Abstract:Objective To explore the experience in the perioperative treatment for osteoporotic fracture complicated with chronic renal insufficiency and its influence on the prognosis of the patients. Methods Sixty-nine patients with osteoporotic fracture complicated with chronic renal insufficiency were retrospectively analyzed. All the patients received surgery and were treated by team during the perioperative period. The comprehensive evaluation for treatment, preoperative retrospective analysis of the disease and treatment, and postoperative follow-up were carried out. Follow-up guidance, and the patient's perioperative renal function, anemia, wound conditions and fracture prognosis were analyzed and followed up. Results Among the 69 patients, there were 27 males and 42 females with an average age of (72.5±14.0). And there were 9 cases of spinal compression fracture, 39 hip fracture, 3 distal radius fracture, 7 proximal humerus fracture and 11 other limb fracture. There were 66 cases of chronic nephrogenic renal insufficiency, 1 case of pre-renal chronic renal insufficiency and 2 cases of post-renal chronic renal insufficiency. The mean preoperative hemoglobin content was (102.5±20.2)g/L, total protein (65.1±7.8)g/L, albumin (36.5±5.4)g/L, serum potassium (4.5±0.8)mmol/L, creatinine (293.5±267.8)μmol/L and glomerular filtration rate (GFR) (41.5±84.0)ml/min in the 69 patients. Towards chronic kidney disease (CKD), 11 cases were in stage Ⅱ, 27 cases were in stage Ⅲ, 9 in stage Ⅳ and 22 in stage Ⅴ. All the wounds had first intention healing after the operation. Sixty-two patients were followed up for (62.3±44.8) months. Ten patients died by the end of the follow-up. Nine of them were hip fracture patients. At the end of follow-up, the mortality rate was 16.1%. Conclusions The predilection sites of osteoporotic fracture complicated with chronic renal insufficiency are the hip and spine. Hip fracture is still the main disease affecting the prognosis of the patients. The controlling primary diseases by means of medical and surgical co-management is the premise to ensure the perioperative safety of the patients, but chronic renal insufficiency may still affect the patients' rehabilitation and prognosis after discharge.
