Abstract:BACKGROUND AND PURPOSE:We aimed to establish the prevalence, characteristics, and outcomes of intracranial atherosclerosis (ICAS) in China by a large, prospective, multicenter study.METHODS:We evaluated 2864 consecutive patients who experienced an acute cerebral ischemia<7 days after symptom onset in 22 Chinese hospitals. All patients underwent magnetic resonance angiography, with measurement of diameter of the main intracranial arteries. ICAS was defined as ≥50% diameter reduction on magnetic resonance angiography.RESULTS:The prevalence of ICAS was 46.6% (1335 patients, including 261 patients with coexisting extracranial carotid stenosis). Patients with ICAS had more severe stroke at admission and stayed longer in hospitals compared with those without intracranial stenosis (median National Institutes of Health Stroke Scale score, 3 versus 5; median length of stay, 14 versus 16 days; both P<0.0001). After 12 months, recurrent stroke occurred in 3.27% of patients with no stenosis, in 3.82% for those with 50% to 69% stenosis, in 5.16% for those with 70% to 99% stenosis, and in 7.27% for those with total occlusion. Cox proportional hazards regression analyses showed that the degree of arterial stenosis, age, family history of stroke, history of cerebral ischemia or heart disease, complete circle of Willis, and National Institutes of Health Stroke Scale score at admission were independent predictors for recurrent stroke at 1 year. The highest rate of recurrence was observed in patients with occlusion with the presence of ≥3 additional risk factors.CONCLUSIONS:ICAS is the most common vascular lesion in patients with cerebrovascular disease in China. Recurrent stroke rate in our study was lower compared with those of previous clinical trials but remains unacceptably high in a subgroup of patients with severe stenosis.
Meta-analysis of long-term outcome of symptomatic intracranial artery stenosis: comparison of stenting and medical therapy

Abstract:Objective To compare the long-term outcomes between the stenting and the medical therapy in patients with symptomatic intracranial artery stenosis.Methods The articles that compared between the stenting and the medical therapy in patients with symptomatic intracranial artery stenosis searched from PubMed,Cochrane Lirary,EMBASE,and Google Scholar.A meta-analysis was conducted to evaluate risks of stroke recurrence and composite vascular events at 1 years,and composite vascular events risk and clinical outcomes (a good outcome was defined as the modified Rankin scale score 0-2) at 2 years after stent therapy or medical therapy.Results Six studies,comprising a total of 920 patients with symptomatic intracranial artery stenosis were included,458 in the stenting group,and 462 in the medical therapy group,their mean age was 61.8 years and 66％ were males.The meta-analysis showed that there were no significant differences in the risks of stroke recurrence (pooled odds ratio [OR] 1.74,95％ confidence interval [CI]0.95-3.20;P =0.07) and composite vascular events (pooled OR 1.27,95％ CI 0.74-2.16;P =0.39) at 1 year,as well as composite vascular events risk (pooled OR 1.30,95％ CI 0.89-1.89;P =0.17) and clinical outcome (pooled OR 1.37,95％ CI 0.32-5.82;P =0.67) at 2 years between stenting group and medical therapy group.Conclusions There were no significant differences in the risks of stroke recurrence and composite vascular events at 1 year,as well as composite vascular events risk and clinical outcome at 2 years after stenting or medical therapy in patients with symptomatic intracranial artery stenosis.

Effect of Endovascular Primary Stent-Assisted Angioplasty on Atherosclerotic Intracranial Stenosis

Abstract:Objective To analyse the preliminary outcome and its relevant factors of endovascular stent assisted angioplasty for intracranial stenosis. Methods Sixty one atherosclerotic intracranial stenoses in 57 patients (23 stenoses in posterior and 38 in anterior circulation) were treated with endovascular angioplasty using balloon expandable coronary stents. Results Technical success was achieved in all patients, with no procedural and associated complications except one of intracranial hemorrhage. Angiographic results immediately after stenting suggested that the residual postprocedure stenosis was 0%～32% (mean 8.7%±4.4%), with significant reduction from preprocedure stenosis (74.5%±6.7%). The patients were clinically asymptomatic and had no recurrent TIA and stroke. Follow up angiogram in 43 patients revealed no restenosis (50%), though 3 patients revealed mild endothelial hyperplasia. Conclusions Endovascular stenting for intracranial stenosis is safe and feasible, and would be favorable for decreasing incidence of stroke during short term follow up.

Abstract:Background: Stroke is currently the second-leading cause of death just behind ischaemic heart disease. Drug therapy is currently the standard of care for patients with symptomatic intracranial artery stenosis (sICAS). Stenting is an important treatment for the prevention and treatment of ischemic stroke. It has been suggested that vertebral artery stenting might reduce this risk, but operation-related complications limit the application of stenting in the treatment of ischemic stroke. The differences in the safety and efficacy of stenting combined with drugs and drugs alone in the treatment of sICAS are unclear. The aim of this study was to assess the impact of both treatment modalities on the prognosis of patients with sICAS through a systematic review and meta-analysis. Methods: The Chinese databases (CNKI, Wanfang, VIP, CBM, DUXIU) and English databases (Pubmed, Embase, Ovid_medline, Cochrane library, Web of science)were searched to identify all studies describing sICAS. The "Risk of Bias Assessment" tool and the "Jadad Scale" provided by the Cochrane Collaboration were used to evaluate the risk of bias and quality of the collected literature. The risk ratio (RR) and its 95% confidence interval (CI) were determined using Stata statistical software version 14.0. Results: A total of 11 studies were included, comprising a total of 1,915 patients. The combined results of the study showed no significant difference between the incidence of transient cerebral ischemia (TIA)and stroke in patients with sICAS treated with drugs in combination with stents versus drugs alone. The incidence of death or stroke, cerebral haemorrhage, disabling stroke or death was significantly higher in patients receiving stent-combined drug therapy versus drug therapy alone for sICAS. Conclusion: Studies suggest that stenting combined with medication for patients with sICAS may increase the incidence of death or stroke, cerebral haemorrhage, stroke or death, but has no significant effect on the incidence of TIA and stroke. The studies report inadequate and conflicting data and therefore the safety and efficacy of stenting for sICAS should be interpreted with caution. Clinical Study of Endovascular Angioplasty for Intracranial Stenosis with Different Stents

Abstract:Objective The aims of this study is to evaluate the safety,curative effects and restenosis of endovascular angioplasty with different stents and its influence on treating symptomatic intracranial stenosis. Method 264 patients with 287 symptomatic intraeranial stcntosis treated by stent-assised angioplasty were retrospectively reviewed. We analyzed relationship of types of stents with success rate, primary endpoints and restenosis. Results The success rate of BMSs,DESs and self-expanding stents were 95.3% ,97. 3% and 97.8% ,respectively. The cumulative probability of primary endpoint within 12 months in BMSs, DESs and self-expanding group was 4. 8%, 6. 5% and 6.0%, respectively. In patients with angiographic follow-up, the restenosis rate in BMSs group (28.5%) was significantly higher than that in DESs group (4. 8%) and self-expanding group (0%) . Conclusions Stent-assisted angioplasty is technically feasible and safe for symptomatic intracranial stenosis. The type of stent has no influence on success rate and primary endpoint.
Stent-assisted Angioplasty for Symptomatic Stenosis of Intracranial Arteries: a Systematic Review

Abstract:Objective To assess the efficacy and safety of stent-assisted angioplasty for symptomatic stenosis of intracranial arteries. Methods The databases including Medline, EMBASE, Pubmed and CMBase, and reference lists of relevant articles from the Journals of cerebrovascular diseases were searched respectively by computer and manual method. Results No relevant randomized control trial was found. A total of 1 168 patients with symptomatic stenosis of intracranial arteries were reported in 21 articles related to stent-assisted angioplasty and 264 patients with symptomatic stenosis of intracranial arteries received stent-assisted angioplasty in our department. The successful rate of stent-assisted angioplasty for intracranial artery stenosis was 96.61%.The probability of primary endpoint within 30 days was 12.53%, and the cumulative probability of ischemic stroke, hemorrhagic stroke and death after 30 days was 5.97%. The rate of restenosis after stent-assisted angioplasty ranged from 0 to 32.4%. Conclusions The endovascular stenting for intracranial artery stenosis is feasible, but it may produce higher mortality and disabled rate. The prognosis is good in the patients with symptomatic stnosis of intracranial arteries in whom the stent-assisted angioplasty is successfully performed.

Abstract:Stroke is one of the diseases that seriously threaten the survival and health of human beings. In Europe and the United States, stroke is the second leading cause of death after heart disease and tumors.([1])Stroke is also one of the fatal diseases in Asian countries. On average, about 1.5 million new stroke patients are added each year in China, and the incidence of stroke is increasing year by year. About 80% of all stroke patients are ischemic stroke, and symptomatic internal carotid atherosclerotic stenosis is another important cause of ischemic stroke. Ipsilateral carotid stenosis >= 50% increases the incidence of transient cerebral ischemia and stroke in the carotid artery region by 10% to 15%, and is also closely related to the recurrence of acute and long-term stroke in patients.([2])Therefore, the clinical application and efficacy of carotid stenting in patients with symptomatic internal carotid artery stenosis are analyzed and evaluated to provide a basis for the selection of clinical treatment options. The clinical data of patients with symptomatic carotid stenosis who underwent carotid stenting and the clinical data of conservative treatment of patients with symptomatic carotid stenosis were retrospectively analyzed, and the carotid stenosis rate, symptoms, and National Institute of Health stroke scale where compared before and after surgery. And activities of daily living score. Because the control group treatment method in this article is completely free for patients to choose, and belongs to a retrospective analysis, the results suggest that it can provide a high-quality treatment approach for the treatment of patients with symptomatic carotid stenosis, without causing any harm to the patient, So no ethical approval is needed, and no patient informed consent is required. In recent years, with the continuous advancement of science and technology and new stent materials, intravascular interventional technology has developed rapidly. In continuous clinical practice and research, the safety and effectiveness of stent technology have also been gradually improved. Arterial stenting has gradually become an important method for the treatment of atherosclerotic carotid stenosis. This technique can not only improve the symptoms and prognosis of patients with symptomatic internal carotid stenosis, but also prevent the occurrence of ischemic events. The promotion of this technology has the effect of reducing disability and mortality. The alternative therapy of drug therapy, namely arterial stent implantation, has become a new way to treat atherosclerotic stroke. This treatment technology can quickly relieve the abnormal hemodynamics of distal blood vessels caused by arterial stenosis, which is ischemic. Cerebrovascular disease provides new ideas for treatment. Carotid angioplasty and stenting for symptomatic internal carotid stenosis under a distal cerebral protection device is a safe and effective treatment.

Abstract:Objective To observe the effectiveness and safety of endovascular stent assisted angioplasty for symptomatic intracranial arteriostenosis.Methods 4 patients with symptomatic intracranial arteriostenosis were treated with endovascular angioplasty using balloon expandable stents. Therapeutic effects were evaluated by clinical manifestations, digital subtraction angiography (DSA) and transcranial Dopple (TCD) before and after the operation. Follow up was made for 7~10 months.Results Technical success was achieved in all patients, with no complication of cerebral embolism and hemorrhage except one of transient seizure during operation. Angiographic results immediately after stenting suggested that the residual postoperation stenosis was 8%(0%~17%), with significant reduction from preoperation stenosis of 75%(70%~80.7%). No recurrent TIA and stroke occurred and no restenosis was found with serial TCD in all 4 patients during follow up of 7~10 months.Conclusions Endovascular stenting is a safe and effective therapeutic tool for symptomatic intracranial arteriostenosis.
Safety and Efficacy on Cerebral Protection with Filter Devices During Carotid Artery Stenting in Chinese Patients

Abstract:OBJECTIVE:To assess the efficacy, safety, procedural success and long-term clinical outcome in patients underwent percutaneous carotid stenting with distal device.METHODS:Percutaneous carotid stents with distal device were implanted to 58 patients with 59 significant (> 75%) carotid artery stenosis (49 men, mean age 68 years) between January 2000 to December 2005. Forty-five out of 58 patients were symptomatic, 35 had coronary artery diseases and 10 had previous strokes.RESULTS:Sixty one carotid stenting were implanted to 59 lesions in 58 patients. Stents with filter devices were successfully implanted in 57 out of 58 (98%) patients. Angioplasty success rate was 100%. Aspirin (300 mg/d) and Clopidogrel (75 mg x 2/d) were administered 3 days prior operation and clopidogrel was discontinued 30 days post stenting and aspirin was continued at dose of 100 mg/d. The percentage of stenotic carotid artery reduced from 85.3% to 6.2% after stenting and the diameter increased from 1.3 +/- 0.9 mm to 5.2 +/- 1.1 mm. Two minor strokes (3.4%) occurred during operation and at 14 days post stenting. All patients were discharged from the hospital after an average of 2.5 days hospitalization. At 14 +/- 2 months follow up, all patients survived and there were 2 asymptomatic restenosis (50% and 70% and the latter underwent successful balloon angioplasty), 2 myocardial infarctions (1 non-Q wave and 1 Q wave myocardial infarction, all underwent successful emergent PCI) and 2 minor strokes occurred at 6 and 8 months post stenting.CONCLUSION:Carotid stenting with distal device appears to be safe and effective in treating patients with carotid artery stenosis.
Randomized controlled trial of symptomatic middle cerebral artery stenosis: endovascular versus medical therapy in a Chinese population.

Abstract:Background and Purpose-To investigate the efficacy and safety of percutaneous transluminal angioplasty and stenting (PTAS) for symptomatic middle cerebral artery stenosis compared with standard medical treatment in a low-risk Chinese population. Methods-A prospective, randomized, controlled, single-center clinical trial was conducted comparing PTAS with medical treatment for symptomatic middle cerebral artery stenosis (>= 70%). Patients were enrolled according to 1: 1 enroll ratio (PTAS: medical). The PTAS group received stenting or balloon angioplasty, whereas the medical treatment group received standard medical treatment (aspirin 100 mg plus clopidogrel 75 mg/d), and all the patients were under strict control of the risk factors. The end point events were any kind of ipsilateral stroke or transient ischemic attack, or death from any origin during 1-year follow-up. Results-The enrollment was stopped after 70 patients were enrolled from August 2007 to December 2010, with a 30-day rate of end point events of 8.3% versus 5.9% (P=0.69) for PTAS and medical group, respectively, and 1-year rate of end point events of 19.4% versus 17.6% (P=0.85), respectively. There was no significant difference in baseline characteristics between the 2 groups. The mean follow-up time, which was ongoing, was 9.9 +/- 3.9 and 9.7 +/- 4.4 months, respectively. Among the risk factors, hypertension was the independent related to the outcome (P=0.015). Conclusions-This study showed that endovascular treatment is as safe but not better than medical treatment for symptomatic middle cerebral artery stenosis in a low-risk Chinese population. History of hypertension increases the risk of recurrent ischemic events. (Stroke. 2012; 43: 3284-3290.)

Abstract:Introduction Intracranial atherosclerotic stenosis (ICAS) is a common cause of stroke worldwide. However, whether the treatment options for symptomatic ICAS is stent placement or medical therapy alone is still controversial. At present, three multicentre randomised controlled trials (RCTs) have been published, but their research designs are also slightly different and the conclusions are not completely consistent. Therefore, we plan to conduct a systematic review and individual patient data (IPD) meta-analysis of randomised clinical trials to ascertain safety and efficacy of stenting versus medical therapy alone for symptomatic patients with intracranial arterial stenosis. Methods and analyses We will identify RCTs comparing stenting vs medical therapy alone in patients with symptomatic ICAS stenosis (70%–99%) through a systematic search, mainly including PubMed, MEDLINE, EMBASE, the Cochrane Library and ClinicalTrials.gov. Individual-level patient data for a prespecified list of variables will be sought from authors of all eligible studies. The primary outcome was a composite of stroke or death within 30 days, or stroke in territory of qualifying artery beyond 30 days after randomisation. IPD meta-analysis will be conducted with a one-stage approach. Ethics and dissemination Ethical approval and individual patient consent will not be required in most cases since this IPD meta-analysis will use pseudoanonymised data from RCTs. Results will be disseminated through peer-reviewed journals and international conferences. PROSPERO registration number CRD42022369922.

Abstract:Background: The value of percutaneous transluminal angioplasty and stenting (PTAS) in the context of aggressive medical treatment for severe intracranial artery stenosis (ICAS) is under debate. This study compared the effects of PTAS and aggressive medical treatment in patients with severe ICAS and transient ischemic attack or stroke.Material/Methods: A retrospective cohort study was performed. Patients with severe ICAS were assigned to a PTAS group or aggressive medical treatment group, according to the angiographic features of the stenotic lesions. The primary outcome was defined as stroke or death within 30 days or cerebral ischemia occurring ipsilaterally to the qualifying artery beyond 30 days.Results: We included 220 patients: 48 in the PTAS group and 172 in the medical group. The median follow-up was 32 months. PTAS was not associated with an increased incidence of the primary outcomes (10/42 vs. 39/172, p = 0.96) or increased risks of the secondary outcomes of stroke, cardiovascular events, major bleeding, or mortality. The results of log-rank tests did not support a significant difference in event-free survival as a primary outcome between the 2 groups (chi-square = 0.07, p = 0.79). Moreover, although not significantly greater, the mean survival of patients in the PTAS group appeared to be better than that among patients in the medical group, as indicated by the curve for cumulative survival.Conclusions: A suitable PTAS procedure is safe for patients with severe ICAS, and no significant differences in incidence of recurrent stroke or death were found between PTAS and aggressive medication treatment.

Abstract:Objective To evaluate the curative effect,safety,and medium-term effect of intracranial artery stenosis stent (ICASS) for the treatment of the symptomatic intracranial artery stenosis.Methods ICASS was performed in 21 cases with the symptomatic intracranial artery stenosis.The stenting success rate,the improvement rate of stenosis post-stenting and the incidence of complications were observed.The change of the scores of the Modified Rankin Scale (MRS)and National Institutes of Health Stroke Scale (NIHSS)between pre-and post-stenting (1 month,3 months,6 months,12 months and 2 years)and the occurrence of ischemic cerebrovascular events after stenting were compared and analyzed respectively.Results Except one case failed because of intracranial hemorrhage during the operation,other 20 patients were treated with endovascular stent plasty successfully,the total technical achievement ratio was 95.2%.The aver-age stenosis ratio decreased from (73.4±7.9)% to (13.7±9.2)%.The complications incidence was 4.8% within the peroperative. 9 patients were reviewed with DSA in 6 months,the average ratio of stenosis was (29.5 ±12.2)%,no patient was found with the restenosis.Compared with pre-stenting,the score of MRS and NIHSS at the follow-up duration of 3 months,6 months,12 months and 2 years post-stenting of patients were significant improved (all P < 0.05 ).During the follow-up period,2 patients were found with new transient ischemia attack (TIA),1 patient had non-targeted vascular CI without other target vascular therapy-related com-plications.Univariate analysis indicated that patients with elderly (≥ 75 years old),or hypertension,or diabetes,or dyslipidemia,or in the set of events to endovascular treatment for longer than 1 month,or with more than two independent risk factors for patients with underlying diseases treatment,or without systemic therapy,whom would be much more easily to reoccur the cerebrovascular events in post-operation(all P <0.05 ).Conclusion ICASS should be an effective and safe therapeutic method to the symptomatic intracra-nial arterial stenosis.The medium-term efficacy is significant.Serious complications might still be emerge with endovascular stent plasty,so close attention must be paid with.

Abstract:Objective To evaluation the efficacy and safety of endovascular stenting for intracranial artery stenosis.Methods By the checking of DSA,39 patients with intracranial artery stenosis were divided into the anterior circulation group [26 cases,including 11 cases of transient ischemic attack(TIA),15 cases of cerebral infarction(CI)] and posterior circulation group(13 cases,including 3 cases of TIA,10 cases of CI).All the patients were treated with endovascular stents.The rates of the stenting success,improvement rate of stenosis post-stenting and incidence of complications were compared between the two groups.In 6～12 months follow-up,DSA reexamination was performed,and the scores of mRS and BI in patients with CI between pre and post-stenting were compared.Results The rate of stenting success in anterior circulation group was 92.3%,and 100% in posterior circulation group.The improvement rates of stenosis compared pre-stenting in the two groups were all above 70%.The incidence of complications was 19.2% in anterior circulation group and 7.7% in posterior circulation group.There was no statistical difference between the two groups.There were 10 cases in anterior circulation group and 5 cases in posterior circulation group received DSA reexamination,and 50% restenosis was found in the 2 cases(1 case in each group).In the period of the follow-up,the score of mRS and BI at the 6 months and 12 months post-stenting of patients with CI were significant improved than pre-stenting(all P0.05).There was no TIA and CI occurred in the two groups.Conclusion The endovascular stents is a safe and effective method for improving intracranial artery stenosis,and decreasing the incidence of cerebrovascular events.

Abstract:Objective To investigate the effect of stenting on the clinical outcome of acute cerebral artery occlusion(ACAO)in the anterior circulation.Methods The clinical data of 42 patients with ACAO in the anterior circulation who were treated by stenting from January 2016 to June 2017 were retrospectively analyzed. Of 42 patients, 13 patients with orthotopic stenosis were treated with stent implantation after thrombectomy (N1 group), 11 with orthotopic stenosis were treated with thrombectomy (N2 group), and 18 without orthotopic stenosis were treated with thrombectomy (N3 group). Results The use rate of tirofiban of N1 group (69.2%, 9/13) was significantly higher than N2 group(9.1%,1/11;P<0.05),which was significantly higher than N3 group(0%;P<0.05).The operation time of N3 group was significantly shorter than N1 and N2 groups(P<0.05).The percentage of 90-day modified Rankin scale score of 0 to 2 points after operation in N2 group was significantly lower than that in N1 and N3 groups(P<0.05).There was no statistical difference among the 3 groups in 24-hour National Institutes of Health Stroke Scale score after operation,the percentage of grade 2b-3 of cerebral infarction thrombolytic therapy,the incidence of intracranial symptomatic hemorrhage,the incidence of cerebral palsy,and the mortality rate(P>0.05).Conclusions The stent implantation after thrombectomy may have benefit for the patients with patients with ACAO in the anterior circulation with orthotopic stenosis.The use of antiplatelet agents does not increase the associated bleeding complications.

Abstract:Intracranial atherosclerotic stenosis (ICAS), also known as intracranial atherosclerotic disease, is a common etiology of ischemic stroke worldwide, especially in Asia. [ 1 ] Among Chinese populations, ICAS-related strokes account for 30% to 50% of all ischemic strokes, ranking as a leading cause of death and disability and significant associated morbidity and mortality. [ 2–4 ] In addition, ICAS is linked to an elevated risk of Alzheimer disease, [ 5 ] which will undoubtedly have a substantial impact on patients' quality of life and imposing a considerable economic burden on society.

Abstract:OBJECTIVETo evaluate the safety and short-term outcome of endovascular stent-assisted angioplasty for the treatment of intracranial arterial stenosis.METHODSAngioplasty and stent placement were administered to treat 46 patients with lesions of atherosclerotic intracranial stenosis, including 16 lesions of basilar artery, 12 of vertebral artery, 13 of internal carotid artery and 9 of middle cerebral artery.RESULTSTechnical success was achieved in 49 of 50 vessels (98%), with no procedure-related death or cerebral ischemic attack. Extracranial internal carotid artery dissection occurred in 1 patient and was successfully treated with a self-expand stent. One patient had subarachnoid hemorrhage because of perforation by microwire with no permanent neurologic deficit, and another 2 patients had groin hematoma. Angiographic examination immediately after stenting revealed that the stenosis rate was significantly reduced (72.4% +/- 12.3% vs 10.6% +/- 7.8%). There was no cerebral ischemic attack in the 37 patients who were clinically followed up (6 - 18 months, mean of 8.5 months).CONCLUSIONAngioplasty and stenting for the treatment of intracranial stenosis is safe and feasible, and it may be favorable for decreasing the incidence of ischemic attack.
