Dosimetric effects of different MLCs on RapidPlan model application

Abstract:Objective:To explore the modeling and extended use of RapidPlan model with different systems of multi-leaf collimator (MLC).Methods:Included in this study were historic plans of 81 rectal cancer patients who underwent radiotherapy between May 2014 and June 2015 [using accelerator configured with Varian Millennium MLC (MMLC) ] and of 35 lung cancer patients who underwent stereotactic radiotherapy (SBRT) between November 2017 and May 2019 [ (using accelerator configured with Varian High Resolution MLC (HDMLC) ] at Peking University Cancer Hospital. Two different RapidPlan models (M _MMmlc and M _HDmlc) were trained with these historic plans, and then were tested for their use in and across MLC plans. Results:With either M _MMmlc or M _HDmlc, RapidPlan generated exactly same optimization parameters for same individual patients under identical beam arrangements with any MLC configuration. When optimized plans under the two MLC configurations were normalized to the minimum prescription requirements of target region: (1) the mean dose-volume histogram (DVH) of the two MLC plans optimized by M _MMmlc almost overlapped with each other, with a difference below 2% and similar conformability of target PGTV dose, despite a lower homogeneity index (HI) by 20% with HDMLC than with MMLC. Other than these, the overall maximum differences in other dosimetric parameters did not exceed 0.97%; (2) The M _HDmlc optimized PTV and IGTV towards the tail of DVH were higher with HDMLC than with MMLC. Other than these, the rest of DVH almost overlapped between the two MLCs. Compared with the MMLC plans, specific presentation of dosimetric parameters with HDMLC plans showed a lower PTV conformity index (CI) by 0.96%, a higher HI value by 1.75%, more favorable profiles in 9 out of 16 organ-at-risk dosimetric parameters, reductions in V 5 Gy ( P<0.05) , V 10 Gy, V 20 Gy ( P<0.05) and D 1000 cm3 of normal lung tissue, and increases in maximum dose by 2.07%, D 0.35 cm3 by 2.88%, and D 1.2 cm3 by 3.99%, of spinal cord ( P<0.05) . Conclusion:RapidPlan modeling offers good compatibility and adaptability between different MLCs. The dosimetric advantage of HDMLC over MMLC is reflected in SBRT for patients with small lung cancers. In the future, such difference can guide selection of MLC configuration in automated planning.
