Abstract:The misadministrations recently reported by the New York Times clearly emphasize the paramount importance of patient safety in radiation therapy. To avoid such incidents, one possible solution is to implement an in vivo dose-monitoring system during the first and each subsequent fraction of treatment for each patient. However, traditional in vivo dosimetry systems, such as diode, MOSFET and TLD, can only provide a point dose at/or near skin surface, which is not adequate to verify the accuracy of delivered dose. The purpose of this study is to evaluate and implement a method for a 2D/3D in vivo dose verification program using an electronic portal imaging device (EPID). During the treatment for each patient, exit portal images are acquired in integration mode with a EPID (aS500, Varian Medical Systems, Palo Alto, CA) at SDD = 150 cm. The delivered fluence map for each field is reconstructed by correcting predicted scatter from the patient to the imager and primary attenuation estimated from the patient CT images. The reconstructed fluences are then imported into a Varian Eclipse treatment planning system to calculate 2D/3D dose on the patient planning CT images. To evaluate the accuracy of the dose delivery, dose comparison was performed between the dose distribution reconstructed with the measured EPID images and the planned one. The 2D dose distribution at the isocenter plane (SAD = 100 cm) are compared using gamma index (criteria: 3%/3 mm). Ten IMRT and 3D conformal cases with different disease sites were used to evaluate the validation of the proposed method. Our results indicated that the measured dose at isocenter center differs <5% from the planned ones. The measured 2D dose distribution shows good isodose (30-100%) overlay with the plan. Quantitative comparison using gamma index shows a mean of -0.15 and 93.9% of dose points have gamma value less than 1. Comparison of the DVH curves from the measurement and planning shows that the difference for D95 of the PTvs. and maximum dose of most critical structures is within 5%. The proposed EPID-based method for in vivo dose monitoring provides an integrated and seamless tool to detect and avoid protracted radiation misadministration events. Several advantages of the proposed technique include: 1) near real-time determination of delivered dose and 2D/3D dose distributions verification; 2) elimination of conventional TLD, Diode and MOSFET based in vivo dosimetry, thereby eliminating the influence of in vivo dosimeters on delivered dose; and 3) no need of any additional hardware for dose measurement. We believe these steps can further reduce but not entirely eliminate the risk of reportable radiation events.
