The Significance of Transition Zone Volume and Transition Zone Index in the Diagnosis of Benign Prostatic Hyperplasia

Abstract:Purpose:To evaluate the significance of transition zone volume (TZV) and transition zone index (TZI, the ratio between TZV and total prostate volume ) in the diagnosis of benign prostatic hyperplasia ( BPH ).Methods:The study consisted of 262 men, whom were divided into three groups: group A for 220 BPH patients with symptom, but no urinary retention; group B for 19 BPH patients with urinary retention; and another 23 subjects with no BPH served as normal control. Results:All men completed the international prostate symptom score (IPSS), quality of life (QOL), peak urinary flow rate (Qmax), total prostate volume (TPV) and TZV by transrectal ultrasonography (TRUS), the TZI calculated. There were significant differences on TPV, TZV, TZI, IPSS, QOL and Qmax among group A, group B and control group. The age significantly correlated with TPV(r= 0.24 , P 0.01 ) and TZV (r= 0.25 ,P 0.01 ) respectively. The age correlated negtively with Qmax (r= -0.27 ,P 0.01 ).There are weak correlation between age and IPSS (r= 0.15 ,P 0.05 ) and no correlation between age and QOL (r= 0.11 , P 0.05 ). There are a weak correlation between TPV and IPSS (r= 0.26 ), QOL(r= 0.20 ), Qmax (r= -0.29 ), respectively. But there are a strong correlation between TZV and IPSS (r= 0.04 ), QOL (r= 0.32 ), Qmax (r= -0.43 ) and also strong correlation between TZI and IPSS (r= 0.51 ), QLO (r= 0.41 ), Qmax (r= -0.52 ), respectively. Conclusions:①BPH patients' TPV, TZV, IPS, QOL increase and Qmax decreases with age.②TPV, TZV and TZI in BPH patients with urinary retention are larger than that in BPH patients with no urinary retention.③TZV and TZI were stronger correlative with IPSS, QOL, Qmax than TPV and should been stressed on diagnosis of BPH. ④TZI is a valued parameter in assessing BPH and maybe is useful to predict response to therapies.
