Analysis of post-operative effect of 29 patients with iatrogenic bile duct injury

Abstract:Objective To study the operative time and repair methods in 29 cases of iatrogenic bile duct injury,in order to improve the long-term post-operative effects.Methods Twenty-nine cases of iatrogenic bile duct injury admitted from Jan 1999 to Dec 2010 were studied retrospectively.Results According to the the classification quoted from the 《Guidelines of prevention and treatment in biliary tract injury》,formulated by the Biliary Surgery Group of Surgery Branch of the Chinese Medical Association,there were type Ⅰ bile duct injury in 3 patients,type Ⅱ1 in 22 patients,type Ⅱ2 in 3 patients,and type Ⅱ3 in 1 patient.Twelve cases underwent instant repair,6 cases of early repair,and 11 cases of late repair.The operative methods were: aberrant bile duct ligation in 1 case,bile duct repair in 1 case,bile duct repair with T-tube drainage in 9 cases,biliary duct end-to-end anastomosis with T-tube drainage in 2 cases and cholangiojejunal Roux-en-Y anastomosis in 16 cases.Among the 29 patients,6 were lost to follow-up and one died.The average follow-up time was 4.7 years.Among the 22 followed-up patients,20 cases recovered from a single repair procedure.All of the 22 followed-up patients had good long-term effect without biliary duct stricture and jaundice from the last operation.Conclusions Laparoscopic cholecystectomy is one of the main causes of proximal hepatic bile duct injury.Good long-term prognosis can be obtained if we choose a good operative time,and the right repair technique.T-tube drainage is placed for about 3 months,and it can reduce the incidence of biliary leakage and bile duct stricture.
