena cava (SVC), the orifice of SVC, and the left atrium, respectively. The patient underwent the repair of PAPVC with division of the SVC and re-implantation on the right atrial appendage to restore normal systemic venous drainage. Postoperative course was uneventful. In conclusion, PAPVC is a rare congenital cardiac pathology. MSCTA could contribute to an accurate anatomic and functional definition of this variant.
Clinical Significance of Partial Anomalous Pulmonary Venous Connections (Isolated and Atrial Septal Defect Associated) Determined by Cardiovascular Magnetic Resonance

Abstract:Background: Partial anomalous venous connections (PAPVC) are associated with left to right shunting and right heart dilatation. Identification of PAPVC has increased with widespread use of cross-sectional imaging modalities. However, management strategies are mostly based on expert opinion given the scarcity of data from large series. We aimed to define types and significance of isolated and atrial septal defect (ASD) associated PAPVC detected by cardiovascular magnetic resonance. Methods: We retrospectively reviewed our cardiovascular magnetic resonance database from 2002 to 2018 to identify isolated or ASD-associated PAPVC cases. Results: A total of 215 patients (median age 46 years; range, 6-83) with isolated or ASD-associated PAPVC were identified among 102 135 clinical cardiovascular magnetic resonance studies. Of these, 104 were isolated and 111 were associated with an ASD. Anomalous connection of right upper pulmonary vein was the most common single venous anomaly (99/215), but in the isolated PAPVC group there were more anomalous left than right upper pulmonary veins (39 versus 34). The Qp/Qs was significantly higher for isolated anomalous single right upper pulmonary vein than left upper pulmonary vein (1.6 versus 1.4 respectively; P=0.01) as were right ventricular end-diastolic volumes (113.7±30.9 versus 90 [57-157] mL/m2, P=0.004). In the PAPVC with an ASD group, sinus venosus ASDs (82%) were associated with right-sided PAPVCs while both right and left-sided venous anomalies were seen in secundum ASDs (18%). In a substantial number of patients (30 out of 91) with sinus venosus ASDs, PAPVCs were more complex and involved more than a single anomalous right upper pulmonary vein; and in 5 patients with ASD, PAPVC was identified only after the ASD closure. Conclusions: This large series provides descriptive and hemodynamic features for isolated and ASD-associated PAPVCs. Anomalous isolated right upper pulmonary vein may cause a significant shunt (Qp/Qs >1.5). PAPVC associated with sinus venosus and secundum ASDs might be more complex than a single anomalous pulmonary vein and missed before ASD correction.

Abstract:Background Partial anomalous venous connection (PAPVC) is a rare congenital heart disease where the blood flow from one or more pulmonary veins (but not all) returns to the right atrium or systemic venous circulation and is often associated with a sinus venosus atrial defect (SVD). Transthoracic echocardiography (TTE) can provide limited information for this anomaly and the diagnosis of this congenital defect has been a clinical challenge. Case presentation We report here a case of a 75-year-old female with adult-onset pulmonary arterial hypertension (PAH), hypoxemia and right-sided chamber dilatation. The diagnosis of PAPVC was made incidentally by multidetector computed tomographic angiography (MCTA) that was performed to exclude pulmonary embolism. In this type of PAPVC, the atrial septum is intact, the right upper pulmonary vein (RUPV) connects to the superior vena cava (SVC), and the SVC overrides across the atrial septum and has bi-atrial connection, all of which are clearly manifested by MCTA. Conclusions This case indicates the need to exclude a PAPVC and SVD in unexplained pulmonary hypertension, and MCTA is a reliable non-invasive imaging technique with high resolution and wide anatomic coverage. The case also demonstrates that the coexisting SVD with PAPVC is an anomalous venous connection instead of atrial septal defect (ASD) and its key feature is the overriding of SVC or IVC across the intact atrial septum.

Abstract:Objective:To evaluate the value of Doppler echocardiography in diagnosing partial anomalous pulmonary venous connection(PAPVC).Methods:A retrospective echocardiography review of 132 patients with PAPVC confirmed by surgical operation.Results:According to Brody's classification.11/14 patients(78.6%) with type A PAPVC were diagnosed accurately by Doppler echocardiography.57/103 patients(55.3%) with type B were diagnosed.3/4 patients(75%) with type C were diagnosed.3 patients with type D and 8 patients with mixed type were unconfirmed by Doppler echocardiography.Amorg the 132 patierts,that of right anomalous pulmonary venous connection covers 90.1%,left anomalous pulmonary venous connection 7.4% and bilateral cases 2.5%.The most common associated anomaly was an atrial septal defect(ASD) in 121 patients(91.7%),40 patients(33.1%) having sinus venous ASD,18 patients(14.9%) having large ASD or single atrium,63 patients(52%) having secondary ASD,9 patients(6.8%) had an intact atrial septum.2 patients (1.5%) having partial sinus septal defects.Conclusion:The reflux of pulmonary vein and the continuity of atrial septeum could be demonstrated and PAPVC could be diagnosed accurately by Doppler echocardiography.

Abstract:OBJECTIVE:To investigate partial anomalous pulmonary venous connection (PAPVC) with echocardiography.METHODS:The right ventricular volume overload was detected by routine echocardiography in 37 child patients, who underwent further echocardiography to find the abnormal locations of pulmonary vein opening at superior, inferior vena cava and right atrium. The ultrasound results were compared with surgical findings.RESULTS:In 30 patients the ultrasound diagnosis was consistent with surgery results, 7 were misdiagnosed by ultrasound with a detective rate of 81.1 %. All 37 PAPVC patients presented varying degrees of right heart enlargement; PAPVC combined with atrial septal defect (ASD) was found in 34 cases.CONCLUSION:The possibility of PAPVC should be considered when unexplained right heart volume overload was detected by echocardiography. Superior, inferior vena cava and right atrium should be inspected when the pulmonary veins were not seen in echocardiography.

Abstract:Objective: To explore the features of echocardiographic partial anomalous pulmonary venous connection (PAPVC) with intact interatrial septum. Methods: Three patients with intact interatrial septum of PAPVC were examined by transthoracic echocardiography (TTE) using HP Sonos 5500 color Doppler machine. Imaging was performed at a frequency of 2-4 MHz, or 12 MHz. Results: Accuracy of the diagnosis by TTE was 100%. The patients had isolated PAPVC. The connection site was the coronary sinus in two, right atrium in one. There was a single anomalous connection vein in one patient, two in the others. Apparent right heart dilation was found in all the patients. Conclusion: Unexplained right heart volume overload that was examined by TTE should have a high-degree suspicion for PAPVC. The origin site of pulmonary veins and their flow should be examined routinely. Transesophageal echocardiography (TEE) and TTE are complementary in the diagnosis.
Features and Diagnostic Value of Echocardiography on Partial Anomalous Pulmonary Venous Connection

Abstract:AIM: To study the diagnostic value and the imaging characteristics of partial anomalous pulmonary venous connection (PAPVC) by echocardiography. METHODS: Thirty four patients with PAPVC and 20 patients with secondary atrial septal defect (SASD), who had been confirmed by operation or angiography, were involved in the study. PAPVC and SASD were compared in heart morphology. RESULTS: The connection of pulmonary veins in patients with PAPVC was as follows: 2 case in superior vena cava (SVC), 1 in left vertical vein, 24 in right atria (RA), 6 in coronary sinus and 1 in both SVC and RA. 7 cases (20.6%) had single connection of an anomalous vein, 25 cases (73.5%) had the connection of two anomalous veins and 2 cases (5.9%) had the connection of three anomalous veins. By echocardiography, the accuracy of diagnosis was 70.6% (24/34). Of the 10 cases were misdiagnosed, SASD was found in 9 cases and SASD with ventricular septal defect in 1 case. In PAPVC, the most commonly accompanied malformation was SASD (22/34). Corrected diameter of right atrium (RA) and right ventricle (RV) increased in both PAPVC and SASD. Ratios of RA to LA and RV to LV were significantly different ( P 0.05) between PAPVC and SASD although they all increased in PAPVC and SASD. CONCLUSIONS: The ratio of RA/LA1.5 is an important index for the diagnosis of PAPVC and the pulmonary venous connection should be included in routine examination.

Abstract:Introduction and importance: Partially anomalous pulmonary venous connection (PAPVC) is a rare congenital heart disease, often concomitant with atrial septal defects (ASDs). PAPVC usually tends to be treated by surgery, but the case we report will open up new perspectives for the interventional treatment of PAPVC present with ASD. Case presentation: We present a case of a 2-year-old 11 kg boy transthoracic echocardiography showed secundumtype ASD. A supracardiac-PAPVC was accidentally detected during cardiac catheterization, and an abnormal pulmonary vein connection was detected with a vertical vein (VV) opening. Ultimately, ASD and VV were both occluded. Clinical discussion: Surgical therapy of PAPVC is the first line treatment of most centers in the world. However, the main complications after surgical repair of PAPVC raise our concerns, such as pulmonary stenosis, caval vein stenosis and sinus node dysfunction. Therefore, percutaneous closure of PAPVC can be an alternative method. This case of percutaneous interventional closure of ASD and supracardiac PAPVC through a vertical vein in the same surgery was first reported. Patients with ASD tend to have missed diagnoses of PAPVC. We can evaluate it by transesophageal echocardiography (TEE), cardiac magnetic resonance imaging (CMR) and computed tomography (CT). Conclusions: This case suggests that the effect of interventional therapy is quite reliable. For children with ASD, attention should be paid to the omission of the presence or absence of PAPVC before surgery. During interventional therapy, a guide wire rather than a catheter should be preferred to explore the atrial septum and pulmonary veins to avoid a missed diagnosis of PAPVC.

Abstract:Congenital heart defect (CHD) is one of the most common birth defects and the leading course of infant mortality. Total anomalous pulmonary venous connection (TAPVC) is a rare type of cyanotic which accounting for approximately 1-3% of congenital heart disease cases. Based on where the anomalous veins drain, TAPVC can be divided into four subtypes: supracardiac, cardiac, infracardiac, and mixed. In TAPVC, all pulmonary veins fail to link to the left atrium correctly but make abnormal connections to the right atrium or systemic venous system. The mortality of TAPVC patients without proper intervention is nearly 80% in the first year of life and 50% of them died within 3 months after birth. However, the pathogenesis and mechanism of TAPVC remains elusive. In this chapter, we systematically review the epidemiology, anatomy, and pathophysiology of TAPVC and give an overview of the research progress of TAPVC pathogenesis.

Abstract:Rationale: Pulmonary artery sling (PAS) is a rare congenital anomaly. Associated airway anomalies and/or those of the cardiovascular system are present in about half the patients. Situs inversus is a rare disease in which organs of the chest and/or abdomen are arranged in a mirror image reversal of their normal position. Herein, we report a rare case of pulmonary artery sling and situs inversus incompletus, which has not yet been reported. Patient concerns: A 10-year-old girl was admitted because of heart murmur for more than 9 years. On physical examination, the second heart sound was prominent, and a grade 2/6 systolic murmur was heard at the left mid-sternal border. Echocardiography revealed PAS and atrial septal defect (8.6 mm). A chest computer tomography angiograph demonstrated that she had lung inversus, right aortic arch, and right lung hypoplasia in addition to PAS, with a normal positioning of the heart. The PAS intersected and twisted across the bronchus, which was obviously narrowed. The PAS was type II B, since the carina was at the T6 level without a separate right upper lobe bronchus. Diagnoses: Her final diagnosis was that of PAS, tracheal stenosis, situs inversus incompletus, right lung hypoplasia, right aortic arch, ASD and PDA. Interventions: She underwent one-stage total correction for her initial cardiovascular defects through median sternotomy under cardiopulmonary bypass support. Outcomes: She had an uneventful recovery and completely healthy following the procedure. Lessons: A thorough examination before PAS surgery was essential in discovering and carefully evaluating complicated heart and lung anomalies.

Abstract:Objective To evaluate the features and the regularity of partial anomalous pulmonary venous connection(PAPVC)by echocardiography.Methods Sixty-eight patients with PAPVC confirmed by operation were enrolled in the study,then their transthoracic echocardiographic data were analyzed retrospectively.Results In 68 patients with PAPVC,there were 10 cases of supracardiac type(14.7%,10/68),56 cases of intracardiac type(82.4%,56/68),1 case of infracardiac type(1.5%,1/68),and 1 case of mixed type(1.5%,1/68).And 18 cases(26.5%,18/68) had single connection of an anomalous vein,47 cases(69.1%,47/68) had the connection of two anomalous veins,and 3 cases(4.4%,3/68) had the connection of three anomalous veins.Fifty-eight cases(85.3%,58/68) with PAPVC and atrial septal defect(ASD) were proved.The rate of right upper pulmonary venous connection with superior vena cava type of atrial septal defect was 81.8%(9/11).Accurate rate of transthoracic echocardiography in diagnosis of PAPVC was 83.8%(57/68).Conclusion By observing the location and blood flow direction of pulmonary venous connection,transthoracic echocardiography can partly replace cardiac angiography in diagnosis of PAPVC.
A Rare Pulmonary-to-systemic Venous Connection Associated with the Partially Anomalous Pulmonary Venous Connection

Abstract:Background We report an extremely rare case of pulmonary-to-systemic venous connection associated with the partially anomalous pulmonary venous connection. Our case highlights that computed tomography angiography has a significant diagnostic and differential value for this rare anomaly. Methods and Results A 32-year-old male presented with chest discomfort for 2 weeks. The electrocardiogram and laboratory were unremarkable abnormalities. Transthoracic echocardiography suggested an enlargement of the right atrium and right ventricle. Cardiac computed tomography angiography images showed partial anomalous drainage of the right superior pulmonary vein into the superior caval vein, and a large pulmonary-to-systemic venous connection from the right middle pulmonary vein into the superior vena cava resulting in left atrial to systemic venous shunt. The patient underwent a successful thoracoscopic correction of partially anomalous pulmonary venous connection. Post-procedural course was uneventful, and he was discharged after 7 days. Discussion Pulmonary-to-systemic venous connection may also be called a levoatriocardinal vein. The embryological origin of this venous connection is an abnormal persistency of the splanchnic plexus connecting the pulmonary venous plexus and the cardinal system. The diagnosis of these rare types of the pulmonary to systemic communications has important clinical implications. We report an extremely rare case with pulmonary-to-systemic venous connection from the right middle pulmonary vein into the superior caval vein, although the connection might have been considered a levoatriocardinal vein, there is no separate formation of the right middle pulmonary vein at the time of existence of the splanchnic plexus, so it is inappropriate to explain the collateral channel encountered in our patient on the basis of an embryological remnant, a more appropriate term for these anomalies would be a "pulmonary-to-systemic collateral vein". Because the venous collateral pathway may be a source of the persistent left-to-right shunt and cause clinical manifestations of chronic right heart volume overload later in life. Therefore, we have performed successful thoracoscopic correction of partial anomalous pulmonary venous drainage. The Post-procedural course was uneventful. Conclusion The pulmonary-to-systemic venous connection associated with the partially anomalous pulmonary venous connection is a rare congenital malformation. Computed tomography angiography as a noninvasive imaging method plays an important diagnostic and differential value for this entity.
