Abstract:INTRODUCTION:The aim of this study was to implement our technique for the initial dissection of the inferior hypogastric plexus and protection of the autonomic nerve supply to the corpora cavernosa in laparoscopic radical cystoprostatectomy with an orthotopic ileal neobladder and report the initial outcomes.METHODS:Eleven normally potent patients with preoperative cT2N0 bladder cancer who underwent bilateral nerve-sparing laparoscopic cystoprostatectomy performed by the same surgeon were selected from May 2018 to September 2020. In this procedure, the anterior part of the inferior hypogastric plexus was dissected first between the prehypogastric nerve fascia and rectal proper fascia medial to the distal ureter. Then the Denonvilliers' fascia and the nerves around the prostate were preserved according to current intrafascial principles. The preliminary operative, oncologic, and functional results are presented.RESULTS:The median follow-up duration was 18 months. We observed early and late complications in 5 patients, but none exceeded grade III. Of the 11 patients, ten gained daytime continence (90.9%), and 8 (72.7%) showed nocturnal continence at the last follow-up. Regarding postoperative potency, 10 of the 11 patients (90.9%) remained potent with or without oral medications, excluding one who had partial tumescence but did not follow our recommendations regarding medication use. No local recurrence or positive surgical margins were noted.CONCLUSION:In addition to emphasizing our cavernosal nerve-sparing procedure, this report on the precise dissection and protection of the inferior hypogastric plexus could be of clinical significance, providing potentially ideal short-term functional results.
[Effects of Electroacupuncture on Postoperative Functional Recovery in Patients with Gynaecological Laparoscopic Surgery].

Abstract:OBJECTIVE:To evaluate electroacupuncture on the efficacy and the safety of gastric and systemic functional recovery in patients with carbon dioxide pneumoperitoneum undergoing gynaecological laparoscopic surgery.METHODS:Sixty patients undergoing gynaecological laparoscopic surgery were randomly divided into a regular anesthesia group (group A) and a combination electroacupuncture anesthesia group (group B), 30 cases in each group. The group A were treated with regular general anesthesia, and on the basis of general anesthesia, the group B were treated with electroacupuncture at bilateral Zusanli (ST 36) and Liangqiu (ST 34) by continuous wave and a frequency of 2 Hz 30 minutes before the surgery until the end of the operation. Sedation scale by Ramsay, nausea, vomiting and mental state scores of postoperative were evaluated and compared between the two groups.RESULTS:Compared with the group A, the sedation scale (1.19 +/- 0.56 vs 1.96 +/- 0.90), nausea score (0.59 +/- 0.80 vs 1.81 +/- 0.88), vomiting score (0.48 +/- 0.94 vs 1.89 +/- 0.93) and mental state score (0.96 +/- 0.73 vs 1.69 +/- 0.75) were all decreased significantly in the group B (all P < 0.01).CONCLUSION:Combination electroacupuncture anesthesia can relieve postoperative irritable, nausea and vomiting, improve mental state after the operation and promote the gastric and systemic functional recovery. It is a good method for the adjustment of carbon dioxide pneumoperitoneum during laparoscopic surgery.
The Effect of Transcutaneous Electrical Stimulation Treatment in Combination with Intraoperative Nerve Staining on Sexual Function after Radical Hysterectomy: a Pilot Study

Abstract:Objective: Cervical cancer treatment by radical hysterctomy is usually associated with sexual dysfunction. The aim of the study was to evaluate the effect of transcutaneous electrical stimulation treatment in combination with intraoperative nerve staining on sexual function after radical surgery. Methods: Eligible patients (n = 88) who had undergone laparoscopic nerve sparing radical hysterectomy (Type C1, nerve sparing radical hysterectomy, NSRH), pelvic lymphedenectomy, vaginal extension and ovarian transposition were recruited in the study and allocated (no random assignment) to four groups. Group 1 (biofeedback) received a short-term bio-feedback transcutaneous electrical stimulation treatment after surgery. Group 2 (NSRH) received NSRH with intraoperative nerve staining. Group 3 (electrical) received transcutaneous electrical stimulation after surgery combined with intraoperative nerve staining. Group 4 (placebo) did not receive either transcutaneous electrical stimulation after the surgery or intraoperative nerve staining. Sexual function of the patients and their partners were evaluated using the Female Sexual Function Index (FSFI) 3 months after completion of treatment. Six domains were assessed: desire, arousal, lubrication, orgasm, satisfaction and pain, while the visual analog score (VAS) was used to evaluate partner satisfaction. Results: On average, overall sexual function reduced from baseline to 3-month follow-up for Groups 1 (biofeedback), 2 (NSRH) and 4 (placebo) . In Group 1, all domain scores reduced from baseline to 3-month follow-up. In Group 2, reductions were observed in desire, arousal and orgasm domains. In Group 3 (electrical) there were no reductions observed in any domains, while in Group 4 reductions were observed in desire, arousal, orgasm and pain. The results revealed that there was no statistical difference between group 2, 3 and 4 in lubrication and satisfaction domains. And there were no statistical difference between group 2 and 3 in pain domain. For the patient partner satisfaction, the difference between baseline and 3 months after treatments of group 3 was significantly lower than the other three groups. And the differences of group 2 and 4 were significantly lower than group 1 respectively. Conclusions: The use of transcutaneous electrical stimulation treatment combined with intraoperative nerve staining could improve the sexual function for the patients and the enhance sexual satisfaction for their partners after radical hysterectomy.

Abstract:Introduction: Post-radical-hysterectomy (RH) patients suffer from a series of problems resulting from neurovascular injury, such as bladder dysfunction, which reduce their quality of life. We have designed this study to evaluate the efficacy of transcutaneous electrical stimulation (TENS) on patient rehabilitation after RH for early cervical cancer. Materials and methods: A total of 97 patients were enrolled in a randomized-controlled trial (from January 2015 to December 2019) involving 7 medical centers nationwide. Patients were assigned to either the intervention group ( n = 46), or the control group ( n = 51). TENS was given to patients in the intervention group from the 7 th day after surgery for a total of 14–21 days. The control group received no TENS. Primary outcomes were measured for residual urine volume and recovery of urination function. Secondary outcomes were measures for urodynamics (UDS), pelvic floor electromyography function examination (PFEmF), and quality of life (QoL). Results: Residual urine volume and improvement in the rate of urination were found to show no significant differences on the 14 th , 21 st , and 28 th days after surgery. The maximum flow rate (Qmax) in the intervention group was significantly higher than that in the control group on the 28 th day, but there were no significant differences in average flow rate, voiding time, time to Qmax, muscle fiber strength, muscle fiber fatigue, and the abnormal rate of A3 reflection on the 28 th day and the 3 rd mo., as well as in the QoL at 3 rd mo., 6 th mo., and 12 th mo. after surgery. Conclusion: Our study showed no sufficient evidence to prove that TENS under the trialed parameters could improve the subject's voiding function, PFEmF, and QOL after RH. Curative efficacy of low frequency electrical stimulation in preventing urinary retention after cervical cancer operation

Abstract:<h3 class="Heading">Background</h3><p class="Para">To evaluate the clinical significance of low-frequency electrical stimulation in preventing urinary retention after radical hysterectomy.</p><h3 class="Heading">Methods</h3><p class="Para">A total of 91 women with stage IA2–IB2 cervical cancer, who were treated with radical hysterectomy and lymphadenectomy from January 2009 to December 2012, were enrolled into this study and were randomly divided into two groups: trail group (48 cases) and control group (43 cases). Traditional bladder function training and low-frequency electrical stimulation were conducted in the trail group, while patients in the control group were only treated by traditional bladder training. The general condition, rate of urinary retention, and muscle strength grades of pelvic floor muscle in the perioperative period were compared between these two groups.</p><h3 class="Heading">Results</h3><p class="Para">The incidence of postoperative urinary retention in the electrical stimulation group was 10.41%, significantly lower than that in the control group (44.18%), and the difference was statistically significant (<em class="EmphasisTypeItalic">P</em> < 0.01). The duration of postoperative fever and use of antibiotics were almost the same between these two groups. Eleven days after surgery, the difference in grades of the pelvic floor muscle between these two groups was not statistically significant. However, 14 days after the operation, grades of the pelvic floor muscle were significantly higher in the trail group than in the control group, and the difference was statistically significant (<em class="EmphasisTypeItalic">P</em> < 0.01). In addition, although there was no significant difference between the two groups with different parameters (<em class="EmphasisTypeItalic">P</em> = 0.782), the incidence of urinary retention was lower in the endorphins analgesia program group than in the neuromuscular repair program group (9.09% < 11.54%).</p><h3 class="Heading">Conclusion</h3><p class="Para">Low-frequency electrical stimulation is more effective than conventional intervention in preventing urinary retention after radical hysterectomy. It also intensifies the recovery of pelvic muscle strength.</p>

Biofeedback electrical stimulation for treatment of urinary incontinence after laparoscopic radical prostatectomy

Abstract:AIM: To investigate the efficacy of biofeedback electrical stimulation and pelvic floor muscle training in the treatment of urinary incontinence after laparoscopic radical prostatectomy (LRP). METHODS: Forty-one patients who underwent LRP in Department of Urinary Surgery, Third Affiliated Hospital of Sun Yat-sen University from July 2005 to June 2007 were selected, including 12 with mild disease, 23 with moderate and 6 with severe. The patients were treated by UROSTIM biofeedback electrical stimulation (Laborie, Canada), once per day for five times and pelvic floor muscle training. One or two courses of treatment were performed according to the degree of urinary incontinence. Therapeutic outcome was assessed 4 weeks after treatment according to the criteria: recovery if self-conscious urinary incontinence disappeared, urine was controllable, urination was normal, and urinal pad test was negative; effective, if self-conscious urinary incontinence significantly decreased, and urinal pad test was negative for more than 3 successive times; ineffective, if urinary incontinence was not improved, and urinal pad test was positive. The patients were followed up for 3 to 12 months. RESULTS: Of the 41 patients, the total efficacy rate was 83% (34/41), curative rate was 56.1% (23/41), efficacy rate 26.9% (11/41) and ineffective rate 17.0% (7/41). Of the cases with mild urinary incontinence, 11 were cured and 1 was effectively improved; of the cases with moderate urinary incontinence, 11 were cured and 8 were significantly improved; of the cases with server urinary incontinence, 1 was cured, 2 were improved, and 3 were failed. CONCLUSION: Biofeedback electrical stimulation can facilitate rehabilitation of urinary control after prostatectomy.
Effect of Pelvic Floor Function Exercises Based on the ERAS Concept on Lower Urinary Tract Symptoms after Radical Hysterectomy for Cervical Cancer: A Retrospective Cohort Study

Abstract:Objective: This study aimed to analyse the effect of pelvic floor function exercises based on the enhanced recovery after surgery (ERAS) concept on lower urinary tract symptoms after radical hysterectomy for cervical cancer. Methods: The retrospective study was conducted using clinical data of patients admitted to our hospital from January 2021 to December 2022 and underwent radical hysterectomy for cervical cancer. In accordance with the nursing method, the patients were divided into the routine group (conventional care) and the ERAS pelvic floor function exercise group, for which ERAS concept-based pelvic floor function exercises were carried out on the basis of the former group. The confounders between the two groups were matched by propensity score, and the scores of urination, pelvic floor muscle strength, lower urinary tract symptoms and quality of life (QoL) were compared between the two groups after matching. Results: A total of 180 patients were included in the study, of whom 60 patients were identified after propensity score matching, consisting of 30 patients each in the ERAS and routine groups. The baseline characteristics of the two groups were balanced, and the difference was not statistically significant (p > 0.05). Compared with the routine group, the observation group showed significantly shorter first spontaneous urination time, greater first urination volume (p < 0.05), better pelvic floor muscle strength, considerably lower urinary tract symptoms and better QoL at discharge and 3 months after discharge (p < 0.05). Conclusions: Pelvic floor function exercises based on the ERAS concept can relieve lower urinary tract symptoms, accelerate urination and improve pelvic floor muscle strength and QoL after radical hysterectomy for cervical cancer.
Clinical Outcomes of Pelvic Lymph Node Dissection Before Versus after Robot-Assisted Laparoscopic Radical Cystectomy

Abstract:Objective: The purpose of this study was to compare the clinical outcomes of bladder cancer patients treated with extended pelvic lymph node dissection (ePLND) before or after cystectomy under robotic-assisted radical cystectomy (RARC).Methods: A retrospective study to identify 348 patients with bladder cancer who underwent RARC was performed. Of the patients, 152 (42.8%) underwent ePLND before radical cystectomy (RC) (group A) and 196 (56.3%) underwent ePLND after RC (group B). The clinical, pathological, and overall survival were compared.Results: The total and RC operation time in Group A (total: 130.68 +/- 29.25 minutes, RC: 59.45 +/- 28.63 minutes) were both shorter than Group B (total: 154.17 +/- 38.18 minutes, RC: 94.81 +/- 41.21 minutes) (P < .05). However, no significant difference in time of ePLND. The estimate blood loss (EBL) of RC part and total operation (RC+ePLND) in group A was less than group B (both P < .05), while the ePLND part did not show significance. The result of vascular and nerve injury and surgical drain withdrawal time were similar in two groups. The total number of lymph nodes in group A was fewer than group B (16 versus 26; P < .05). Moreover, the number of bilateral internal iliac and presacral lymph nodes of group A was fewer than group B significantly, whereas the number of bilateral external iliac, common iliac, and obturator lymph nodes was similar in two groups. The lymph node density of group A was significantly lower than group B. The median follow-up of all patients was 33.0 months. Importantly, the survival of group B was better than group A (hazard ratio: 1.412; 95% confidence interval: 1.004-1.987; P = .048).Conclusions: Performing ePLND before RC reveals better result on operation time and EBL, while, when ePLND after RC, the total number of lymph nodes dissected is more and the survival is better. It recommended ePLND be performed before RC, and it is necessary to recheck the internal iliac and presacral area after cystectomy.

Abstract:Background Class III radical hysterectomy (RH III)_plus pelvic lymphadenectomy is the standard surgery for early stage cervical cancer (CC) patients, the 5 year survival rate is about 90%, but pelvic floor disorders especially bladder dysfunction are common due to damaged vessels and nerve fibers following surgery. Transcutaneous electrical stimulation (TENS) treatment has been used to treat bladder disorders for many years, but its effect on cervical cancer patients, the best treatment time point and stimulated protocol, had never been assessed. The aim of this study is to investigate the efficacy of TENS treatment on lower urinary tract symptoms (LUTS) after RH III in CC patients. Methods/Design The study will be conducted as a clinical, multicentre, randomised controlled trial with balanced randomisation (1:1). The planned sample size is 208 participants (at 1:1 ratio, 104 subjects in each group). At 5–7 days after RH III, patients are screened according to operative and pathological findings. Enrolled participants are randomised into an intervention group (TENS plus conventional clinical care) or control group (conventional clinical care), with stratification by menopausal status (menopause vs. non-menopause) and surgical modality (laparoscopic RH or abdominal RH). Participants in both groups will be followed up at 14 days, 21 days, 28 days, 3 months, 6 months, 12 months, 18 months and 24 months after surgery. The primary endpoint is improvement rate of urination function which is defined as recovery (residual urine ≤50 ml) or improvement (residual urine 50–100 ml). Secondary endpoints include urodynamic parameter, urinary incontinence, anorectal function, pelvic function, quality of life (QOL), disease-free survival and adverse events. Primary endpoint analyses will be carried out by Cochran-Mantel-Haenszel tests taking into center effect. Discussion To our knowledge this is the first trial to investigate the effect of TENS treatment on bladder function recovery after RH III among CC patients. This study will provide new information on TENS efficacy for bladder function recovery. Once confirmed, it may help to provide a new, non-invisive treatment for those postoperative CC patients with poor pelvic function, which would help improve their quality of life. Feasibility of real-time pelvic sphincterial electromyographic monitoring during laparoscopic radical prostatectomy

Abstract:Objective To evaluate whether real-time pelvic sphincterial electromyographic(PSE) monitoring during laparoscopic radical prostatectomy contributes to postoperative continence recovery.Methods Eighty patients with stage T2a-T3a prostate cancer(PC) were enrolled.All patients had preoperative full continence and were randomly divided into two groups(n=40 per group).Patients in the PSE group underwent real-time PSE monitoring during laparoscopic radical prostatectomy and those in control group received laparoscopic radical prostatectomy alone.NIM-Response 1.0 electromyographic monitor system(Medtronic ENT Medtronic USA,Inc.) was used for the dissection,explosion and protection of the periprostatic nerve during the surgery.Follow up was done at outpatient clinic or by telephone to evaluate the urinary continence status on the day of catheter removal,3rd month and 6th month postoperatively.Cystourethroscopy and urodynamic examination were performed on the 30th day after surgery.Results Surgery was successfully performed in all patients.There were no significant differences between two groups in baseline variables(P>0.05).In the PSE group,PSE showed the innervation of urethral sphincter was like a network,and thus continence sparing techniques were used in the laparoscopy radical prostatectomy.Significant difference in full continence rate was found on the day of catheter removal(65.0% vs 37.5%,P<0.05,respectively),but absent on 3rd month and 6th month after surgery(80.0% vs 75.0% and 92.5% vs 90.0%).On the 30th day after surgery,urodynamic examination and cystourethroscopy demonstrated urethral closure and urethral sphincter,respectively,in both groups.Conclusion Our findings demonstrate that real-time PSE monitoring during laparoscopic radical prostatectomy is beneficial for the early continence recovery after radical prostatectomy.
Clinical Study on Low-Frequency Electrical Pulse Acupoint Stimulation Combined with Pelvic Floor Muscle Exercise in the Treatment of Urinary Incontinence after Radical Prostatectomy

Abstract:Background: Urinary incontinence (UI) is a common complication after radical prostatectomy (RP). It has a great influence on the postoperative quality of life of patients. This study aims to explore the clinical efficacy of low-frequency electrical pulse acupoint stimulation combined with pelvic floor muscle exercise in the treatment of UI after RP. Methods: The clinical data of 129 patients with UI after receiving RP in our hospital from July 2020 to July 2023 were retrospectively analysed. A total of 65 patients who received pelvic floor muscle exercise from July 2020 to January 2022 were set as the reference group. Of these patients, four were excluded, resulting in the inclusion of 61 cases. A total of 64 patients who received low-frequency electrical pulse acupoint stimulation combined with pelvic floor muscle exercise from February 2022 to July 2023 were classified into the observation group. Of these patients, four were excluded, and 60 cases were finally included. SPSS 23.0 was used to analyse the use of urine pads, recovery time of urinary control and improvement of urination in the two groups. Results: Before treatment, no significant difference existed in the use of urine pads, urination condition, maximum flow rate, maximum cystometric capacity, maximum urethral closure pressure, abdominal leak point pressure and scores on Short-Form-36 Health Survey (SF-36) in both groups (p > 0.05). After treatment, the observation group had significantly lower use of urinary pads, urination frequency and leakage times; Significantly shorter recovery time of urinary control (p < 0.05); And significantly higher maximum flow rate, maximum cystometric capacity, maximum urethral closure pressure, abdominal leak point pressure and SF-36 scores than the reference group (p < 0.05). Conclusions: The combination of low-frequency electrical pulse acupoint stimulation and pelvic floor muscle exercise can improve clinical symptoms, shorten the recovery time of urinary control and improve urodynamics and quality of life in patients with UI after RP.
Preliminary study of pelvic autonomic nerve-sparing sub-radical/radical hysterectomy on preserving postsurgical bladder function in patients with cervical and endometrioid cancer

Abstract:Objective:To investigate the effects of the technique for preserving the hypogastric nerves and pelvic splanchnic nerves on postsurgical bladder function.Methods:Between July 3,2006 and August 11,2006,a nerve-sparing cardinal ligament resection was carried out on 12 consecutive patients with uterine cervical cancer and endometrioid cancertreated at Fudan University Cancer Hospital.Results:Catheter was routinely removed 3~4 days after sub-radical hysterectomy in patients with endometrioid cancer,and 7 days after radical hysterectomy in 5 patients withcervical cancer.One patient with stage Ⅰ_ b1 cervical cancer was failed to remove the catheter at 7 days after surgery and removed at 14 days.The catheter was removed at 10 and 14 days after ultra-radical hysterectomy in 2patients with stageⅡ_b cervical cancer.Conclusions:Pelvic autonomic nerve-sparing cardinal ligament-resection may be strongly recommended in sub-radical hysterectomy,and those procedures should be improved in radical cardinal ligament resection.However,it is difficulty to preserve pelvic autonomic nerve plexus in patients with stage Ⅱ_b cervical cancer.

Abstract:Background: Bladder dysfunction, notably urinary retention, emerges as a significant complication for cervical cancer patients following radical hysterectomy, predominantly due to nerve damage, severely impacting their postoperative quality of life. The challenges to recovery include insufficient pelvic floor muscle training and the negative effects of prolonged postoperative indwelling urinary catheters. Intermittent catheterization represents the gold standard for neurogenic bladder management, facilitating bladder training, which is an important behavioral therapy aiming to enhance bladder function through the training of the external urethral sphincter and promoting the recovery of the micturition reflex. Nevertheless, gaps remain in current research regarding optimal timing for intermittent catheterization and the evaluation of subjective symptoms of bladder dysfunction. Methods: Cervical cancer patients undergoing laparoscopic radical hysterectomy will be recruited to this randomized controlled trial. Participants will be randomly assigned to either early postoperative catheter removal combined with intermittent catheterization group or a control group receiving standard care with indwelling urinary catheters. All these patients will be followed for 3 months after surgery. The study's primary endpoint is the comparison of bladder function recovery rates (defined as achieving a Bladder Function Recovery Grade of II or higher) 2 weeks post-surgery. Secondary endpoints include the incidence of urinary tract infections, and changes in urodynamic parameters, and Mesure Du Handicap Urinaire scores within 1 month postoperatively. All analysis will adhere to the intention-to-treat principle. Discussion: The findings from this trial are expected to refine clinical management strategies for enhancing postoperative recovery among cervical cancer patients undergoing radical hysterectomy. By providing robust evidence, this study aims to support patients and their families in informed decision-making regarding postoperative bladder management, potentially reducing the incidence of urinary complications and improving overall quality of life post-surgery. Abstract:In 1967 Shealy first used electrical stimulation of the spinal cord to treat spasticity and pain. This therapy proved to be effective for bladder dysfunction too. The effect of electrical stimulation of the spinal cord at thoracic level has been evaluated in 18 neurogenic patients suffering from hyperreflexia with detrusor-sphincter dyssynergia. Bladder function improved significantly in 13 (73%). Partial or complete relief of bladder hyperreflexia, marked increased of bladder capacity and reduction of residual urine were recorded. The beneficial effect of stimulation indicates that it is a safe and effective alternative treatment for the neuropathic bladder and careful trials with further investigations should be carried out.
Effect of Psychological Intervention on Pelvic Floor Function and Psychological Outcomes After Hysterectomy

Abstract:Background Hysterectomy is one of the most frequently performed operations worldwide. However, postoperative complications and body image changes may induce psychological distress after hysterectomy. The study aimed to evaluate the effect of psychological intervention on pelvic floor function and psychological outcomes following hysterectomy among patients with benign indications. Methods Ninety-nine patients underwent hysterectomy were randomly divided into intervention group ( n = 50) and control group ( n = 49). Patients in the control group received routine postoperative nursing care, while extra psychological intervention was provided to patients in the intervention group, including psychological support, regular lectures and family support. After 6 months, patient's psychological statuses were assessed by Generalized Anxiety Disorder scale (GAD-7) and Patient Health Questionnaire-9 (PHQ-9). The pelvic floor function of patients was evaluated using Pelvic Floor Impact Questionnaire (PFIQ-7) and Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12). Furthermore, the incidence of postoperative complications, including uracratia, pelvic organ prolapses, sexual dysfunction and chronic pelvic pain, was evaluated. Results After 6-month intervention, the GAD-7 and PHQ-9 scores were significantly decreased in the intervention group ( p < 0.001 and p = 0.018 respectively). Both scored were significantly lower than that in the control group ( p < 0.001 and p < 0.001). Compared with control group, the incidence of uracratia, pelvic organ prolapse, sexual dysfunction and chronic pelvic pain for intervention group was significantly lower ( p = 0.003, p = 0.027, p = 0.001, p = 0.002 respectively) and the pelvic floor muscle strength was significantly stronger ( p = 0.001). Besides, the postoperative Urinary Incontinence Impact Questionnaire (UIQ-7), Pelvic Organ Prolapse Impact Questionnaire (POPIQ-7), and Colorectal-Anal Impact Questionnaire (CRAIQ-7) scores were significantly lower ( p = 0.025, p = 0.04, p < 0.001) and PISQ-12 score was significantly higher in intervention group ( p < 0.001). Conclusion Psychological intervention could effectively improve the psychological condition of patients with hysterectomy, which may facilitate patients' postoperative recovery in pelvic floor function. These findings emphasized the necessity of psychological intervention in routine postoperative nursing care.

Abstract:To evaluate the role of electrical stimulation (ES) in the recovery of postprostatectomy urinary incontinence (UI).We performed a meta-analysis of all available randomized controlled trials comparing ES enhanced pelvic floor muscle training (PFMT) with PFMT alone for postprostatectomy UI. We separated in the analysis the continence rate within 3 months or longer than 6 months after operation, which stand for the early and late recovery of UI after operation, respectively. Relative risk (RR) reductions and their 95% confidence intervals (CIs) were calculated for categorical outcomes.Four studies randomizing 210 cases were included in the meta-analysis. Three studies enrolling 186 cases reported the continence rate within 3 months after radical prostatectomy. The pooled analysis did not show that ES improved early recovery of UI better than did PFMT (RR 1.21; 95% CI = 0.95-1.54, P = .12). All 4 studies provided data for 6-12 months after RP; the pooled analysis did not show a relative benefit (RR = 1.03; 95% CI = 0.88-1.20, P = .73).Based on available evidence, ES enhanced PFMT did not improve the return to continence more than PFMT in men with postprostatectomy UI.

Abstract:BackgroundFacilitating the recurrence of spontaneous voiding is considered to be a way to prevent urinary retention after surgery, which is of great importance in cholecystectomy. This study aimed to assess the effect of transcutaneous electrical acupoint stimulation (TEAS) on spontaneous voiding recovery after laparoscopic cholecystectom.MethodsParticipants who underwent elective laparoscopic cholecystectomy were randomly assigned to either the TEAS group or the sham group. Active TEAS or sham TEAS at specific acupuncture points was conducted intraoperatively and postoperatively. The primary outcome was the recovery speed of spontaneous voiding ability after surgery and secondary outcomes included postoperative urinary retention (POUR), voiding dysfunction, pain, anxiety and depression, and early recovery after surgery.ResultsA total of 1,948 participants were recruited and randomized to TEAS (n = 975) or sham (n = 973) between August 2018 and June 2020. TEAS shortens the time delay of the first spontaneous voiding after laparoscopic cholecystectomy (5.6 h [IQR, 3.7–8.1 h] in the TEAS group vs 7.0 h [IQR, 4.7–9.7 h] in the sham group) (p < 0.001). The TEAS group experienced less POUR (p = 0.020), less voiding difficulty (p < 0.001), less anxiety and depression (p < 0.001), reduced pain (p = 0.007), and earlier ambulation (p = 0.01) than the sham group.ConclusionsOur results showed that TEAS is an effective approach to accelerate the recovery of spontaneous voiding and reduce POUR which facilitates recovery for patients after laparoscopic cholecystectomy.

Abstract:[Purpose] To investigate the effect of electrical stimulation and pelvic floor muscle training on muscle strength, urinary incontinence and erectile function in men with prostate cancer treated by radical prostatectomy. [Subjects and Methods] One hundred twenty-three males were randomized into 3 groups 1 month after RP: (G1, n=40) control; (G2, n=41) guideline: patients were instructed to perform three types of home exercises to strengthen the pelvic floor and (G3, n=42) electrical stimulation: patients in this group were also instructed to perform exercises as group G2, and also received anal electro-stimulation therapy, twice a week for 7 weeks. The primary outcome assessment was based on the measurement of the recovery of pelvic floor muscle strength between groups. Secondary outcomes were: 1 hour Pad Test, ICIQ-SF, IIEF-5 and IPSS. Data were obtained preoperatively and at 1, 3 and 6 months after surgery. [Results] There was no significant difference in the demographic data among groups. Greater urinary leakage and pelvic floor muscle weakness in the first month compared to pre treatment improved after 3 and 6 months postoperative, without difference among groups. [Conclusion] The muscle strength recovery occurs independently of the therapy employed. Pelvic floor exercises or electrical stimulation also did not have an impact on the recovery of urinary continence and erectile function in our study.

Abstract:Background: Low anterior resection syndrome (LARS) is a distressing condition that affects approximately 25-80% of patients following surgery for rectal cancer. LARS is characterized by debilitating bowel dysfunction symptoms, including fecal incontinence, urgent bowel movements, and increased frequency of bowel movements. Although biofeedback therapy has demonstrated effectiveness in improving postoperative rectal control, the research results have not fulfilled expectations. Recent research has highlighted that stimulating the pudendal perineal nerves has a superior impact on enhancing pelvic floor muscle function than biofeedback alone. Hence, this study aims to evaluate the efficacy of a combined approach integrating biofeedback with percutaneous electrical pudendal nerve stimulation (B-PEPNS) in patients with LARS through a randomized controlled trial (RCT). Methods and analysis: In this two-armed multicenter RCT, 242 participants with LARS after rectal surgery will be randomly assigned to undergo B-PEPNS (intervention group) or biofeedback (control group). Over 4 weeks, each participant will undergo 20 treatment sessions. The primary outcome will be the LARS score. The secondary outcomes will be anorectal manometry and pelvic floor muscle electromyography findings and the European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire-Colorectal 29 (EORTC QLQ-CR29) scores. Data will be collected at baseline, post-intervention (1 month), and follow-up (6 months). Discussion: We anticipate that this study will contribute further evidence regarding the efficacy of B-PEPNS in alleviating LARS symptoms and enhancing the quality of life for patients following rectal cancer surgery.
