Correlation between facial nerve evoked-electromyographic responses and degree of neuromuscular blockade in patients undergoing tympanoplasty

Abstract:Objective To evaluate the correlation between facial nerve evoked-electromyographic (EEMG)responses and degree of neuromuscular blockade(NMB)during tympanoplasty.Methods Forty ASA ⅠorⅡpatients of both sexes aged 20-64 yr undergoing tympanoplasty were divided into 2 groups:group A facial nerve exposed(n=16)and group B facial nerve unexposed(n=24).Anesthesia was induced with midazolam 2-3 mg.fentanyl 2 μg/kg and propofol 2 mg/kg.Tracheal intubation was facilitated with succinyl choline 2 mg/kg.The patients were mechanically ventilated(VT 8-10 ml/kg,RR 10-12 bpm).PET CO2 was maintained at 35-40 mm Hg.Anesthesia was maintained with isoflurane inhalation,remifentanil infusion(0.2 μg·kg-1·min-1)and rocuronium.Controlled hypotension was performed during operation with nitroglycerin.MAP was maintained at 55-65 mm Hg.ECG,BP,HR,SpO2 and PET CO2 were monitored.NMB was monitored by responses of adductor pollicis muscle to stimulation of ulna nerve using TOF-Watch (R)SX Monitor(Organon,Netherlands).The facial nerve EEMG responses were measured when the degree of NMB(T1)wag at 0,25%,50%,75%and 100%of the control height.The stimulation threshold and amplitude of EEMG responses were recorded.Results No EEMG response was elicited at the degree of NMB≥75%in 4 patients.There was a linear positive correlation between stimulation threshold of EEMG and degree of NMB and a linear negative correlation between the amplitude of EEMG and degree of NMG.The stimulation threshold in group A was significantly lower at all degrees of NMB than that in group B.With deepening muscle relaxation,the stimulation threshold of EEMG was gradually increasing,while the amplitude of EEMG was gradually decreasing.Conclusion There is a positive correlation between stimulation threshold of EEMG and degree of NMB and a negative correlation between amplitude of EEMG and degree of NMB.
