Abstract:Objective:To explore the clinical value of interhemispheric-callosum approach in the treatment of intraventricular hemorrhage with acute obstructive hydrocephalus.Methods: The clinical data of 24 cases of intraventricular hemorrhage with hydrocephalus treated by microsurgery through interhemispheric-callosum approach were retrospectively analyzed.Results: All patients survived and no postoperative intracranial infection occurred.Eight cases were classified as Grade I,six cases as Grade II,five cases as Grade III,and five cases as Grade IV,under a prognosis grading system of activities of daily living(ADL) after a follow-up of 3 months.Conclusions: Interhemispheric-callosum approach could help to improve the prognosis of intraventricular hemorrhage with hydrocephalus.
Clinical Analysis of Surgical Treatment for 189 Infants with Ventricular Septal Defect

Abstract:Objective To explore the effect of surgical treatment on ventricular septal defect in infants, including the operation indications, surgical techniques and perioperative therapy.Methods All clinical data of 189 consecutive infants were collected from July. 2001 to June 2003.The infant ages were 2～12 months (mean 7.22±3.68) with the body weight 2.5～11 kg(mean 6.05±1.73 kg).VSD located on perimemberanous region in 141 cases, subpulmonary region in 47 cases, perimemberanous-subarterial region in 1 case.Results One case died after operation for pulmonary hypertensive crisis. The mortality was 0.53%. 17 cases were complicated with lung infect and 6 cases with pulmonary hypertensive crisis, 5 cases with arrhythmia, 2 cases with low cardiac output and pulmonary edema, 2 cases with a little shunt. 188 cases grow and develop well after 2～48 months follow-up survey. Conclusion The operation for VSD in infants is safe and effective. Proper opportunity for operation, the improvement of surgical technique and proper perioperative management are key factors in successful outcome of surgical treatment.
Effect of Ventricle-Peritoneal Shunt in Treatment of Patients with Post-Traumatic Hydrocephalus Combined with Severe Consciousness Disturbance

Abstract:Objective To investigate the effect of ventricle-peritoneal (V-P) shunt in treatment of patients who developed post-traumatic hydrocephalus (PTH) with no symptoms or with atypical symptoms due to an excessively severe traumatic brain injury (TBI).Methods A retrospective study was performed in patients who developed PTH with atypical symptoms undergone V-P shunt from January 2004 to June 2007.Patients' general information,TBI data,PTH severity and postoperative follow-up data were collected and applied to assay the improvement rate in prognosis and its associated factors.Results Thirty-one patients were involved in this study.After 12 months of follow-up,20 (65％) patients revealed significant improvements in clinical symptoms.Among 10 patients who developed PTH after decompressive craniectomy,cranioplasty was performed following V-P shunt and significant improvements were observed in nine patients.Patients' age and severity of PTH based on CT evaluation before shunt placement were strongly correlated with the surgical outcome.Conclusions Most PTH patients with atypical symptoms can benefit from V-P shunt.Additionally,younger patients and those with less severe PTH before V-P shunt are expected a better outcome.

Abstract:Objective To compare the effect of microsurgery for ruptured intracranial supratentorial uneurysms during intermediate period versus delayed period and to evaluate the feasibility of surgical clipping during intermediate period. Method A retrospective analysis of 95 patients with Hunt-Hess Grade Ⅰ～Ⅲ on admission who were admitted 4 to 10 days after initial supratentorial aneurysmal subarachnoid haemorrhage. Forty-one patients underwent intermediate clipping while 54 patients accepted delayed obliteration of ruptured aneurysm (days 11-30). Outcome after 6 months was assessed using Modified Rankin Scale(mRS). Results There was no difference in outcome between two groups according to mRS at 6 months follow-up. Thirty patients(73.2%) in the intermediate group and 42 patients(77.8%) in the late group had a favorable functional outcome with a mRS score of 0 to 2. In 39.1% of the 23 patients who had poor outcome, rebleeding before active treatment was the leading factor contributing to poor outcome. Conclusions Surgical clipping of ruptured intracranial aneurysms during intermediate period provided good opportunity for patients to achieve favorable outcomes especially for those in good clinical condition.
Hypertensive intracerebral hemorrhage: surgical timing and prognosis

Abstract:Objective To investigate the effect of ultra-early or early surgical treatment on the aged and middle-aged patients with hypertensive intracerebral hemorrhage(ICH).Methods Patients with hypertensive ICH were classified into the aged(range from 60 to 83 years old,57 cases) and middle-aged(range from 39 to 59 years old,46 cases) groups.Of 103 patients,26(45.6%) in the aged group and 22(47.8%) in the middle-aged group underwent ultra-early surgical treatment(within 7 h),while 31(54.4%) in the aged group and 24(52.2%) in the middle-aged group were treated by early surgical management(within 7-48 h).The activity of daily living(ADL) measure was used to assess the outcome of patients with hypertensive ICH after surgery.Results Mortality in the aged group was significant lower after ultra-early surgical treatment compared with early surgical treatment(7.0% vs 32.2%,χ2= 5.134,P < 0.05).In addition,in the aged group 12 patients after ultra-early surgery and 6 patients after early surgery were in ADL grade Ⅰ(46.1% vs 19.3%,χ2 = 4.7,P < 0.05).However,there was no statistically significant difference in outcome in the middle-aged group after ultra-early compared with that after early surgical treatment(all P > 0.05).Conclusion Ultra-early surgical treatment can bring on a better outcome for aged but not for middle-aged patients with hypertensive ICH in comparison to early surgical treatment.
Treatment of Patent Ductus Arteriosus and Short-Term Outcomes among Extremely Preterm Infants: a Multicentre Cohort Study

Abstract:Background:The optimal treatment strategy for patent ductus arteriosus (PDA) in extremely preterm infants is currently highly controversial. This study aimed to evaluate the association between PDA treatment and short-term outcomes among extremely preterm infants.Methods:This cohort study included all extremely preterm infants (≤27 and 6/7 weeks) who were admitted to hospitals participating in the Chinese Neonatal Network from January 2019 to December 2021, and were diagnosed to have PDA by echocardiogram. PDA treatment was defined as medical treatment and/or surgical ligation of PDA during hospitalization. Short-term outcomes included death, bronchopulmonary dysplasia (BPD), death/BPD, retinopathy of prematurity, necrotizing enterocolitis, and severe brain injury. Multivariate logistic regression was used to evaluate the association between PDA treatment and outcomes. Subgroup analysis were performed among infants with different respiratory support on 3 and 7 days of life.Findings:A total of 2494 extremely preterm infants with the diagnosis of PDA were enrolled, of which 1299 (52.1%) received PDA treatment. PDA treatment was significantly associated with lower risk of death (adjusted odds ratio, 0.48; 95% confidence interval, 0.38-0.60). The decreased risk of death was accompanied by increased risk of BPD and death/BPD. In subgroup analysis according to respiratory support, PDA treatment was associated with lower risk of death among infants who required invasive ventilation. However, the beneficial effect on death was not significant among infants who did not require invasive ventilation.Interpretation:PDA treatment was associated with reduced mortality in extremely preterm infants, but this beneficial effect was mainly present among infants who required invasive ventilation.Funding:This study was funded by the Shanghai Science and Technology Commission's Scientific and Technological Innovation Action Plan (21Y21900800) and the Canadian Institutes of Health Research (CTP87518).
Microsurgery for severe brainstem hemorrhage (a report of 34 cases)

Abstract:Objective To explore the indication and skills of microsurgery for severe brainstem hemorrhage and its curative effect. Methods The clinical data of 34 patients with severe brainstem hemorrhage (the volume of bleeding ranged from 5 to 18 ml), who underwent microsurgery from February, 2006 to April, 2010 in our hospital, were analyzed retrospectively. GCS ranged from 4 to 7 scores on admission in the patients. Results The hemotomas were successfully removed by 36 operations in all the patients. The subtemporal craniotomy through keyhole approachs were performed in 16 of 34 patients. The consciousness was recovered 2 days after the surgery in 2 patients with 5 scores of GCS and 2 patients recovered their consciousness 7 days after the surgery. Three patients died after the operation. Of 31 patients followed up from 6 months to 3 years, 4 were recovered well, 9 mildly disabled, 7 severely disabled, 5 vegetatively survived and 6 died. Conclusion The prognosis can be significantly improved in patients with severe brainstem hemorrhage by removal of the hematomas early after the hemorrhage and enhancing perioprative comprehensive treatment.
Effect of Nonoperative Treatment on the Outcome of Patients with Posttraumatic Hydrocephalus.

Abstract:OBJECTIVE:To compare the outcome of non-operative treatment with the outcome of surgical intraventricular drainage for patients with posttraumatic hydrocephalus including complications, mortality and favorable outcome.METHODS:Thirty-nine patients were assigned to a nonoperative treatment group and 38 patents to a surgical intraventricular drainage group. Each patient's outcome was evaluated 3 years after treatment by using Glasgow Outcome Scale.RESULTS:In the nonoperative treatment group the rate of favor able outcome (good recovery or mild disability) was 89.74% (35 of 39 patients) and the mortality was 2.56% (1 of 39 patients). In the surgical intraventricular drainage group the rate of favorable outcome was 71.05% (27 of 38 patients) and the mortality was 13.16% (5 of 38 patients; P<0.05). Mortality was not significantly different between the two groups.CONCLUSIONS:Nonoperative treatment may be better than surgical intraventricular drainage for patients with posttraumatic hydrocephalus.
Treatment and Prognosis of Pulmonary Hemorrhage in Preterm Infants During 2007-2016

Abstract:OBJECTIVE:To study the treatment and prognosis of pulmonary hemorrhage in preterm infants. METHODS:A total of 106 preterm infants diagnosed with pulmonary hemorrhage, who were hospitalized in the neonatal ward of Peking University Third Hospital between 2007 and 2016, were enrolled. These patients were divided into 2007-2011 group (34 cases) and 2012-2016 group (72 cases) according to the time of hospitalization, divided into conventional-frequency ventilation group (43 cases) and high-frequency oscillatory ventilation (HFOV) group (63 cases) according to the respiratory support method used after the development of pulmonary hemorrhage, and divided into non-operation group (34 cases) and operation group (14 cases) according to whether PDA ligation was performed for the unclosed PDA before pulmonary hemorrhage. The general data, treatment, and prognosis were compared between different groups. RESULTS:Compared with the 2007-2011 group, the 2012-2016 group had higher rates of HFOV and PDA ligation (P<0.05), a lower mortality rate during hospitalization (P<0.05), a longer length of hospital stay (P<0.05), and higher incidence rates of intracranial hemorrhage and bronchopulmonary dysplasia (P<0.05). Compared with the conventional-frequency ventilation group, the HFOV group had a lower mortality rate during hospitalization (P<0.05), a longer length of hospital stay (P<0.05), and higher incidence rates of intracranial hemorrhage and bronchopulmonary dysplasia (P<0.05). Compared with the non-operation group, the operation group had a lower mortality rate during hospitalization (P<0.05), a longer length of hospital stay (P<0.05), and higher incidence rates of intracranial hemorrhage and bronchopulmonary dysplasia (P<0.05). CONCLUSIONS:The application of HFOV and PDA ligation can improve the survival rate of preterm infants with pulmonary hemorrhage, but the incidence of intracranial hemorrhage and bronchopulmonary dysplasia is also increased.
Effect of minimally invasive surgery for intracerebral hemorrhage: a single center retrospective analysis

Abstract:Objective:To evaluated the safety, efficacy and longterm prognosis of different minimally invasive surgery treatments in patient with intracerebral hemorrhage (ICH).Methods:Patients with basal ganglia hemorrhage, admitted to Neurosurgery Department of Second Affiliated Hospital, School of Medicine, Zhejiang University from January 2019 to December 2020, were divided into three groups: small bone window craniotomy (SBWC) group, endoscopic surgery (ES) group and stereotactic hematoma aspiration followed by urokinase (SHAU) group. The baseline characteristics, perioperative information and lone-term outcome data of the three groups were analyzed to evaluate the safety and effectiveness of different minimally invasive treatments in the treatment of ICH.Results:There were 356 patients in this group, including 116 cases in ES group, 135 cases in SHAU group and 105 cases in SBWC group. No significant differences were observed among all three groups the baseline characteristics, including age, sex, hematoma location, GCS score, NIHSS score and hematoma volume (P>0.05). There were significant differences among the three groups with respect to the operative blood loss and operative time (P<0.05). The rebleeding rate in the SHAU groups was significantly higher than in the other two groups (P<0.05). The evacuation rate 24 h after operation in AMWC and ES groups was significantly higher than SHAU group (P<0.05). The evacuation rate 3 d after operation in ES group was better than that in SBWC group and SHAU group (P<0.05), but there was no significant difference between SBWC group and SHAU group (P>0.05). On 7 d after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05). Six months after surgery, the NIHSS score in ES and SHAU groups were significantly higher than the AMWC group, the good prognosis rate in ES and SHAU groups were significantly higher than the AMWC group (P<0.05).Conclusion:Minimally invasive surgery has a definite effect on the treatment of ICH and can significantly improve the long-term outcome. Among them, endoscopic surgery has high hematoma removal efficiency and fewer complications.
Outcomes and Prognostic Factors of Infantile Acquired Hydrocephalus: a Single-Center Experience.

Abstract:AimTo assess the etiologies and adverse outcomes of infantile acquired hydrocephalus and predict prognosis.MethodsA total of 129 infants diagnosed with acquired hydrocephalus were recruited from 2008 to 2021. Adverse outcomes included death and significant neurodevelopmental impairment which was defined as Bayley Scales of Infant and Toddler Development III score < 70, cerebral palsy, visual or hearing impairment, and epilepsy. Chi-squared was used to evaluate the prognostic factors of adverse outcomes. A receiver operating characteristic curve was calculated to determine the cutoff value.ResultsOf 113 patients with outcome data, 55 patients (48.7%) had adverse outcomes. Late surgical intervention time (13 days) and severe ventricular dilation were associated with adverse outcomes. The combination of surgical intervention time and cranial ultrasonography (cUS) indices was a better predictive marker compared with any of them (surgical intervention time, P = 0.05; cUS indices, P = 0.002). Post-hemorrhage (54/113, 48%), post-meningitis (28/113, 25%), and hydrocephalus arising from both hemorrhage and meningitis (17/113, 15%) accounted for a large proportion of the etiologies in our study. Hydrocephalus occurs secondary to post-hemorrhage and had a favorable outcome compared with other etiologies in both preterm and term groups. A significant difference in adverse outcomes between the inherited error of metabolism as a cause and other etiologies (P = 0.02).ConclusionLate surgical treatment times and severe ventricular dilation can predict adverse outcomes in infants with acquired hydrocephalus. It is crucial to identify the causes of acquired hydrocephalus to predict the adverse outcomes. Research into measures of improving adverse outcomes following infantile acquired hydrocephalus is urgently necessary.
Curative effect analysis of robot-assisted drainage surgery in treatment of spontaneous hypertensive brainstem hemorrhage

Abstract:Objective: Spontaneous hypertensive brainstem hemorrhage (HBSH) is characterized by sudden onset, rapid progression and poor prognosis. There has been a growing tendency of surgical treatment for HBSH. This study aimed to investigate outcomes and potential factors associated with the prognosis of robot-assisted drainage surgery for HBSH treatment. Methods: Patients with HBSH from July 2016 to March 2023 at a single neurosurgery center were included and divided into conservative group and surgical groups. Baseline and clinical data, radiographic characteristics, complications, and outcome evaluations were recorded and analyzed. Results: A total of 125 patients, with 74 in the conservative group and 51 in the surgical group, were enrolled in the study. Mortality at 6 months was 59/74 (79.7%) in the conservative group and 9/51 (17.6%) in the surgical group. Twenty-four patients (47.1%) achieved favorable outcomes in the surgical group, whereas this rate in the conservative group was only 5.4% (4/74). There was a significant difference in NIHSS, GCS, and mRS at 6 months between surviving patients in the conservative and surgical groups. In prognostic analysis in the surgical subgroup, initial GCS score [5 (IQR 4–7) vs. 3 (IQR 3–4), p < 0.001], NIHSS [36 (IQR 32–38) vs. 40 (IQR 38–40), p < 0.001], smoking history [45.8% (11/24) vs. 74.1% (20/27), p = 0.039], hematoma volume [6.9 (IQR 6.2–7.6) vs. 9.6 (IQR 7.3–11.4), p = 0.001], and hemorrhage location ( p = 0.001) were potential risk factors for poor 6-month prognosis after robot-assisted surgery for HBSH. Conclusion: Based on the results of this study, robot-assisted minimally invasive drainage of brain stem hematoma may significantly reduce mortality and improve prognosis. Surgery should be conducted for selected patients.

Abstract:Objective To study the clinical and etiological characteristics of the newborn with hydrocephalus,and to compare the treatment methods and outcomes for hydrocephalus with different causes. Methods Neonatal onset of hydrocephalus cases admitted to neonatal ward of our Hospital from September, 2011 to June,2015 were retrospectively enrolled for study.Data of demographic characteristics,cranial imaging findings,treatment methods and efficacy of therapy of these patients were collected.Infants enrolled in this study were followed up after three to six month of discharge from hospital.Response and outcome of different treatment for hydrocephalus with different etiologies were compared.The causes of hydrocephalus and the treatment methods were also compared between term and preterm infants.Results A total of 38 cases of hydrocephalus were enrolled,of which 23 cases were premature infants (60.5%),15 cases were term infants (39.5%).Etiologies among our cohort were intracranial hemorrhage (18 /38,47.4%), central nervous system infection (11 /38,28.9%),intracranial hemorrhage complicated with central nervous system infection (8 /38,21 .1 %),and inborn error of metabolism (methylmalonic aciduria and hyperhomocysteinemia)(1 /38,2.6%).Etiological spectrum of hydrocephalus between preterm infants and term infant were of no significant difference statistically (P >0.05 ).The majority (14 /18,77.8%)of hydrocephalus following intracranial hemorrhage were treated by intermittent lumbar puncture,external drainage of ventricles,liquid storage sac of ventricle and recovered.Among infants with intracranial hemorrhage plus central nervous system infection and infants with central nervous system infection without hemorrhage,majority (9 /11 and 6 /8 respectively)finally received permanent shunt (ventriculo-peritoneal shunt).Percentage of shunt in hydrocephalic infants with central nervous system infection was significantly higher than that in infants with intracranial hemorrhage alone (P <0.05 ).After hospital discharge, 37 (97.4%)infants had no recurrence of hydrocephalus,whereas 16 (42.1 %)infants had varying degrees of neurological sequelae.Conclusion Intracranial hemorrhage and central nervous system infection are the common causes of hydrocephalus.Majority of hydrocephalus patients who without intracranial hemorrhage do not need permanent shunt treatment.However,nearly half of the infants have neurological sequelae even after the hydrocephalus has recovered.
Observation of the effect of ventriculo peritoneal shunt combined with Tongqiao Huoxue Lishui method on pediatric hydrocephalus

Abstract:Objective It is to evaluate the clinical effect and safety of ventriculo peritoneal shunt combined with Tongqiao Huoxue Lishui method for pediatric hydrocephalus.Methods 80 children with hydrocephalus were randomly divided into treat-ment group and control group with 40 cases in each group.Both the groups were treated with ventriculo peritoneal shunt, and the treatment group was added with self-made Huoxue Tongluo decoction at one week before operation, the decoction was stopped on the operative day, and continued when the patients could have liquid diet, and the treatment lasted for four weeks after operation.The clinical curative effect and correlated complications between the two groups were compared.Results The curative effect of treatment group was much better than that in the control group (P<0.05), all the healing rates of symptoms and signs, such as development of language and movement, convulsions, nausea, low spirits and blakish complexion except head width were higher in treatment group than those in control group ( all P<0.05);there were one case with peritoneal cav-ity infection, on case with shunt tube obstruction in treatment group, two case with peritoneal cavity infection, on case with shunt tube obstruction in control group, the differences between the two groups were not significant;no one with hypotension syndrome or viscera perforation was found in both groups.Conclusion Ventriculo peritoneal shunt combined with Tongqiao Huoxue Lishui method can significantly improve the effect of operation and clinical symptoms with less complications in the treatment for pediatric hydrocephalus.

Abstract:Objective: The aim of this study is to identify the factors predicting persistent hydrocephalus after periventricular tumor resection in children and assess the need and efficacy of perioperative cerebrospinal fluid (CSF) intervention. Methods: We performed a retrospective analysis of pediatric patients who underwent resection surgery of a periventricular tumor between March 2012 and July 2021 at the Department of Neurosurgery in Zhujiang Hospital of South Medical University. Demographic, radiographic, perioperative, and dispositional data were analyzed using univariate and multivariate models. Results: A total of 117 patients were enrolled in our study. Incidence of postoperative persistent hydrocephalus varied with tumor pathology ( p = 0.041), tumor location ( p = 0.046), surgical approach ( p = 0.013), extension of resection ( p = 0.043), tumor volume ( p = 0.041), preoperative Evan's index ( p = 0.002), and preoperative CSF diversion ( p = 0.024). On logistic regression, posterior median approach (OR = 5.315), partial resection (OR = 20.984), volume > 90cm 3 (OR = 5.768), and no preoperative CSF diversion (OR = 3.661) were independent predictors of postoperative persistent hydrocephalus. Preoperative Evan's index is significantly correlated with tumor volume ( p = 0.019). Meanwhile, the need for preoperative CSF drainage in patients in this cohort was significantly correlated with tumor location ( p = 0.019). Conclusion: Tumor pathology, location, surgical approach, the extension of resection, tumor volume, preoperative Evan's index, and preoperative CSF diversion were considered to be predictive factors for postoperative persistent hydrocephalus. Notably, posterior median approach, partial resection, and tumor volume > 90cm 3 , without preoperative CSF diversion, were identified as independent risk factors for persistent postoperative hydrocephalus. Preoperative identification of children at risk of developing persistent postoperative hydrocephalus would avoid delays in planning the cerebrospinal fluid diversion. Active and effective preoperative hydrocephalus intervention in children with periventricular tumors is beneficial to reduce the incidence of persistent hydrocephalus and ventriculoperitoneal shunt surgery after resection.

Abstract:Objective To evaluate the effect of early single-stage synchronous ventriculoperitoneal shunt(VPS) and cranioplasty for the patients with postoperative skull defect and hydrocephalus following severe craniocerebral injury.Methods The 53 patients with postoperative skull defect and hydrocephalus following severe craniocerebral injury operation were divided into two groups,including 36 cases(group A) underwent early synchronous operation and 17 cases(group B) underwent two-stage operations.Results There were no significant differences in the underdrainage or scalp infection between the two groups,but the rates of curative effect were significantly higher and the occurrent rates of shunt infection were significantly lower in group A than those in group B.Conclusion Adoption of early single-stage ventriculoperitoneal shunt and cranioplasty has a significantly better curative effect than two-stage operations,and can decrease the occurrent rates of shunt infection,so it is an effective,economical and practical therapeutic means to the patients with postoperative skull defect and hydrocephalus following severe craniocerebral injury.
Postnatal treatment and prognostic analysis for intracranial cyst diagnosed in the fetal period

Abstract:Objective To evaluate the influence of intracranial cyst diagnosed in the fetal period on postnatal growth and development and effectiveness of early surgical treatment. Methods Clinical data of 27 children with intracranial cyst diagnosed by prenatal ultrasound and MRI were analyzed retrospectively. The cysts increased to different extents in all the children after birth, including no obvious clinical symptoms in 17 patients, hydrocephalus in 6, weak limb activity in 2 and epileptiform seizure in 2. Cyst decollement under endoscope was performed in 21 patients and cyst decollement under microscope in 6. Results Early complications included subdural effusion in 4 patients, in which absorption by self in 3, and improvement after external drainage in 1 patient with chronic hematoma in hydrops. Cyst recurrence occurred in 2 patients, and improved after endoscopic surgery again. During a mean follow-up period of 13.1 months, the growth and development of these children were good, the cysts shrunk to different extents and hydrocephalus relieved in all the patients. Conclusions Early surgery via opening the window between cyst and cerebral cistern or ventricle for intracranial cyst diagnosed by prenatal ultrasound timely can establish sufficient cerebrospinal fluid circulation, shrink the cyst and promote brain development. Endoscopic surgery can be the first choice for this kind of disease.

Abstract:Abstract Background Neonatal hydrocephalus is a serious neurological disease, however, with the progress of medical technology, and the increasing survival rate of extremely premature infants, its incidence rate, etiology and diagnosis and treatment have also undergone great changes. In this study, we aimed to summarize the above clinical changes of neonatal hydrocephalus at our hospital over the past 12 years to provide a reference for clinical practice. Methods The medical records of infants with neonatal hydrocephalus admitted to the neonatal unit of Peking University Third Hospital from January 1, 2010, to December 31, 2021, were retrospectively examined. For comparative analysis, the infants were divided according to the time of admission into a group for the first 6 years and one for the latter 6 years. We compared the gestational age, birth weight, etiology, treatment, and prognosis of two groups. Results Sixty-two patients (28 in the first 6 years; 34 in the latter 6 years) were diagnosed with neonatal hydrocephalus. Their gestational ages ranged from 25 to 39+ 6 weeks (median, 30 weeks), and their birth weights ranged from 600 to 4046 g (median, 1255 g). The gestational age and birth weight were significantly lower in the latter 6-year group than in the first 6-year group. There was a significant increase in severe periventricular–intraventricular hemorrhage in infants and cerebral parenchymal hemorrhage in the latter 6-year group. In terms of surgical treatment, there are more applications of Venturar Access Device(VADs)in the latter 6-year group. Twenty-one infants (35%) died after treatment withdrawal. Of the surviving infants, 22 had normal development, and 19 had developmental delay. Conclusion Gestational age and birth weight were significantly lower in the latter 6-year group than in the first 6-year group, and there was a significant increase in severe intracranial hemorrhage and cerebral parenchymal hemorrhage in this group.

Abstract:Objective To analyze the influencing factors of surgical management for neonatal complicated congenital heart disease(CCHD),sumarize the clinical experience and improve the surgical effect.Methods The clinical data of 154 neonates with complicated congenital heart disease who were undergone surgical management from August 2008 to August 2013 were retrospectively analysed.Results There were 14 cases of death with a mortality rate of 9.1％ (14/154),including 4 cases of preoperative death.The operative motality is 6.7％ (10/150),including 3 cases of single malformations(D-TGA/VSD,PA/VSD,IAA),and 7 cases of multiple malformation(HRHS/PS in 2,D-TGA/COA in 1,D-TAG/PS in 1,COA/SV in 1,SV/PA/IVS in 1 and D-TGA/COA/SV in 1).140 cases accepted regular follow-up after hospital discharge from 3 months to 5 years,8 of them underwent secondary surgery and all survived.Conclusion Timely diagnosis,actively improvement for preoperative internal environment,fine operative technique,individual cardiopulmonary bypass and intensive postoperative care are all important influencing factors of surgical effect for neonatal severe complicated congenital heart disease.

Abstract:Objective To analyze the influencing factors of surgical management for Blalock-Taussig shunt during neonatal period,and to summarize the clinical experience,therefore,to improve the surgical outcome.Methods The clinical data between Jan 2011 and Dec 2016 were reviewed,42 neonates(26 males,16 females)with the mean age from 1 to 29 days,and weight from 2.3 to 4.1 kg,underwent a Blalock-Taussig shunt.The additional operation included PDA retention in 3 patients,right ventricular outflow tract retention in 12 patients,B-T conduit banding in 2 patients and PDA banding in 1 patient.Results The early mortality was 28.6％ (12/42).Univariate analysis revealed low birth weight,waiting time for surgery,preoperative acidosis or cardiac shock,dual pulmonary blood supply,conduit/weight,high IS (inotropic drugs score),unplanned intervention as risk factors for early death.In the multivariate analysis,preoperative acidosis or cardiac shock,conduit/weight,high IS were independent risk factors of early death.Conclusion The mortality rate after the neonatal modified Blalock-Taussig shunt remains high.It can be improved by proper perioperative treatment,immediate surgical treatment and choose suitable conduit size,maintain the stable circulation.
