Comparison of different techniques and pathological samples of spinal tuberculosis for culture and drug sensitivity test of Mycobacterium tuberculosis

Abstract:[Objectives] To explore the detection rate of Mycobacterium tuberculosis using different techniques and pathological specimens of spinal tuberculosis for culture and drug sensitivity test.[Methods] The BacT/ALERT 3D and improved Lowenstein-Jensen (L-J) culture techniques with drug sensitivity tests were respectively used to culture three pathological specimens including pus,granulation tissue and lesion wall harvested in 262 patients with spinal tuberculosis who underwent the anterior,posterior,or combined anteroposterior surgical treatment.The detection rates were compared between the two methods and among the three pathological specimens.[Results] The positive rates of BacT/ALERT3D method was significantly different with that of improved L-J culture (32.82% vs.20.99%,P<0.05).In addition,the positive rate of the pus,granulation tissue and lesions wall detected by BacT/ALERT 3D method proved 34.17%,23.08% and 15.58% respectively,with statistical differences among the three samples (P<0.05),while the positive rate were 26.13%,12.43%,5.84% respectively detected by improved L-J culture,likewise,the difference was statistically significant (P<0.05).The drug sensitivity rates of 78 strains isolated from positive tuberculosis culture to isoniazid,rifampicin,pyrazinamide,streptomycin,ethambutol proved 87.18%,89.74%,98.72%,98.72% and 97.44%,conversely,the resistance rate was 12.82% 10.26%,1.28%,1.28% and 2.56% respectively.[Conclusion] Compared with the improved L-J culture technique,BacT/ALERT3D liquid culture system has obviously advantages of shorten detection time and improved the detection rate,therefore it has more application value for early diagnosis and evaluation of treatment.Pus can be used as the preferred specimen for the cultivation of spinal tuberculosis specimens because it is more available with higher detection rate of culture than other specimens.
