Value of high-spatial-resolution MRI imaging in preoperative staging of rectal carcinoma

Abstract:Objective: To evaluate the accuracy of high-spatial-resolution MRI imaging in preoperative staging of rectal carcinoma. Methods: Fifty-six patients with rectal carcinoma histologically proved underwent high-spatial-resolution MRI imaging and were staged preoperatively, and the rectal carcinoma stage was performed according to TNM staging system. Preoperative MRI staging of rectal carcinoma and postoperative histopathologic TNM staging were compared. The accuracy of the MRI T staging and N staging were calculated. Results: The accuracy of the MRI T staging was 83.9%(47/56): T2 (71.4%, 10/14), T3 (86.8%, 33/38). The sensitivity, specificity, positive predictive value, and negative predictive value of MRI in detecting invasion through the bowel wall were 87.8% (36/41), 73.3% (11/15), 90.0% (36/40) and 68.6% (11/16), respectively. The accuracy of MRI N staging was 67.9% (36/53): N0 (52.2%, 12/23), N1 (58.8%, 10/17), N2 (66.7%, 6/9), N3 (75.5%, 3/4). The sensitivity, specificity, positive predictive value, and negative predictive value of MRI in detecting lymph node invasion were 63.3% (19/30), 52.2% (12/23), 65.5% (19/29) and 52.5% (12/23), respectively. Conclusions: High-spatial-resolution MRI imaging accurately indicates the tumor staging of rectal carcinoma and depth of extramural tumor infiltration, but still has a low accuracy rate for detecting metastatic lymph node. It provides valuable information to choose the optimal therapeutic procedure.
