Abstract:Abstract Background There are few case reports in which non-cardiac surgery under regional anesthesia has been described in a patient with a pulmonary artery aneurysm. We report a case in which we managed a 74-year-old lady with a hip fracture for fixation under spinal anesthesia after thorough evaluation and meticulous planning. The patient was discharged home after 5 days without any perioperative events. Case presentation A 74-year-old, hypertensive lady with an incidentally detected pulmonary artery aneurysm was posted for a dynamic hip screw. Two-dimensional echocardiography revealed normal biventricular function with an ejection fraction of 60%, no regional wall motion abnormalities, concentric right ventricular hypertrophy (tricuspid annular plane systolic excursion or TAPSE of 19 mm), severe calcific pulmonary stenosis (PS) with pressures 90/61 mm Hg, pulmonary valve annulus of 12 mm, and pulmonary artery systolic pressure of 35 mm Hg. The left and right pulmonary artery diameters were 17 mm and 20 mm, respectively. She was clinically asymptomatic with an oxygen saturation of 98% on room air. A sequential lumbar epidural anesthesia was planned for this patient given her age, overall frail condition, and to avoid airway instrumentation. As epidural was difficult due to fused spaces, a sub-arachnoid block was administered with continuous monitoring. Surgery was uneventful and the patient was discharged on the 5th postoperative day. Conclusions With this case report, we suggest that a central neuraxial blockade could be administered in presence of severe PS with meticulous monitoring and careful planning.

Abstract:Aortic transection, a near-complete tear through the layers of the aorta, is a critical condition often resulting from trauma such as motor vehicle collisions. The urgency of managing aortic transection underscores the critical need for effective interventions. We report the case of a male in his early 50s with no significant medical history who presented to the emergency department following a motor vehicle collision, sustaining multiple injuries including a descending thoracic aortic transection. Rapid diagnostic assessment confirmed the severity of the injury, necessitating immediate intervention. Endovascular aortic repair was successfully employed, highlighting its efficacy in managing acute aortic injuries. The patient responded well to treatment, underscoring the importance of timely intervention in improving patient outcomes. This case emphasizes the critical role of rapid diagnostic assessment and endovascular intervention in managing life-threatening thoracic aortic injuries, particularly in the acute setting.

Anesthetic management of a patient with congenital cyanotic heart anomaly for emergency surgery: A case report

Abstract:Among congenital cyanotic heart diseases (CHDs), situs inversus totalis with transposition of great vessels with a large ventricular septal defect (VSD) has a very low incidence of around 1 in 10,000. Hereby, we present a 16-year-old man with the aforementioned cardiac anomaly with cardiac arrhythmias, septic shock, and a history of road traffic accident-causing osteomyelitis of the left thigh requiring incision and drainage. The patient was admitted to the intensive care unit with a high-grade fever, narrow pulse pressure, and atrial fibrillation. The patient was operated on under general anesthesia with endotracheal intubation after optimization. Invasive monitoring, antiarrhythmics, and vasopressors were required intraoperatively, and surgery progressed uneventfully. Furthermore, the patient had undergone a series of debridements after 8 days, which were performed under regional anesthesia uneventfully. This case report represents a plan of action for perioperative anesthetic management and anticipates the difficulties for CHD patients in the course of surgery and subsequential prudence.

Abstract:Total endovascular aortic arch repair is nowadays a promising alternative for selected patients with aortic arch disease. These endovascular procedures are technically challenging and require a diligent planning among all members of a multidisciplinary "Aorta Team" integrating cardiovascular anesthesiologists. In fact, endovascular aortic arch repair is a major challenge for anesthesiologists because of the risk of hemodynamic instability, cerebral events and acute kidney injury. In order to achieve the success, it is fundamental to discuss each patient in an individual basis, including perioperative management and care, and to be aware of surgical steps and their potential complications. Considering our previous experience with endovascular thoracoabdominal aortic surgery, we herein summarize our experience with anesthetic management of patients who underwent total endovascular aortic arch repair and its principal outcomes.

Abstract:Background: Blunt traumatic aortic injury (BTAI) is the second leading cause of death due to traumas in young patients. The primary presentation might be chest or interscapular pain, difficulty in breathing, and, in severe cases, hypotension. Considering the rapid deterioration of these patients' clinical conditions, prompt diagnosis and treatment initiation are crucial. In these injuries, the most involved parts of the aorta are the isthmus (distal to the left subclavian artery) and the descending part in the thorax. Therefore, the main diagnostic strategies include transthoracic echocardiography, CT angiography, and endovascular diagnostic approaches. Case presentation The patient was a 19-year-old male presenting with the symptoms of chest pain, dyspnea, and extremities excruciating pain after a car turnover. The initial evaluation showed no abnormal cardiovascular finding except bilateral hemothorax, addressed with chest tubes. Twelve hours later, when the patient was under observation for orthopedic surgeries, his chest pain and dyspnea started, and TTE and CTA showed a grade three descending aneurysm of the aorta. The patient was treated immediately with an endovascular procedure of stent implantation. A delayed debranching surgery was also performed, which resulted in desirable outcomes and uneventful follow-up. Conclusion: Although open thoracic surgery is the main and almost the only option for treating aneurysms of the aorta in hemodynamically unstable patients, the endovascular procedure has shown superior outcomes in selected patients with appropriate anatomy. Debranching surgery, which can be done simultaneously or with delay after the initial procedure, has proven protective against thromboembolic cerebral events. Clinical key point: Patients with an aneurysm of the aorta should be transported to a medical center with a multidisciplinary team for an urgent evaluation and treatment. The initial resuscitation and diagnosis are challenging, considering the fatal nature of these injuries, and the selection of the treatment is based on the patient's clinical condition and evaluated anatomy in cardiovascular imaging.

Abstract:Summary New innovations in the diagnostic laboratory achieve their full potential when they can be automated. Increasingly molecular biology (DNA) techniques are being utilised in traditional pathology disciplines, as well as the more recent ones of cytogenetics and molecular genetics. Molecular biology was first exploited for diagnostic purposes when Southern blotting became established. However the time‐consuming nature of the methodology, as well as the skills required, made it difficult for Southern blotting to be used routinely in the service laboratory. Subsequently the invention of PCR facilitated the approach to DNA diagnostics. Today PCR in commercial or home‐made kits is used for a range of procedures. The steps required to amplify DNA with PCR can also be fully automated. However the analysis of PCR products, which frequently requires slab gel electrophoresis and toxic chemicals or radioisotopes for visualisation, remains difficult to automate. An alternative way for analysing PCR products is now available through capillary electrophoresis. With this technique, automation can be extended to sample loading, electrophoresis and data analysis. An Unusual Presentation of Type B Aortic Dissection as Out-of-Hospital Cardiac Arrest Complicated by Spinal and Renal Ischaemia Along With Atrial Fibrillation, Stroke, and Severe Stenosis in Obtuse Marginal Branch: A Therapeutic Dilemma

Abstract:Aortic dissection (AD) is a catastrophic cardiovascular problem that can be challenging to diagnose sometimes. Despite diagnostic challenges, it requires a high degree of suspicion and prompt treatment is vital to its successful management. AD can be divided into type A aortic dissection (TAAD) and type B aortic dissection (TBAD). TAAD is characterised by dissection in the ascending aorta whereas TBAD does not have dissection in the ascending aorta. TBAD is usually managed conservatively, and patients receive medical therapy such as antihypertensive medications, analgesia, and rehabilitation. This, however, is complicated by malperfusion of certain organs, which can be life-threatening. Patients who have malperfusion of certain organs should be managed aggressively and endovascular aortic repair should be considered in such cases. We present a case of a 63-year-old patient who presented with out-of-hospital pulseless electrical activity cardiac arrest and was successfully resuscitated. An electrocardiogram showed new-onset atrial fibrillation with ST-segment depression and a coronary angiogram showed severe stenosis in the obtuse marginal branch of the left circumflex artery. A computed tomography scan of the thorax and abdomen showed TBAD with an occluded right renal artery and the patient was conservatively managed. The patient was discharged home after prolonged hospital admission and was conservatively managed for TBAD. This case was complicated by the fact that the patient had an out-of-hospital cardiac arrest and a coronary angiogram showed severe stenosis in the obtuse marginal branch of the left circumflex artery. The patient also had new-onset atrial fibrillation, which made his clinical management very challenging. It is important to avoid unnecessary coronary intervention that can create more challenges in managing such patients.
An Endovascular Treatment of a Thoracic Aortic Injury Caused by a Misplaced Pedicle Screw: Case Report and Review of the Literature

Abstract:Aortic injuries are devastating complications after spinal surgery. We here would like to share our experience with a successful endovascular treatment of an iatrogenic thoracic aortic injury caused by misplaced pedicle screw after spinal surgery. A patient underwent posterior spinal surgery for L1 burst fracture was transferred to our department for instrumentation removal. An iatrogenic thoracic aortic injury at T11 level caused by a pedicle screw was recognized after routine CT scans. Complete screw extraction and instrumentation removal were performed after the placement of a thoracic aortic stent graft covering the injured region. The patient had an uneventful postoperative period and no complications were observed in the 1 and 12-month follow-up by contrast-enhanced CT scans. For the delayed thoracic aortic injuries which usually were usually shown on CT scans as pseudoaneurysm or penetration of the aorta, stent graft implantation would provide a preferred solution with high-effectiveness, low-invasiveness and fewer complications compared with conventional open surgery.
Aortic Outcome after Implementation of a Treatment Strategy for Type B Aortic Dissection Involving the Aortic Arch

Abstract:Objective: To evaluate the outcomes achieved after implementing a treatment strategy for non-A non-B (NANB) (B 1-2 D according to the latest consensus document of the Society of Vascular Surgery (SVS) and Society of Thoracic Surgeons (STS) acute aortic dissection (AAD)). Methods: This retrospective observational study adhered to the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) checklist. All cases of NANB AAD (B 1-2 D) treated at our institution between January 2016 and December 2022 were reviewed. Morbidity, mortality, aortic-related reintervention, and remodeling were analyzed. Results: Among 519 cases of acute aortic syndrome, n = 22 (4.2%) patients presented with NANB AAD (B 1-2 D) (n = 16,72.7% men, mean age 61.5 years+/14.7). Eleven cases were managed with best medical treatment (BMT) alone. Among them, one patient (9.1%) died suddenly 2 days after diagnosis for aortic rupture. Frozen elephant trunk procedure (FET) was required in the remaining 11 patients: 7 (31.8%) needed emergent operation for risks of impending aortic rupture or retrograde aortic dissection extension, and 4 (26.7%) underwent delayed surgery within a month from initial presentation. Overall, in-hospital mortality was 9.1% with both FET and BMT. At a median follow-up of 40 months (range 2 days-200 months) no other deaths occurred. A statistically significant differences in the rate of false lumen thrombosis (100% vs 55.5%, P = 0.033), and a significant positive aortic remodeling in zone 3 (P < 0.001) and 4 (P = 0.038) were reported in operated versus medically managed patients. Conclusions: The best treatment for NANB is not established. We advocate for medical stabilization with an operative approach that favors open surgery in the acute post dissection period, promotes aortic remodeling, and carries acceptable risk in centers where FET is performed routinely.

Abstract:Articular cartilage protects opposing bone extremities in diarthrodial joints in the form of a highly resilient, shock-absorbing cushion lined with a well-lubricated surface. Built during development, it undergoes limited turnover in adulthood and responds poorly to the various environmental and genetic adverse conditions that it can be subjected to in a lifetime. Consequently, damage to the tissue is generally irreversible and becomes gradually worse, leading to common joint degenerative diseases collectively referred to as osteoarthritis (1). These diseases can cause severe joint pain, deformation, and incapacitation. Intense research efforts have therefore been deployed over the last decades to decipher mechanisms that underlie articular cartilage development, adult maintenance and degeneration, and to discover ways to prevent osteoarthritis and regenerate healthy joints (2–4). Recent findings have further stimulated these efforts by eliciting hope that articular stem cells (ASCs) may be present or inducible in adult joints and could thus be used to design efficient cell-based therapies (5–8) Some studies have pinpointed ASCs in the superficial layers of articular cartilage by exploiting cell quiescence (very slow cycling), a defining property of stem cells, and others have proposed the existence of ASCs in articular cartilage, synovium and intrapatellar fad pad surfaces based on the ability of cells to express mesenchymal markers in vivo and to undergo chondrocyte differentiation in vitro. Lineage-tracing experiments in the mouse using a Gdf5Cre transgene along with a Rosa26floxlacZ CRE recombinase reporter have demonstrated that articular chondrocytes and other joint cell types arise during development from a common pool of mesenchymal progenitors and that these progenitors are distinct from those for growth plate chondrocytes and endochondral bone osteoblasts (9,10). It is thus believed that some of these embryonic joint progenitors may persist as ASCs in adult articular tissues. However, since this Gdf5Cre/Rosa26floxlacZ genetic strategy activates lacZ expression in all embryonic joint precursor cells and transmits this property to all daughter cells for life, it has not allowed addressing a number of important questions, such as the following: do joint surface-lubricating cells and articular chondrocytes arise as independent lineages from joint progenitors or do joint surface-lubricating cells give rise to articular chondrocytes? An elegant study published in this issue of Arthritis and Rheumatology by the Harvard University groups of Andrew Lassar and Matthew Warman describes a novel lineage-tracing strategy that will greatly help answer such questions and that already starts tackling some of them (11).

Abstract:This case report illustrates the complexity and severity of acute aortic dissection. This condition has one of the highest mortality rates of any cardiovascular emergency and is often extremely challenging to treat with both open and endovascular intervention often required. This patient presented with a ruptured aortic dissection which is rare and often immediately fatal. He survived urgent extensive aortic endovascular stenting, but, despite preventative measures, developed spinal cord ischaemia post-intervention. The contemporary management of acute aortic dissection, and the pathophysiology and prevention of spinal cord ischaemia are covered in this fantastic case report.

Abstract:Background: Aortic dissection is a life-threatening condition characterized by a tear in the aortic wall that separates its layers. Complete aortic dissection involves both the ascending and descending aorta, necessitating prompt diagnosis and intervention. We present a case report of a 65-year-old male with a complete aortic dissection and discuss the challenges and management strategies employed in his treatment. Case Presentation: A 75-year-old male with a history of hypertension presented to the emergency department with sudden-onset severe chest and back pain. Physical examination revealed unequal blood pressure readings in both arms, suggesting aortic involvement. Urgent computed tomography angiography (CTA) and TTE confirmed the diagnosis of a complete aortic dissection extending from the aortic root to the iliac arteries. The patient was promptly transferred to the operating room for surgical intervention. Discussion: The management of complete aortic dissection requires a multidisciplinary approach involving cardiovascular surgeons, interventional radiologists, and anesthesiologists. In this case, emergency surgery was performed to address the life-threatening condition. Aortic root replacement and ascending aorta repair were performed, followed by stent graft placement in the descending aorta to exclude the false lumen. Postoperative care involved close monitoring for potential complications such as renal dysfunction, paraplegia, and cardiac ischemia. Conclusion: Prompt diagnosis and appropriate surgical intervention are crucial in the management of complete aortic dissection. Collaboration among healthcare professionals is essential for optimizing patient outcomes. Further studies are warranted to assess long-term outcomes and identify strategies for preventing complications in patients with complete aortic dissection.

Abstract:Journal of Endovascular Therapy, Ahead of Print. Purpose:Instances of aortic lesions combined with thoracolumbar spine fractures have been rarely described, primarily occurring in high-speed motor vehicle collisions and falls from heights.Case Report:We present a unique case of a 71-year-old woman with a thoracic aortic rupture caused by a thoracolumbar spine fracture during an accidental domestic fall. She experienced an injury at the level of the celiac trunk on the posterior aspect of the aorta. Hemorrhagic shock and profound hemodynamic instability prompted the swift initiation of a percutaneous endovascular approach, involving the use of REBOA (Resuscitative Endovascular Balloon Occlusion of the Aorta) followed by the parallel graft technique in a sandwich configuration. Subsequently, thoracolumbar spinal fixation was carefully scheduled and performed 20 days after achieving clinical stabilization.Conclusions:This case underscores the importance of a multidisciplinary approach in managing complex aortic and spinal injuries. The combined use of REBOA and the sandwich graft technique can be life-saving in managing acute thoracic aortic injuries, allowing for the staged stabilization and repair of associated spinal fractures.Clinical ImpactThis case highlights a novel approach to managing a rare thoracic aortic rupture caused by an accidental domestic fall. The application of REBOA for immediate hemodynamic stabilization, combined with the use of a sandwich graft configuration, underscores the adaptability of advanced endovascular techniques in complex, life-threatening situations. These interventions demonstrate the importance of tailoring treatment strategies to individual patient needs, especially in cases involving atypical mechanisms of injury. Abstract:We present the case of a 59-year-old male who sustained an ascending aortic injury and a subdural hematoma after a head on collision. After undergoing emergent craniotomy for evacuation of the subdural hematoma, he was maintained with strict blood pressure control. Once able to be safely anticoagulated, he underwent replacement of the ascending aorta. This exceedingly rare case was managed by a multidisciplinary team approach that led to an optimal outcome given his disastrous multi-traumatic injuries.

Abstract:Objective: To summarize the anesthetic management experiences of patients with Stanford A aortic dissection( AD) undergoing surgical treatment under cardiopulmonary bypass( CPB). Methods: From January2012 to January 2013,a total of 55 patients with Stanford A AD underwent replacement of the ascending aorta and aortic arch,implantation of descending aorta stent under CPB with deep hypothermic circulatory arrest( DHCA) and selective cerebral perfusion. All the patients underwent general anesthesia and endotracheal intubation. In the operation,we closely monitored the hemodynamic parameters,electrocardiogram and oxygen saturation. We maintained stable breathing,circulation and internal environment. Results: CPB time was 162-493( 255. 31 ±42. 36) min and aortic cross-clamp time was 86-282( 170. 61 ± 43. 58) min. The time of DHCA was 13-55( 36. 23 ± 18. 14) min. All the patients were sent to intensive care unit. Eight patients died. Cause of death after the operation was respectively bleeding and heart failure,heart failure,infection and multiple organ failure,cerebral infarction and liver and kidney failure. Conclusion: Strengthening the vital organs and blood protection is the key to reduce the postoperative complications and mortality.
Aortic arch surgery for type B aortic dissection: How far should we go? The value of a hybrid approach

Abstract:Traditionally, the management of type B aortic dissection has been the domain of the vascular surgeons. Timing and type of intervention still generate debate. We sought to review our early experience with the treatment of this condition based on a hybrid approach following an aortic multi-disciplinary team meeting involving close cooperation between cardiac surgeons, vascular surgeons, interventional radiologists, vascular anesthetists, and cardiac anesthetists. Four patients (age 41-56 years; 3 males; 1 female) with type B aortic dissection underwent aortic arch surgery through a hybrid approach: one elective procedure consisting of ascending aorta and hemi-arch replacement with debranching followed by thoracic endovascular aortic repair (TEVAR); one redo procedure requiring aortic arch replacement with hybrid frozen elephant trunk; two acute presentations (aortic arch replacement and debranching followed by TEVAR; AVR with ascending aorta, arch, and proximal descending thoracic aorta replacement with conventional elephant trunk and debranching). Deep hypothermic circulatory arrest was required in three patients. Despite respiratory complications and slightly prolonged postoperative course, all patients survived without onset of stroke, paraplegia, malperfusion, endoleak, or need for re-exploration. Follow-up remains satisfactory. Different factors may affect outcome following complex aortic procedures. Nevertheless, close cooperation between cardiac surgeons, vascular surgeons, and interventional radiologists may reduce potential for complications and address aspects that may not be completely within the domain of individual specialists.
