Retroperitoneal Laparoscopic Radical Nephrectomy: Report of 82 Cases

Abstract:Objective To evaluate the safety and efficacy of retroperitoneoscopic radical nephrectomy.Method A total of 82 patients(50 males and 32 females;age range:19~81 years;mean age 56.2 years)underwent retroperitoneoscopic radical nephrectomy.For 48 tumors were located in the left and 34 tumors were in the right,6 due to painless hematuria,11 due to the pain of waist and abdomen,the rest 65 were detected by ultrasonography during physical examination.All the patients can not touch the renal carcinoma.The masses by ultrasonography was on average 3.8 cm(range 1.7～8.0 cm)in diameter,CT scan was performed on all the 82 cases.The tumor size detected by CT was consistent with that by ultrasonography.Preoperatively,54 cases had the tumors of clinical stage T1,18 of T2 and 10 of T3.Results The operative time was 65～300 min(mean 126.6 min),and blood loss was 50～500 ml(mean 116.3 ml).24 h postoperative drainage flow was 20~500 ml(mean 141.3 ml);The time left the sickbed after operation was 1~3 d(average 1.3 d);the postoperative exhaust time were 1.5~3.3 d(average 2.1 d);The hospital stay were 8~27 d(average 12.6 d);4 renal carcinoma diameter 7 cm,and conglutinate with all around tissue,changing to open operation;2 fatness and BMI30,the operate space can not expos,changing to open operation.Pathology showed that 71 cases were of renal clear cell carcinoma,5 of papillary renal cell carcinoma,3 of oxyphil cell tumor,2 of chromophobe carcinoma,1 of renal adenocarcinoma.During the follow up of 6~64 months(mean 22 months),80 patients survived;only 2 died of live rmetastasis one year after operation.Conclusion Retroperitoneoscopy radical nephrectomy is safe and effective.
