Complete isolation of prefrontal lobe for recurrent refractory epilepsy: conception, indication and surgical technique

Abstract:Objective To summarize 8 cases of prefrontal isolation brain surgery for the treatment of intractable epilepsy.Methods Based on cerebral hemisphere and the posterior quadrantic disconnection, we introduced the concept of "completely isolation of epileptogenic zone" for this kind of cases, and performed an operation of total prefrontal lobe isolation for 8 cases who had an invalid surgery of epileptic zone resection.In order to ensure the completeness of prefrontal isolation, the anterior commissure, the front part of the corpus callosum and prefrontal lobe were disconnected in a singular procedure.All the patients were followed up for 16 months after the operation;the patients were assessed by Engel classification.Results The operation was successful in all the patients.Two cases underwent lumbar drainage because of fever after operation, and both recovered well.The postoperative efficacy evaluation showed that 5 belonged to Engel grade Ⅰ(no postoperative epileptic attacks), 2 grade Ⅱ (1 ~ 2 attacks per year), and 1 grade Ⅲ (seizure frequency reduced more than 75%).Conclusions The prefrontal isolation technique is one of the effective surgical solutions for the treatment of refractory frontal lobe epilepsy.
