Abstract:ALTHOUGH the value of intensive psychotherapy for persons with schizophrenia has been challenged (e.g., Gunderson et al. 1984; May 1968), there are still many clinicians and others who believe that some kind of interpersonal work with severely disordered patients, including people with schizophrenia, is extremely helpful and perhaps essential (Tomecek 1990). A crucial problem for exploring this contention is the complex interaction among four components: the measurement of efficacy, the specific nature of the interpersonal encounters, the patients' broader lives and treatment contexts, and the exact characteristics of the patients for whom these encounters may be helpful. The former blanket acceptance among many groups of the value of psychotherapy for schizophrenia is clearly not valid. But this by no means indicates that intensive interpersonal contacts of some kind for some people with schizophrenia are not important. For example, Bullard's (1960) classic paper on dealing with paranoid patients may still be extremely valuable for working with such people. In order not to bring excessively quick closure to this issue, there needs to be far more inquiry to identify specifically what kinds of contacts may be important for which people. Then the efficacy and optimal context of these contacts can be tested.

Abstract:To have a complete human science in the mental health field it is essential to give adequate attention to both the objective and the subjective data related to people with psychiatric disorders. The tendency in the past has been to ignore or discount one or the other of these data sources. Subjective data are particularly neglected, sometimes considered (only) part of the "art" of medicine since the usual methodologies of the physical sciences in themselves are not adequate to reflect the nature, elusiveness, and complexity of human subjective experience. The complete experience of hallucinated voices, for instance, often includes not only the voices themselves but also terrible anguish and terrifying inability to concentrate. But even such descriptors fall unnecessarily short of reflecting the data of the experience, thus leaving research, theory, and treatment with incomplete information. To represent adequately the subjective data it is essential to recognize that besides the usual discursive knowledge and methods of traditional physical science, a second kind of knowledge and method is required to reflect the depth of human experience. To accomplish this, we must employ approaches to narrative and the arts that are uniquely capable of capturing the nature of these experiences. Only by attending seriously in our research, training, theory, and practice to the unique nature of subjective data is it possible to have a true human science for our field.

Abstract:This report develops the notion that the mental health field often ignores or avoids many aspects of patients' reports about their experiences of schizophrenia and related disorders. When closer attention is paid to patients' reports of their experiences, one key phenomenon suggested is the importance of the interaction between the person and the disorder. This interaction evolves over time and has numerous implications for understanding, studying, and treating schizophrenia and related disorders. Specifically, the interaction involves the person as a goal-directed being, the person's feelings as helping to drive the phases of disorder and improvement, and the existence of psychological regulatory mechanisms.

Abstract:Clinical experiences and the questions they generate are a rich potential source for increased understanding of psychopathology and its treatment. Yet a chasm frequently appears to exist isolating clinical observations from the "real" research viewed as essential to developing and testing their implications. This report suggests that a concept of real research limited to large-sample elaborate studies is too narrow for optimal progress in the field. Research principles for developing a clinical intuition systematically, in the clinical context, are described. The important values as well as limitations of this kind of research are noted.

Abstract:The recent emphasis for research on psychiatric disorders focuses on supposed underlying processes and their biological elements. Although such a direction may be fruitful, it would be a mistake to ignore more integrated approaches for understanding the mysteries of schizophrenia or other psychiatric disorders. The complexity of the biopsychosocial paradigm should not lead to ignoring its potential value for understanding these disorders. The role of subjective data other than those identified as "symptoms" is also likely to be crucial.

Abstract:Although dichotomously defined for clinical purposes, psychosis may exist as a continuous phenotype in nature. A random sample of 7076 men and women aged 18-64years were interviewed by trained lay interviewers with the Composite International Diagnostic Interview (CIDI). Those with evidence of psychosis according to the CIDI were additionally interviewed by psychiatrists. For the 17 CIDI core psychosis items, we compared a psychiatrist's rating of hallucinations and/or delusions (Clinical Psychosis; sample prevalence 4.2%) with three other possible positive CIDI ratings of the same items: (i) symptom present, but not clinically relevant (NCR Symptom; sample prevalence 12.9%); (ii) symptom present, but the result of drugs or somatic disorder (Secondary Symptom; sample prevalence 0.6%); (iii) symptom appears present, but there is a plausible explanation (Plausible Symptom; sample prevalence 4.0%). Of the 1237 individuals with any type of positive psychosis rating (sample prevalence 17.5%), only 26 (2.1%) had a DSM-III-R diagnosis of non-affective psychosis. All the different types of psychosis ratings were strongly associated with the presence of psychiatrist-rated Clinical Psychosis (NCR Symptom: OR=3.4; 95% CI: 2.9-3.9; Secondary Symptom: OR=4.5; 95% CI: 2.7-7.7; Plausible Symptom: OR=5.8; 95% CI: 4.7-7.1). Associations with lower age, single marital status, urban dwelling, lower level of education, lower quality of life, depressive symptoms and blunting of affect did not differ qualitatively as a function of type of rating of the psychotic symptom, were similar in individuals with and without any CIDI lifetime diagnosis, and closely resembled those previously reported for schizophrenia. Presence of any rating of hallucinations was strongly associated with any rating of delusions (OR=6.7; 95% CI: 5.6-8.1), regardless of presence of any CIDI lifetime diagnosis. The observation by Strauss (1969. Hallucinations and delusions as points on continua function. Arch. Gen. Psychiatry 21, 581-586) that dichotomously diagnosed psychotic symptoms in clinical samples are, in fact, part of a continuum of experiences, may also apply to the general population. The boundaries of the psychosis phenotype may extend beyond the clinical concept of schizophrenia.

Abstract:The author discusses problems of the typological diagnostic system currently used in psychiatry, which focuses primarily on symptoms in classifying psychiatric disorders. He describes a comprehensive diagnostic system that would involve routine evaluation of five patient characteristics: symptoms, circumstances associated with symptoms, previous duration and course of illness, quality of personal relationships, and level of work function. He suggests that such a multivariable approach to psychiatric diagnosis can provide valuable information about the patient to clinicians, researchers, and other mental health workers.

Abstract:Introduction: The last decade has witnessed a resurgence of interest in the clinician's subjectivity and its role in the diagnostic assessment. Integrating the criteriological, third-person approach to patient evaluation and psychiatric diagnosis with other approaches that take into account the patient's subjective and intersubjective experience may bear particular importance in the assessment of very young patients. The ACSE (Assessment of Clinician's Subjective Experience) instrument may provide a practical way to probe the intersubjective field of the clinical examination; however, its reliability and validity in child and adolescent psychiatrists seeing very young patients is still to be determined. Methods: Thirty-three clinicians and 278 first-contact patients aged 12-17 years participated in this study. The clinicians completed the ACSE instrument and the Brief Psychiatric Rating Scale after seeing the patient, and the Profile of Mood State (POMS) just before seeing the patient and immediately after. The ACSE was completed again for 45 patients over a short (1-4 days) retest interval. Results: All ACSE scales showed high internal consistency and moderate to high temporal stability. Also, they displayed meaningful correlations with the changes in conceptually related POMS scales during the clinical examination. Discussion: The findings corroborate and extend previous work on adult patients and suggest that the ACSE provides a valid and reliable measure of the clinician's subjective experience in adolescent psychiatric practice, too. The instrument may prove to be useful to help identify patients in the early stages of psychosis, in whom subtle alterations of being with others may be the only detectable sign. Future studies are needed to determine the feasibility and usefulness of integrating the ACSE within current approaches to the evaluation of at-risk mental states.

Abstract:Our reintroduction of the concept of negative symptoms in schizophrenia (Strauss et al, 1974) has been followed by much productive research on this topic. As in the work of Andreason, Crowe, and others, the focus of most of this research has been on improving descriptive assessments of these symptoms (Andreason, 1982) and exploring their biological correlates (Crowe, 1981). This work has been extremely important, but psychological and social aspects of negative symptoms are also important (Barton, 1959; Wing & Brown, 1970; Gruenberg, 1967), and in these areas there has been little research. Nevertheless, it is crucial to explore possible psychological and social factors in negative symptoms in order to understand the symptoms more fully and to provide a basis for more adequate prevention and treatment. It is also important to clarify psychological and social factors in these symptoms to provide a basis for more effective biological research, since the heterogeneity of negative symptoms at a psychological level could hide significant biological correlates.
Greater than the sum of the parts: a qualitative content analysis of what constitutes a good treatment in the inpatient setting

Abstract:Background The evaluation of psychotherapy is guided by established concepts, such as efficacy and effectiveness, and acceptability. Although these concepts serve as valid proxies, little is known about corresponding criteria for those directly involved in this treatment. This study aimed to explore inpatients' and health professionals' definitions of a good treatment in the inpatient setting. Methods Fifteen semi-structured interviews were conducted in a private psychiatric clinic in Switzerland and structured by qualitative content analysis. Different subsamples of the inpatient setting (patients N = 5; psychiatrists N = 5; other health professionals N = 5) were interviewed. Results In total, 546 text passages were grouped in 10 superordinate categories and identified as relevant for the concept of a good treatment. Participants stressed patient-specific (i.e., new insights; basic attitudes), treatment-specific (i.e., therapy methods and expertise; treatment success; therapy setting), and relationship-based (i.e., communication and feedback; relationships within the clinical setting; overcoming challenges and hurdles) components that are indispensable for a good therapeutic process. Components that are related to the clinical inpatient setting (i.e., setting and organization of the clinic; code of conduct) were also highlighted. Conclusions Patients' and health professionals' definitions of what constitutes a good treatment entails a wide array of aspects. The clinical setting is seen to offer unique components that are emphasized to have a healing effect.

Abstract:A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions. Implementing mental health care in physical care services is still insufficient. Thus, interdisciplinary communication across treatment providers is essential. The standardized language provided by the diagnostic statistical manual favors a clear conceptualization. However, this approach might not focus on the individual, as thinking in categories might impede recognizing the continuum from healthy to diseased. Psychoanalytic theory is concerned with an individual's unconscious conflictual wishes and motivations, manifested through enactments like psychic symptoms or (maladaptive) behavior with long-term consequences if not considered. Such modifiable internal and external factors often are inadequately treated. However, together with the physical chronic condition constraints, these factors determine degrees of freedom for a self-determined existence. The effect of therapeutic interventions, and especially therapy adherence, relies on a solid therapeutic relationship. Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis's focus on attachment problems. This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research. A clinical example is discussed. Additionally, further directions for research are given. The theoretical frame in psychoanalytic therapies is the affect-cognitive interface. Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other; shaping this awareness is the main intervention point. The interactional ingredients, the patient's inherent bioenvironmental history meeting the clinician, are relevant variables. Several intrinsic, subliminal parameters relevant for changing behavior are observed. Therapeutic interventions aim at supporting the internalization of the superego's functions and at making this ability available in moments of self-reflection. By supporting mentalization abilities, a better understanding of oneself and higher self-regulation (including emotional regulation) can lead to better judgments (application of formal logic and abstract thinking). Thus, this facilitates enduring behavior change with presumably positive effects on mental and physical health.

Abstract:Background: The hospital intensive care unit (ICU) environment encompasses sick patients who present for care in health crisis. Healthcare in this setting is complex, often involving the co-ordination of multiple professional teams, all under significant time pressures. The sequelae for staff interacting in this dynamic and often volatile setting are variable, depending upon their coping skillset and their familiarity with the stressors. Aims: The primary aim of this study was to describe and in doing so, normalize the behavioural responses expressed by ICU doctors (Intensivists) in response to stressful workplace events. The secondary aim was to identify those responses that contributed to resilience. Methods: A prospective qualitative study of senior Intensivists using a semi-scripted iterative interview. Data were transcribed and thematically analysed with verbatim quotations selected to support coding choices. Results: Nineteen experienced Intensivists from three sites in Australia and Israel participated. Clinicians described conscious, physiological and professional responses to stressors, including sense-making and taking time to process information with appropriate support. Two of the most important mitigation processes revealed were the use of reflective learning and preventative practice changes to prevent future errors. These were overlaid with the importance of disclosure and transparency in clinical work. Conclusions: Repeated exposure to stressful events potentiates burnout, wherein staff no longer experience satisfaction and enjoyment in what they do. This paper presents the behavioural responses that experienced Intensivists described in relation to stressful events in the ICU, including steps taken to mitigate the effects of these events on their personal well-being.

Abstract:Are there patterns of characteristics in psychiatric patients, different from traditional diagnostic considerations, that could provide important information for understanding, treatment, and research? To pursue this question, this report describes an investigation of clinical and demographic characteristics in a representative sample of first admissions for functional psychiatric disorder. Analyzing the patterns of these characteristics showed that social class had a particularly key role relating to a larger number of characteristics than did symptom and functioning measures. Symptoms when analyzed together revealed replicated factors not corresponding to diagnostic types. The implications of these and other findings for considering a broad perspective in conceptualizing, studying, and treating psychiatric disorder are discussed.

Abstract:BACKGROUND: A number of studies have documented psychosocial problems, psychiatric morbidity and impaired quality of life in primary care patients.OBJECTIVE: The aim of this trial was to test the usefulness of the joint use of different diagnostic interviews and self-rated questionnaires.METHODS: Two hundred consecutive patients in a primary care practice in Italy underwent the Structured Clinical Interview for DSM-5 and the Semi-Structured Interview for the Diagnostic Criteria for Psychosomatic Research (DCPR) in its recently revised form. As self-rated evaluations, the PsychoSocial Index, the Short-Form Health Survey and the Illness Attitude Scales were administered.RESULTS: There were 46 patients (23%) with at least 1 DSM-5 diagnosis. Eighty-eight patients (44%) had at least 1 DCPR diagnosis, mainly maladaptive illness behavior (26.5%), allostatic overload (15.5%) and demoralization (15%). There were 47 (23.5%) patients who had a DCPR diagnosis only; 5 subjects (2.5%) had a DSM diagnosis only. Patients with DCPR syndromes displayed significantly higher self-rated levels of stress, psychological distress and maladaptive illness behavior and significantly lower levels of quality of life and well-being than patients with no diagnoses.CONCLUSIONS: In a busy clinical setting, a simple self-rated questionnaire such as the PsychoSocial Index may afford a useful tool to unveil patient current distress. The DCPR can provide clinical information for a substantial number of patients who do not satisfy DSM-5 classification criteria and yet present with psychosocial problems, as measured by self-rated scales. The DCPR may improve the assessment and treatment plan of primary care psychologists or consulting psychiatrists.

Abstract:One improvement from the second to the third edition of the Diagnostic and Statistical Manual of Mental Disorders was to replace clinicians' subjective interpretations of mental disorders with objective descriptions of signs and symptoms that could be rated reliably across investigators. Along with clinicians' subjective impressions, however, the subjective experiences of the person with the mental disorder were minimized. This information could be valuable, as people's subjective experiences of disorders may indicate major underlying processes and be different from how characteristics of disorders appear objectively to outside observers. The authors suggest that empirically derived, patient-subjective characteristics of mental disorders be incorporated into future editions of the DSM and the ICD. Not only will these data offer important information that will help to enhance the accuracy of the diagnostic categories of the DSM and ICD, but such data also may serve to enhance clinicians' abilities to conceptualize accurately and empathically treat these disorders in their patients. Examples of patient-subjective criteria and their relationship to current DSM criteria are examined for borderline personality disorder and schizophrenia and suggestions for DSM-V and ICD-11 are offered. Diagnostic criteria that accurately reflect patients' subjective experience could also increase clinicians' ability to empathize with patients, one of the most important variables in treatment alliances.

Abstract:Kraepelin's description of "dementia praecox" was a synthesizing influence in a field that had for too long been fragmented in its thinking about severe psychiatric disorders. Now, once again, the field is attempting to deal with some of the complexities involved in psychopathology and its evolution, complexities that have been reemphasized by newer descriptive research methods. To account for patients deviations from stereotyped diagnostic and prognostic concepts, multiple aspects of course of disorder have been defined and principles of multiaxial diagnosis supported. These principles can provide a basic structure for practice and research using a biopsychosocial orientation within the context of a system model.
