Iatrogenic bile duct injury:management and re-operation time

Abstract:Objective To investigate the management and the re-operation time of iatrogenic bile duct injury.Method The clinical data of 37 patients with iatrogenic bile duct treated from January 2004 to January 2008 were analyzed retrospectively.Results The main causes for iatrogenic bile duct injury were open cholecystectomy,laparoscopic cholecystectomy and common bile duct exploration,which accounted for 94.6 %(35/37).For 36 patients were performed operations among 37 patients,3 were found iatrogenic bile duct injury in the operation and were performed bile duct reconstruction operation immediately,accounted for 8.1%,9 patients were performed exploratory laparotomy within 24 h,accounted for 24.3%,38 bile duct reconstruction operations were performed,34 patients were followed up for 6 months to 1 year and the rate of satisfactory outcome was 82.3%.Conclusion Iatrogenic bile duct injury often occurs during cholecystectomy.Good knowledge of the anatomy and the possible variations of the bile ducts,strictly controlling of operative indications and recognition of bile cyst,common hepatic duct and common bile duct are the keys to prevent iatrogenic bile duct injury.Management according to the time and types of bile duct injury,early finding and timely correct management are very important to the prognosis.Roux-en-Y biliary-enteric anastomosis is the most common and the most effective method for the bile duct reconstruction.
