Abstract:AIM: To investigate the expression characteristics of P-glycoprotein (P-gp), topoisomerase II (Topo II) and glutathione S-transferase π (GST-π) in gastric carcinoma as well as their effects on the prognosis of patients. METHODS: The expression of P-gp, Topo II and GST-π in 130 patients with gastric carcinoma were detected by immunohistochemistry and some patients were followed up for more than two years to analyze the relationship between the expression of P-gp, Topo II and GST-π and the clinical prognosis of patients. RESULTS: The positive rate of P-gp, Topo II and GST-π expression in gastric carcinoma were 23.85%, 80.77% and 75.38% respectively. The expression of P-gp and GST-π were associated with tumor size, and the positive rates of P-gp and GST-π in patients with tumor size more than 5 cm in diameter were significantly higher than those in ones with tumor size less than 5 cm in diameter (χ 2 = 4.56, P = 0.033; χ 2 = 5.545, P = 0.020). P-gp and GST-π expression were also correlated with the vessel invasion (invasion vs non-invasion: χ 2 = 17.69, P < 0.001; χ 2 = 5.40, P = 0.020) and 2-year survival rate (++∼+++ vs -∼+: χ 2 = 3.964, P = 0.047; χ 2 = 4.2576, P = 0.039). The positive rate of P-gp expression was markedly higher in patients with lymph node metastasis than that in ones without metastasis (χ 2 = 5.84, P = 0.016). The expression of Topo II was associated with the types and differentiations of histology. The positive rate of Topo II in patients with signet-ring cell carcinoma was obviously lower than that in ones with other kinds of carcinoma (χ 2 = 7.29, P = 0.007), and it was apparently lower in lowly-differentiated carcinoma than that in highly- and moderately-differentiated ones (χ 2 = 7.79, P = 0.005). CONCLUSION: The expression of P-gp and GST-π can be regarded as prognostic factors in gastric carcinoma. Combined detection is necessary for evaluating the degree of drug resistance and prognosis of patients.
