Comparison Between Endoscopy and Conventional Drainage Operations in Treatment of 66 Cases with Hypertensive Intraventricuar Hemorrhage

Abstract:Objective To explore the clinical value of neurosurgical endoscopy in treatment of hypertensive intraventricular hemorrhage.Methods 66 patients with hypertensive intraventricular hemorrhage in the First Affiliated Hospital of Medical School of Xi'an Jiaotong University from February 2010 to August 2012 were recruited and divided into endoscopy treatment group(n = 36) and conventional drainage group(n = 30).Patients in endoscopy treatment group were treated with endoscopic hematoma evacuation operations,patients in conventional drainage group were treated with lateral ventricular puncture and drainage operations.Hematoma evacuation rate,infection rate,hydrocephalus occurrence rate,ADL grades 6 months after operation were compared.Results ①Hematoma evacuation rate 90% were found in 12 cases in endoscopy treatment group,hematoma evacuation rate 60%-90% were found in 21 cases in endoscopy treatment group,hematoma evacuation rate 60% were found in 3 cases in endoscopy treatment group 48 hours after the treatment;hematoma evacuation rate 90% were found in 1 case in conventional drainage group,hematoma evacuation rate 60%-90% was found in 8 cases in conventional drainage group,hematoma evacuation rate 60% were found in 21 cases in conventional drainage group 48 hours after the treatment,the differences were statistically significant(Zc = 5.146,P 0.01).②1 case(2.7%) of intracranial infection was found in endoscopy treatment group,4 cases(13.3%) of intracranial infection were found in conventional drainage group,the difference was not statistically significant(χ2 = 1.315,P = 0.252);2 cases(5.6%) of hydrocephalus were found in endoscopy treatment group,11 cases(36.7%) of hydrocephalus were found in conventional drainage group,the difference was statistically significant(χ2 = 8.143,P = 0.004).③6 months after the operation,17 cases of ADL Ⅰ grade,9 cases of ADL Ⅱ grade,6 cases of ADL Ⅲ grade,2 cases of ADL Ⅳ grade,1 case of ADL Ⅴ grade,1 case of dead were found in endoscopy treatment group,good recovery rate was 88.89%(32/36);5 cases of ADL Ⅰ grade,8 cases of ADL Ⅱ grade,7 cases of ADL Ⅲ grade,2 cases of ADL Ⅳ grade,1 case of ADL Ⅴ grade,2 cases of dead were found in conventional drainage group,good recovery rate was 66.67%(20/30),the difference was statistically significant(Zc = 2.907,P = 0.004).Conclusion Endoscopy treatment in evacuation of intraventricular hemotoma shows better effect than conventional drainage treatment,it has less complications and sequelae,can improve prognosis,and shows exact effect.
