Abstract:Background: Microwave is a method for improving fracture repair. However, one of the contraindications for microwave treatment listed in the literature is surgically implanted metal plates in the treatment field. The reason is that the reflection of electromagnetic waves and the eddy current stimulated by microwave would increase the temperature of magnetic implants and cause heat damage in tissues. Comparing with traditional medical stainless steel, titanium alloy is a kind of medical implants with low magnetic permeability and electric conductivity. But the effects of microwave treatment on fracture with titanium alloy internal fixation in vivo were not reported. The aim of this article was to evaluate the security and effects of microwave on healing of a fracture with titanium alloy internal fixation. Methods: Titanium alloy internal fixation systems were implanted in New Zealand rabbits with a 3.0 mm bone defect in the middle of femur. We applied a 30-day microwave treatment (2,450MHz, 25W, 10 min per day) to the fracture 3 days after operation. Temperature changes of muscle tissues around implants were measured during the irradiation. Normalized radiographic density of the fracture gap was measured on the 10th day and 30th day of the microwave treatment. All of the animals were killed after 10 and 30 days microwave treatment with histologic and histomorphometric examinations performed on the harvested tissues. Findings: The temperatures did not increase significantly in animals with titanium alloy implants. The security of microwave treatment was also supported by histology of muscles, nerve and bone around the implants. Radiographic assessment, histologic and histomorphometric examinations revealed significant improvement in the healing bone. Conclusion: Our results suggest that, in the healing of fracture with titanium alloy internal fixation, a low dose of microwave treatment may be a promising method.
Effects of different doses of microwave therapy on adjacent tissue of titanium alloy implants after fracture surgery

Abstract:BACKGROUND: Microwave treatment is a common physical therapy method that can increase the temperature and blood circulation of deep tissues, and is used for improving fracture repair. However, microwave treatment cannot be used if there is surgically implanted metal plate or screw. OBJECTIVE: To observe the dame of microwave treatment to the tissues surrounding the titanium alloy implants. METHODS: Forty-four New Zealand white rabbits were randomized into experimental and control groups. The model of the fracture at the middle of the femur was established in all rabbits, and the rabbits in the experimental group were implanted with titanium alloy internal fixation systems. A 30-day microwave treatment (2 450 MHz, 20 W or 40 W, 20 minutes daily) was applied to the fracture site in all rabbits at 3 days after operation. RESULTS AND CONCLUSION: After 20 W of wave microwave treatment, the temperature of tissues around the implants showed no significant increase or severe heat injury. While, 40 W of wave microwave treatment significantly increased the temperature of tissues around the implants and the tissue was damaged severely. Our results indicate that, the low-dosage microwave treatment may be a promising method in the rehabilitation therapy of fractures with titanium alloy internal fixation.
Study on TiO2 Nanofilm That Reduces the Heat Production of Titanium Alloy Implant in Microwave Irradiation and Does Not Affect Fracture Healing

Abstract:Background. Metal implants can produce heat and damage adjacent tissues under microwave irradiation, which makes local metal implants in the body a contraindication for microwave therapy. However, with the wide application of titanium alloy implants which have low permeability and low conductivity, this concept has been challenged. Our team members have confirmed through previous research that continuous low-power microwave irradiation does not cause thermal damage to the surrounding tissues of the titanium alloy. Is there any other way to further increase the dose of microwave irradiation while reducing the heat production of titanium alloy implants? In this study, the effect of TiO2 nanofilm on reducing the heat production of titanium alloy implants in microwave field was verified by animal experiments, and the effect of TiO2 nanofilm on fracture healing was observed. Methods. 30 rabbits were selected. In the experiment of temperature measurement, 10 rabbits were randomly divided into experimental group (n=5) and control group (n=5), and the contralateral lower limb of the rabbits in experimental group was set as the sham operation group. The right femurs in the experimental group were implanted with Ti6Al4V plates coated with TiO2 nanofilm, and the right femurs in the control group were implanted with common titanium alloy plates without TiO2 nanofilm. The same surgical procedure was used in the sham operation group, but no plate was implanted. The temperature of the deep tissue above the metal implant was measured with an anti-interference thermocouple thermometer during 20 minutes of microwave irradiation. The other 20 rabbits were randomly divided into two groups, experimental group (n=10) and control group (n=10). The femoral shaft fracture models were established again. Ti6Al4V plates coated with TiO2 nanofilm and common titanium alloy plates were implanted in the two groups, respectively, and both groups were exposed to continuous microwave irradiation with a power of 40 W or 60 W for 30 days after operation. The fracture healing was evaluated by X-ray at 0 day, 14 days, and 30 days after microwave irradiation, respectively. The animals were sacrificed at 30 days after operation for histopathological assessment. Results. The temperature in the experimental group, control group, and sham operation group increased significantly after 40 W and 60 W microwave irradiation (2.18±0.15°C~6.02±0.38°C). When exposed to 40 W microwave, the temperature rise of the control group was 4.0±0.34°C, which was significantly higher than that of the experimental group 2.82±0.15°C (P<0.01) and the sham operation group 2.18±0.33°C (P<0.01). There was no significant difference in temperature rise between the experimental group and the sham operation group (P=0.21). When exposed to 60 W microwave, the temperature rise of the control group was 6.02±0.38°C, which was significantly higher than that of the experimental group 3.66±0.14°C (P<0.01) and sham operation group 2.96±0.22°C (P<0.01), and there was no significant difference between the experimental group and the sham operation group (P=0.32). X-ray evaluation showed that there was no significant difference in callus maturity between the experimental group and the control group at 14 days (P=0.554), but there was significant difference in callus maturity between the two groups at 30 days (P=0.041). The analysis of bone histologic and histomorphometric data at 30 days was also consistent with this. Conclusion. Under the animal experimental condition, compared with the common titanium alloy implant, the TiO2 nanofilm can reduce the heat production of the titanium alloy implant in the 2450 MHz microwave field and has no adverse effect on fracture healing. This study opens up a promising new idea for the application of microwave therapy to metal implants in human body.
Thermal Effects of 2450 Mhz Microwave Exposure Near A Titanium Alloy Plate Implanted in Rabbit Limbs

Abstract:This study aimed to examine the safety profile of microwave therapy on limbs with metal implants. New Zealand white rabbits were implanted with titanium alloy internal fixation plates. Femurs were exposed to 20, 40, 60, or 80W of microwave radiation for 30min (microwave applicator at 2450MHz), and temperatures of the implants and muscles adjacent to implants were recorded. To evaluate thermal damage, nerves were electrodiagnostically assessed immediately after radiation, and histologic studies performed on nerve and muscle sections. As expected, implant temperature was highest in the exposure field. Temperatures of limbs with titanium alloy implants increased significantly at 60 and 80W, with a significant decline in the nerve conduction velocity and acute thermal injuries in nerves and muscles adjacent to implants. However, temperature remained unchanged and no adverse effects were observed in nerves and muscles at 20 and 40W. These findings are inconsistent with the current notion that surgical metal implants in the treatment field are contraindications for microwave therapy. Comparative Study of the Proliferative Ability of Skeletal Muscle Satellite Cells under Microwave Irradiation in Fractures with Titanium Alloy Internal Fixation in Rabbits

Abstract:The aim of the present study was to investigate the proliferation of skeletal muscle satellite cells (MSCs) under different amounts of microwave irradiation in fractures with titanium alloy internal fixation. A total of 45 male New Zealand adult white rabbits were used to establish a femoral shaft fracture and titanium alloy internal fixation model. The rabbits were randomly divided into the control group (group A) and the experimental groups (groups B and C). For 15 days, groups B and C were exposed to microwave treatment (25 or 50 W, respectively) for 10 min per day. The quadriceps femoris muscle was used for the isolation and culture of MSCs in vitro. The cultured cells were identified using cellular immunohistochemical staining. Transmission electron microscopy was used to observe mitochondrial ultrastructure damage, MTT assays were used to detect cell viability and cell cycle phases were analyzed by flow cytometry. The results revealed that, following 48 or 72 h of culture, cell viability was significantly greater in group B compared with group A, and was significantly lower in group C compared with group A (P<0.05). Compared with group A, the percentage of the cell population in the G0/G1 phase in group B was significantly decreased (P<0.05) and the proportion in the S and G2/M phases was increased (P<0.05). These results were reversed in group C; the percentage of cells in the S and G2/M phases was significantly lower (P<0.05) and in the G0/G1 phase was significantly higher (P<0.05) than in group A. These results suggested that in the healing of fractures with titanium, the proliferation of MSCs is significantly affected by microwave radiation in a dose-dependent manner.

Abstract:Background Research studies on the influence of radiofrequency electromagnetic radiation on implants in vitro have failed to investigate temperature changes in the tissues adjacent to the implants under microwave therapy. We therefore, used a rabbit model in an effort to determine the impact of microwave therapy on temperature changes in tissues adjacent to the titanium alloy implants and the safety profile thereof. Methods Titanium alloy internal fixation plates were implanted in New Zealand rabbits in the middle of femur. Microwave therapy was performed by a 2450 MHz microwave generator 3 days after the surgery. Temperature changes of muscles adjacent to the implants were recorded under exposure to dose-gradient microwave radiation from 20w to 60w. Results Significant difference between control and microwave treatment group at peak temperatures (T peak ) and temperature gap (T gap= T peak -T vally ) were observed in deep muscles (T peak , 41.63 ± 0.21°C vs. 44.40 ± 0.17°C, P < 0.01; T gap , 5.33 ± 0.21°C vs. 8.10 ± 0.36°C, P < 0.01) and superficial muscles (T peak , 41.53 ± 0.15°C vs. 42.03 ± 0.23°C, P = 0.04; T gap , 5.23 ± 0.21°C vs. 5.80 ± 0.17°C, P = 0.013) under 60 w, and deep muscles (T peak , 40.93 ± 0.25°C vs. 41.87 ± 0.23°C, P = 0.01; T gap , 4.73 ± 0.20°C vs. 5.63 ± 0.35°C, P = 0.037) under 50w, but not under 20, 30 and 40w. Conclusion Our results suggest that low-dose (20w-40w) continuous-wave microwave irradiation delivered by a 2450 MHz microwave generator might be a promising treatment for patients with titanium alloy internal fixation, as it did not raise temperature in muscle tissues adjacent to the titanium alloy implant.

Abstract:Objective To observe the influence of low or middle power microwave radiation on tissues adja-cent to titanium alloy implants. Methods Titanium alloy internal fixation plates were implanted into the upper femur of the right side of New Zealand white rabbits, the left femurs served as the control. Then the femurs were exposed to 20, 40, 60, or 80 W of microwave radiation at a frequency of 2450 MHz for 30 minutes, and real-time temperature of the implants and its adjacent muscles were recorded. The nerve conduction study of the sciatic nerve was performed by stimulating the nerve and recording from gastrocnemius in both groups. Finally, all the rabbits were sacrificed and histo-logical studies were performed on nerve and muscle sections. Results Compared to the control limbs, temperatures of limbs with titanium alloy implants increased significantly when exposed to microwave radiation at 60 and 80 W ( P<0.05) but no significant differences were revealed when exposed to radiation at 20 and 40 W ( P<0.05) . For the sciatic nerve conduction, significantly prolonged distal latency of CMAP ( P<0.05) and slower nerve conduction velocity ( P<0.05) were observed in the experimental limbs when exposed to microwave radiation at 60 and 80 W, but the decrease of CMAP amplitude was not significant (P>0.05). Pathological examination showed obvious congestion and bleeding in the intercellular spaces of skeletal muscle cells in the experiment group when exposed to radiation at 60 W, with no ob-vious pathological changes found in the sciatic nerve. However, when exposed to radiation at 80 W, vacuoles appeared in myocytes, the cellular structure was destroyed and the myelin sheath of the sciatic nerve swelled. Conclusion Ex-posure to microwave irradiation at 20 and 40 W, but not 60 and 80 W exerts no adverse effects on nerves and muscles adjacent to titanium alloy internal fixation plates. Consequently, continuous wave microwave irradiation at low powers (20~40 W) is a safer auxiliary treatment for limbs with titanium alloy implants.

Abstract:Clinical hyperthermia induced by microwave is an effective treatment to the rehabilitation of muscle and skeletal injures. However, due to the excessive heat caused by the microwave, those patients implanted with metal orthopedic devices are not recommended to have microwave diathermy treatment. In this paper, TiO2 films were fabricated and coated on Ti6Al4V substrates. Preliminary study was carried out to investigate whether TiO2 encapsulated Ti6Al4V could provide better performance towards the influence of microwave. Results showed that by using sol-gel method a smooth and thin TiO2 film could be coated on the Ti6Al4V substrate. Compared with the uncoated samples, the average temperature increment of TiO2 film encapsulated samples was much lower, which indicated less joule heat was generated during the exposure procedure. According to this preliminary in vitro study, TiO2 film may have the potential application in the diathermy rehabilitation treatment of orthopedic metal implants.

Abstract:Microwave diathermy treatment has been successfully applied to such patients as for it can induce clinical hyperthermia to healing muscle skeletal injuries. However, as a contraindication, patients implanted with metallic implants for internal fixation of bone fracture could not take the microwave diathermy treatment. This is mainly due to microwave induced sudden temperature rise in metal implants, which will lead to heat damages in local tissues. The aim of this study is to improve the heat-resisting effect of titanium implants by modifying its surface. A uniform TiO2 coating has been designed and fabricated on the surface of Ti (TA3) substrate by the method of anodic oxidation to inhibit the excessive heat generation of metallic implants. In vitro microwave irradiation experiment, the temperature of Ti substrate increased 2.5 degrees C, TiO2 coated substrate increased only 1.4 degrees C, decreasing almost 50% under the same testing condition. The results indicate that the anodized TiO2 coating may be an effective method to reducing the temperature rise of the metal implants during microwave diathermy treatment, which will provide a potential rehabilitation solution to internal fixation of bone fracture.

Abstract:Microwave processing has been used to heat treat a TiC/Si3N4 ceramic composite. The strength degradation from thermal shock was substantially recovered. The recovery rate was much greater with microwave heating than with conventional heat treatment at equivalent average temperature, The mechanism of strength recovery was hypothesized to be microwave radiation, which caused selective heating of TiC particles, enhanced the reaction of grain boundaries, and resulted in the sintering of microcracks around TiC particles.

Abstract:Objective: To verify that the TiO2 nanofilm dip-coated by sol-gel can reduce titanium alloy implants (TAI)'s heat production after microwave diathermy (MD). Methods: The effect of 40W and 60W MD on the titanium alloy substrate coated with TiO2 nanofilm (Experimental Group) and the titanium alloy substrate without film (Control Group) were analyzed in vitro and in vivo. Changes in the skeletal muscle around the implant were evaluated in ex vivo by histology. Results: After 20 min of MD, in vitro the temperature rise of the titanium substrate was less in the Experimental Group than in the Control Group (40W: 1.4 degrees C vs. 2.6 degrees C, p < .01, 60W: 2.5 degrees C vs. 3.7 degrees C, p<.01) and in vivo, the temperature rise of the muscle tissue adjacent to TAI was lower in the Experimental Group than in the Control Group (40W: 3.29 degrees C vs. 4.8 degrees C, p < .01, 60W: 4.16 degrees C vs. 6.52 degrees C, p < .01). Skeletal muscle thermal injury can be found in the Control Group but not in the Experimental Group. Conclusion: Sol-gel dip-coated TiO2 nanofilm can reduce the heat production of TAIs under single 40(similar to)60W and continuous 40W MD and protect the muscle tissue adjacent to the implants against thermal injury caused by irradiation. Effects of Low Power Microwave Irradiation (20 W) on the Proliferation and Differentiation of Skeletal Muscle Satellite Cells in a Rabbit Model of Femoral Fracture with Internal Fixation

Abstract:BACKGROUND:Low power microwave irradiation has been shown to promote the healing of fractures with internal fixation;however, its action mechanisms on the skeletal muscle around the fracture site are unclear. OBJECTIVE:To study the effects of low power microwave irradiation (20 W) on the proliferation ability of skeletal muscle satel ite cel s in a rabbit model of femoral fracture with internal fixation. METHODS:Forty male New Zealand rabbits were used to establish femoral fracture fol owed by internal fixation models, and then were equal y randomized into spontaneous recovery and microwave treatment groups. Low power microwave irradiation (20 W) was given for 30 consecutive days in the microwave treatment group on day 4 after modeling, while no microwave irradiation was given in the spontaneous recovery group. Rabbit thigh muscles adjacent to the implant were obtained to isolate skeletal muscle satel ite cel s. Immunohistochemical staining, hematoxylin-eosin staining and quantitative RT-PCR were used to evaluate the ability of the proliferation and differentiation of skeletal muscle satel ite cel s. RESULTS AND CONCLUSON:Hematoxylin-eosin staining showed that there was no significant difference in the morphology and histology of skeletal muscle tissues between the spontaneous recovery and microwave treatment groups. However, the relative mRNA expression of MyoG in the cultured skeletal muscle satel ite cel s in vitro and the number ofα-sarcometric actin-postive cel s in the microwave treatment group were significantly increased compared with the spontaneous recovery group (P<0.05). The proliferative ability of skeletal muscle satel ite cel s was inhibited at the early stage, but not at the later stage. Our results suggest that low power microwave irradiation (20 W) can promote the proliferation and differentiation of skeletal muscle satel ite cel s around the implant in a rabbit model of femoral fracture with internal fixation, and thereby confirm the efficacy and safety of low power microwave irradiation for the internal fixation of fractures.
Effects of thermomechanical process on the microstructure and mechanical properties of a fully martensitic titanium-based biomedical alloy

Abstract:Thermomechanical treatments have been proved to be an efficient way to improve superelastic properties of metastable β type titanium alloys through several studies. In this paper, this treatment routes, already performed on superelastic alloys, are applied to the Ti-24Nb alloy (at%) consisting of a pure martensite α'' microstructure. By short-time annealing treatments performed on the heavily deformed material, an interesting combination of a large recoverable strain of about 2.5%, a low elastic modulus (35 GPa) and a high strength (900 MPa) was achieved. These properties are shown to be due to a complex microstructure consisting of the precipitation of nanoscale (α+ω) phases in ultra-fine β grains. This microstructure allows a superelastic behavior through stress-induced α'' martensitic transformation. In this study, the microstructures were characterized by X-ray diffraction and transmission electron microscopy and the evolution of the elastic modulus and the strain recovery as a function of the applied strain was investigated through loading-unloading tensile tests.
The in Vivo Bone Response of Ultraviolet-Irradiated Titanium Implants Modified with Spontaneously Formed Nanostructures: an Experimental Study in Rabbits

Abstract:Purpose: Acid etching in conjunction with storage in an aqueous solution can induce nanostructures forming spontaneously on titanium surfaces, but an inevitable biologic degradation is suspected to accompany the evolution of nanostructures. The aim of this study was to investigate whether ultraviolet (UV) irradiation is effective to solve this problem and further enhance the bioactivity. Materials and Methods: Surface characteristics of five groups of titanium implants (SLAnew, SLAold, modSLA, UV-SLA, and UV-modSLA) and their in vivo bone response were analyzed in this study. The surface characteristics were evaluated with contact angle measurements, scanning electron microscopy, and x-ray photon spectroscopy. A total of 160 implants (32 for each group) were inserted into the tibial metaphyses and femoral condyles of 40 rabbits. After 3 and 6 weeks, the rabbits were euthanized for removal torque tests and histomorphometric analysis. Results: Spontaneously formed nanostructures were observed on water-stored surfaces with a size of approximately 15 nm in diameter, and an inevitable contamination of hydrocarbons accompanied the evolution of nanostructures. UV irradiation effectively eliminated hydrocarbon contamination that accompanied nanostructure evolution. UV-modSLA implants showed the highest removal torque value, and UV-irradiated implants exhibited higher bone-to-implant contact and bone area. Conclusion: UV irradiation can effectively eliminate the hydrocarbon contamination accompanying the evolution of nanostructures and further enhance the osseointegration. Nanostructures and UV treatment have combined effects in enhancing the interfacial strength between titanium and bone, while UV photofunctionalization has much more overwhelming effects on histologic and histomorphometric performance.

Abstract:During the past decade, numerous research has been conducted to evaluate the effectiveness of microwave treatment in improving rock fragmentation. While these studies focused on investigating the effect of microwave treatment on rock parameters, the current research approaches the problem from another perspective by studying the applicability of mode I fracture toughness parameters to evaluate the effectiveness of the microwave treatment as a rock pre-conditioning method based on energy analysis. Different rock sizes are investigated, and it is shown that for the basalt rock used in this study, the minimum required diameter for the fracture toughness experiments is 70 mm. In a novel approach, a finite element based numerical model is employed to maximize the energy absorption of the sample in the cavity. It is shown that optimizing the location of the rock sample in the cavity can increase the heat over microwave efficiency (HOME) and weakening over microwave energy (WOME) by more than 40% and 30%, respectively. Furthermore, it is concluded that although increasing the exposure time results in more K IC value reduction, it decreases the WOME value by more than 50% in some cases. Finally, the applicability of fracture toughness experiments in evaluating the effectiveness of microwave treatment is discussed in detail.

Abstract:The use safety of laser additive manufactured (LAM) titanium alloys is closely related to each one’s fracture failure mode. In this study, in situ tensile tests were carried out to study the deformation and fracture mechanisms of LAM Ti6Al4V titanium alloy before and after annealing treatment. The results showed that plastic deformation promoted slip bands to occur inside the α phase and shear bands to generate along the α/β interface. In the as-built sample, cracks initiated in the equiaxed grains and propagated along the columnar grain boundary, showing a mixed fracture mode. However, it transformed into a transgranular fracture after annealing treatment. The widmanstatten α phase acted as a barrier for slip movement and improved the crack resistance of grain boundaries.

Controlling the microstructure and fracture toughness of the Ti-5Al-5Mo-5V-1Cr-1Fe alloy by multiple heat treatments

Abstract:This study presents the effects of the microstructure on the corresponding fracture toughness of the Ti-5Al-5Mo-5V-1Cr-1Fe (Ti-55511) alloy by undergoing STA (alpha/beta solution treatment plus aging) and STAA (alpha/beta solution treatment plus first-step aging and second-step aging) processes. The results show that the microstructural evolution of Ti55511 as well as the corresponding fracture toughness is very sensitive to the heat treatment mechanism. Here, an increase in the solution temperature can reduce the content of alpha(p), while increasing the content of alpha precipitates during the STA and STAA processes. An increase in the aging temperature coarsens the alpha precipitate and decreases its content, which deflects the crack and consumes additional energy, thus resulting in an enhanced fracture toughness. Note that the STAA process results in the further formation of alpha precipitates, which will cause a larger stress concentration that promotes the growth and coalescence of voids; notably, this is inevitably accompanied by a decrease in toughness. In addition, ductile fracture dominated by dimples is mainly experienced in the crack instability and growth stage, and a higher toughness increases the size of the dimples. Overall, this study indicates that the toughness of the alloy can be controlled from 38.2 to 110 MPa.m(0.5) by multiple heat treatments. (C) 2022 The Authors. Published by Elsevier B.V.
The Healing Process of Intracorporeally and In Situ Devitalized Distal Femur by Microwave in a Dog Model and Its Mechanical Properties In Vitro

Abstract:BACKGROUND: Limb-salvage surgery has been well recognized as a standard treatment and alternative to amputation for patients with malignant bone tumors. Various limb-sparing techniques have been developed including tumor prosthesis, allograft, autograft and graft-prosthesis composite. However, each of these methods has short- and long-term disadvantages such as nonunion, mechanical failures and poor limb function. The technique of intracorporeal devitalization of tumor-bearing bone segment in situ by microwave-induced hyperthermia after separating it from surrounding normal tissues with a safe margin is a promising limb-salvage method, which may avoid some shortcomings encountered by the above-mentioned conventional techniques. The purpose of this study is to assess the healing process and revitalization potential of the devitalized bone segment by this method in a dog model. In addition, the immediate effect of microwave on the biomechanical properties of bone tissue was also explored in an in vitro experiment.METHODS: We applied the microwave-induced hyperthermia to devitalize the distal femurs of dogs in situ. Using a monopole microwave antenna, we could produce a necrotic bone of nearly 20 mm in length in distal femur. Radiography, bone scintigraphy, microangiography, histology and functional evaluation were performed at 2 weeks and 1, 2, 3, 6, 9 and 12 months postoperatively to assess the healing process. In a biomechanical study, two kinds of bone specimens, 3 and 6 cm in length, were used for compression and three-point bending test respectively immediately after extracorporeally devitalized by microwave.FINDINGS: An in vivo study showed that intracorporeally and in situ devitalized bone segment by microwave had great revitalization potential. An in vitro study revealed that the initial mechanical strength of the extracorporeally devitalized bone specimen may not be affected by microwave.CONCLUSION: Our results suggest that the intracorporeal microwave devitalization of tumor-bearing bone segment in situ may be a promising limb-salvage method.

Abstract:Objective Microwave hyperthermia was applied on tumor-bearing bone in order to study its effectiveness and complication. Methods Ten patients were treated with microwave hyperthermia. The soft tissue lesions were resected routinely and the alveolar processes were resected. The residue bone including the lesion and some normal bone close to the tumor was subjected to heat at 55℃for 15 minutes by microwave, with surrounding normal soft tissue protected by copper mesh. Resnlts There was no swelling and pain in the local region. The wounds healed well in 7 patients,wound infection and pathological fracture occurred in 3 patients. No local recurrences of tumor occurred in all of patients. Conclusion Microwave hyperthermia has effective inactivation of tumor cells in tumor bearing bones. If the indication was strict to chose, there is no serious complication. As the time of follow - up was short, the long-term effectiveness was waited for further observation.
Free Radical Production, Inflammation and Apoptosis in Patients Treated With Titanium Mandibular Fixations-An Observational Study

Abstract:Despite high biocompatibility of titanium and its alloys, this metal causes various side effects in the human body. It is believed that titanium biomaterials may induce an innate/adaptive immune response. However, still little is known about changes caused by titanium mandible implants, particularly with regard to bone healing. The latest studies showed disturbances in the antioxidant barrier, increased oxidative/nitrosative stress, as well as mitochondrial abnormalities in the periosteum covering titanium mandible fixations; nevertheless, the impact of titanium implants on free radical production, inflammation, and mandible apoptosis are still unknown. Because severe inflammation and apoptosis are among the main factors responsible for disturbances in osteointegration as well as implant rejection, this study is the first to evaluate pro-oxidant enzymes, cytokines as well as pro- and anti-apoptotic proteins in the periosteum of patients with a broken jaw, treated with titanium miniplates and miniscrews. The study group consisted of 29 patients with double-sided fracture of the mandible body requiring surgical treatment. We found significantly higher activity of NADPH oxidase and xanthine oxidase as well as enhanced rate of free radical production in the periosteum of patients in the study group compared to the control group. The markers of inflammation [interleukin 1 (IL-1), interleukin 6 (IL-6), tumor necrosis factor α (TNF-α), transforming growth factor β (TGF-β) and β-glucuronidase (GLU)] as well as apoptosis [Bax, Bax/Bcl-2 ratio, caspase-3 (CAS-3) and nitric oxide (NO)] were significantly elevated in periosteum covering titanium fixations compared to the control group. In the study group, we also demonstrated an increased content of titanium on the periosteum surface, which positively correlated with CAS-3 activity. The study led us to the conclusion that titanium mandible implants increase the production of pro-inflammatory cytokines, and enhance free radical generation in the periosteum covering titanium miniplates and miniscrews. Additionally, exposure to Ti6Al4V titanium alloy induces apoptosis in the mandible periosteum. However, no clinical signs of the said phenomena have been observed.
