Controlled-release tablets of Doxazosin Mesylate and Phenoxybenzamine in pre-operative preparation and blood pressure control for pheochromocytoma patients

Abstract:Objective To compare the effects of Doxazosin Mesylate controlled release (CR) tablets and Phenoxybenzamine in the pre-operative management and blood pressure control for pheochromocytoma patients. Methods Totally 40 patients with left pheochromacytoma were operated on in Ruijin Hospital from 1978 to 2006. There were 10 patients in group 1,15 in group 2 and 15 in group 3. The mean age of patients were (45.1±24.5) years old in group 1,(43.2±30.1) years old in group 2 and (44.7±23.4) in group 3. The perioperative systolic blood pressure was analyzed retrospectively in these 40 patients. The patients were divided into the group 1 (non-α receptor blocker),group 2 (Phenoxybenzamine) and group 3 (Doxazosin Mesylate CR) according to the preoperative medicine. The systolic blood pressure at five key time points were recorded as SBP1 (before induction),SBP2 (after anesthesia),SBP3 (during tumor exploration),SBP4 (tumor rescected) and SBP5 (after operation). Results Before anesthesia,the diagnostic pressure of the three groups were significantly different (P<0.01),with the highest level found in group 2,followed by group 1 and group 3. After anesthesia,the diagnostic pressure were decreased in all the three groups,with the lowest level in group 2,followed by group 1 and group 3. During tumor exploration,the pressure was increased in all the three groups,with the lowest level in group 2,followed by group 1 and group 3; there was significant difference between the three groups,but there was no significant difference between group 2 and group 1. After tumor resection,the diagnostic pressure in the three groups was decreased,with no significant difference found in group 2 and group 1,with significant difference between group 1,2 with group 3 (P<0.001). At the end of operation,the pressures were similar between the three groups. The blood pressure in group 3 was more stable and that in group 2 was not so stable. Conclusion For the patients with left pheochromoctyoma,phenoxybenzamine is better in controlling of systolic blood pressure during the preoperative period,and Doxazosin Mesylate CR is more suitable for intra-operation procedures.
