Abstract:To evaluate the therapeutic effect of using micro-implant anchorage (MIA) to rotate the functional occlusal plane (FOP) counterclockwise. Forty skeletal class Ⅱ high-angle patients who had completed orthodontic treatment were enrolled, including 20 patients treated with MIA orthodontic system (MIA group) and the other 20 patients treated with traditional sliding straight wire appliance (control group). Cephalometric measurements on the lateral cranial radiographs before and after treatment were performed, all acquired data were statistically analyzed with SPSS 26.0. At the end of treatment, MIA group obtained better effect of FOP and mandibular plane counter-clockwise rotation than the control group. In the MIA group, the average change of FOP-frankfort horizontal plane (FH), FOP-SN and mandibular plane angle (MP-FH) angle was -4.5(-7.3, -3.7)°, (3.3)° and -1.7(-3.0, -0.9)°, respectively. In the control group, the average change of FOP-FH, FOP-SN and MP-FH angle was -0.1(-4.1, 3.0)°, (-0.1±5.1)° and -0.4(-2.4, 0.7)°, respectively. There was significant difference between the change of the two groups (all <0.05). Compared with the traditional sliding straight wire appliance, counterclockwise rotation of FOP can be more effectively reversed by using MIA orthodontic system, and the MP-FH can be reduced as well.

Abstract:Objective To evaluate the effectiveness and efficiency of mesially inclined second molar correction with uprighting spring.Methods Nine patients with 12 impacted second molars were treated with uprighting spring and fixed appliance.The inclination of second molar on panoramic film before and after treatment was measured and the improvement of the tooth inclination was compared with paired t-test.Results Good results were obtained in all patients, the impacted teeth were corrected.The average inclination of second molar was increased by (37.0 ±15.2) degrees (P＜0.05).The treatment time was between 9 to 36 months.Conclusions The cantilever uprighting spring is an easy and reliable way to correct the mesially inclined second molar.

Abstract:Objective To study the changes of condylar morphology and position in the mandibule-retrusion patients during fixed inclined bite-plate treatment.Methods The subjects were 13 consecutive Class II osseous malocclusions with mandibular retrusion treated with fixed inclined-bite.From each subject,four MRI images of TMJ were obtained and evaluated:before treatment,at the beginning of treatment,during treatment and after treatment.Results MRI-signs of condylar remodeling were observed at the posterior-superior border in all 13 subjects treated with fixed inclined-bite.Condylar-fossa relationship changes were unconspicuous before and after treatment.Conclusion Condylar remodeling seems to contribute significantly to the increase in mandibular growth resulting from fixed inclined bite-plate treatment,while condylar-fossa relationship has no remarkable changes.

Abstract:Objective:To design a new type of appliance to upright the impacted mandibular second molars and observe the clinical effects.Methods:The looped molar retractor (LMR) had three components:bands,lingual arch,and loop with silver solding spurs and ligature regions.LMR was fabricated and used in the treatment of 5 cases with bilateral impacted mandibular second molar and 2 cases with unilateral impacted mandibular second molars.The clinical effects were observed and evaluated.Results:All the impacted second molars were up-righted successfully in short time.The average period of treatment was 4 months.The loss of anchorage was not observed.Condusion:LMR is an ideal tool to up-right the impacted inclined second molar.

Abstract:Background: Clear aligners (CAs) can distalize mandibular molars. The present study aimed to study the efficiency and influencing the factors of CAs for mandibular molar distalization and to provide guidance for clinical design. Materials and Methods: The present study evaluated 32 adult orthodontic patients who needed mandibular molar distalization and were treated with CAs between September 2018 and September 2020 at the school of medicine. The cone-beam computed tomography images of T0 (before orthodontic treatment) and T1 (after mandibular molars were fully distalized) were collected, and the actual movements of mandibular molars were measured and compared with the expected movements. The efficiency of mandibular molar distalization during treatment was comprehensively evaluated, and the relevant factors were analyzed. Results: For mandibular first molar distalization, the average efficiency of the crown was 67.19% 9swn. 13%, and that of the root was 37.87% 7stn. 72%. There was a significant difference between the achieved movement amount and the expected amount (P < 0.05). For mandibular second molars, the average efficiency of the crown was 58.47% 7swn. 07%, and that of the root was 57.03% 3stn. 48%. There was a statistically significant difference between the achieved movement amount and the expected amount (P < 0.05). There was a significant negative correlation between the expected movement and the efficiency of the crown. Conclusions: CAs can achieve mandibular molar distalization, and the movement pattern of molars is mainly a tipping movement. To accomplish bodily movement, overcorrection should be fully considered. In addition, it is suggested that close attention should be given to observing root movement during molar distalization.

Abstract:Objective:To evaluate the effect of segmental approach in the treatment of Angle class Ⅱ division 1 malocclusion.Methods:7 cases of classⅡ,division 1 maloclusion were treated.Upper first premolar and lower second premolars were extracted in all cases.Lower first molars were mesially moved by means of segmental arch. Cephalometric analysis was used to evaluate the effects of the treatment.Results:Facial profile improvement and lower anterior facial height maintenance were achieved.The occlusion plane angle,mandibular plane angle were well controlled with lower anterior teeth upright on the lower basal bone.All patients represented good mandible response.Conclusion:Segmental technique is a simple and effective approach in correcting class Ⅱ,division 1 maloclusion

Abstract:Abstract Surgery combined with orthodontic uprighting of impacted mandibular second molars requires sequential treatment procedures. This article presents a rare case of horizontally impacted bilateral mandibular second molars accompanied by dental crowding. The patient’s mandibular second molars were fully embedded in bone with overlying mesially-impacted mandibular third molars and the mandibular first molars were inclined distally. The treatment involved staged bilateral extraction of the second premolars and mandibular third molars, followed by the surgical implantation of micro-implants for anchorage in the mandibular ramus. At a subsequent stage, the mandibular second molars were successfully orthodontically uprighted, leading to a favourable treatment outcome.

Abstract:Objective The purpose of this study is to evaluate the effectiveness of the segmented arch technique with direct miniscrew anchorage for uprighting mesially inclined mandibular second molars.Methods The sample comprised 18 implant-prosthodontic patients(aged 32.8±5.7 years).There were 25 mesially inclined mandibular second molars that need to be uprighted before implant denture therapy.The periodontal treatments were performed before orthodontic treatment if necessary.One miniscrew (1.6 × 9.0 mm,self-drilling screw;CiBei Co.,Ningbo,China) was placed between the mandibular premolars around mucogingival junction level.After a week,the segmented arch (0.016 × 0.022 inch TMA) was developed and cantileverly mounted on the miniscrews.The cephalometric radiographs before and after the uprighting treatment were measured and compared.Results All the inclined second molars have been uprighted.The average treatment period was 4.0± 1.0 months.After the treatment,the angle or distance between lower second premolar and the mandibular plane did not change significantly,while the angle of the mandibular second molar was significantly increased by 16.5 degree (P＜0.001).The distance between the lower second premolar and second molar was significantly increased by 4.4 mm (P＜0.001).The distal-buccal cusp of the lower second molar did not extruded significantly.Conclusions The segmented arch technique with direct miniscrew anchorage could be used as an efficient method to upright inclined molars,and the first molar space can increase moderately.

Abstract:Objective To design one kind of removable appliance to upright lingual inclined lower second molars. Methods One band with a lingual button was bonded on the crown of lingual inclined lower second molar, then a removable, uprighter was put on, the tipping molar was uprighted by using buccal traction force produced by the latex elastics between the button and the hook of removable uprighter. Results Three cases had been treated successfully within 2 months. Conclusion Removable uprighter might be an effectiveappliance to upright lingual inclined lower second molars.

Abstract:Although clear aligner can be efficient and predictable in some tooth movements, mandibular molar protraction is the least predictable owing to biological and biomechanical limitations. This case report presents a 20-year-old female with poor prognosed left mandibular first molar (#36), deviated dental midline and mild crowding. After extraction of #36, clear aligners and an Albert cantilever were used for treatment. Through carefully designed dual mechanical system, we achieved uprighting and protraction of #37 within 27 months, with crown and root movements of 9.9mm and 12.1mm, respectively. The predictability of the crown and root movement was 107.6% and 84.6%. Coincident dental and facial midline, class I molar and canine relationship and good root parallelism were also achieved. Large-distance mandibular molar protraction can be achieved effectively with a combination of Albert cantilever arm and clear aligner.

Abstract:Background Maxillary canine impaction has been extensively reported, but studies of mandibular canine impaction are relatively few. Clinical studies and reports of experience treating mandibular canine impaction are of clinical benefit to both orthodontists and dentists. Aims This report introduces a Class II division 2 crowded case with severely impacted, fully transposed, mandibular canines treated by a non-extraction approach and mandibular arch expansion. Methods The completely transposed, impacted, mandibular canines were successfully aligned. The crowding was relieved by arch expansion and incisor proclination without obvious radiographic alveolar bone loss. The two severely labially displaced mandibular canines assisted in the expansion of the narrow mandibular arch. Results The maxillary and mandibular arches were expanded and well aligned, and Class II molar and canine relationships and a normal overjet and overbite were established. The mandibular canines were aligned in advantageous positions. There was no regional alveolar bone resorption around the mandibular canines. Conclusion The treatment approach provided an example of significant bone remodelling. Using the impacted mandibular canines for anchorage, the age of the patient provided an opportunity to adopt a relatively conservative and unique treatment approach which may be applied to relieve crowding.

Abstract:OBJECTIVE:To evaluate dento-skeletal and soft-tissue profile changes after extraction of lower second molars and treatment using fixed appliances in severe class III subjects.METHODS:Fourteen patients with severe skeletal class III malocclusion (male 4, female 10, age rang 12.0 - 17.1 years old, mean age 13.3 +/- 0.8), diagnosed as requiring orthognathic surgery, but rejected surgical therapy were included in the study. Lateral cephalometric films taken at the beginning and end of treatment were analyzed using Pancherz analysis and a traditional cephalometric analysis.RESULTS:After active treatment, inclination of lower incisors was decreased 11.7 degrees when measured to the mandibular plane (P < 0.001). A negative value of the distance between upper and lower lip position to SnPg' at the beginning of treatment changed to a positive value (P < 0.001).CONCLUSIONS:The results of the current preliminary study suggest that success in the treatment of some severe class III deformity in the permanent dentition can be achieved with fixed appliances and extraction of lower second molars. A remarkable soft-tissue change was noted after the treatment and concave facial profiles changed to straight profiles.

Abstract:OBJECTIVE:To compare the angular changes in the developing mandibular third molars in the mandibular second premolar extraction and non-extraction cases and to determine whether the mandibular second premolar extraction causes favorable rotational changes in the angulations of the developing mandibular third molars and benefits its later eruption.METHODS:Pretreatment and posttreatment panoramic radiographs were taken from 45 subjects who had been treated by the extraction of the mandibular second premolars and 48 subjects who had been treated without extraction. The horizontal reference plane was used to measure and compare the changes in the angulations of the developing mandibular third molars.RESULTS:The mean uprighting of the mandibular third molars seen in the extraction group was (5.9±2.5) degrees on the right side and (8.0±3.4) degrees on the left side following treatment. For the nonextraction group the mean uprighting was (1.8±2.4) degrees on the right side and (1.5±2.1) degrees on the left side. There was a statistically significant difference between the groups (P<0.05).CONCLUSION:Mandibular second premolar extraction might be a helpful procedure on the angulations of the developing mandibular third molars.

Abstract:Objective: To construct a three-dimensional (3D) finite element model and analyze the biomechanical characteristics during mandibular second molar mesialization using customized lingual appliances and mini-implant. Methods: One adult student volunteer from The First Affiliated Hospital of Zhengzhou University with lower left first molar extraction was selected. After CT scanning, Mimics, Geomagic, ANSYS were employed to develop a 3D finite element model including customized lingual brackets, stainless steel lingual arch wire, buccal buckles, lower dentition, periodontal ligaments and alveolar bone. Four different loading methods (1. the force of 1.470 N loaded at the lingual side only; 2, 3, 4. the forces of 0.490, 0.735 and 0.980 N loaded at both buccal and lingual sides, respectively.) were included. The initial displacements of the lower second molar and stress distribution in the periodontal ligaments were analyzed. Results: More uniform stress distributions in the periodontal membrane of mandibular left second molar were found when the mesial force were loaded at both buccal and lingual sides than the force loaded at lingual side only. More inclination of the second molar was observed when force loaded at both sides than at lingual side only. With the values of the force increased, the mesial inclination potential of the second molar, the initial movement of the second molar and the stress distribution in the periodontal membrane increased. Under the force of 0.735 N at both buccal and lingual sides, mesially inclined initial displacements of the mandibuar second molar were observed and the stress values of von Mises was in the suitable area. Conclusions: Less second molar rotation was found when the force loaded at both buccal and lingual sides than loaded at lingual side only. However, force loaded at both sides would increase the measial inclination potential of the second molar.
Clear aligner treatment of an adult open bite with bilateral missing mandibular first molars through molar protraction with Albert cantilever arms

Abstract:Premature loss of first permanent molars is a common occurrence in clinical practice, leading to abnormal occlusion and inefficient mastication for patients. This case report presented the orthodontic retreatment of a 32-year-old female patient who suffered from premature loss of mandibular bilateral first molars. The mesial tipping of the mandibular second molars led to an occlusal interference, resulting in a clockwise rotation of the lower jaw and an anterior open bite (AOB). We achieved long-distance molar mesialization using clear aligners with Albert cantilever arms. After 42 months of treatment, the patient's occlusion and facial profile significantly improved. The cantilever combined with the cleat aligner treatment has been clinically effective for the protraction of mandibular posterior teeth, extending the field of application of clear aligners.
Orthodontic treatment for substitution of impacted mandibular second molars with their adjacent wisdom teeth

Abstract:A distinctive method for extraction of the bilateral impacted mandibular second molars (MM2s) and moving the bilateral mesioangularly impacted mandibular third molars (MM3s) to the MM2 positions with appropriate occlusion by orthodontic uprighting and protraction was demonstrated in a 24-year-old woman and a 20-year-old woman, respectively. Although the surgical procedures should be performed under general anesthesia, rapid orthodontic treatment after tooth extraction aided in the initial teeth movement of the bilateral MM3s. The proposed treatment could be implemented and completed in 2 years or less. After undergoing the orthodontic treatment, the patient achieved robust bone support no matter with or without bone grafting. With this less technique-sensitive method, the patient still achieved improved posterior occlusion and masticatory function. Oral and maxillofacial surgeons should be consulted before this treatment option was adopted.

Abstract:Purpose: This case report describes the lingual orthodontic treatment of an adult skeletal Class II division 1 patient with lip protrusion, mandibular retrusion, severe overjet and deep overbite, proclined upper and lower incisors and mild crowding in both arches. Materials and methods: The overjet was corrected with upper first premolar extraction with the help of skeletal absolute anchorage. Also, 4 third molars were extracted and miniscrew-assisted total distalization was performed in mandibular arch to decrease the proclination of lower incisors after aligning and levelling. Results: After 25-month treatment, satisfactory facial aesthetics and good occlusion were achieved. A two-year follow-up after confirmed that the outcome was stable. Conclusion: Maxillary first premolar extraction combined with total arch distalization with skeletal anchorage could greatly reduce the excessive overjet and lip protrusion in skeletal Class II cases.
Correcting Unfavourable Intermaxillary Relationship in Partial Edentulous Area and Simultaneous Implant

Abstract:OBJECTIVE:This study was to evaluate the result of correcting the unfavourable intermaxillary relationship with orthognathic surgery in the partial edentulous area and simultaneous implant.METHODS:9 cases with jaw discrepency underwent orthognathic surgery for correcting the intermaxillary relationship and simultaneous implant surgery. The orthognathic operations were Le Fort I osteotomy with mandibular SSRO in 3 cases; simple mandibular SSRO in one case, subapical osteotomy of mandibular front area in 4 case; posterior maxillary osteotomy in one case, 25 implants were simultaneously placed in the edentulos and partial edentulos area.RESULTS:Result showed all 9 cases got the normal intermaxillary relationship post-operation, and implant prosthesis 8 months later, 3 cases with implant supported partial denture, one case with implant supported overdenture, 5 cases with ceramic crowns. Follow-up means 11 months, no implant lost.CONCLUSIONS:Base on this work, the authors believe combining orthognathic surgery and implant surgery is a feasible and reliable method in correcting unfavourable intermaxillary relationship in the implant related area for indicated patients.
