Abstract:Background: An early approach to Class III malocclusions is crucial. From diagnosis to treatment, the aim is be to correct and harmonise maxillary and mandibular growth as quickly as possible. Case report: A new treatment protocol for the early treatment of Class III malocclusions to be used in children in deciduous teething and/or early mixed teething was proposed. Ten children, 6 females and 4 males, with an overall average age of 6.5 years (6 years and 6 months) at time T0 were included in this experimental study. Twelve months orthopaedic interceptive therapy using two protraction facemasks (Delaire and Petit mask) and two types of extraoral elastic bands (8 and 16 oz) were given. Parents/guardians were involved as much as possible to increase compliance with the treatment. Conclusion: The orthopaedic-interceptive treatment for Class III malocclusion intends to provide a potential improvement of the growth response so that significant results can be achieved quickly. The motivational role of the parent can positively influence the child's treatment cooperation. The clinician should then try to make parents understand the importance of early treatment of Class III malocclusion thus obtaining a full cooperation with the parent/ guardian and a good compliance from the child.

Abstract:An 8-year-old Chinese girl sought treatment for a severe skeletal Class III malocclusion and open-bite skeletal pattern. Traditionally, patients with a skeletal Class III malocclusion are treated after they have stopped growing, and then they are treated with a combined orthodontic and orthognathic surgery approach. But the risks and expenses of this treatment plan are not acceptable to all patients. This young patient was treated with facemask therapy, a maxillary expansion device, and a molar occlusal splint for maxillary developmental stimulation with control of vertical jaw growth. After the completion of orthopedic therapy, 2 × 4 technology was used to adjust molar positions. A bonded tongue crib was used in the early permanent dentition to help the patient break her bad tongue habits. Straight-wire appliances were used for 16 months to adjust the occlusal relationship. This achieved significant improvement in anterior tooth relationships and facial profile esthetics. At the 2-year posttreatment follow-up, the results were satisfactory. The success of the sagittal relationship correction between the maxilla and the mandible for a skeletal Class III malocclusion depends on the coordination of transverse and vertical relationships combined with the growth potential of each patient.

Abstract:Abstract A skeletal Class II malocclusion is an anteroposterior discrepancy resulting from a combination of skeletal and dental factors. In adolescent patients, it is possible to leverage their growth potential to address bone deformities and minimise the need for future surgical interventions. In the present case report, a combination treatment approach was adopted which included a functional appliance, fixed appliances, and temporary anchorage support, to treat a 13-year-old Chinese male with a severe skeletal Class II malocclusion. After a treatment duration of 36 months, the patient achieved a favourable occlusion and an improved facial profile. Subsequently, a stable occlusal relationship and balanced facial proportions were maintained during a 2-year retention period.

Face Mask Therapy and Comprehensive Orthodontic Treatment for Skeletal Class III Malocclusion: A Case Report

Abstract:Aim: This article aims to report a case of face mask therapy and comprehensive orthodontic treatment for skeletal class III malocclusion in a 16-year-old girl. Background: Treating skeletal class III malocclusion in a growing patient is crucial, as it can help avoid the need for additional surgery. Early treatment also lessens the negative impacts of the patient's facial abnormality on their social life because surgery is only done later. Case description: In this case report, a 14-year-old female patient presented with skeletal class III malocclusion with primary complaints of anterior crossbite. There was no relevant medical history. Face mask therapy and fixed appliance therapy were components of the treatment approach that successfully corrected the malocclusion. The total period of treatment was 20 months. Conclusion: The treatment resulted in a harmonious face, a well-aligned smile arch, stable dental and skeletal relationships, and significant esthetic improvements, including improved facial symmetry and profile. Significance: A growing teen who has a skeletal class III malocclusion and a maxillary deficit may be helped by a combination of face mask therapy and thorough orthodontic treatment. This case report outlines the use of the aforementioned technique to successfully treat a 14-year-old child with class III malocclusion and maxillary deficiencies.Early management of skeletal class III malocclusion in developing adolescents is vital as it can potentially eliminate the necessity for future surgical intervention, leading to improved treatment outcomes.Careful case selection, patient cooperation, and long-term stability enable a successful, stable, and esthetically pleasing treatment outcome. Abstract:Early orthodontic treatment is an important means of preventing and treating dentofacial deformities during the period of growth and development. In this stage, children have great potential in growth and development, high adaptability of muscles and temporomandibular joint, and good responsiveness to orthodontic force. Therefore, orthodontic intervention and treatment in this stage can prevent and guide the normal growth and development of dentition, occlusion and maxillofacial complex. This article summarizes the commonly used orthodontic techniques and appliances in the mixed dentition, including interceptive treatment of oral habits, application of functional appliances, fixed appliances, clear aligners, as well as management of severe crowding and space maintenance. This article comprehensively explains the application and indications of different orthodontic techniques in design and appliance selection in the treatment of malocclusions in the mixed dentition.

Abstract:Skeletal Class III malocclusion is one type of dentofacial deformity that significantly affects patients' facial aesthetics and oral health. The orthodontic treatment of skeletal Class III malocclusion presents challenges due to uncertainties surrounding mandibular growth patterns and treatment outcomes. In recent years, disease-specific radiographic features have garnered interest from researchers in various fields including orthodontics, for their exceptional performance in enhancing diagnostic precision and treatment effect predictability. The aim of this narrative review is to provide an overview of the valuable radiographic features in the diagnosis and management of skeletal Class III malocclusion. Based on the existing literature, a series of analyses on lateral cephalograms have been concluded to identify the significant variables related to facial type classification, growth prediction, and decision-making for tooth extractions and orthognathic surgery in patients with skeletal Class III malocclusion. Furthermore, we summarize the parameters regarding the inter-maxillary relationship, as well as different anatomical structures including the maxilla, mandible, craniofacial base, and soft tissues from conventional and machine learning statistical models. Several distinct radiographic features for Class III malocclusion have also been preliminarily observed using cone beam computed tomography (CBCT) and magnetic resonance imaging (MRI).

Abstract:Abstract Background The treatment of a skeletal Class III malocclusion with accompanying mandibular asymmetry is an orthodontic challenge. A skeletal Class III may be associated with a retrognathic maxilla, a prognathic mandible and a mandibular asymmetry can be of dental or skeletal origin or in various combinations. Timely treatment with appropriate biomechanics is crucial for an acceptable and stable result. Aims The aim of this article was to introduce a clinically effective asymmetric magnetic reverse Twin-block appliance (AMRTB) for growth modification treatment of an 11-year-old girl who presented with a skeletal Class III malocclusion and accompanying mandibular asymmetry. Methods The treatment was comprised of two phases, the first of which was growth modification using the AMRTB for the stimulation of maxillary growth, restraint of mandibular growth, and improvement of the mandibular asymmetry. This was followed by a second phase involving full fixed appliances to optimise the interdigitation and occlusion. Results The skeletal Class III relationship and mandibular asymmetry was successfully corrected after two years of treatment. A good facial appearance and pleasing smile aesthetics were achieved. The treatment result was stable after six years of follow-up. Conclusion This case demonstrated that the AMRTB was a clinically effective appliance for growth modification treatment in patients with a skeletal Class III malocclusion and mandibular asymmetry.
Component Analysis of Craniofacial Type of Xi'an Chinese with Class III Malocclusion in the Early Permanent Dentition

Abstract:Objective:To analyze craniofacial type composition of Class Ⅲ malocclusion in early permanent dentition in the Xi'an region. Methods:We performed a retrospective study on craniofacial type of the Class Ⅲ malocclusion patients admitted to the Department of Orthodontics,Stomatology Hospital,the Fourth Military Medical University,during 2003-2008. The computer-aided cephalometric X technology had been used and the features of class III malocclusion patients in early permanent dentition were studied. Results:In early permanent dentition,craniofacial types in sagittal directions were mainly manifested as maxillary and mandibular normal or maxillary normal with mandibular hyperplasia. Craniofacial types in vertical direction were mainly manifested as average angle,and seldom as low-angle and high-angle. The cross-analysis of sagittal and vertical craniofacial morphology was performed. Both maxillary and mandibular normal and maxillary normal with mandibular hyperplasia in average angle patients were predominant,and followed by maxillary hypoplasia with mandibular normal combined with average angle. Conclusion:It was demonstrated that class III malocclusion patient in early permanent dentition has either sagittal or vertical features. The component analysis can play a very important role in understanding the detailed features of class III malocclusion. And more important,it is helpful for the future study on the early treatment of class III malocclusion.

Abstract:The developing class III malocclusion is one of the most challenging problems for the practicing orthodontist to manage clinically. True class III malocclusion is rare in our region as compared to Class II and Class I malocclusion. With adults, orthognathic surgery and dental camouflage is the viable treatment option. A variety of treatment alternatives exists for patients in the developing stages of a Class III malocclusion. In the past much of the therapy has focused on restriction of mandibular growth with chin cups and functional appliances. This is based on the traditional thought that developing Class III malocclusions were the result of prognathic mandible. Recently, however, there has been a growing awareness that the majority of patients with a developing Class III skeletal pattern exhibit a maxillary deficiency with a normal or only slightly prognathic mandible. Therefore, considerable attention has been given to early treatment using maxillary protraction therapy. Using facemask therapy in conjunction with maxillary expansion has been shown in clinical reports to be a successful and predictable treatment option. Treatment should be carried out as early as possible with the aim to prevent it from becoming severe. A case treated with biphasic therapy – orthopaedic appliance followed by fixed orthodontic treatment is presented here. Abstract:Angle, Tweed, and Moyers classified Class III malocclusions into 3 types: pseudo, dentoalveolar, and skeletal. Clinicians have been trying to identify the best timing to intercept a Class III malocclusion that develops as early as the deciduous dentition. With microimplants as skeletal anchorage, orthopedic growth modification became more effective, and it also increased the scope of camouflage orthodontic treatment for patients who were not eligible for orthognathic surgery. However, orthodontic treatment combined with orthognathic surgery remains the only option for patients with a severe skeletal Class III malocclusion or a craniofacial anomaly. Distraction osteogenesis can now be performed intraorally at an earlier age. The surgery-first approach can minimize the length of time that the malocclusion needs to worsen before orthognathic surgery. Finally, the use of computed tomography scans for 3-dimensional diagnosis and treatment planning together with advances in imaging technology can improve the accuracy of surgical movements and the esthetic outcomes for these patients.

Abstract:Objective To analyze the sagittal craniofacial morphology composition in Class Ⅲ patients in Xi'an area.Methods The craniofacial type of the Class Ⅲ malocclusion in 1177 patients were analyzed.The features of Class Ⅲ malocclusion patients in mixed dentition,early permanent dentition and permanent dentition were studied by computer-aided cephalometric X-ray.Then the component analysis of sagittal craniofacial morphology was performed.Results In the mixed dentition,the craniofacial types in sagittal directions mainly manifested as normal maxilla with mandiblar hyperplasia followed by normal mandible with maxillary hypoplasia.Craniofacial types in sagittal directions were normal maxilla and mandible or normal maxilla with mandibular hyperplasia in early permanent dentition.In permanent dentition,craniofacial types in sagittal directions mainly showed normal maxilla with mandibular hyperplasia followed by maxillary hypoplasia with mandibular hyperplasia.Conclusions Class Ⅲ patient has different sagittal features in different ages,but mandibular hyperplasia is still the main manifestation.

Abstract:Vasopressin injection (pitressin®) has become widely accepted as a useful aid in roentgen visualization of the gallbladder. Through its ability to increase peristalsis, vasopressin injection results in rapid expulsion of gas from the intestine, thus removing obscuring gas shadows. The drug may cause only transient and minor toxic reactions. Collins and Root 1 recognized three contraindications: coronary heart disease, hypertension and hypotension (systolic blood pressure under 100). Kirklin 2 in reporting on the use of vasopressin injection in 100 cases, noted no serious ill effects although numerous minor reactions were observed, the most frequent being pallor. Recently, Mills and co-workers 3 directed attention to the possibility of serious myocardial effects of the drug. We observed two cardiac accidents following vasopressin injection during cholecystography. On the basis of generally accepted principles governing its use, no contraindication to the use of the drug was evident in these cases. REPORT OF CASES Case

Abstract:OBJECTIVE: Early interfering orthodontics were used to treat maxillary hyperplasia by face mask and extra-oral bow technique in teenages.The position changes of maxilla after orthodontic treatment were evaluated.METHODS:The cephlometric variables before and after treatment were measured.Variables after treatment were compared with normal values in children with same ages,and changes of the maxillary position was determind.RESULTS:The position of the maxilla after orthopedics was significantly advanced (P<0.001),but there was still difference from normal values.CONCLUSION:In the peroid of mixed dentition of patients with cleft lip and palate,early orthodontics on maxillary hyperplasia could stimulate maxillary development and growth,remove or improve crossbite,extra-oral bow and face mask were efficient tools for this kind of patients.

Abstract:A patient with skeletal Class III malocclusion was treated in 2 phases during the early permanent dentition. In phase 1, maxillary protraction was combined with rapid palatal expansion; in phase 2, fixed appliances were placed. The results were good posttreatment, and, 1 year later, a favorable growth tendency could be observed. This report shows that treatment for a patient with skeletal Class III malocclusion can be started in the early permanent dentition, with very good final results.

Abstract:Objective The purpose of this study was to evaluate the effect of alternate maxillary expansions and constrictions (Alt-MEC) with lighter maxillary protractionforce in treatment of Class Ⅲ malocclusions.Methods Twenty children with Class Ⅲ malocclusion were selected and randomly divided into two groups.In experimented group (n =10),an alternate rapid maxillary expansion and constriction protocol (Alt-MEC) and then maxillary protraction (MP)commenced with 300g force each side.In control group (n=10),rapid maxillary expansion was conducted for 1 week,and then maxillary protraction (MP) is started with about 500 g at each side.Pretreatment (T1) and posttreatment (T2) cephalometric radiographs were taken and then analyzed.Results The sagittal skeletal variables all showed significant improvements (P＜0.05) in the experimental group (SNA,2.62°;ANB,2.97°;WITS appraisal,2.48mm;point A protracted 2.47mm) and the control group (SNA,1.80°;ANB,2.06°;WITS appraisal,2.39mm,point A protracted 2.44mm),but the changes between two groups showed insignificant differences (P＞0.05).Conclusions Alternate rapid maxillary expansions and constrictions (Alt-MEC) with lighter force of maxillary protraction is effective in maxilla advancement in the treatment of the children with Class Ⅲ malocclusions.

Abstract:Objective: To identify the morphological characteristics of craniofacial structure in patients with skeletal class Ⅲ malocclusions.Methods: The study samples consisted of 63 lateral cephalometric radiographs of adult with classⅠ malocclusion(32 male and 31 female,ages>17 years) and 27 patients with skeletal class Ⅲ malocclusion(14 male and 13 female,ages>17 years,ANB>-3).The proportion of posterior cranial base and the mandibular length were measured.Results: There was an increase in total mandibular length(Go-Gn,Cd-Gn) in female patients with class Ⅲ malocclusion.There was a decrease in the posterior cranial base length(S-Ar) and an increase in mandibular length (Go-Gn) in male patients with class Ⅲ malocclusion.Conclusion: The major craniofacial characteristics of skeletal class Ⅲ malocclusion compared with those of class Ⅰmalocclusion are smaller posterior cranial base in males and larger mandibular length in both sexes.

Abstract:Objective To describe the surgical procedure in correcting Class Ⅲ patients with long face syndrome and to evaluate the lateral cephalometric changes resulting from surgicalorthodontic treatment.Methods 36 Chinese Class Ⅲ patients with long face syndrome were selected.All patients underwent bimaxillary surgery.The surgical approaches involved Lefort I osteotomy (advancement and superior repositioningof the maxilla) and bilateral sagittal split ramus osteotomy (counterclockwise rotation and setback of the mandible).Lateral cephalometric records were taken at pre-treatment (T1) and post-treatment (T2).The data were analyzed statistically by using paired t tests.Results Anteroposterior and vertical dysplasia were corrected by surgery.Slanted occlusal plane was flattened and the gonial angle was closed.The S-Go/N-Me increased 2％ (P＜0.01),whereas ANS-Me/N-Me displayed no significant difference.Conclusions The surgical procedure makes it possible to obtain an optimal esthetic and functional result in cases of Class Ⅲ long face patients.This approach is a useful option to treat these deformities.

Abstract:Class II malocclusion is a recurrent problem that may occur at a young age. If treated initially, the malocclusion can be corrected by redirecting the growth without invasive modalities and avoiding orthognathic surgeries. A female patient aged 10 years three months came to the department of orthodontics having a complaint of upper front teeth placed forwardly, diagnosed with skeletal class II due to retrusive mandible and vertical maxillary excess with hyper divergent growth pattern with increased anterior facial height, with Angle's molar class II division 1 malocclusion, increased overjet of 13 mm and overbite of 7 mm, acute nasolabial angle, deep mentolabial sulcus, and hyperactive mentalis. It was treated using an activator with medium-high-pull headgear (modified Herren activator) passing through the maxillary center of resistance. A fixed mechanotherapy with high-pull headgear was given using the anterior inclined plane acrylic plate in the maxilla and McLaughlin, Bennett, and Trevisi (MBT). Begg's wrap was used for the retention plan. This case report shows the significance of proper treatment results obtained due to correct identification and planning in treating malocclusion.
