Abstract:Intracranial atherosclerotic stenosis (ICAS) is one of the most common causes of stroke worldwide, in particular in Chinese, Black and Hispanic populations compared with Caucasians [ 1 Wong L.K. Global burden of intracranial atherosclerosis. Int. J. Stroke. 2006; 1: 158-159 Crossref PubMed Scopus (445) Google Scholar , 2 Gorelick P.B. Wong K.S. Bae H.J. Pandey D.K. Large artery intracranial occlusive disease: a large worldwide burden but a relatively neglected frontier. Stroke. 2008; 39: 2396-2399 Crossref PubMed Scopus (518) Google Scholar ]. In addition to the medical therapy, angioplasty and stenting are alterative surgical approaches. However, recent AHA/ASA guideline on stroke prevention recommended not to adopt stenting with the Wingspan stent system as an initial treatment for patients with stroke or transient ischemic attack attributable to severe (70%–99%) ICAS (class III, level C) [ [3] Kernan W.N. Ovbiagele B. Black H.R. Bravata D.M. Chimowitz M.I. Ezekowitz M.D. et al. Guidelines for the prevention of stroke in patients with stroke and transient ischemic attack: a guideline for healthcare professionals from the American Heart Association/American Stroke Association. Stroke. 2014; 45: 2160-2236 Crossref PubMed Scopus (3181) Google Scholar ]. This recommendation was formulated based on the only randomized clinical trial — Stenting and Aggressive Medical Management for Prevention Recurrent Stroke and Intracranial Stenosis (SAMMPRIS) trial, which demonstrated that stenting had higher risk for 30-day and long-term stroke and death than aggressive medical therapy in patients with severe ICAS [ 4 Chimowitz M.I. Lynn M.J. Derdeyn C.P. Turan T.N. Fiorella D. Lane B.F. et al. Stenting versus aggressive medical therapy for intracranial arterial stenosis. N. Engl. J. Med. 2011; 365: 993-1003 Crossref PubMed Scopus (1397) Google Scholar , 5 Derdeyn C.P. Chimowitz M.I. Lynn M.J. Fiorella D. Turan T.N. Janis L.S. et al. Aggressive medical treatment with or without stenting in high-risk patients with intracranial artery stenosis (SAMMPRIS): the final results of a randomised trial. Lancet. 2014; 383: 333-341 Abstract Full Text Full Text PDF PubMed Scopus (576) Google Scholar ]. However, this trial was performed in Caucasians [ [4] Chimowitz M.I. Lynn M.J. Derdeyn C.P. Turan T.N. Fiorella D. Lane B.F. et al. Stenting versus aggressive medical therapy for intracranial arterial stenosis. N. Engl. J. Med. 2011; 365: 993-1003 Crossref PubMed Scopus (1397) Google Scholar ]. Asians have more ICAS, and more diffuse lesions, whether the SAMMPRIS result validate in Chinese patients, also needs further investigation.
