Resection of the epileptogenic focus for nonlesional perirolandic epilepsy

Abstract:Objective To review experiences in the diagnosis and surgical treatment of epileptogenic focus in nonlesional central epilepsy.Methods The clinical data of 11 patients with nonlesional central epilepsy were analyzed retrospectively.All the patients were transplanted with intracranial electrodes.The epileptogenic area was localized by electrocorticography and functional area localized by cortical electrical stimulation.All the patients underwent resection of the epileptogenic zone in the condition of intraoperative awakening.Results Eleven patients were followed up for 17 to 32 months,and the results were class Ⅰ in 3 patients,class Ⅱ in 3,class Ⅲ in 2 and class Ⅳ in 3 according to Engel classification.Limb dysfunction occurred in 3 patients including 2 recovered 2 weeks and 3 months after the operation respectively and there remained finger function limitation in 1 patient.Conclusions Based on the functional area and epileptogenic foci are localized exactly by intracranial electrodes,resection of the epileptogenic zone can be performed to treat nonlesional central epilepsy and achieve a good outcome.
