Different bone grafts with guided bone regeneration for treatment of bone defects around implants

Abstract:Background: Recent researches have demonstrated that, the bone grafts with guided bone regeneration technique are an important method of clinical repairing of peri-implant bone defects. Objective: To evaluate the bone formation of different bone grafts combined with guided bone regeneration for the repairing of peri-implant bone defects. Design, time and setting: A controlled observation was performed in the Central Laboratory and Animal Experimental Center of Dalian Medical University (Dalian, Liaoning, China) between May 2004 and March 2005. Materials: Four male healthy mongrel dogs, weighing 11-15 kg; Bio-oss bone dust was purchased from USA; Titanium screw and titanium membrane were offered by Xi'an Zhongbang Biomaterials Co., Ltd (China). Methods: After the mandibular premolars were extracted for 12 weeks, surgical implantation of 4 titanium screws was performed on each side of the mandible of four mongrel dogs. Three-wall bone defects at a size of 3 mm high, 3 mm wide and 3 mm long, were created at the buccal side of each implant. All mandibles were divided into four groups: The osteotomy sites were separately filled with autogenous chip bone, Bio-oss mineral bone, and the mixture of both bones. Control group was not subject to the implantation. The surgical areas of the left mandible were covered with titanium membranes, whereas the right mandible without membrane was used as controls. Main outcome measures: The microstructure of bone defects area, the mass and structure of newly formed bone, as well as the degradation of Bio-oss bone dust were detected under light microscopy. Results: All 4 dogs were involved in the result analysis. No titanium membrane exposed at 9 weeks postoperatively. Other two dogs appeared the exposure and shedding of titanium membrane at 10 and 12 weeks, respectively. They were healed by debridements. All 32 titanium screws binded with the bone well, without any loosening or drop. Pseudomembrane formed on the surface of bone defects area, where the titanium membrane dropped. The mass of bone resorption was the most highest in the control group compared to other groups (P < 0.01); The mass in the mixture group was obviously lower than that in autograft group (P < 0.05). Both quantity and quality of regenerated bone in the mixture graft specimens were superior to that of single Bio-oss grafted or single autologous bone grafted specimens. Whether the membranes were utilized or not, the mass of bone resorption was larger at 16 weeks than those at 9 weeks in all the groups. Bone grafts with titanium membranes were subjected to greater resorption than those without membrane coverings (P < 0.01). Conclusion: It is feasible to repair the peri-implant bone defects using autologous bone in combination with Bio-oss mineral bone. The mixture may significantly promote the bone regenerating, and titanium membrane promotes the absorption of bone graft.
