Abstract:Photoplethysmography (PPG) is a widely used technology for health monitor based on pulse wave measurement by monitoring the blood volume of blood vessels via electro-optic technique. As a kind of non-electrophysiological signal with low amplitude and low frequency, PPG signal may be easily disturbed by motion artifact. This paper proposes a simulation method based on a new reflection model which includes a skin-friendly flexible substrate with a narrow-band full-reflection film plating on it and an embedded system accordingly to study anti-motion artifacts detection in pulse wave monitoring. Monte-Carlo method is presented to simulate the dynamic human skin model and the results demonstrate the effectiveness of the proposed model. A wrist worm artifact-resistive pulse wave monitoring platform (PWMP) is presented accordingly, the measurement accuracy of pulse rate by the platform is within +/- 2 beats per minute(bpm) at the range of 30bpm to 240bpm compared with the output of Fluke Index2 (produced by Fluke Corp, USA) in stationary situation. Three kinds of typical postures are performed to verify the proposed model experimentally, results show that the proposed platform has good correlation as compared to PC-60B Medical Pulse Oximeter from Heal Force in the measurement of pulse rate, and the pearson correlation coefficient is 0.953(p<0.01), which reveals that the proposed model has the potential to recover pulse wave signal for pulse rate monitoring.

Abstract:Pulse wave contains much information on a cardiovascular system. Pulse wave variability during exercise is of great significance as it reflects more information combining with pulse wave under stationary state. This paper studied the morphology variability of radial pulse wave during exercise. Radial pulse waves were collected from 30 subjects with two pressure pulse sensors worn at the wrists of the right and left hands, respectively. Electrocardiography (ECG) was also detected synchronously. After data preprocessing and feature point extraction, the variability of several parameters of pulse wave and ECG were analyzed. It is notable that pulse rate (PR) and heart rate (HR) change synchronously. During the exercise period, both systolic phase and diastolic phase of a radial pulse shorten but the latter is more obvious. The amplitude of the dicrotic notch decreases and even turns negative. Aligning the radial pulse waveforms together, the radial pulse waveforms prior to, during and after exercise coincide with each other except for some details like the tidal wave which fades away during exercise.

Abstract:Analysis of pulse signals has the following problems: low accuracy of frequency domain analysis,lack of stability analysis and over-dependence on experience to extract features in frequency domain.Combined with Lasso regression,the wavelet multi-resolution decomposition was used to extract features in precise frequency bands in order to mine the diseases features of pulse signals in frequency domain.A class of pulse signals of patients with cardiovascular diseases was analyzed to verify the stability of frequency features in time domain.The analysis of atrial fibrillation and coronary heart diseases was implemented to show how to extract feature bands and classify the two diseases.Then a linear classification model was constructed to realize the automatic identification of both diseases.

Abstract:The arterial pulse wave is a physiological signal which can reflect the function of the human cardiovascular system. Owing to the measurement being convenient and safe, radial pulse waves have been often used in noninvasive monitoring of human health, which can further reflect the health status of the cardiovascular system, including information of the aorta and peripheral arteries. Moreover, radial pulse waves can assist clinical diagnosis in the prevention and treatment of cardiovascular disease. This paper systematically reviews the progress in analysis of pulse wave analysis, including pulse wave acquisition, waveform processing, pattern classification, parameter estimation, and clinical applications. In terms of waveform acquisition, this paper reviews a variety of methods to obtain radial pulse waves, including tonometry, photoplethysmography, ultrasound manometry, and flexible tactile pressure sensors. In the aspect of waveform processing, this paper summarizes the methods of radial pulse waves preprocessing and feature extraction. With the rapid development of machine learning and deep learning algorithms, radial pulse waves can be used to identify the status of human cardiovascular systems and to estimate parameters related to cardiovascular function. This paper also discusses the applications of radial pulse waves in clinical practice, including cardiovascular function evaluation and pulse diagnosis in traditional Chinese medicine. Some open-source databases and analysis software are also listed. The current development trend, challenges, and future directions of analysis of radial pulse waves are also offered.

Abstract:We investigated the beat-to-beat fluctuation of the photoplethysmography (PPG) waveform. The motivation is that morphology variability extracted from the arterial blood pressure (ABP) has been found to correlate with baseline condition and short-term surgical outcome of the patients undergoing liver transplant surgery. Numerous interactions of physiological mechanisms regulating the cardiovascular system could underlie the variability of morphology. We used the unsupervised manifold learning algorithm, Dynamic Diffusion Map, to quantify the multivariate waveform morphological variation. Due to the physical principle of light absorption, PPG waveform signals are more susceptible to artifact and are nominally used only for visual inspection of data quality in clinical medical environment. But on the other hand, the noninvasive, easy-to-use nature of PPG grants a wider range of biomedical application, which inspired us to investigate the variability of morphology information from PPG waveform signal. We developed data analysis techniques to improve the performance and validated with the real-life clinical database.

Abstract:Pulse diagnosis is an essential part in four basic diagnostic methods (inspection, listening, inquiring and palpation) in traditional Chinese medicine, which depends on longtime training and rich experience, so computerized pulse acquisition has been proposed and studied to ensure the objectivity. To imitate the process that doctors using three fingertips with different pressures to feel fluctuations in certain areas containing three acupoints, we established a five dimensional pulse signal acquisition system adopting a non-contacting optical metrology method, 3D digital image correlation, to record the full-field displacements of skin fluctuations under different pressures. The system realizes real-time full-field vibration mode observation with 10 FPS. The maximum sample frequency is 472 Hz for detailed post-processing. After acquisition, the signals are analyzed according to the amplitude, pressure, and pulse wave velocity. The proposed system provides a novel optical approach for digitalizing pulse diagnosis and massive pulse signal data acquisition for various types of patients. (C) 2017 Elsevier Ltd. Abstract:Monitoring of arterial blood pressure via cuffless pulse waveform measurement at the wrist has an important clinical value for the early diagnosis and prevention of cardiovascular disease. However, accurate measurement of the radial pulse waveform is challenging owing to its subtle, wideband, and preload-dependent variation characteristics. Evidence shows that uncertainties or variations of wearing pressure and skin temperature can cause artifact signals in wrist pulse measurements, thus degrading blood pressure estimate accuracy and hindering precise clinical diagnosis. Herein, we report a flexible multisensory pulse sensor utilizing natural piezo-thermic transduction of human skin in conjunction with thin-film thermistors for the accurately measuring radial artery pulse waves with high fidelity and good anti-artifact performance. The flexible pulse sensor achieved a wide pressure measuring range (228.2 kPa), low detection limit (4 Pa), good linearity ( R 2 = 0.999), low hysteresis (2.45%), fast response (88 ms), and good durability and stability, thereby enabling accurate pulse measurement with high fidelity. The pulse sensor also monolithically integrated the simultaneous detections of skin temperature and wearing pressure for resisting artifact effects in pulse measurements. Through the fusion of multiple features extracted from the pulse waveform, wearing pressure, skin temperature and user's personal physical characteristics using an efficient multilayer perceptron, blood pressure is accurately estimated and good generalizability is achieved.

Comparison of pulse wave velocity computed by different characteristic points

Abstract:To determine which couple of characteristic points is proper for calculating pulse wave velocity (PWV), 10 sets of PWV based on various points of electrocardiogram (ECG) and plethysmogram (PPG) were obtained and analyzed. PWV values from 468 subjects were compared by mean deviations and their standard deviations in computation. Repeatability coefficient and correlation coefficient between different PWV values were computed. Meanwhile, correlations between PWV and age, blood pressure and low density lipoproteins cholesterol (LDL-C), etc. were analyzed to determine which set of PWV is sensitive to physiological parameters. The results show that PWV computation based on the R wave of ECG and the foot of PPG computed by maximum second derivation method (A 1) and intersecting tangent method (A 3) has the minimum deviation and best reproducibility. Also these two sets of PWV are closely related to clinical physiological parameters and exhibit the ability in distinguishing normotensive and hypertensive.

Abstract:Pulse signals contain abundant information which is useful in healthcare. Various approaches have been suggested with an aim of acquiring high-quality pulse signals, but are usually limited by either low accuracy or complicated apparatus. In this work, we proposed a "quasi applanation tonometry" method for pulse detection, where an adjustable static pressure was applied on wrist to compress artery and pulse signals were simultaneously recorded at the same position. To realize the method, a wearable compound sensing system, consisting of a piezoresistor and a piezoelectret sensor, was developed to measure the static pressure and pulse signals, respectively. The results showed that clear pulse signals with detailed information were successfully acquired with the sensing system, and by increasing the static pressure the pulse amplitude increased accordingly until a maximum was achieved. Further increase of the static pressure would lower the pulse amplitude and destroy the waveform gradually. A linear relation between the mean arterial pressure and the static pressure for maximum pulse amplitude was fitted, which would support the proposed method for the application of blood pressure estimation. In addition, heartbeat and respiration monitoring were also realized based on the recorded pulse signals. In general, this work has demonstrated a simplified pulse detection method by developing a piezoresistor-piezoelectret compound sensing system and realized some applications of pulse detection in health monitoring.

Reproducibility of Pulse Wave Measurement

Abstract:Objective To assess the reliability of central and peripheral pulse wave velocity measurement. Methods Carotid-femoral and carotid-radial pulse wave velocity(CFPWV, CRPWV) measurements were performed in five healthy female volunteers by two observers in two separate days, with 3 exams each day at 9:00, 11:00, 14:00 respectively. Results The differences of mean absolute values between two observers were 0.07±0.70 m/s(9.54±1.02 m/s vs 9.47±0.82 m/s) and 0.64±1.40 m/s(9.60±1.10 m/s vs 8.96±0.91 m/s) for the CFPWV and CRPWV. Pearson coefficients were 0.73(P<0.01) and 0.1(P=0.86) and total intra-class coefficients were 0.50 and 0.15 for CFPWV and CRPWV. Bland-Altman plots showed good inter-observer reproducibility both for CFPWV and CRPWV. The inter-occasion reproducibility was not content enough for clinical application, with intra-class coefficient 0.23-0.68 and 0.01-0.32 for CFPWV and CRPWV. Conclusion Under standardized measurement conditions, CFPWV and CRPWV showed good inter-observer reproducibility but less satisfactory inter-occasion reproducibility.

Network Modeling Analysis in Health Informatics and Bioinformatics

Abstract:Ayurveda and traditional Chinese medicine (TCM) have used wrist pulse analysis for centuries. It is a non-invasive, novel, convenient, and highly successful disease diagnosis technique that analyses the pulse pattern from the patient's wrist. During the wrist pulse diagnostic, the clinician uses three fingers to palpate the patient's wrist to analyze the pressure unbalance as an imbalance of force under the finger may be the cause of disease. However, the unsatisfactory results of clinical rating scales pave the path for future research. Usage of radial pulse to analyze and diagnose disease seems to be a complicated process. As a result, early automatic evaluation can improve life quality and longevity. The GRADE (Grading of Recommendations Assessment, Development, and Evaluation) and SPIDER (Sample, Phenomenon of interest, design, evaluation and research type) search methodology schemes were used to find data and relevant studies for this study. Approximately 800 articles were extracted using relevant search phrases. After a step-by-step mapping and removal of various studies, sixty papers were found appropriate for this review. Following the quality assessment of the collected studies, seven inhibitors from the patient's Wrist are identified for analysis, five of which are significant. The pulse features of many machine learning categorization techniques in previous studies are also compared in this review. The term "future views" refers to the research gaps that have been identified. According to the researchers, wrist pulse analysis findings can help practitioners to understand early biomarkers for disease diagnosis, categorization, and quantification. The significance of this technique of disease diagnosis is low-cost and noninvasiveness. It provides an in-depth understanding of different pulse features that affect disease identification and categorization by encapsulating the wrist pulse.

There is an association between the morphologic variation extracted from arterial blood pressure (ABP) signals and short-term surgical outcomes. The underlying physiology could be the numerous regulatory mechanisms on the cardiovascular system. We hypothesized that similar information might exist in PPG waveform. However, due to the principles of light absorption, the noninvasive PPG signals are more susceptible to artifacts and necessitate meticulous signal processing. Approach: Employing the unsupervised manifold learning algorithm, Dynamic Diffusion Map, we quantified multivariate waveform morphological variations from the PPG continuous waveform signal. Additionally, we developed several data analysis techniques to mitigate PPG signal artifacts to enhance performance and subsequently validated them using real-life clinical database. Main results: Our findings show similar associations between PPG waveform during surgery and short-term surgical outcomes, consistent with the observations from ABP waveform analysis. Significance: The variation of morphology information in the PPG waveform signal in major surgery provides clinical meanings, which may offer new opportunity of PPG waveform in a wider range of biomedical applications, due to its non-invasive nature.

Simulation study of pulse wave in the radial artery

Abstract:An extended mathematical model of human cardiovascular system was established for the study of pulse wave diagnostic method. This model can be used to generate pulse waves at three points along the radial artery on the wrist. Using this model, a number of simulation experiments were performed. The effects of hemodynamic parameter variations, such as the blood viscosity and vessel wall elasticity, upon the pulse waveforms are investigated. The impacts of pathological changes, such as heart failure, hypertension, and arteriosclerosis, have been studied through computer simulation by using the model established in this work. To verify the model and the simulation results, clinical data from 31 subjects with known health status have been collected. The clinical data agree with our simulation results. Thus, the results of this work suggest that using modeling and simulation method to study the pulse wave is possible and the model established in this work provide a useful tool for the pulse wave analysis.

Abstract:We apply the recently developed adaptive non-harmonic model based on the wave-shape function, as well as the time-frequency analysis tool called synchrosqueezing transform (SST) to model and analyze oscillatory physiological signals. To demonstrate how the model and algorithm work, we apply them to study the pulse wave signal. By extracting features called the spectral pulse signature, {and} based on functional regression, we characterize the hemodynamics from the radial pulse wave signals recorded by the sphygmomanometer. Analysis results suggest the potential of the proposed signal processing approach to extract health-related hemodynamics features.

Pressure wrist pulse signal analysis by sparse decomposition using improved Gabor function

Abstract:Background and objective: In traditional Chinese medicine and Ayurvedic medicine, wrist pulse wave fluctuations are an important indicator for distinguishing different health states. Owing to the development of modern sensing technology, computational methods have been used in the analysis of pulse wave signals. The description and quantification of the peaks in the pulse wave is significant for the identification of health status. Methods: In this study, we decomposed the pressure pulse waveform of the radial artery into several components by sparse decomposition with an improved Gabor function. To better represent the position, shape, and relationship of the peaks, we designed an improved Gabor function structure based on the characteristics of the pulse waveform to generate a time-frequency dictionary. Compared with conventional representation methods, the shape of the Gabor function is more variable. In addition, owing to the limitation of windowing, the Gabor function can reduce the influence on other positions when it represents a specific position. Feature vectors consisting of decomposed components can be used for computerized pulse signal analysis and disease diagnosis. Results: In the binary classification of healthy and diseased pulse signals, the proposed method achieved the best results for health/diabetes, health/cardiac disease, health/hypertension, and health/nephropathy with accuracies of 93.54%, 73.42%, 88.42%, and 82.28%, respectively. The multi-classification performance of the different types of features was evaluated by six classifiers, and the proposed method obtained the highest classification performance with support vector machine-radial basis function for both balanced and imbalanced data. Conclusions: The results indicated that the proposed method enabled to obtain a smaller representation error and exhibited superior performance in distinguishing between the signals collected from patients and healthy individuals. Moreover, for the multi-classification of the pulse signals, the proposed method performed better than the state-of-the-art methods.
Non-invasive Blood Pressure Measure Based on Wavelet Transform

Abstract:A novel method to calculate arterial blood pressure by using wavelet transform to detect the peak value of pulse wave and precisely calculate the pulse transit time was introduced and applied in a self-developed non-invasive continuous blood pressure measure system.The sensors used in the system have simple structure and wear comfortably.The system solved the problems,such as low accuracy and lack of synchronism in collecting electrocardiogram(ECG) and pulse wave signal,which exist in other systems based on pulse wave velocity.Experimental results showed that the novel method can detect the peaks of pulse wave precisely and calculate the artery blood pressure based on the pulse transit time.

Abstract:Electrocardiography (ECG) is recording of heart electrical activity. For analyzing and diagnosis of heart diseases ECG is very important. In graphical ECG which used for clinical diagnosis all features are not visible. Different types of signal processing methods are present which can be used for extracting ECG signal features. Wavelet transforms is one kind of signal processing tool which is used for analyzing ECG signal. For features extraction multi-resolution wavelet transform can be used. During recording of ECG different kind of noise are added with ECG. So noise should be removed from ECG, than R peaks were detected which amplitude is higher than the other peaks. Referring to R peaks the others peak as P, Q, S and T were detected. Then different feature of the ECG signal were detected. Time differences between R peaks were calculated and then heart rate calculated from mean RR interval. In ECG RR interval indicate the change between consecutive heart rate (HR). Heart rate variability (HRV) explored how RR interval varies over time. HRV is calculated from RR interval series obtained from ECG signal analysis. From the RR intervals time domain indices of HRV were determined by using MATLAB programming and MIT-BIH database signal were used as input. In the time domain method SDNN, RMSSD, and pNN50 etc were determined here.

Abstract:Heart Rate Variability (HRV) signal analysis provides a quantitative marker of the Autonomic Nervous System (ANS) function. A wristband-type wireless photoplethysmographic (PPG) device was custom-designed to collect and analyze the arterial pulse in the wrist. The proposed device is comprised of an optical sensor to monitor arterial pulse, a signal conditioning unit to filter and amplify the analog PPG signal, a microcontroller to digitize the analog PPG signal, and a Bluetooth module to transfer the data to a smart device. This paper proposes a novel model to represent the PPG signal as the summation of two Gaussian functions. The paper concludes with a verification procedure for HRV signal analysis during sedentary activities.
