Abstract:Objective To analyze the causes of failure of hydroxyapatite(HA) coated hemispheric cup (ABGⅠ) and to evaluate its long period results. Methods We retrospectively studied the patients' records and X-ray films of 299 patients (336 hips) with consecutive primary HA coated hip in Anyung hospital in Yuso, Korea between March 1992 and December 1995. Until July 30, 2004, 121 cases(141 hips) were followed up. Among them, 77 acetabular cups (68 cases) which included 53 hips in 48 males and 24 hips in 20 females were revised. The average age of the patients was 51.4 years old (range, 30-72 years old). 7 hips were performed with total hip revision; Acetabular cup revisions were carried out in 70 hips. The position of acetabular prosthesis was measured. The acetabular malposition was defined as abduction 50°, anteversion 20°, insufficient covering of cup 10%, and the polyethylene liner wear rate was calculated according to offset distance of femoral head, which was measured on AP X-ray. Results 11 hips were revised winth 5 years post-operation, and 66 hips were revised within 6-12 years. The majority of revisions were done between 7 to 9 years post-operation. The causes for revision were analyzed based on X-ray films. There were prosthesis loosening in 42 hips, severe osteolysis in 43, hips mild osteolysis in 12 hips. All prosthesis liners wore in different degree. Except for some factors that probably affected the results, such as 18 cases of cup malposition, 6 cases of acetabular dysplasia with acetabular bone graft and 2 cases of trauma, 2 cases of dislocation, there were 49 revisions probably due to prosthetic reason. The liner wear rate was (0.26±0.10)mm/y. No good ongrowth bone was found on the surface of the cups taken off from the patients. Conclusion This HA coated cup has a good medium term result, and poor long-term result. The main causes for acetabular cup revision rate are the acetabular dysplasia, malposition of acetabular prosthesis, high liner wear rate and osteolysis related to too many screws.
