Intravascular Ultrasound Guided Balloon Fenestration in Aortic Dissection

Abstract:Objective To study the role of intravascular ultrasound (IVUS) in the balloon fenestration of aortic dissection.Methods Eight dogs underwent surgical creation of a dissection in the descending thoracic aorta followed with balloon fenestration under the distal blood pressure and blood flow,transluminal pressure gradient,lumen area and configuration before and after fenestration.The accuracy was assessed by comparing the dissection after fenestration.Results Distal blood pressure and blood flow were dropped from (104±8) mm Hg and (61±12) ml/min to (52±20) mm Hg and (27±11) ml/min after dissection (P0.01),and returned to (100±8) mm Hg and (59±12) ml/min after fenestration (P0.05).Transluminal pressure gradient,(59±15) mm Hg after dissection,was dropped to (11±4) mm Hg after fenestratiom (P0.01).False lumen area and percentage,(56.4±10.3) mm2 and (64.0±10.9)% after dissection,was dropped to (44.5±6.4) mm2 and (51.3±5.8)% after fenestration (P0.01).Dissection configuration transformed from the concave form to the convex form after fenestration.Dissection length by IVUS was correlated well with the anatomic length (r=0.97, P0.01).The residual dissection lengths were 3 mm or less after fenestration.Conclusion IVUS guided balloon fenestration is an effective means to relieve the peripheral artery ischemia caused by aortic dissection.
