Abstract:ObjectiveTo discuss the application of SUS2 appliance in early Angle classⅡmalocclusion.MethodsA total of 23 cases of orthodontic patients, aged 12 to 15 years old, were selected. All patients were treated with straight wire appliance, and then the upper and lower teeth were well stand. After that, apply to the SUS2 appliance 6-9 months. Lateral cephalogram measurement was carried out before and after treatment and the results were statistically analyzed. Results 23 patients achieved improvement in the proifles, normal overbite, overjet and molar relationship after treatment. The cephalometric analysis showed that there were signiifcant decreased in ANB, UL-E, LL-E, OB, OJ, PP-FH and Y axis after treatment (P<0.05). SNB, Go-Gn, Ar-Go and L1-MP increase signiifcantly after treatment (P<0.05). OP-FH, SNA, SN-MP and U1-SN had no obvious change after treatment (P>0.05).Conclusion SUS2 appliance and MBT can be used efifciently in the orthodontic treatment of Angle II malocclusion at an early stage of permanent dentition. Becase it can effectively use the growing potential of mandibular and stimulate its growing.

Abstract:Objective: To study the feasibility of distalization of maxillary first molar for the correction of class II malocclusion with impaction of second premolar.Methods: 24 subjects with crowding mixed late dentition and impacted second premolar were treated by distalization of maxillary first molar unilaterally or bilaterally with removable appliance and followed by straight wire appliance.Cephalometric analysis was performed with pre-treatment and post-treatment cephalograms.Paired t-test was conducted to assess the treatment effects.Results: The crowns of the maxillary first molars were distalized by an average of 4.5 mm into a Class I molar and canine relationship,Labioclination of the maxillary anterion teeth increased.Conclusion: Malocclusions of class II with impaction of second premolar can be treated by distalization of maxillary first molar with removable distalizing appliance and straight wire appliance in the mixed late dentition.

Abstract:Objective: To compare the effects of different functional appliances in the treatment of skeletal class Ⅱ malocclusion.Methods: A literature survey was performed by applying the database of MEDLINE and CNKI.The Data of clinical randomized controlle trials(RCTs) and controlled clinical trials(CCTs) were selected,Meta analysis was conducted with the software Review 4.2.Results: The search strategy resulted in 111 English articles and 43 Chinese articles.19 articles were qualified for the final analysis.In the qualified articles the treatment effects of 5 kinds of functional appliance were reported.The decrease of SNA was not significantly different between the groups of Activatoer and Binator,SNA decreased more in Twin-block group than in Herbst.The increase of SNB in group Twin-block=in Activator>in Herbst>in Bionator.The decrese of ANB in group of Twin-block>in Activator>in Bionator>in Herbst>in Frnkel-Ⅱ.The decrease of overjet in group of Twin-block>in Herbst>in Frnkel-Ⅱ.The decrease of U1-NA in group of Frnkel-Ⅱ>in Activator>Herbst.The decrease of U1-PP in group Twin-block>in Frnkel-Ⅱ>in Bionator.The increase of IMPA in group Herbst>in Bionator>in Twin-block.IMPA was not significantly different between the groups of Activator and Frnkel-Ⅱ.Conclusion: Frnkel-Ⅱ is not effective in the treatment of sever skeletal Class Ⅱ malocclusions while Twin-block and Activor are.Patients with low IMPA or with poor treatment compliance should be treated with Herbst.Activator is more effective than Bionator in correction of skeletal sagital problem and lingual inclination of upper anterior teeth.

Abstract:Intermaxillary traction is one of the methods often applied to adjust and reconstruct occlusal relation, utilizing the theory that intermaxillary reciprocal anchorage can establish favorable and stable occlusion by persistent elasticity.Since there is close relationship between temporomandibular joint (TMJ) and occlusion,any changes of the temporomandibular joint structure may result in malocclusion,intermaxillary traction could be used to correct this malocclusion.This article reviews the theory of intermaxillary traction and its application in temporomandibular joint surgery.Supported by Shanghai Leading Academic Discipline Project (Grant No.Y0203).

Abstract:Twelve adult patients with overerupted molars were treated with insertion of microscrew into maxilla on palatal and buccal sides,respectively.The microscrews were connected with the overerupted molars,giving the force toward the roots to intrude the teeth.The overerupted molars of twelve patients were all successfully intruded with an average length of 2.5 mm,and the average duration of treatment was 5.5 months.The technique of microscrew to intrude overerupted molars can provide significant effect,short duration of treatment and satisfactory compliance.

Abstract:Objective:To design a new type of appliance to upright the impacted mandibular second molars and observe the clinical effects.Methods:The looped molar retractor (LMR) had three components:bands,lingual arch,and loop with silver solding spurs and ligature regions.LMR was fabricated and used in the treatment of 5 cases with bilateral impacted mandibular second molar and 2 cases with unilateral impacted mandibular second molars.The clinical effects were observed and evaluated.Results:All the impacted second molars were up-righted successfully in short time.The average period of treatment was 4 months.The loss of anchorage was not observed.Condusion:LMR is an ideal tool to up-right the impacted inclined second molar.

Abstract:OBJECTIVE: Early interfering orthodontics were used to treat maxillary hyperplasia by face mask and extra-oral bow technique in teenages.The position changes of maxilla after orthodontic treatment were evaluated.METHODS:The cephlometric variables before and after treatment were measured.Variables after treatment were compared with normal values in children with same ages,and changes of the maxillary position was determind.RESULTS:The position of the maxilla after orthopedics was significantly advanced (P<0.001),but there was still difference from normal values.CONCLUSION:In the peroid of mixed dentition of patients with cleft lip and palate,early orthodontics on maxillary hyperplasia could stimulate maxillary development and growth,remove or improve crossbite,extra-oral bow and face mask were efficient tools for this kind of patients.

Abstract:Premature loss of first permanent molars is a common occurrence in clinical practice, leading to abnormal occlusion and inefficient mastication for patients. This case report presented the orthodontic retreatment of a 32-year-old female patient who suffered from premature loss of mandibular bilateral first molars. The mesial tipping of the mandibular second molars led to an occlusal interference, resulting in a clockwise rotation of the lower jaw and an anterior open bite (AOB). We achieved long-distance molar mesialization using clear aligners with Albert cantilever arms. After 42 months of treatment, the patient's occlusion and facial profile significantly improved. The cantilever combined with the cleat aligner treatment has been clinically effective for the protraction of mandibular posterior teeth, extending the field of application of clear aligners.

Abstract:Background Extracting wholly impacted maxillary 3rd molars faces difficulty due to the narrow surgical field, adjacent teeth resistances and risk of oroantral communication. This study is designed to introduce and evaluate the applicability of a novel method-buccal rotation to extract maxillary 3rd molars. Materials and methods In this cohort study, from October 1st 2020 to September 30th 2021, 72 wholly impacted maxillary 3rd molars were included. Based on the crowns with coronal 1/3, middle 1/3, apical 1/3 of the adjacent teeth roots, teeth were classified into position I, II, III. Based on the angles < 30°, ≥ 30°but < 60°, ≥ 60° to the adjacent teeth, teeth were classified into angulation A, B, C. Traditional method and novel method-buccal rotation were applied based on the surgical simulations. Surgical results were recorded. To analyze the data, Chi-square test was applied. Results 82.00% of teeth in position I and 50.00% in position II were designed to use traditional method, 83.33% in position III were using the novel method ( p < 0.05). 81.25% of teeth in angulation A and 52.63% in angulation B were designed to use traditional method, 80.00% in angulation C were using the novel method ( p < 0.05). Four cases got temporary complications. Conclusion Buccal rotation was applicable to extract the deep impacted maxillary third molars with large angles towards the adjacent teeth.

Abstract:Asymmetries are among the most challenging problems in orthodontics. Proper diagnosis is critical to discern first whether the asymmetry is dental or skeletal. If it is dental, one must then determine whether one dental arch or both are at fault. Once diagnosed, the next challenge is determining not only an appropriate treatment plan, but also the appropriate mechanics plan. This aim of this article is to present a patient with a severe asymmetry to emphasize the importance of a problem-based differential diagnosis to develop both a sound treatment plan and a mechanics plan that successfully integrates miniscrews from the start of the process. An 18-year-old woman had a Class III subdivision left malocclusion, an asymmetric lower facial third, and a deviated midline. The treatment plan consisted of asymmetric distalization of the maxillary right and mandibular left posterior dentitions to create space to resolve the deviated midlines, correct the canted occlusal plane, and obtain an ideal occlusion. Active treatment with Clarity ceramic 0.022 x 0.028-in appliances (3M Unitek, Monrovia, Calif), temporary anchorage devices, and a pendulum appliance lasted 22 months. The final result and the 2-year retention records demonstrate that a harmonious facial balance, an attractive smile, ideal occlusal relationships, and a stable outcome were achieved. This case report shows that with proper planning, asymmetric use of temporary anchorage devices in multiple posterior quadrants can be used to obtain molar distalization, and this approach is an effective alternative to dental extraction therapy.

Abstract:More and more attention has been paid to the early treatment of malocclusion, especially the early treatment of class Ⅲ malocclusion. Class Ⅲ malocclusion not only affects maxillofacial growth and development, occlusal function and facial beauty, but also leads to serious physical and mental problems. Focusing on class Ⅲ malocclusion, the definition, classification, etiological mechanism, craniofacial growth and development rule, and the influence of early treatment on craniofacial growth and development were reviewed. In class Ⅲ patients, the craniofacial growth and development should be evaluated and analyzed when the early intervention carried out. The purpose of treatment of class Ⅲ malocclusion is achieved by using the appliance to affect the craniomaxillofacial growth.

Abstract:Early orthodontic treatment is an important means of preventing and treating dentofacial deformities during the period of growth and development. In this stage, children have great potential in growth and development, high adaptability of muscles and temporomandibular joint, and good responsiveness to orthodontic force. Therefore, orthodontic intervention and treatment in this stage can prevent and guide the normal growth and development of dentition, occlusion and maxillofacial complex. This article summarizes the commonly used orthodontic techniques and appliances in the mixed dentition, including interceptive treatment of oral habits, application of functional appliances, fixed appliances, clear aligners, as well as management of severe crowding and space maintenance. This article comprehensively explains the application and indications of different orthodontic techniques in design and appliance selection in the treatment of malocclusions in the mixed dentition.
Combined orthopedic-orthodontic treatments of adolescent skeletal open-bite with severe molar-incisor hypomineralization: a case report and literature review

Abstract:The treatment of adolescent skeletal open-bite malocclusion with severe molar-incisor hypomineralization (MIH) remains challenging. Though conducive to open-bite treatment and endodontic management, early molar extraction may trigger a series of negative impacts on occlusion and stomatognathic development. In addition, molars' crown restoration was shown to worsen open-bite malocclusion considering the intrinsic vertical increment of hyperdivergent growth. This case report describes the successful multidisciplinary therapy combined with orthopedic and orthodontic treatment of a 10.2-year-old girl with mixed dentition, a protruding profile and skeletal open-bite malocclusion with severe MIH and crowding. During the mixed and early permanent dentition, function regulator-4 (FR-4), resin-bonding transpalatal arch (TPA) and modified spring-loaded bite blocks were implemented to correct abnormal swallowing and control the facial vertical growth. Radiographic results, including the counterclockwise rotation of the occlusion plane, decreasing mandibular angle and increasing posterior-anterior face height ratio accompanied by obvious mandibular vertical growth, indicated that the performed orthopedic treatments efficiently controlled hyperdivergent open-bite growth during puberty. After the maxillary and mandibular second molars were occluded, all first permanent molars were extracted, and fixed appliances combined with implant anchorage were used to correct malocclusion and convex profile. Ultimately, a stable Class I functional occlusion and satisfying facial improvement were achieved and maintained following a 2-year follow-up.
Clinical finite element analysis of mandibular displacement model treated with Twin-block appliance

Abstract:Introduction: The mechanical distribution of the mandible is an important factor that affects functional orthosis during Twin-block (TB) appliance correction. Changes in the mandible before and after TB appliance correction are also key factors in maintaining the therapeutic effect. Finite element analysis, a powerful numerical, analytical tool, is widely used to predict the stress and strain distribution of the craniofacial bone that orthodontics generates. Methods: The sample was a 14-year-old male patient with Class II malocclusion during growth. A cone-beam computed tomography scan was undertaken at pretreatment and posttreatment. In the Finite element analysis of the pretreatment model, the remote displacement model of the mandible was established with the sella point as the center. A mandibular model under TB appliance loading was established. Its mandibular displacement and von Mises stress were compared before and after loading. Three-dimensional registration was conducted on the pretreatment and posttreatment models to measure the sagittal displacement of the centrosome. Results: The force on the mandible occurred mainly in the condyle neck and medial mandible after the TB appliance moved the mandible. After displacement, the posterior upper margin of the condyle was farther away from the articular fossa. Three-dimensional registration results showed that new bone had formed behind and above the condyle after TB appliance treatment. Conclusion: The TB appliance provides additional advantages in treating skeletal Class II malocclusions by helping to reduce the burden on the temporomandibular joint and promoting the adaptive reconstruction of the mandible.
Treating High Angle Bimaxillary Protrusion with Three Kinds of Extraction Method: A Clinical Study

Abstract:OBJECTIVE:To compare the effect of three kinds of extraction model on high angle bimaxillary protrusion patients.METHODS:A total of 30 patients with Class I malocclusion and bimaxillary protrusion, aged 14-25 years old, were selected and divided into three groups. Four first premolars were extracted in the first group. The two maxillary first premolars and two mandibular first molars were extracted in the second group. The two maxillary first premolars and two mandibular first molars were extracted in the third group, and two additional micro-implants used as orthodontic anchorage in maxilla. Three groups were all treated with MBT appliance. Cephalometric analysis were carried out before and after treatment, and the results were analyzed with statistics.RESULTS:1) About the hard tissues, compared with the first group, there were statistically significant differences of N-Me, SGo/NMe, ANS-Me, FH/MP, SN/MP, and ODI in the second and the third group after treatment (P<0.01). 2) About the soft tissues, the teeth and the alveolar bone, compared with the first group, there were statistically significant differences of Pg-Pos, Li-SnPos, Si-LiPos, LL-E, L1-NB, L1/NB, U1/L1, L7-MP in the second and the third group after treatment (P<0.01). 3) All patients received consummate orthodontic treatment and obtained fine occlusion. Facial profiles were improved significantly after orthodontic treatment.CONCLUSION:1) After orthodontic treatment with mandibular first molars extraction, FH/MP, SN/MP, N-Me, ANS-Me, L1-NB and L1/NB decreased respectively, and soft tissue profiles were improved significantly. 2) Additional micro-implant used as orthodontic anchorage in maxilla significantly contributed to the maxillary incisor retraction and subsequent soft tissue change. 3) The first molars extraction and additional micro-implant used as orthodontic anchorage are efficient in improving the facial profiles for high angle bimaxillary protrusion patients.

Abstract:BACKGROUND:Temporomandibular joint (TMJ) disorders are closely related to high-angle and skeletal Class II malocclusion. Sometimes pathological changes in the mandibular condyle can cause open bite to occur after growth is complete.CASE SUMMARY:This article is about the treatment of an adult male patient with a severe hyperdivergent skeletal Class II base, an unusual and gradually occurring open bite and an abnormal mandibular condyle anterior displacement. Because the patient refused surgery, four second molars with cavities and root canal therapy were extracted, and four mini-screws were used for intrusion of the posterior teeth. The treatment duration was 22 mo, and after the treatment, the open bite was corrected and the displaced mandibular condyles were seated back to the articular fossa as shown by cone-beam computed tomography (CBCT). Based on the patient's open bite history, the result of clinical examinations and CBCT comparisons, we believe it is possible that the occlusion interference was eliminated after the four second molars were extracted and the posterior teeth were intruded, and the patient's condyle spontaneously returned to its physiologic position. Finally, a normal overbite was established, and stable occlusion was achieved.CONCLUSION:This case report suggested that identifying the cause of open bite is essential, and the TMJ factors for hyperdivergent skeletal Class II cases should be particularly examined. For these cases, intruding posterior teeth may place the condyle in a more appropriate position and provide an environment suitable for TMJ recovery.

Abstract:Objective:To treat maxillary fractures with orthodontic appliances at early stage,stabilize the fragments,and to regain normal occlusion.Methods:21 patients with maxillary fractures were selected.Orthodontic brackets and wires were used intra-orally,and protraction face masks and complex headgears were used extra-orally within one week after the trauma.Inter-maxillary elastics and extra-oral tractions were performed.Results:Normal occlusion and better fractures reduction were obtained in 20 patients by this means.Conclusion:Treatment of maxillary fractures,with slight displacement of segments and loss of a few or no teeth,can be performed with orthodontic appliances.
