Effect of total knee arthroplasty on osteoarthritis and rheumatoid arthritis

Abstract:Objective To compare the clinical effect and safety of total knee arthroplasty (TKA) in the treatment of osteoarthritis (OA) and rheumatoid arthritis (RA).Methods Totally 305 patients with OA (OA group) and 86 patients with RA (RA group) underwent TKA,and were compared the operation lasting time,postoperative drainage volume,hospitalization stay,preoperative and postoperative 1-,3-and 12-month visual analogue scale (VAS) scores,Hospital for Special Surgery (HSS) knee score,knee joint activity,postoperative 3-month joint survival rate,and the incidences of 12-month postoperative infection,prosthesis loosening,lower extremity deep venous thrombosis and joint revision.Results The postoperative drainage volume was significantly less and the hospitalization time was significantly shorter in OA group than that in RA group (P＜0.05),and there was no significant difference in the operation lasting time between two groups (P＞0.05).There were no significant differences in preoperative joint activity ((88.65 ± 12.44)° vs (80.63± 12.71)°),VAS score (3.93±0.87 vs 4.02±0.65) and HSS knee score (55.74±12.47 vs 49.13±49.13) between OA group and RA group (P＞0.05).The postoperative 1-,3-and 12-month joint activities ((91.65±6.16)°,(108.52± 5.94)°,(120.64±6.82)°) and HSS knee scores (69.55±8.56,83.62±6.47,92.43±8.39) were significantly higher and VAS scores (2.19±0.63,1.07±0.45,0.36±0.34) were significantly lower than those before operation in OA group and than those after operation in RA group (P＜0.05).The postoperative 1-,3-and 12-month joint activities ((89.49±6.53)°,(98.45±4.84)°,(115.72±7.92)°) and HSS knee scores (65.43±6.33,81.52±7.82,86.81± 9.74) were significantly higher and VAS scores (3.82±0.57,1.95±0.72,0.84±0.63) were significantly lower than those before operation in RA group (P＜0.05).The postoperative 12-month infection rate and joint revision rate were significantly lower in OA group (0.33％,0.33％) than those in RA group (5.36％,6.58％) (P＜0.05),and there were no significant differences in the postoperative 12-month incidence of deep venous thrombosis in lower extremities (11.80％ vs 6.98％),and postoperative 3-month joint survival (99.67％ vs 98.94％) between OA group and RA group (P＞0.05).Conclusion TKA achieves a satisfactory result for both OA and RA,while the postoperative pain is milder,the joint activity is better and infection rate is lower for OA patients.
