Surgical Management and Timing of Iatrogenic Mjnry to Bile Duct

Abstract:Objective:To review iatrogenic bile duct injury and evaluate the factors influencing the outcome in different situations related to iujury type?timing for surgical repair?repair technique and the procedure.Methods:All 28 patients had bile duct injuries after the primary cholecystectomy performed previously. According to their injury types,the postoperative period for surgical biliary reconstruction and the selected surgical procedure,we analyzed the affecting factors and evaluated the results of treatment.Results:46.4% patients had successful reconstruction of bile duct in the initial repair. The reasons for failure in reconstruction include:①The surgical made was chosen inadequately.②The biliary stent was inserted at wrong portion,or not good timing.③The period of second stage repair was either too long or too short.Conclusion:①End to end anastomosis is only applied to those patients whose biliary injury is Bismuth type Ⅰ~Ⅱ lesions no cauterizing biliary damage.②When patients are not suitable for end to end anastomosis hepaticojejunostomy Roux Y anastomosis first is the first choice.③If the biliary injury is not recognized by the operator at the time of the initial operation,the delayed repair of bile duct may be carried out unless the biliary peritonitis or biliary fistula is present.④If the anastomotic orifice is less than 10 mm in diameter,the biliary stent should be inserted during the repair taken out about one year later.
