Abstract:OBJECTIVE:To evaluate the results of total knee arthroplasty (TKA) in patients with posttraumatic degenerative arthritis due to a previous fracture around the knee.METHODS:We analyzed the results of 15 TKAs, performed from 1997 to 2003, in 15 patients with post-traumatic degenerative arthritis due to a previous fracture around knee. There were 3 women and 12 men with an average age of 58 years (range, 31-76 years). The time from fracture to arthroplasty averaged 8.2 years (range, 2-27 years). Internal fixation had previously been performed in 8 patients resulting in retained hardware. At the time of arthroplasty a femoral fracture malunion was present in two knees. Lateral retinacular release (4 knees), extensor mechanism realignment (1 knee) or medial collateral ligament reconstruction (1 knee) were needed at the time of arthroplasty.RESULTS:Follow-up averaged 35 months (range, 12-73 months). No patient was lost for follow-up. According to the Knee Society Score scale, the mean preoperative knee score was 37 (range, 10-70) and functional score was 41 (range, 0-60). They were improved significantly to a mean of 84 (range, 10-100) and 76 (range, 20-100) points, respectively at the latest follow-up. The mean knee arc of motion were improved from 84 degree preoperation to 94 degree at the latest follow-up. Postoperative manipulation under anesthesia for poor motion was carried out in 4 knees. No knee had aseptic loosening that required subsequent revision. Two knees developed superficial infection and were treated with debridement. It subsequently recovered with the retention of components.CONCLUSIONS:Significant improvement in function and relief of pain has been achieved in patients with previous fractures undergoing subsequent TKA. However, this procedure is technically demanding and patients are at increased risk for restricted motion and need more care following TKA. This study suggests that the outcome of TKA may be improved further by making special efforts to restore limb alignment, to ensure correct component positioning, and to manage soft tissue balance.

Abstract:Total knee arthroplasty (TKA) is a widely performed surgical intervention designed to relieve pain and restore function in patients with severe knee joint degeneration, most commonly due to osteoarthritis. This procedure involves replacing the damaged knee joint with a prosthetic implant, providing significant improvements in mobility and quality of life. While TKA is beneficial for many, the individuals who stand to gain the most are those with advanced knee arthritis unresponsive to conservative treatments, typically aged 60 and above, experiencing activity‐related pain and functional limitations. Identifying patients with the highest potential for positive outcomes is critical to maximizing the benefits of this transformative surgery. Goal of this editorial is to give a critical view on today's common practice of arthroplasty surgery.

Clinical application and prospect of outpatient total hip/knee arthroplasty in China

Abstract:Total hip/total knee arthroplasty (THA/TKA) can effectively alleviate the pain and restore joint function as well as improve the quality of life of patients who are suffering from all kinds of moderate or late stage joint diseases. In recent years, with the development of surgical technology and Enhanced Recovery After Surgery (ERAS), THA and TKA have achieved to be more secure and mature, which encourage surgeons to attempt outpatient THA and TKA and relevant experi-ences have been summarized. In China, there is a growing number of THA and TKA and a serious imbalance between medi-cal supply and demand. Outpatient THA and TKA can effectively reduce cost and shorten length of stay, which will be bene-ficial to increase the utilization efficiency of medical resources and to promote medical health service progress. This review will kick ideas to prospect clinic application and development of outpatient THA and TKA in China. We hope THA and TKA will be further promoted and efficiency of medical resources will be improved.
Primary total knee arthroplasty for complex supracondylar femoral fractures in patients with knee arthritis: A retrospective study of a patient cohort.

Abstract:When elder arthritis patients suffered from complex supracondylar femoral fractures, their joints condition and general health condition elevate the difficulties in operation and post-surgical recovery. Here, we aimed to simplify the operation procedure by using one-step Total Knee Arthroplasty (TKA) with a stemmed femoral implant. We also investigated if this method could improve the patients' experience after the operation. The surgery including femoral osteotomy and implantation was performed on all fourteen patients by the same orthopedic specialist. The patients' hospitalization time was recorded. The recovery of knee function and patient satisfaction was evaluated by a systematic follow-up with average time 38 months, up to 5 5 years, using Hospital for Special Surgery (HSS) knee scores, the range of motion (ROM), anteroposterior and lateral radiography, and Visual Analog Scale (VAS) scores. The average of hospitalization days was 16 days. No angular deformity, malunion, or shortening were found at radiography. The average ROM was 105.2 degrees at the end of the follow-up period. The knees in all the patients show adequate stability. All patients had returned to their former daily activities. Seventeen out of 24 patients were satisfied with the outcome of the surgery. The usage of TKA with a stemmed femoral implant is a reasonable method for elderly patients suffering from supracondylar femoral fractures and concomitant knee arthritis.
Analysis of the special clinical features of 1202 cases of total knee arthroplasty

Abstract:Objective A retrospective analysis was conducted on 1 202 cases of total knee replacements (TKR) during the periord of 14 years, in order to study the special clinical features of the patients and their knee deformities, with the final purpose of finding the appropriate methods for the management. Methods From April 1987 to Augest 2001, 1202 cases of total knee arthroplasty were performed for 833 patients. There were 464 unilateral TKRs in 464 patients, and 738 bilateral TKRs in 369 patients. The diagnosis of the clincal conditions were osteoarthritis (include synovial chondromatosis and kaschin beck disease), rheumatoid arthritis, ankylosing spondylitis, also included were some rare rheumatic diseases of the knee such as pigmented villonodular synovitis, tuberculous arthritis, Charcot arthritis, hemophilic arthritis and psoriatic arthritis. Patients in this series also had several kinds of severe deformities such as genu varus with dislocation or medial plateau defect, genu valgus, flexion contracture of the knee, flexion deformity with joint fusion, flexion joint fusion with subluxation, and deformity in the low extremities involving multiple joints in patients with JRA. Results Some severe deformities were considered as contraindication of surfacing total knee replacement, however the life quality of patients had significant improvement after operations. Through the improvement of surgical skill, the complications following TKR in severe knee flexion deformity are less in number than that happened in the early days. Conclusion 1) TKR in China at the moment is facing more challenges and difficulties than that in the developed countries. 2)The proper types of prosthesis should be selected beforehand in order to get the good result. 3) Physical therapy is very important in the rehabilitation following TKR, so that the establishment of full time rehabilitation team is mandatory. 4)The key point for promoting TKR treatment for patients with various knee disease is to develop homemade prosthesis in China. Abstract:Objective To evaluate the results of total knee arthroplasty(TKA) for post-traumatic arthritis due to previous fracture around the knee and acute severe tibial plateau fractures. Methods 16 cases were performed TKA for post-traumatic arthritis due to previous fracture around knee and acute severe tibial plateau fractures.Results All cases were followed up for averaged 32 months(range,12～62 months).The mean Martin function scores was 32 points (range,10 to 59 points) before the arthroplasty,and increased to 80 points (range,30 to 92 points) postoperatively.The mean Knee Society function scores improved significantly from 46 to 79 points.The mean range of motion of knee were improved from 58° to 88°.Conclusions It can save or improve articular function in the treatment of severe acute tibial plateau fracture and post-traumatic arthritis with total knee arthroplasty.The effect is better for severe acute tibial plateau fracture than post-traumatic arthritis.

Abstract:Introduction: Valgus deformity is characterized by an outward angulation of the knee joint. The most common cause of valgus deformity is osteoarthritis (OA), a prevalent progressive joint disease that causes chronic pain and functional limitations. Total knee replacement (TKR) is rarely done in patients with grade-I valgus deformity and young age. To the best of our knowledge, this is the first case report of its kind. Case Report: A 34-year-old man presented to us with 15 years of persistent, progressively worsening right knee pain that was interfering with his daily activities. No non-operative treatment could alleviate his severe pain. Physical examination revealed a positive valgus stress test, limited knee extension, and an asymmetrical gait. He was diagnosed with a grade-I valgus deformity of the right osteoarthritic knee. History, physical examination, and radiological findings confirmed the diagnosis. In consideration of severe pain and impaired quality of life, we opted to perform TKR using a medial parapatellar approach. Regular follow-ups were done after the procedure. He experienced no pain or recurrence of deformity. He was very satisfied with the result. His Western Ontario and McMaster Universities OA Index score at 12 months following surgery was 5, indicating a favorable outcome. Conclusion: This case exhibits the effectiveness of TKR in treating grade-I valgus deformity of the osteoarthritic knee with severe pain in a young adult, resulting in improved pain alleviation, mobility, joint alignment, and overall quality of life. High tibial osteotomy (HTO) and unicompartmental knee arthroplasty (UKA) are useful alternatives, but there is little understanding of which procedure is advantageous. Abstract:Fifty percent of tibial plateau fractures occur in adult older than 50 years old, with 8–24 % of those occurring in elderly patients. The gold standard treatment for these fractures in young patients is open reduction and internal fixation (ORIF). However, because of poor bone quality, soft tissue impairment, and complications related to immobilization, ORIF in the elderly are associated to a high complication rate. For this reason, some authors proposed early total knee arthroplasty (TKA) to treat tibial plateau fractures in elderly patients affected by prior osteoarthritis. Surgeons approaching this surgery should be aware that the outcomes are inferior compared to elective TKA. Furthermore, the complication rate following TKA in tibial plateau fractures are more similar to revision TKA than primary TKA. For these reasons, early arthroplasty in proximal tibial fractures should be reserved to elderly patients affected by prior osteoarthritis, who do not comply with the restricted weight-bearing postoperative protocol in order to avoid further surgery. There are few reports on literature reporting on the outcomes of early TKA in tibial plateau fractures, with small records and short follow-up. However, the authors agree in defining this procedure as a valid option in these patients.This chapter reviewed indications, surgical techniques, and outcomes of TKA to treat acute tibial plateau fracture.

Abstract:Introduction Knee ankylosis can result in major functional impairment, and surgical treatment of knee ankylosis is often regarded as a tremendous challenge due to technical difficulties such as difficult joint exposure and a high incidence of complications. The objective of this study was to review the results of preoperative and postoperative functional scores, range of motion (ROM), and complications of total knee arthroplasty (TKA) for the treatment of knee ankylosis. Surgical Methods Between January 2007 and January 2021, 19 patients (17 patients, 11 women, and 6 men) who had knee ankylosis underwent TKA. The mean age of the patients was 52 years (range, 31 to 71), and the mean follow-up period was 10.2 years (range, 3.1 to 13.9). The surgical procedure involved a total knee arthroplasty, performed via a medial parapatellar approach, a quadriceps snip, and a secondary osteotomy with soft-tissue release. The postoperative clinical outcomes and complications were evaluated using a range of methods, including ROM assessment, Hospital for Special Surgery Knee Score (HSS), and visual analogue scale (VAS) scores. Result At final follow-up, the mean HSS score improved significantly from the preoperative score of (33.63 ± 8.69) to (88.05 ± 5.23) ( P < 0.001), ROM improved from (0 ± 0°) to (100.90±14.07°) ( P < 0.001), the VAS score improved from a preoperative score of 0 to (1.00 ± 0.94) ( P < 0.001), and radiographs showed no aseptic loosening of the knee. There were eight knees (42.1%) that had postoperative complications. Conclusion Total knee arthroplasty in patients who have ankylosing knees resulted in significant improvements in flexion and extension, mobility, and quality of life, despite a high incidence of complications such as skin necrosis.

Abstract:Background The use of total knee arthroplasty (TKA) in treatment of chronic degenerative pathologies of the knee has boasted of an experience of 50 years. The aim of this bibliometric and visualized study is to comprehensively examine the current status and global trends of TKA research. Methods Publications related to TKA research from 2010 to 2019 were retrieved from Web of Science (WoS) and Science Citation Index-Expanded (SCIE) database and then analyzed through bibliometric methodology. As for the visualized study, the software VOS viewer was utilized for bibliographic coupling, coauthorship, cocitation, and co-occurrence analysis, along with further simulation of publication trends in this field. Results A total of 8631 publications were eventually included. The number of publications tends to increase annually worldwide. The USA was the pioneer which has made tremendous contributions, with the most publications and citations, as well as the highest H-index. The Journal of Arthroplasty has published the most papers, while Clinical Orthopaedics and Related Research has the highest citation frequency. The Hospital for Special Surgery has made the greatest contribution when total publication number and coauthorship were taken together. Studies could be divided into five clusters: "alignment study", "revision TKA study", "complication study", "rehabilitation study", and "perioperative management study", which have a trend of balanced development in this field. Conclusions There will be an increasing number of publications on TKA research according to the current global trends, and the USA maintained the leadership in this area. Additionally, a trend of balanced development may exist in the field of TKA research, accompanied with inherent changes of hotspots in each cluster.
[Total Knee Arthroplasty in Charcot Arthritis Patients].

Abstract:OBJECTIVE:To discuss the results of total knee arthroplasty undertaken for Charcot arthritis patients.METHODS:Between September 2000 and January 2007, 4 consecutive patients (2 men and 2 women; 5 TKAs) with Charcot arthritic knees were treated. At the time of surgery, the mean age of the patients was 67.3 years (64 to 72 years). One had bilateral and 2 had unilateral TKA and one had ipsilateral total hip and total knee arthroplasty. The mean knee society score before operation was 33.75 points.RESULTS:The mean follow-up was 65 months (19 to 96 months). The mean knee society score post-operation had improved to 95.25 points in 5 TKAs with no occurrence of infection and loosening. All 4 patients were very satisfactory with the clinical results at the last follow-up.CONCLUSIONS:The midterm follow-up data indicated that total knee arthroplasty to treat Charcot arthritis could get good clinical results. The postoperative results in Charcot arthritis were closed related with the proper prosthesis and surgical technique.

Abstract:This chapter explores the management of end stage knee osteoarthritis (OA) in the aging athlete. In this population, the focus shifts from solely pain relief to enabling a return to specific athletic activities. Different and higher patient expectations cause the need for more careful consideration of surgical techniques, implant selection, and rehabilitation. Total Knee Arthroplasty (TKA) remains the gold standard, offering broad indications, and a high success rate overall. Unicompartmental Knee Arthroplasty (UKA) targets isolated OA and demonstrates higher return-to-sport rates. Literature suggests both options favour low-impact activities. Both UKA and TKA are successful options in the treatment of end stage knee OA, even in an active and athletic population.

Improved osteotomy in total knee arthroplasty

Abstract:BACKGROUND: Recently, total knee arthroplasty has been widely applied in clinic, and the replacement techniques and prosthetic design concept have been greatly improved. But osteotomy order has not been reported. OBJECTIVE: To discuss the clinical effect of the improved osteotomy in total knee athroplasty. METHODS: The total knee arthroplasty was performed in 60 patients (60 knees), including 52 cases of osteoarthritis, six cases of rheumatoid arthritis and two cases of traumatic arthritis, and posterior stabilized knee joints were used for total knee arthroplasty, such as the PFC-Sigma PS prosthesis or LINK Gemini PS prosthesis. The femoral osteotomy was performed firstly under extreme knee bending, including oblique osteotomy anterior and posterior to the distal femur (some types contained intercondylar osteotomy). Patel a un-conventional replacement was performed fol owed by the tibial plateau osteotomy. The operation time, postoperative drainage volume, range of motion of knee joint as wel as Hospital for Special Surgery knee scores before replacement and 6 and 12 weeks after replacement were recorded and statistical y analyzed. RESULTS AND CONCLUSION: Al the 60 patients were fol owed-up for 3-14 months. The average operation time was (51.3±12.5) minutes; the postoperative drainage volume was (302±39) mL. The range of motion of knee joint and Hospital for Special Surgery knee scores 6 and 12 weeks after replacement were improved compared with those before replacement (P < 0.01). At 1 month after replacement, two cases had low-grade infection which was improved after anti-infection therapy without prosthesis rejection. Femoral osteotomy before total knee arthroplasty can obtain the great space for tibial operation, which is beneficial to removal of the meniscus, and further loosens the surrounding soft tissue safely. This method has the advantages of simple operation, shortening the operation time, reducing the intraoperative blood loss, and better repairing effect.
Short to Mid-Term Outcome of Total Hip Arthroplasty with Cementless Implants in Patients Younger Than 25 Years Old.

Abstract:Total hip arthroplasty (THA) was once reserved for old patients with hip arthritis but has become more common in very young patients (≤25 years old) diagnosed with various joint disease that were not relieved with conservative treatment. The purpose of this study was to evaluate short to mid-term outcome of THA in patients younger than 25 years old. This retrospective study included 45 patients younger than 25 years (mean age 21.8 ± 2.14 years) who underwent THA. The surgical indications, implant selection, leg length discrepancy, medication history, radiographic outcome, survival and clinical results including the modified Harris hip score (mHHS) and SF-36 score were assessed. The mean follow-up period recorded for patients was 7.82 years (range 5–12 years). The most common diagnosis was avascular necrosis (37.7%) followed by ankylosing spondylitis (24.4%), developmental dysplasia of the hip (17.7%) and rheumatoid arthritis (8.8%). The preoperative leg length discrepancy (LLD), which ranged from 0.5–7 cm significantly improved after surgery ranging from 0–1 cm. The mean preoperative mHHS was 32.44 ± 18.90 compared with the postoperative score of 94.54 ± 5.81 (p < 0.001). The preoperative p-value of SF-36 was lower compared to the postoperative value in all subgroups of SF-36 (p < 0.001). At the latest follow-up there were no radiological signs of loosening and all the implants were classified as well-integrated. At present total hip arthroplasty is considered to be safe and a good solution for young patients below 25 years suffering from end-stage joint disease; however, longer follow-up is required to evaluate the long-term function and outcome of the prosthesis in order to restore the normal lifestyle of the patients.
Practice of Preoperative Strategy Based on Total Knee Arthroplasty

Abstract:Objective:Based on the information of Emulative surgery of Joint replacement as well as its postoperative dynamic data of Finite Element Analysis on Knee joint overall,a 3-dimension preoperative plan is designed to instruct Total Knee Arthroplasty.Methods:After scanning affected knee joint by CT/MR and scanning knee prosthesis by Laser instrument,a model composed of prosthesis,knee joint as well as its ligament was rebuilt computationally,dynamic lines were measured.Processes of prosthesis installation osteotomy performed by facility of Simulation in Mimics according to the knee joint replacement standard,this postoperative model was imported into ANSYS so as for Meshing,Material assignment,load applied,then,stress distribution was analyzed by Finite Element Method.The 3-dimension preoperative plan was thus abstracted for these data so as to direct 20 cases of clinical joint replacement in Practical Group,another 20 cases of clinical joint replacement acted as Matched Group was undergone without 3-dimension preoperative plan traditionally.Results:The best finite element model of postoperative TKA 3D knee joint was obtained and selected,dynamic data is tested to be approximately agreeable to those previous studies of direct measurement upon prosthesis,and it's confirmed to be reasonable.Supervised by the 3-dimension preoperative plan concluded by those virtual data,the accuracy of Practical Group is much higher than the Matched Group.Conclusion:The information for this experiment of analyzing structural deformation,stress distributioninternal energy change provides reliable base of preoperative planning,and it benefits to search the best position for prosthesis installation,osteotomy and surgical result prediction,consequently,the successful ratio of joint replacement is improved.

Abstract:Case: We performed total knee arthroplasty (TKA) on a 62-year-old male patient who presented with symptomatic knee osteoarthritis resulting from habitual dislocation of the patella. The patient had a painless unassisted gait with stable patella and 5 to 95° knee range of motion, 23 months after surgery. Conclusion: TKA can be an effective treatment option for geriatric patients with habitual patellar instability and concomitant knee osteoarthritis, but may require surgical modifications based on individual pathoanatomy.
