Abstract:The objective of this study it to simulate the blood flow in a Three-Dimensional intracranial aneurysm model which is made from a DSA(digital subtraction angiography) or other clinic images.Hemodynamic parameters are obtained from the simulation.We observe the difference in these Hemodynamic parameters before and after embolism treatment.Compared with 2-D aneurysm model,3-D aneurysm model is much more direct and visual.The study can provide useful information for the rupture mechanism and clinic diagnosis of intracranial aneurysms.

Abstract:Abstract Background It is still a challenge to choose the most appropriate endovascular treatment strategy for intracranial aneurysms. In this study, we aimed to investigate the application of AneuPlan™, based on computational fluid dynamics analysis, in the surgical planning of intracranial aneurysms. Methods: 9 patients with intracranial aneurysm were enrolled, whose surgical planning was made according to the Surgical Planning Program analysis, before endovascular treatment. Clinical characteristics, procedural details, stimulated hemodynamics analysis after surgery, site effect and 6-month follow-up were recorded. A descriptive analysis was performed. Results: Among the 9 patients, 5 patients received flow diverter implantation, 3 patients received stent-assisted coiling and 1 patient received simple coiling. The velocity of blood flow into the aneurysm and wall shear stress of the aneurysm were obviously reduced. No hemorrhagic or ischemic complication occurred in all patients. No recurrence of the aneurysm at 6-month follow-up. Conclusions: The good clinical prognosis obtained according to the Surgical Planning Program shows its potential for the optimization of the endovascular treatment for intracranial aneurysms.
Numerical simulation of blood flow in image-based cerebral aneurysms

Abstract:Objective Simulate the blood flow numerically in patient-specific cerebral aneurysms and help assessing the risks of rupture and reoccurrence of cerebral aneurysms in clinic. Method Geometrical models were constructed from three-dimensional rotational angiography data,Herschel-Bulkley non-Newtonian fluid model was chosen for the blood,and the commercial CFD software was employed to simulate the blood flow in the two patient-specific computational models. Result The streamlines,the velocity fields in important cross sections,the wall shear stress distribution and total pressure distribution on the aneurysmal wall were obtained. Furthermore,the graphs of wall shear stress and total pressure,at 20 points of the neck and top of each aneurysm respectively,at systolic time,were plotted. Conclusions Hemodynamic factors play important roles in the growth and rupture of cerebral aneurysms and patient-specific CFD simulation of blood flow in cerebral aneurysm can make a contribution in studying the hemodynamics in cerebral aneurysms. Distribution of wall shear stress is different from that of unsteady index of wall shear stress at the neck of the aneurysm,and the relationship between the oscillation of wall shear stress and the risks of rupture and reoccurrence of the aneurysm should be studied.

Abstract:Objective To classify intracranial aneurysms according to ellipticity index (EI), simulate the aneurismal models with computational Fluid Dynamic (CFD) softwares and to analyze the hemodynamical features of aneurysms. Method According to EI, 39 aneurysms were divided into type A (EI > 1.8)and type B(EI ≤ 1.8) Matlab, Ansys, and Fluent softwares were used to simulate the blood flow of aneurysms. Results The dynamic pressure, velocity and wall shear stress on the top of the aneurismal models were the lowest. The parameters near the "inflow tract" were the highest. The rupture risk of type A aneurysms were higher than that of type B. The WSS's ratio of type A was positive correlation with the EI. Conclusions The hemodymamic factors, such as dynamic pressure, velocity and wall shear stress, are the lowest on the top of aneurysm, which contribute to its rupture. The rupture risk of aneurysms can be reflected by EI and WSS's ratio in the inflow tract and the top of the aneurysms. Following the increasing of EI and WSS's ratio, the rupture risk of aneurysms is increasing too.

Abstract:Objective To use numerical simulation for analying hemodynamic changes in intracranial aneurysm with elastic wall,and to analyze the effect of mechanical properties of the aneurysm wall on the hemodynamic changes.Methods Based on the DSA images of a patient with multi-intracranial aneurysm,the numerical simulation of the hemodynamics of the elastic intracinal aneurysm was processed using a software of finite element method of computational fluid dynamics(CFD) and the computational fluid-structure interaction analyses.We also investigated the effect of the coupling of hemodynamics and aneurysm wall movement.Results The morphology of the elastic aneurysm underwent deformation,and the angle between aneurysm and parent artery also changed,affecting the inflow jet dispersed into the flow field of aneurysm and the distribution of wall shear stress on the surface of aneurysm.Conclusion The numerical simulation of CFD can directly reflect the hemodynamic characteristics of aneurysms.More accurate elastic wall aneurysm model is needed to improve the quantitative analysis of the hemodynamics of intracranial aneurysms.

Abstract:Objective To identify the difference of intra-aneurysmal hemodynamics between wide-necked and narrow-necked aneurysms in curved-vessels.Methods Wide-necked and narrow-necked sidewall aneurysm models in curved-vessels were created by using computational fluid dynamics simulations.The intra-aneurysmal hemodynamics including velocity,wall shear stress and pressure were studied by Fluent software.Results Peak velocity in wide-necked aneurysm was higher than that of narrow-necked aneurysm.The wall shear stress was more prominent at the neck than the other areas of aneurysm.The changes of pressure in aneurismal sac were coincident with that of parent vessels.Conclusion Compared with narrow-necked aneurysms,the wide-necked aneurysms present more complicated intra-aneurysmal hemodynamics.And numerical simulation is helpful for the individual management.

Abstract:The objective of this study is to investigate the effect of hemodynamic parameters on the formation, growth and rupture of an aneurysm. The numerical simulation of elastic aneurysm model was made from a DSA or other clinic images. Comparing of the result of simulation of elastic aneurysm model with that of the rigid one at their velocity vector and shear stress distribution, many differences were found. There were great differences in the distribution of velocity magnitude at some sections which were predicted by rigid and elastic wall models. And the distribution of velocity magnitude of one outlet was obviously eccentric, which affected the distribution of wall shear stress and exchange of substance through the wall. The current of the distribution of wall shear stress along the wall of aneurysm simulated by rigid and elastic wall models were similar. But there were obvious differences between the two models on the values of wall shear stress especially at the neck of aneurysm. Through above analysis, our study demonstrated clearly that the results of 2-D elastic numerical simulation were in more consistance with the clinical and pathological practice.

Abstract:Intracranial aneurysms are pathological dilatations which endanger people's health. Hemodynamics is thought to be an important factor in the pathogenesis and treatment of aneurysms. To date, the bulk of investigations into hemodynamics have been conducted by making use of mathematically idealized models for rigid aneurysms and associated arteries. However the walls of aneurysms and associated arteries are elastic in vivo. This study shows the differences of the simulation between elastic and rigid wall models. The numerical simulation of elastic aneurysm model is made from a representative Digital Subtraction Angiography (DSA) image and calculated with CFD software to get the wall deformation and the velocity field. Then the results are analyzed. By comparing the simulation results of the two models from their velocity vectors and shear stress distribution, many differences can be noted. The main difference exists in the distribution of velocity magnitude at some sections, with one outlet having obviously off-center distribution for the elastic wall model. The currents of the distribution of wall shear stress along the wall of aneurysm simulated in rigid and elastic wall models were similar. But there were apparent differences between the two models on the values of wall shear stress especially at the neck of aneurysm. The off-center distribution of velocity magnitude affects the distribution of wall shear stress and the exchange of substance through the wall. The analysis demonstrated clearly that the results of 2-D elastic numerical simulation were in good agreement with the clinical and pathological practice. The results of this study play an important role in the formation, growth, rupture and prognosis of an aneurysm on clinic application.

Abstract:Objective To investigate the hemodynamic characteristics of intracranial terminal aneurysms and analysis the possible mechanism of growth and rupture of intracranial aneurysms. Methods Some images of intracranial terminal aneurysms were handled by some image processing software. The profile meshed by some computational fluid dynamics (CFD) software were used to simulate the flow and analysis wall shear stress. Results From the beginning to 0.22 T in every cardiac cycle, the wall shear stress had a change sharply to the maximum and then decreased dramatically to the zero from 0.22-0.55 T accompany with velocity. There were maximum stress and minimum stress when 0.22 T and 0.55T at neck, but meanwhile there was no wall shear stress at the wall. Conclusion Pressure is one of risk factors which will make the aneurysm rupture. CFD simulation can provide key hemodynamic information for pathophysiological mechanisms and clinical treatment of intracranial terminal aneurysms.

Abstract:Objective To explore the hemodynamic characteristics of the intracranial aneurysm associated with the fenestration of vertebrobasilar artery, and to analyze the reason for the fenestration of the vertebrobasilar artery frequently associated with intracranial aneurysm. Methods The hemodynamic digital simulation in one patient with the intracranial aneurysm associated with the vertebroba-silar artery fenestration was processed by the computational fluid dynamics(CFD) software according to DSA images. Results The hemodynamic characteristics of the intracranial aneurysm associated with the fenestration, in which special hemodynamic characteristics of two arteries inflow-combination were found, was consistent with that of intracranial aneurysm in the arterial bifurcation. Conclusions The digital simulation by CFD can directly reflect the hemodynamic characteristics of the intracranial aneurysms. The formation of the intracranial aneurysm frequently associated with the vertebrobasilar artery fenestration is related to the special morphology similar to the arterial bifurcation in the fenestration segment.
An Analysis of 2D Hemodynamic Numerical Simulation of Intracranial Saccular Aneurysms with an Associated Daughter Aneurysm

Abstract:Objective To explore the hemodynamic characteristics of intracranial saccular aneurysms with an associated daughter aneurysms.Methods The numerical simulation of hemodynamics in 2 patients with intracranial saccular aneurysms(A and B) with an associated daughter aneurysms were processed with software of finite element method of computational fluid dynamics(CFD) according to DSA images,and their data were analyzed with statistical methods.Results ①Velocity of blood flow:The mean velocity of blood flow in parent arteries was 0.135±0.060 m/s,and the velocity of blood flow in aneurysms was 0.018±0.012 m/s(t=8.458,P=0.000);low-speed vortex was formed in the main sac of aneurysms and their daughter aneurysms,and the blood flow velocity of aneurysms was the highest during T=0.2 s cardiac cycle.②Changes of dynamic pressure and wall shear stress:The mean dynamic pressure of the parent arteries was 12.85±14.06 Pa,and the mean wall shear stress was 0.870±0.769 Pa;the dymamic pressure in aneurysms was 1.60±1.75 Pa,and wall shear stress was 0.235±0.232 Pa.There were significant differences between parent arteries and aneurysms in dynamic pressure and wall shear stress(t=3.153,P=0.009;t=4.001,P=0.004).Dymamic pressure and wall shear stress decreased rapidly after blood flow entering into the aneurysm,and it decreased to the lowest when the blood flow reached the top the aneurysm.In contrast with the other part of an aneurysm,dymamic pressure and wall shear stress at the neck of an aneurysm were the highest(P0.05).In contrast to other cardiac cycle,dymamic pressure and wall shear stress in an aneurysm were the highest at T=0.2 s(P0.05).The changes of dymamic pressure and wall shear stress in an aneurysm were correlated with the changes of velocity of blood flow,and they were positively coreelated(r=0.895,P=0.000;r=0.663,P=0.000).The increase of wall shear stresses was not found at the juncture of daughter aneurysm and main sac of aneurysm.Conclusions Hemodynamic characteristics of intracranial saccular aneurysms with an associated daughter aneurysms is accord with the general hemodynamic characteristics of intracranial aneurysms.No apparent specific hemodynamic factors have been found for making their formation,growth and rupture.The microanatomical structure of the daughter aneurysmal wall should be considered the more significantly factors for easy to rupture.

Abstract:OBJECTIVE:To establish a model of three-dimensional numerical simulation of intracranial longitypical aneurism and analyze the hemodynamic features thereof. METHODS:Ten patients with intracranial longitypical aneurism underwent surgical treatment. Intracranial vascular ultrasonography was conducted before operation. Intra-operatively three-dimensional angiography of the aneurysm was conducted and multifunctional physiological monitor was used to record the dynamic pressure and pressure wave form of the main branch parent artery. Matlab, Ansys, and Fluent software were used to simulate the blood flow of the longitypical aneurysms. RESULTS:The hemodynamic parameter levels were the highest at the inflow tract, followed by those at the outflow tract, and were the lowest at the top of aneurysm, for example, the blood flow velocity was (1.07+/-0.23) m/s, the dynamic pressure was (574+/-186) Pa, and the wall shear stress was (7.7+/-2.0) Pa at the inflow tract, all significantly higher than those at the top [(0.15+/-0.07) m/s, (37+/-13) Pa, and (0.40+/-0.13) Pa respectively, all P<0.05]. No eddy or just simple eddy occurred in the aneurysm. CONCLUSION:The dynamic pressure, velocity, and wall shear stress are the lowest at the top of longitypical aneurysm which may contribute to the rupture of aneurysm.
Establishment of 3D Computational Fluid Dynamic Model of Saccular Intracranial Aneurysm

Abstract:Purpose:To explore the establishment of patient-specific 3D computational fluid dynamic model of saccular intracranial aneurysms.Methods:3D model was established from 2 patients with intracranial aneurysms through the rotation DSA,and then it was underwent numerical simulation with finite volume methods.Results:Vortex can be found in the streamline figures in 2 aneurysms.Throughout one cardiac cycle,the maximum wall shear stress around the neck of the aneurysm were 17.08±8.82Pa and 25.22±7.62Pa,and the average wall shear stress were 3.40±2.29Pa and 1.85±1.66Pa.Conclusion:Patient-specific 3D computational fluid dynamic model of intracranial aneurysm can show the hemodynamic features of the parent artery and aneurysms,which can be used for further analysis and research.
Hemodynamics of the Turbulent Flow in the Cranial Aneurysm

Abstract:Objective To simulate the computational hemodynamics of cranial aneurysms and analyze the hemodynamics of cranial aneurysms with broad neck and narrow neck. Methods Representative DSA images of ICA-PCoA aneurysm patients (broad neck and narrow neck) were chosen. Photoshop, Matlab,Ansys and Fluent softwares were used to simulate the computational hemodynamics of cranial aneurysm in a whole cardiac cycle. Results The aneurismal simulation of hemodynamics was performed in the computers successfully. It was found there was turbulent flow in the cranial aneurysms with broad neck,but not in the ones with narrow neck. The velocity of flow in the cranial aneurysms with broad neck was between 0.057-0.156 m/s,and that in the cranial aneurysms with narrow neck was between 0.001-0.014 m/s. Conclusion As the existence of the turbulent flow in the cranial aneurysms with broad neck,cranial aneurysms with broad neck are greatly different to the ones with narrow neck in hemodynamics. Henceforth,in the interventional treatment of aneurysms,suggesting in the interventional treatment of cranial aneurysms,the velocity of turbulent flow should be lowered or the turbulent flow disappeared before embolization of the aneurysm.

Abstract:One example of grid reconstruction and blood flow simulation for a patient with internal carotid artery aneurysms was presented. The method accurately duplicates the geometry to provide computer simulations of the blood flow. Initial images were obtained by using CT angiography in DICOM format. The image was processed by using MIMICS software, and the 3D fluid model (blood flow) and 3D solid model (wall) were generated. The subsequent output was exported to the Ansys workbench software to generate the volumetric mesh for further homodynamic study. The fluid model was defined and simulated in CFX software, while the solid model was calculated in ANSYS software. The force data calculated firstly in the CFX software were transferred to ANSYS software, and after receiving the force data, total mesh displacement data were calculated in Ansys software. Then the mesh displacement data were transferred back to CFX software. The data exchange was processed in workbench software. The results of simulation could be visualized in CFX-post. The wall shear stress, wall total pressure and von mises stress were in good agreement with the clinical and pathological practice. The results of this study play an important role in the formation, growth, rupture and prognosis of an aneurysm on clinic application, and maybe a practical tool for planning treatment and follow-up of patients after neurosurgical or endovascular interventions with 3D angiography.

Abstract:Objective To establish the individualized rigid hemodynamic model of intracranial aneurysms based on computational fluid dynamic(CFD) technique.Methods The original DICOM format image data from a patient with intracranial aneurysms were imported by the MIMICS software directly,the 3D objects were constructed.The simulation of rigid model was made with ANSYS-CFX software,the hemodynamic parameters such as the wall shear stress related to the growth and rupture of aneurysms were analysed.Results The accurate rigid model of the intracranial aneurysm were established successfully,which can simulate the blood flow of the intracranial aneurysm directly.The stress field distribution of the aneurysm could be exported with this technique.The wall shear stress on the neck impinged by the blood flow was the highest compared with the other sites of the aneurysm.Conclusions This method can construct the rigid model of intracranial aneurysm fast and accurately.The hemodynamic parameters such as wall shear stress are involved in the origin,growth and rupture of intracranial aneurysms.The results of the numerical simulation of the model are reliable to the diagnosis and treatment of aneurysms.

Abstract:Additional hemodynamic parameters are highly desirable in the clinical management of intracranial aneurysm rupture as static medical images cannot demonstrate the blood flow within aneurysms. There are two ways of obtaining the hemodynamic information—by phase-contrast magnetic resonance imaging (PCMRI) and computational fluid dynamics (CFD). In this paper, we compared PCMRI and CFD in the analysis of a stable patient’s specific aneurysm. The results showed that PCMRI and CFD are in good agreement with each other. An additional CFD study of two stable and two ruptured aneurysms revealed that ruptured aneurysms have a higher statistical average blood velocity, wall shear stress, and oscillatory shear index (OSI) within the aneurysm sac compared to those of stable aneurysms. Furthermore, for ruptured aneurysms, the OSI divides the positive and negative wall shear stress divergence at the aneurysm sac.

Abstract:According to recent studies, there are various potential predictors for surgical outcome for cerebral aneurysms. An accurate surgical outcome assessment would help make better-informed decisions and avoid the risk of rebleeding. It is well known that hemodynamic factors play an important role in the pathogenesis and treatment of intracranial aneurysms. In this article, a computational fluid dynamic analysis is applied to one patient-specific model of the cerebral aneurysm located at the tip of basilar artery, by which the differences of hemodynamic parameters before and after endovascular treatment may be evaluated. Based on the model, we show that the flow behavior near the neck of the aneurysm sees great differences after endovascular treatment as compared with that before treatment, which also affects the wall shear stress and the displacement distribution. In addition, our whole simulation process is based on a series of CFD commercial software packages, which are easily available for doctors to implement such a method in their daily practice. These results would be used to assess the outcome of endovascular treatment for the aneurysm occlusion.
