Abstract:OBJECTIVE:To evaluate the effect of intensive treatment on the blood sugar, blood lipids and blood pressure levels in incipient diabetes II patients.METHODS:One hundred and sixty incipient diabetes patients were allocated into two groups according to chronological order: 80 cases received routine treatment and 80 cases received intensive treatment. Fasting blood-glucose (FBG), glycosylated hemoglobin (HbA1C), blood pressure, blood cholesterol (TC), triglyceride (TG), LDL cholesterol-C (LDL-C), alanine aminotransferase (ALT) and aspertate aminotransferase (AST) were tested before treatment. For intensive treatment group blood pressure, blood sugar and blood lipids were regularly tested, and the therapeutic protocols were adjusted according to the test results until the therapeutic target reached. After six months, HbA1C, blood pressure, TC, LDL-C, ALT and AST were tested again and comparison was made between the two groups.RESULTS:There was a significant decrease in TC and LDL-C in the intensive treatment group compared with those in the routine treatment group (P <0.05).CONCLUSION:The intensive treatment on the incipient diabetes II patients facilitate the control of the blood lipids and blood sugar.

Abstract:Short-term intensive insulin therapy is unique amongst therapies for type 2 diabetes because it offers the potential to preserve and improve beta-cell function without additional pharmacological treatment. On the basis of clinical experience and the promising results of a series of studies in newly diagnosed patients, mostly in Asian populations, an expert workshop was convened to assess the available evidence and the potential application of short-term intensive insulin therapy should it be advocated for inclusion in clinical practice. Participants included primary care physicians and endocrinologists. We endorse the concept of short-term intensive insulin therapy as an option for some patients with type 2 diabetes at the time of diagnosis and have identified the following six areas where additional knowledge could help clarify optimal use in clinical practice: (1) generalizability to primary care, (2) target population and biomarkers, (3) follow-up treatment, (4) education of patients and providers, (5) relevance of ethnicity, and (6) health economics. Abstract:Type 2 diabetes is a metabolic disease characterized by the progressive deterioration of islet B cell function and insulin resistance. Early intensive insulin therapy,as a novel strategy of type 2 diabetes,can improve islet B cell function and insulin resistance by achieving restrictive glycemic control using insulin. This therapy enables patients to maintain normal blood glucose level after the therapy merely on lifestyle interventions and diet control. However,early intensive insulin therapy may cause some side effects,including increased risk of hypoglycemia and weight gain. This article reviews the definition,the therapeutic schemes,the effects and possible problems of early intensive insulin therapy. We hope that we can help physicians and medical students learn more about this therapy.

Abstract:Background: Type 1 diabetes (T1D) is more than an insulin-deficiency disease-it is an autoimmune disease, and the field is moving towards adopting disease-modifying immunotherapy as part of clinical care during T1D development. Summary: Recent successful immunotherapies as well as therapies that missed the mark are reviewed. T cell-directed therapies may allow for the greatest preservation of β cell function but also come with more side effects. Anti-cytokine therapies are very promising but likely need chronic administration. Antigen-specific therapies while safe have not produced meaningful results. Most successful trials have been conducted in adolescents and adults with stage 3 T1D (clinical T1D) with preserved C-peptide (up to 60% more compared to placebo) demonstrated 1-2 years post treatment. HbA1c and total insulin dose are less likely to be significantly different between treated and placebo groups because most participants in studies are meeting glycemic targets and because of the heterogeneous nature of these measures. In the prevention space (delaying progression from stage 2 to stage 3 T1D) the outcome is more discrete, and a T cell-directed therapy, teplizumab, has received FDA approval. Even negative studies with promising mechanistic and safety profiles have added value. Key messages: What is clear, a single administration or short course of an immunotherapy is unlikely to provide sustained freedom from exogenous insulin.

Abstract:ABSTRACT Type 1 diabetes is an autoimmune condition resulting in insulin deficiency and eventual loss of pancreatic β cell function requiring lifelong insulin therapy. Since the discovery of insulin more than 100 years ago, vast advances in treatments have improved care for many people with type 1 diabetes. Ongoing research on the genetics and immunology of type 1 diabetes and on interventions to modify disease course and preserve β cell function have expanded our broad understanding of this condition. Biomarkers of type 1 diabetes are detectable months to years before development of overt disease, and three stages of diabetes are now recognized. The advent of continuous glucose monitoring and the newer automated insulin delivery systems have changed the landscape of type 1 diabetes management and are associated with improved glycated hemoglobin and decreased hypoglycemia. Adjunctive therapies such as sodium glucose cotransporter-1 inhibitors and glucagon-like peptide 1 receptor agonists may find use in management in the future. Despite these rapid advances in the field, people living in under-resourced parts of the world struggle to obtain necessities such as insulin, syringes, and blood glucose monitoring essential for managing this condition. This review covers recent developments in diagnosis and treatment and future directions in the broad field of type 1 diabetes.

Abstract:It is difficult to estimate the importance of plant pigments in plant biology. Chlorophylls are the most important pigments, as they are required f or photosynthesis. Carotenoids are also necessary for their functions in photosynthesis. Ot her plant pigments such as flavonoids play a crucial role in the interaction between plants an d imals as visual signals for pollination and seed scattering. Studies related to plant pigme ntation are one of the oldest areas of work in plant science. The first publication about carot en ids appeared in the early nineteenth century, and the term "chlorophyll" was first used in 1818 (Davies, 2004). Since then, the biochemical structure of plant pigments has been re veal d, as have the biosynthetic pathways for the major pigments that provide a useful variet y of colors to blossoms and other plant organs. There is widespread interest in the applica tion of molecular methods to improve our knowledge of gene regulation mechanisms and changes i plant pigment content. Genetic modification has been used to alter pigment product ion in transgenic plants. This review focuses on flower pigmentation, its biochemistry an d biology. It presents a general overview of the major plant pigment groups as well as rarer plant dyes and their diversity and function in generating the range of colors observed in plant s.
Therapy concepts in type 1 diabetes mellitus treatment: disease modifying versus curative approaches

Abstract:For many autoimmune diseases, including type 1 diabetes mellitus (T1DM), efforts have been made to modify the disease process through pharmacotherapy. The ultimate goal must be to develop therapies with curative potential by achieving an organ without signs of parenchymal cell destruction and without signs of immune cell infiltration. In the case of the pancreas, this means regenerated and well-preserved beta cells in the islets without activated infiltrating immune cells. Recent research has opened up the prospect of successful antibody combination therapy for autoimmune diabetes with curative potential. This goal cannot be achieved with monotherapies. The requirements for the implementation of such a therapy with curative potential for the benefit of patients with T1DM and LADA (latent autoimmune diabetes in adults) are considered.

Abstract:Following the seminal discovery of insulin a century ago, treatment of individuals with type 1 diabetes (T1D) has been largely restricted to efforts to monitor and treat metabolic glucose dysregulation. The recent regulatory approval of the first immunotherapy that targets T cells as a means to delay the autoimmune destruction of pancreatic β-cells highlights the critical role of the immune system in disease pathogenesis and tends to pave the way for other immune-targeted interventions for T1D. Improving the efficacy of such interventions across the natural history of the disease will probably require a more detailed understanding of the immunobiology of T1D, as well as technologies to monitor residual β-cell mass and function. Here we provide an overview of the immune mechanisms that underpin the pathogenesis of T1D, with a particular emphasis on T cells.

Abstract:Objective:To discuss clinical curative effect of the intensivetherapy of short-acting insulin for type 2 diabetes.Methods:64 patients with diagnosed type 2 diabetes patients were randomly divided into treatment group and control group,and 32 cases constituted a group.All cases were given the conventional diabetes health education,kept the amount of exercise and diet relatively constant.At the same time,cases profess to convinced DMBG,and were subcutaneous injected insulin glargine.In control group,cases oral raigor column nye 0.5 mg ～ 1.0 mg everyday,before meals.In treatment group cases had the conventional treatment on the basis of recombinant human insulin's spirit R(100 U/ml) as a pump use insulin line insulin pump therapy,and initial quantity according to the daily 0.5 U/kg basis before meal quantity and quantity 1:1 infusion.Hypoglycemia decision criteria:blood sugar < 3.5 mmol/L.Two groups treated 2 weeks.Results:Treatment group total effectiveness was 92.59%,and the comparison group total effectiveness was 75.00%.Compared total effective rates,p < 0.05.After treatment in short-acting insulin strengthening treatment group and the control group,FBC,2 HPBG,HbAI,FNS and IRI were significantly lower,differences are significant(P<0.01,P<0.05);After treatment,in short-acting insulin strengthening treatment group,FBC,2 HPBG,HbAI,FNS and IRI improve were significantly superior to control group,the difference is statistically significant(P<0.01,P<0.05).The treatment group was 9.38% incidence of hypoglycemia and control group was 31.25% in low incidence rate,the difference between the two groups have statistical significance(P<0.05).Conclusion:Strengthening short-acting insulin treatment for type 2 diabetes curative effect was be conformed,and hypoglycemia was happened lower,it is worth clinical promotion to be used.
Induction of Long-Term Good Glycemic Control in Newly Diagnosed Type 2 Diabetic Patients by Transient Intensive Insulin Treatment

Abstract:Objective To study the impact of short term intensive insulin treatment on β cell function and long term glycemic control in newly diagnosed type 2 diabetic patients. Methods Twenty two newly diagnosed type 2 diabetic patients were treated with continuous subcutaneous insulin infusion (CSII) for 2 weeks and their β cell function were assessed before and after the insulin treatment by intravenous glucose tolerance test. After the treatment, all patients were followed up for their glycemic control level. Results Two weeks after the treatment, the AUC 0 10 INS, the insulin peak, the increment of the insulin peak(ΔINS), the ratio of total amount insulin to total amount glucose, the ratio of ?NS/ΔG and Homa B were significantly increased ( P <0.05). Homa A was also significantly decreased ( P <0.05). The insulin peak before and after the treatment were positively correlated ( r=0.521,P =0.013). Fifteen patients were followed up for an average of 6 months with mean HbA 1c of 5.97±0.59%, fasting plasma glucose levels of 6.34±1.07 mmol/L, 2 h OGTT plasma glucose levels of 7.81±2.41 mmol/L, fasting insulin levels of 8.63±4.43 μU/ml and 2 h OGTT insulin levels of 19.85±5.33 μU/ml at six month follow up point. Conclusion Short term intensive insulin treatment can effectively induce long term good glycemic control without medication by improving the β cell function as well as decreasing the insulin resistance and restoring the first phase of insulin secretion via eliminating the glucose toxicity. The patients might be brought back in a early time along the natural history of type 2 diabetes to a stage where non pharmacological therapys are still quite efficacious in term of blood glucose control. This suggests that the transient intensive insulin treatment might be therapys used as primary approach in the management of newly diagnosed type 2 diabetes patients.

Abstract:This study aims to explore new treatments for type 1 diabetes that could serve as an alternative or adjunct to insulin therapy. This is a literature review based on a search of relevant scientific articles in PubMed, Scopus, Google Scholar, and Cochrane Library databases. The scrutiny of publications revealed that the introduction of glucagon-like peptide-1 agonists into insulin therapy can improve disease control and reduce the frequency of hypoglycaemic episodes. While immune therapy is pathogenetically justified, its utility is limited in patients with recent onset of type 1 diabetes. It may, however, find application in prophylaxis in individuals at increased risk of developing this type of diabetes. Concurrently, stem cell therapy is under active investigation in clinical trials and has shown promise in reducing insulin dependence, improving β-cell function and controlling glucose levels. In addition, stem cells have demonstrated efficacy in treating complications of diabetes such as diabetic nephropathy, peripheral neuropathy and diabetic angiopathy. There is compelling evidence supporting the significant potential of gene-editing technology. Intravenous administration of T-regulatory cells, as one method of cell therapy, shows potential in stabilising the course of diabetes and slowing its progression. However, further research is warranted to confirm efficacy. While gene therapy holds promise, much of its research is currently in the preclinical stage. Further development of innovative therapies for type 1 diabetes has the potential to enhance the quality of life of patients, improve disease control and prevent the development of complications (Fig. 1, Ref. 54). Abstract:Intensive insulin therapy has been extensively used to control blood glucose levels because of its ability to reduce the risk of chronic complications of diabetes. According to current guidelines, intensive glycemic control requires individualized glucose goals rather than as low as possible. During intensive therapy, rapid blood glucose reduction can aggravate microvascular and macrovascular complications, and prolonged overuse of insulin can lead to treatment-induced neuropathy and retinopathy, hypoglycemia, obesity, lipodystrophy, and insulin antibody syndrome. Therefore, we need to develop individualized hypoglycemic plans for patients with diabetes, including the time required for blood glucose normalization and the duration of intensive insulin therapy, which deserves further study.

Abstract:OBJECTIVE:To investigate the effects of early intensive therapy on beta cell function and long-term glycemic control in newly diagnosed type 2 diabetic patients with different recruiting fasting plasma glucose (FPG) levels.METHODS:A total of 382 newly diagnosed type 2 diabetic patients with FPG 7.0 - 16.7 mmol/L were randomly assigned to therapy with insulin in the form of continuous subcutaneous insulin infusion (CSII) or multiple daily injection (MDI) or oral hypoglycemic agents (OHA, by using gliclazide and/or metformin) for initial rapid correction of hyperglycemia. The treatments were stopped after euglycemia had been maintained for 2 weeks. The patients were followed longitudinally on diet alone for 1 year. Intravenous glucose tolerances tests (IVGTTs) were performed and blood glucose, insulin and proinsulin were measured before and after therapy as well as at 1-year follow-up. Homeostasis model assessment (HOMA) of beta cell function and insulin resistance index (HOMA-beta and HOMA-IR) were calculated. All the patients were stratified on the recruiting FPG: stratum A (7.0 mmol/L </= FPG < 11.1 mmol/L), stratum B (11.1 mmol/L </= FPG </= 16.7 mmol/L).RESULTS:More patients in stratum A achieved target glycemic control (94.4% vs 89.8%) and in shorter time [(5.9 +/- 3.8) d vs (6.9 +/- 3.6) d, P < 0.05] as compared with those in stratum B. beta cell function represented by HOMA-beta and acute insulin response (AIR) improved significantly after intensive interventions in both stratum A and B patients. However, the remission rate at 1 year was significantly higher in stratum A patients (47.8%) than those in stratum B (35.7%, P < 0.05). The patients treated with insulin (especially with CSII) had higher remission rates and better improvement of AIR at 1 year follow-up irrespective of the recruiting FPG (CSII or MDI vs OHA: 57.1%, 51.8% vs 32.8% in stratum A, P < 0.05; 44.4%, 38.7% vs 18.6% in stratum B, P < 0.05).CONCLUSIONS:Compared with OHA, early short time intensive insulin treatment had more favorable outcomes on maintaining AIR and prolonged glycemic remission in newly diagnosed type 2 diabetic patients irrespective of the recruiting FPG levels.

Abstract:Objective To investigate the clinical outcome of early intensive insulin therapy in newly diagnosed type 2 diabetes.Methods 388 patients with newly diagnosed type 2 diabetes admitted in our hospital from January 1998 to January 2008 were randomly divided into 2 groups:treatment group(n=108) and control group(n=280).Cases in treatment group were treated with insulin,which were regulated according to blood sugar.And the acarbose or metformin could be combined with insulin.This treatment lasted for 2 weeks to 3 months.Cases in control group were treated with oral hypoglycemic agents.Observe the two groups for 3 months to 10 years.BMI,the fasting and 2-hour postprandial blood glucose,glycated hemoglobin,the fasting and postprandial C peptide were detected regularly during the period.Clinical remission was followed up for 1 to 10 years.Results Three months after treatment,the level of blood glucose was significantly lower in treatment group than in the control group,and the level of C peptide was significantly higher in treatment group than in the control group(P0.05).1 year after treatment,34 patients(31.5%) could get rid of insulin and oral hypoglycemic therapy,only need the diet and exercise to control the blood glucose(the time can last for 12 year).46 patients(42.6%) needed oral hypoglycemic agents,of which 30 patients(27.8%) only needed one kind of non-insulin secretagogue agents and 16 cases(14.8%) required combined with 2 or more than 2 kinds of oral hypoglycemic agents.28 cases(25.9%) continued the treatment with insulin,of which 16 cases(57.1%) need of insulin dose was less than 30 U/d,12 cases′s(42.9%) dose over 30 U/L.In the control group,18 patients(6.4%) only need the diet and exercise to control the blood glucose,180 cases(64.3%) needed oral hypoglycemic agents.The difference in the outcomes between two groups was significant(P0.01).Conclusion The early intensive insulin treatment on newly diagnosed type 2 diabetes patients could improve the function of islet B cell,reversed part of the course of disease,separated some patients from drug therapy for short term and have a lasting beneficial effect for the disease.Therefore,the early intensive insulin treatment is better than oral antidiabetic drugs treatment for newly diagnosed type 2 diabetes patients.

Abstract:Type 1 diabetes is characterized pathologically by autoimmune insulitis-related islet β-cell destruction. Although intensive insulin therapy for patients with type 1 diabetes can correct hyperglycemia, this therapy does not prevent all diabetes-related complications. Recent studies have shown that autologous hematopoietic stem cell transplantation (HSCT) is a promising new approach for the treatment of type 1 diabetes by reconstitution of immunotolerance and preservation of islet β-cell function. Herein we discuss the therapeutic efficacy and potential mechanisms underlying the action of HSCT and other perspectives in the clinical management of type 1 diabetes.

Abstract:Type 2 diabetes mellitus (T2DM) is a progressive disease characterized by worsening insulin resistance and a decline in β-cell function. Achieving good glycemic control becomes more challenging as β-cell function continues to deteriorate throughout the disease process. The traditional management paradigm emphasizes a stepwise approach, and insulin has generally been reserved as a final armament. However, mounting evidence indicates that short-term intensive insulin therapy used in the early stages of type 2 diabetes could improve β-cell function, resulting in better glucose control and more extended glycemic remission than oral antidiabetic agents. Improvements in insulin sensitivity and lipid profile were also seen after the early initiation of short-term intensive insulin therapy. Thus, administering short-term intensive insulin therapy to patients with newly diagnosed T2DM has the potential to delay the natural process of this disease, and should be considered when clinicians initiate treatment. Although the early use of insulin is advocated by some guidelines, the optimal time to initiate insulin therapy is not clearly defined or easily recognized, and a pragmatic approach is lacking. Herein we summarize the current understanding of early intensive insulin therapy in patients with newly diagnosed T2DM, focusing on its clinical benefit and problems, as well as possible biological mechanisms of action, and discuss our perspective.

Abstract:Type 1 diabetes mellitus is a disorder of glucose regulation characterized by autoimmune destruction of the insulin -producing pancreatic β-cells and the need for lifelong insulin replacement therapy. Both glucose and HbA1c criteria are available for the diagnosis of diabetes. Achieving and maintaining near-normal blood glucose concentrations with appropriate administration of insulin is critical for successful long-term care of patients with diabetes mellitus and prevention of diabetic complications, but it is very difficult to achieve. Efforts to intensify therapy usually result in hypoglycemia. Point-of-care blood glucose testing provides the information needed by patients for clinical decision-making. Despite the ease of use and rapid reporting of modern point-of-care glucose monitoring devices, they provide measurements only intermittently. Continuous glucose monitoring devices can provide new glucose measurements every 5 minutes and may facilitate better glucose control. The development of new technologies that integrate continuous glucose monitoring technology with insulin pumps and a control algorithm to create a system for automated insulin delivery has the potential to significantly improve glycemic control, reduce hypoglycemia, and reduce the burden of care in type 1 diabetes mellitus.

Abstract:Type 1 diabetes (T1D) is a chronic autoimmune disease in which destruction or damaging of the beta-cells in the islets of Langerhans results in insulin deficiency and hyperglycemia. We only know for sure that autoimmunity is the predominant effector mechanism of T1D, but may not be its primary cause. T1D precipitates in genetically susceptible individuals, very likely as a result of an environmental trigger. Current genetic data point towards the following genes as susceptibility genes: HLA, insulin, PTPN22, IL2Ra, and CTLA4. Epidemiological and other studies suggest a triggering role for enteroviruses, while other microorganisms might provide protection. Efficacious prevention of T1D will require detection of the earliest events in the process. So far, autoantibodies are most widely used as serum biomarker, but T-cell readouts and metabolome studies might strengthen and bring forward diagnosis. Current preventive clinical trials mostly focus on environmental triggers. Therapeutic trials test the efficacy of antigen-specific and antigen-nonspecific immune interventions, but also include restoration of the affected beta-cell mass by islet transplantation, neogenesis and regeneration, and combinations thereof. In this comprehensive review, we explain the genetic, environmental, and immunological data underlying the prevention and intervention strategies to constrain T1D.
