Abstract:Purpose: Six issues in pharmaceutical reimbursement arising from recent or proposed changes in Medicare rules or rates or the health care environment are described, along with possible strategies for addressing these issues. Summary: Recent reductions in Medicare pharmaceutical reimbursement rates, increases in use of specialty pharmacies, proposed changes in rules for erythropoiesis-stimulating agent (ESA) reimbursement, increases in need for patient assistance programs, proposed changes in the clinical trial policy, and possible reductions in reimbursement for cases involving medication errors or other negative events as part of pay-for-performance initiatives are cause for concern. A multidisciplinary team approach to improving reimbursement can reduce institutional financial losses and increase options for Medicare beneficiaries. Health-system pharmacists can address concerns about the use of specialty pharmacies by collaborating with payers and specialty pharmacies on the development of workable models for safely obtaining, handling, and administering medications. Proposed rules for ESA reimbursement contain numerous flaws that could compromise patient outcomes. Pharmacists should provide the Centers for Medicare and Medicaid Services and their fiscal intermediaries with feedback on the potential impact of the proposed rules. The use of patient assistance programs can decrease financial losses associated with providing pharmaceuticals to underinsured or uninsured patients, but these programs can present pharmacists with logistical challenges. The proposed clinical research policy gives priority for research efforts and limited health care dollars to the health issues that are most important to Medicare beneficiaries, but details pertaining to implementation and funding have not yet been worked out. Pharmacists should voice opposition to pay-for-performance models that do not support a nonpunitive culture of openness in reporting quality indicators. Conclusion: An understanding of issues in the current health care environment arising from recent and proposed changes in Medicare rules and rates for pharmaceutical reimbursement can help position pharmacists to manage these issues.
Strategies to Help Patients Navigate High Prescription Drug Costs

Abstract:Importance: In the US, many patients struggle to afford prescription drugs, leading to adverse health outcomes. To improve cost-related medication nonadherence, prescribers and clinical staff must understand how to assist patients in overcoming high prescription drug costs. Observations: We reviewed the benefits and limitations of 7 strategies to help patients afford prescription drugs: co-payment cards, patient assistance programs, pharmacy coupons, direct-to-consumer pharmacies, public assistance programs, international online pharmacies, and real-time prescription benefit tools. We created an algorithm to help clinicians identify appropriate strategies based on a patient's health insurance and the type of drug (brand-name vs generic). For example, co-payment cards can lower out-of-pocket costs for privately insured patients taking brand-name prescription drugs. For uninsured individuals or those with public insurance like Medicare Part D who meet financial eligibility criteria, patient assistance or public assistance programs may be available. All patients, regardless of health insurance, can forgo insurance and purchase drugs directly using pharmacy coupons or direct-to-consumer pharmacies, which sometimes offer lower prices for generic drugs compared to insurance. For insured patients, such purchases do not count toward insurance deductibles or annual out-of-pocket maximums. Online international pharmacies provide a last resort for patients in need of brand-name drugs who lack affordable domestic options. Increasingly, prescribers can use real-time prescription drug benefit tools to estimate patient out-of-pocket costs and identify alternative lower-cost treatments for insured patients, but these tools can be inaccurate or incomplete. Conclusions and relevance: The current patchwork of strategies to help patients manage high prescription drug costs highlights the structural and policy challenges within the US prescription drug market that impede affordable access for some patients. While these strategies provide tangible solutions for clinicians to help patients access medically appropriate but costly medications, they do not address the root causes of high drug prices.

Abstract:Objective Evaluate the drug-use state which was infected by the adjustments of medicine price's evaluation methods in Campus hospital of Zhejiang University,objective to provide evidence based strategies on guiding reasonable clinical medication and the drug manufacturing and trade system.maintenance.Methods The consumption of medicine in terms of quantity and amount of money in our hospital during 2007～2008 was investigated.The date was analyzed using DDDs sequence analytical methods.Results In 2008,the money of collecting medicine increased by 26.78 percent but the retail sales only increased by 19.75 percent,over 2007.No significant changes were found in medicine using.Conclusion The bubble price of medicine and the burden on patients could be reduced by the adjustments of medicine price's evaluation methods.

Charging for hospital pharmaceutical services: product cost, per diem fees and fees for special clinical services

Abstract:A method of charging for pharmaceutical services is described which includes the cost of drug products, dispensing fees for intravenous drug admixtures, per diem fees for basic dispensing and clinical services (according to patient type), and fees for special clinical services. The basic per diem charge covers drug purchasing and inventory control, department management, drug information services, dose preparation, drug order interpretation, drug therapy monitoring, and the availability of pharmacists to answer the questions of nurses and physicians. The clinical services for which special fees are charged are: hemogram drug report, drug liver function report, aminoglycoside dosing guidelines, heparin i.v. infusion, oral anticoagulation, patient drug history, operant conditioning, parenteral nutrition guidelines, and pharmacokinetic drug level interpretations and consultations. The reasons for changing to the system, the services provided and experiences with the system are discussed. Implementation of the system is discussed in relation to the calculation of fees; comparisons with alternate charging methods; approval of special clinical service charges; computer billing; information about pharmacy charges for patients; and third-party payers.
Cheaper is not always better: Drug shortages in the United States and a value-based solution to alleviate them

Abstract:Drug shortages threaten patients' access to medications and are associated with adverse health outcomes and increased costs. Drug shortages disproportionately occur among generic drugs of limited profitability, most notably drugs administered by injection. In this perspective, we discuss how reimbursement and purchasing practices that were meant to create an efficient marketplace for generics have generated strong price pressure that threatens profitability in certain markets. We further explain how, faced with limited profitability, manufacturers lack incentives to invest in resilient supply chains, and in some cases, engage in cost-containment strategies or decide to exit the market, ultimately contributing to shortages. We propose the development and implementation of value-based reimbursement to provide needed incentives for drug purchasers and manufacturers to establish a more reliable supply chain as part of the policy solution to reduce the number and extent of drug shortages. This reimbursement model would necessitate the development of a rating system that measures supply chain resilience and maturity for each generic product. This rating would then be applied as a value-based modifier to reimbursement rates for generic products. The proposed model would result in higher reimbursement rates for generic products from more dependable supply chains, generating incentives for manufacturers to invest in supply chain resiliency. We propose the application of this reimbursement system originally in Medicare given Congressional interest on reforming Medicare payment to prevent drug shortages.

Abstract:All tasks performed by hospital pharmacies concern the management of medicinal products. The aim of this work is to analyze the functioning and implemen-tation costs of unit-dose and multi-dose automatic drug distribution systems in comparison with the classical mode of so-called ward medicine cabinets. The study was conducted from January 2020 to October 2021. It included quantitative study, conducted to verify the functioning of hospital phar-macies based on an original questionnaire, which was completed by 26 masters of pharmacy em-ployed in pharmacies hospitals, of which 92.4% were pharmacy head, and in-depth face-to-face interviews conducted in 4 facilities with unit-dose systems and one center that uses the multi-dose system. The work presents the functioning of hospital pharmacies, and the advantages and disadvantages of classical distri-bution model. Opinions on the operation of unit-dose and multi-dose systems as well as experiences related to their implementation in the hospital, including the necessary changes in the structure and organization of the hospital, were also presented. In conclusion, despite high purchase costs, auto-mated drug distribution systems bring many benefits in the field of hospital functioning, mainly in relation to patient care, improving the effectiveness, safety and individualization of pharmacothera-py. The main barriers to implementation of these systems are high costs, lack of IT infrastructure adaptation, the need to change the organization of staff work and the lack of factory-prepared col-lective packaging. In the opinion of pharmacists, changes in the distribution of drugs are necessary and beneficial from proper pharmacotherapy perspective.

Abstract:Several interrelated strategies involving physician leadership and participation have been proposed to contain health care costs while preserving or improving quality. These include programs targeting the 10% of the population that incurs 70% of health care expenditures, disease management programs to prevent costly complications of chronic conditions, efforts to reduce medical errors, the strengthening of primary care practice, decision support tools to avoid inappropriate services, and improved diffusion of technology assessment. An example of a cost-reducing, quality-enhancing program is post-hospital nurse monitoring and intervention for patients at high risk for repeated hospitalization for congestive heart failure. Disease management programs that target groups with a chronic condition rather than focusing efforts on high-utilizing individuals may be effective in improving quality but may not reduce costs. Error reduction has great potential to improve quality while reducing costs, although the probable cost reduction is a small portion of national health care expenditures. Access to primary care has been shown to correlate with reduced hospital use while preserving quality. Inappropriate care and overuse of new technologies can be reduced through shared decision-making between well-informed physicians and patients. Physicians have a central role to play in fostering these quality-enhancing strategies that can help to slow the growth of health care expenditures.

Abstract:The Inflation Reduction Act (IRA) requires Medicare to negotiate lower prices for some medicines with high Medicare spending. Using historical data from public and proprietary sources to apply the IRA's negotiation criteria retrospectively, we identify all drugs that met the eligibility criteria from 2012 to 2021 to classify drugs that would have had a negotiated price in effect in 2022 and to calculate associated decreases in industry revenues. Our results suggest that the IRA's reduction in overall industry revenue will be modest, will not affect most top-selling drugs and will not likely result in large-scale defunding of research and development. Changes in the net present value of drug-development projects will be concentrated in medicines where Medicare is a notable purchaser and where the ratio between expected revenue and development costs was only marginally positive before the IRA. Policymakers considering narrowing or expanding the scope of Medicare negotiation should carefully consider the tradeoffs across medicines with diverse characteristics.

Abstract:Background and objectives: Governments in Asia Pacific (APAC) are increasingly using pharmaceutical pricing strategies to contain rising healthcare costs. The objective of this narrative review is to discuss formal pricing strategies for reimbursed prescription medication in APAC, supported by relevant examples of implementation differences across countries. In the discussion section, we examine key advantages and disadvantages of each strategy. Methods: A narrative review of the peer-reviewed and grey literature was undertaken to retrieve information, including strategy definitions, practising countries, country-specific implementation considerations, and merits and demerits of each strategy. Results: Seven strategies (Internal Reference Pricing, External Reference Pricing, Special Pricing Agreements, Pharmacoeconomic Evaluation, Cost plus pricing, Price Maintenance Premium, and Tendering and negotiations) were identified as most commonly practised in APAC through the review process. Most countries use multiple strategies that differ in how they are implemented. Conclusion: APAC countries use multiple strategies simultaneously with varying implementation methods, including different formulae and sub-types of medication that a strategy applies to, whether the strategy is a mandate or guideline, and the extent of negotiations and transparency. Strategies are instituted partly with the aim of cost containment, and may also promote price stability, innovation, and increased access in the short and longer term. Administrative action on drug pricing: Lessons and opportunities for the Center for Medicare and Medicaid Innovation

Abstract:Background: In October 2022, the Biden administration issued an executive order to the Center for Medicare and Medicaid Innovation (CMMI) to develop new health care payment and delivery models to lower prescription drug costs and promote access to innovative therapies. In response, the agency proposed 3 novel drug payment models for testing. Objective: To understand the impact that CMMI demonstration projects can have on the prescription drug market. Methods: We examined each of the models listed on the CMMI website and searched the Federal Register and news articles for additional models that contained interventions related to patient out-of-pocket drug costs, Medicare drug spending, or Medicaid drug spending. We excluded models with indirect effects on drug costs (for example, bundled payments). We comprehensively reviewed all previous cases in which CMMI has attempted models addressing prescription drug costs and spending and evaluated the circumstances, impact, and lessons learned that may aid policymakers in the design and implementation of new models. Results: We identified 9 CMMI models containing direct interventions related to drug costs. Among prior models addressing drug prices, nearly half (44%, 4/9) were not implemented because of their scope, voluntary nature, and procedural challenges. No implemented models met the CMMI standard for expansion, although key elements of the Senior Savings model limiting monthly insulin costs to $35 were later incorporated into the Inflation Reduction Act. Conclusions: In future CMMI implementation efforts, we suggest maximizing voluntary collaboration when selection bias concerns are minimal, using mandatory models when not, ensuring that the geographic scope is not overly ambitious, and adhering closely to statutory authority and established administrative procedure to minimize legal challenges and maximize model demonstration utility.
Identification and proposed control of helicopter transmission noise at the source

Abstract:Helicopter cabin interiors require noise treatment which is expensive and adds weight. The gears inside the main power transmission are major sources of cabin noise. Work conducted by the NASA Lewis Research Center in measuring cabin interior noise and in relating the noise spectrum to the gear vibration of the Army OH-58 helicopter is described. Flight test data indicate that the planetary gear train is a major source of cabin noise and that other low frequency sources are present that could dominate the cabin noise. Companion vibration measurements were made in a transmission test stand, revealing that the single largest contributor to the transmission vibration was the spiral bevel gear mesh. The current understanding of the nature and causes of gear and transmission noise is discussed. It is believed that the kinematical errors of the gear mesh have a strong influence on that noise. The completed NASA/Army sponsored research that applies to transmission noise reduction is summarized. The continuing research program is also reviewed.

Abstract:Formularies are a tool for managing costs, optimizing patient access, and improving overall health outcomes. The general goal of formularies is to provide access to appropriate therapy while promoting effective resource utilization, which allows the managed care pharmacy organization to operate sustainably. Traditional formulary strategies have included open and closed formularies as well as tiered formularies. However, other formulary structures have emerged in support of the focus on product value. The formulary development process is primarily driven by the pharmacy and therapeutics (P&T) committee and value committee within an organization. Key considerations such as member population, regional differences, regulatory/compliance implications, and benefit design strategies may influence payers to create a customized formulary to provide additional value to their members while managing costs. With the rise of high-cost and specialty products, formularies continue to serve as an important tool for managed care pharmacy organizations. Ongoing trends, such as biosimilars, prescription digital therapeutics, and addressing health equity, will shape future strategy and management of formularies.

Abstract:The USA pays more for brand-name prescription drugs than any other country and new legislation from August 2022 gives Medicare the authority to directly negotiate certain drug prices with manufacturers starting in 2026—something the federal insurer had been prohibited from doing for its prior history. As the USA prepares for negotiations, we therefore surveyed how comparable industrialized countries use statutory requirements and procedures to negotiate brand-name drug prices. Guidance documents, regulations, government and academic publications were reviewed to identify the process of negotiating drug prices in peer countries that have been cited as potential examples for US payment reform: Australia, Canada, France, Germany, and the UK. Processes for arriving at a final price for a drug generally fall under three approaches: statutory rebates, setting a maximum price, and arbitration between national (public) insurers and manufacturers. Each approach to price negotiation could be adopted by Medicare and reduce spending even if Medicare does not adopt an exclusionary or closed formulary. Much remains to be determined about how the new price negotiation authority in the USA will be implemented, and policymakers can learn from comparator countries' statutory and regulatory strategies for price negotiation.

Escalating costs of innovative medicines: perspective and proposals

Abstract:Public healthcare systems are challenged by the soaring costs of medications that require increasing resources, often at the expense of other investments. The increasing pharmaceutical budget poses a threat to the allocation of funds for essential preventive and primary healthcare services while also raising concerns about equitable access, particularly in models where patients bear part of the costs out of their own pockets. Proposals on how to ensure ongoing and long-term accessibility, efficiency, and financial stability are required. The escalating costs of medicines may be explained in part by the mismatch between the traditional value-based pricing and reimbursement frameworks and the type of clinical development of targeted therapies and precision medicine in clinical practice. New appraisal methods and managed access strategies should be adapted to therapies targeting small populations and addressing increased uncertainty. Fair pricing strategies, transparent healthcare investments based on problems and outcomes, regulatory reforms, international cooperation, and critically examining the drug acquisition model are potential solutions. Transitioning from an industry-driven pricing approach to a health-driven payment model can help align the cost of treatments with actual health outcomes, establishing a foundation for a healthcare system that addresses immediate challenges and fosters long-term well-being. Acknowledging the lack of a universally applicable solution, the practical implementation of interventions requires a reframing of the pricing and access system and adaption to the targeted therapeutic approaches. Balancing innovation with financial sustainability necessitates a collaborative, adaptive, and transparent approach, as well as transitioning toward health-driven payment models, moving the focus from the cost of medications to the well-being of populations worldwide.
PharmaTrend as a management tool: implementation in a university hospital

Abstract:The implementation of ASHP's hospital pharmacy management information system, PharmaTrend, in a university hospital is described. The pharmacy department at the University of Wisconsin Hospital and Clinics (UWHC) is a decentralized operation with 63.5 pharmacist full-time equivalents (FTEs) and 81.6 supportive staff FTEs. The multipharmacy capability of PharmaTrend, which allows workload and productivity data for several pharmacies (usually satellites) to be combined into total departmental data for a given period, was used by UWHC to describe the pharmacy department's major cost centers individually. These centers were inpatient services, outpatient services, home health i.v. services, and two offsite pharmacies. Data were divided into elements collected by computer, elements not previously collected or requiring new procedures, and elements requiring manual collection. Data entry forms specific to each cost center were developed. With these forms, collected data can be categorized appropriately by technical staff members. Work hour allocation for each cost center was determined on the basis of PharmaTrend definitions and institution-specific considerations; only those hours reflecting direct fiscal burden of the department on the hospital were included. A manual process for collecting and categorizing data on hours worked and hours paid was automated and adapted for use in PharmaTrend. Finally, some preliminary experience with the system was obtained so that information could be collected for the development of policies, procedures, and training methods and on the use of forms and categorization methods. In particular, the time requirements for collecting, categorizing, and entering selected PharmaTrend data by cost center were determined. Optimal use of PharmaTrend requires creativity and a commitment of time and effort.
How Should We Draw on Pharmacists' Expertise to Manage Drug Shortages in Hospitals?

Abstract:This article argues that drug shortages should be addressed as crises that exacerbate already compromised US health care infrastructure. Clinicians, especially pharmacists, can help limit threats that shortages pose to patients. For example, pharmacists can canvass procurement options, consolidate inventory, and prepare medications to prevent need for some clinical interventions. This article describes how pharmacists' preparation and training equip them to help clinical teams navigate shortages by equitably rationing limited medicines, suggesting appropriate therapeutic alternatives, modifying drug administration routes, or delaying interventions. Pharmacists' roles can be key, since good management of supplies during drug shortages can mitigate risk of worse-than-usual clinical outcomes, mitigate risk of medication errors, and reduce some financial burdens on the overall health care system.
Pricing, profits and pharmacoeconomics--for whose benefit?

Abstract:In today's troubled healthcare climate, it is not uncommon to run across headlines like: 'Health insurance premiums increasing by 10 percent to 30 percent across the country.' This particular New York Times article went on to explain that this premium price hike, the third consecutive double digit increase in 3 years, is driven largely by escalating pharmaceutical costs. The pharmaceutical industry has largely been vilified in the media and in the recent presidential debates, for fueling healthcare inflation and setting what many perceive to be 'unfair' prices in light of the profit margins on their life-saving products. A report released by the Congressional Research Service found that after tax, profits for the pharmaceutical industry averaged 17% of sales, compared with 5% for all other industries. The White House has added its voice to the popular discontent with notices such as this one reported in the New York Times: 'There is a rising tide politically in this country of strong antagonism against the pharmaceutical industry on the dimension of prices. (Without expanded access to insurance) price controls are an inevitable outcome.' Although the prospect of price control remains dubious in America's entrenched laissez-faire economy, David Kessler, former head of the FDA and the Dean of the Yale School of Medicine, described the situation as a 'powder keg,' stating 'the current system is simply not sustainable'. Although there does not appear to be an immediate solution to this escalating crisis, this editorial will examine pharmaceutical pricing, industry profits and the role of pharmacoeconomic analyses amidst the chaos.
