The best way to reflect the subjectivity of a person may be by means of the subjectivity of another. To take account of both subjective experience and objective measures in the course of mental disorders, a new concept is needed. This may be the person's story.

Abstract:This report develops the notion that the mental health field often ignores or avoids many aspects of patients' reports about their experiences of schizophrenia and related disorders. When closer attention is paid to patients' reports of their experiences, one key phenomenon suggested is the importance of the interaction between the person and the disorder. This interaction evolves over time and has numerous implications for understanding, studying, and treating schizophrenia and related disorders. Specifically, the interaction involves the person as a goal-directed being, the person's feelings as helping to drive the phases of disorder and improvement, and the existence of psychological regulatory mechanisms.

Abstract:A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions. Implementing mental health care in physical care services is still insufficient. Thus, interdisciplinary communication across treatment providers is essential. The standardized language provided by the diagnostic statistical manual favors a clear conceptualization. However, this approach might not focus on the individual, as thinking in categories might impede recognizing the continuum from healthy to diseased. Psychoanalytic theory is concerned with an individual's unconscious conflictual wishes and motivations, manifested through enactments like psychic symptoms or (maladaptive) behavior with long-term consequences if not considered. Such modifiable internal and external factors often are inadequately treated. However, together with the physical chronic condition constraints, these factors determine degrees of freedom for a self-determined existence. The effect of therapeutic interventions, and especially therapy adherence, relies on a solid therapeutic relationship. Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis's focus on attachment problems. This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research. A clinical example is discussed. Additionally, further directions for research are given. The theoretical frame in psychoanalytic therapies is the affect-cognitive interface. Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other; shaping this awareness is the main intervention point. The interactional ingredients, the patient's inherent bioenvironmental history meeting the clinician, are relevant variables. Several intrinsic, subliminal parameters relevant for changing behavior are observed. Therapeutic interventions aim at supporting the internalization of the superego's functions and at making this ability available in moments of self-reflection. By supporting mentalization abilities, a better understanding of oneself and higher self-regulation (including emotional regulation) can lead to better judgments (application of formal logic and abstract thinking). Thus, this facilitates enduring behavior change with presumably positive effects on mental and physical health.

[Psychosis sight from the complexity research--a model for the examination of the self organisation of a dysfunctional self]

Abstract:Nowadays the multifactorial genesis of psychotic illnesses is undisputed. The question of the integration of the multilayered results is open from the biological, emotional and social area. Based on the results of the complexity research (chaos theory, theory of the self organisation) the hypothesis was formulated for the emergence of psychotic illnesses that these are understood themselves as an expression of a dysfunctional self. Such a dysfunctional self arises in a self organizing process. Material and method: The definition of the self was newly formulated. This happened on the knowledge of the complexity research and a bio- psycho-social model of illness and health. Ten main categories were defined, which seem essential for the development of a healthy, this means a flexible and autonomous self. In further consequence, based on these categories, a semistructured interview guide was developed. With this interview guide the information about the quality of the self organising process of the development of the self was collected. This guide was tested in the context of a master thesis in a multidimensional interview with a schizophrenic patient and his family. This interview was taken on video tap. The information on video became transcribed. Than the information was examined qualitatively to two aspects. This happened with the help of a qualitative contents analysis. The first aspect concerned the decision of the assignment to one or several of the ten categories. In a 2nd step it was judged, whether the examined information has to be classified as beneficial for the development of a healthy self or as impeding. Results: The developed and used examination model is suitable to make the investigation of a self organizing self possible. At the individual case analysis of a patient with a schizophrenic psychosis the inhibiting influences for the development of a healthy self predominated at a large part of the 10 categories. This corresponded to the hypothesis. Conclusions: The used examination model seems suitable, to examine qualitatively the development of the self in psychotic people. At a more comprehensive study it is necessary to shorten the duration of the interview and to carry out the evaluation directly of video or CD-ROM The result of the qualitative contents analysis showed obviously, in which categories the impeding effect on the development of a healthy self outweighed. This result supports the idea of an individualized multidimensional therapeutic concept, which supports the development of a healthy self. Out of this could surrender to consequences regarding an individual, multidimensional therapy which promotes the development of a healthy itself.

Abstract:Importance Psychiatric disorders may come and go with symptoms changing over a lifetime. This suggests the need for a paradigm shift in diagnosis and treatment. Here we present a fresh look inspired by dynamical systems theory. This theory is used widely to explain tipping points, cycles, and chaos in complex systems ranging from the climate to ecosystems. Observations In the dynamical systems view, we propose the healthy state has a basin of attraction representing its resilience, while disorders are alternative attractors in which the system can become trapped. Rather than an immutable trait, resilience in this approach is a dynamical property. Recent work has demonstrated the universality of generic dynamical indicators of resilience that are now employed globally to monitor the risks of collapse of complex systems, such as tropical rainforests and tipping elements of the climate system. Other dynamical systems tools are used in ecology and climate science to infer causality from time series. Moreover, experiences in ecological restoration confirm the theoretical prediction that under some conditions, short interventions may invoke long-term success when they flip the system into an alternative basin of attraction. All this implies practical applications for psychiatry, as are discussed in part 2 of this article. Conclusions and Relevance Work in the field of dynamical systems points to novel ways of inferring causality and quantifying resilience from time series. Those approaches have now been tried and tested in a range of complex systems. The same tools may help monitoring and managing resilience of the healthy state as well as psychiatric disorders.

Abstract:<p>Beliefs about the malleability of attributes, also known as mindsets, have been studied for decades in social-personality psychology and education. Here, I review the many applications of mindset theory to clinical psychology and psychotherapy. First, I review social psychological and cognitive neuroscience evidence that mindsets and mindset-related messages are, to a large extent, focused on emotional tolerance. Specifically, the growth mindset, or the belief that attributes are malleable, encourages confronting and tolerating anxiety, frustration, and disappointment in healthy and adaptive ways that promote resilience, whereas the fixed mindset and related messages discourage the experience of these emotions and often leads to helplessness. Second, I review the emerging research on the anxiety mindset and discuss its relevance to clinical work. A model is proposed illustrating connections between mindsets, emotion regulation strategies, treatment preferences, and outcomes. Case examples are used to illustrate practical applications. I conclude that mindsets can inform psychotherapy, research, and public policy.</p>

Abstract:INTRODUCTION: This issue of In Session in The Journal of Clinical Psychology offers in depth case studies of psychotherapy for persons with differing types of schizophrenia spectrum disorders across international settings.METHOD: To understand the context and import of this work, this commentary explores commonalities of this work with past work including respect for patients' autonomy, compassion, and attention to the therapeutic relationship. The commentary then discusses themes which harken to the past including framing psychosis as something than can be understood jointly by therapist and patient rather than being unilaterally imposed.RESULTS: It is suggested that this work is emblematic of new movements in the literature which focus on the person who is experiencing schizophrenia.CONCLUSIONS: This focus extends to understanding the purposes, place of the person in the world and their possibilities, which enables them to make sense of what they need and how want to manage their own recovery.

Abstract:Introduction Psychiatry is one of the most fast developing and agile discipline within human medicine. But more work is necessary to complete these advances. Objectives I address the following questions: How does the future of psychiatry look in the eyes of early career psychiatrists? What strengths, weaknesses opportunities and threats will come? And what can we learn from different mental health systems and reagions? Methods Oral or written statements to the raised questions followed optimally by a discussion Results In low- and middle-income countries, a vast majority of people with mental disorders do not receive adequate treatment. Even in high income countries, roughly a third of people with severe forms of mental illness are not receiving the appropriate therapy. Laws concerning mental health are outdated in different countries. The protection of the human rights of the mentally ill is still incomplete and imperfect. The emphasis on economic gain and the digitalization of medicine in recent years has not helped. And new technical advancements such as artifical intelligence are becoming more important. Toward a philosophical structure for psychiatry

Abstract:This article, which seeks to sketch a coherent conceptual and philosophical framework for psychiatry, confronts two major questions: how do mind and brain interrelate, and how can we integrate the multiple explanatory perspectives of psychiatric illness? Eight propositions are proposed and defended: 1) psychiatry is irrevocably grounded in mental, first-person experiences; 2) Cartesian substance dualism is false; 3) epiphenomenalism is false; 4) both brain-->mind and mind-->brain causality are real; 5) psychiatric disorders are etiologically complex, and no more "spirochete-like" discoveries will be made that explain their origins in simple terms; 6) explanatory pluralism is preferable to monistic explanatory approaches, especially biological reductionism; 7) psychiatry must move beyond a prescientific "battle of paradigms" to embrace complexity and support empirically rigorous and pluralistic explanatory models; 8) psychiatry should strive for "patchy reductionism" with the goal of "piecemeal integration" in trying to explain complex etiological pathways to illness bit by bit.

Abstract:Objective: Constructing a meaningful biopsychosocial model for the mental health field has been extremely elusive. Identifying the linkages between the biological, psychological and social domains has been especially daunting. There has been important progress in clarifying general correlations of certain social factors related to the mental health of individuals and in developing training programs to recognize these social factors. However, efforts have usually focused on broad correlations and there have been serious deficiencies in developing methods for understanding and dealing with the specific processes happening at the psychological and social interface. For this reason, it would be important to be able to do such things as for example have a means to clarify the processes that connect the individual's mental health and its specific interactions with his or her social class. In this report we suggest two approaches that can contribute to solving this problem. Methods: We will describe approaches from the fields of anthropology and microhistory that link the specific experiences of the individual and the nature of the social context in which he or she finds him/herself. Results: Careful application of certain anthropological and history study methods that take seriously the specific interactions between the environmental situation and the individual can provide approaches to improved understanding of the relevant variables and the causal links between psycho and social in the biopsychosocial model. Conclusions: Teaching and applying these principles in treatment and research can contribute to a more effective model of biopsychosocial interactions in the mental health field.

Abstract:Arthur Kronfeld (1886–1941) was a major representative of the psychological approach to psychiatry during the interwar period. In his major work from 1930, Perspectives on Nervous and Mental Diseases (Perspektiven der Seelenheilkunde), Kronfeld developed a psychological analysis of the structure of intentionality underlying psychotic manifestations. Yet even though some consider Kronfeld to be perhaps the most outstanding theoretician of psychological psychiatry, psychiatrists from the 1930s to the present day have manifested a peculiar reluctance to deal with the concept of intentionality. This article will illustrate Kronfeld's scientific individuality through a survey of some important aspects of his biography, his philosophical background, and his psychiatric work.

Abstract:Current approaches to the treatment of non-affective psychosis include elements of mentalization-based treatment and the potential in enhancing mentalizing capacity in this patient group has been widely emphasized. This article presents the "psychotic identity dilemma", a concept by Stavros Mentzos, and a therapeutic approach considering this concept as a valuable complementary addition to these treatments. The idea of a dilemma between closeness and distance, which in itself cannot be represented mentally at first, helps to respond to specific needs of patients with psychotic disorders by placing the treatment focus on fundamental interpersonal processes. Following this train of thought, this article attempts to shed light on the importance of the "real relationship" between therapist and patient as well as the exploration of the "here and now", especially at the beginning of psychotherapeutic treatment. Two treatment modes are suggested, one characterized by the experience of interpersonality within the therapeutic relationship and a second one characterized by the reflection of interpersonal phenomena. These modes are framed by Stavros Mentzos' concept of an identity dilemma. We describe how mentalizing first needs to be addressed implicitly in a tolerable, exemplary relationship in which closeness and distance are regulated based on the therapist's countertransference, then explicitly. A series of interventions are described, which allow for moments of shared attention, promote intentionality and contingency and, later in the course of therapy, help to integrate experiences into narratives.

Abstract:The paper by Northoff et al. [1] is an outstanding example of the way to overcome the translational crisis in contemporary psychiatry. We completely agree with their proposal that the focus on first-person experiences of time and space is in an ideal position to address the anomalies of subjectivity that we find in the clinic of psychopathological disorders and, if extended to the brain's spatiotemporal topographic-dynamic structure, is the key to addressing the anomalies of neurobiological mechanism. We hope that this approach to the mind-brain gap will find an increasingly favourable echo in the field of translational research and yet we wish to focus on a common limitation in Northoff et al.'s model and the other translational hypotheses of which we are aware. Northoff et al. are on the right track in that they put subjective experience—and not other non-specific parameters (e.g. behaviour)—at the centre of translational studies. However, this is but a step however great in the right direction. A science of the human brain cannot but take into account the individual peculiarities that characterise the human condition. How this is achievable is not for us to formulate. We can, however, say what the clinical, epistemological and ethical necessities are for insisting in this direction.

Abstract:<p>The current management of patients with primary psychosis worldwide is often remarkably stereotyped. In almost all cases an antipsychotic medication is prescribed, with second‐generation antipsychotics usually preferred to first‐generation ones. Cognitive behavioral therapy is rarely used in the vast majority of countries, although there is evidence to support its efficacy. Psychosocial interventions are often provided, especially in chronic cases, but those applied are frequently not validated by research. Evidence‐based family interventions and supported employment programs are seldom implemented in ordinary practice. Although the notion that patients with primary psychosis are at increased risk for cardiovascular diseases and diabetes mellitus is widely shared, it is not frequent that appropriate measures be implemented to address this problem. The view that the management of the patient with primary psychosis should be personalized is endorsed by the vast majority of clinicians, but this personalization is lacking or inadequate in most clinical contexts. Although many mental health services would declare themselves "recovery‐oriented", it is not common that a focus on empowerment, identity, meaning and resilience is ensured in ordinary practice. The present paper aims to address this situation. It describes systematically the salient domains that should be considered in the characterization of the individual patient with primary psychosis aimed at personalization of management. These include positive and negative symptom dimensions, other psychopathological components, onset and course, neurocognition and social cognition, neurodevelopmental indicators; social functioning, quality of life and unmet needs; clinical staging, antecedent and concomitant psychiatric conditions, physical comorbidities, family history, history of obstetric complications, early and recent environmental exposures, protective factors and resilience, and internalized stigma. For each domain, simple assessment instruments are identified that could be considered for use in clinical practice and included in standardized decision tools. A management of primary psychosis is encouraged which takes into account all the available treatment modalities whose efficacy is supported by research evidence, selects and modulates them in the individual patient on the basis of the clinical characterization, addresses the patient's needs in terms of employment, housing, self‐care, social relationships and education, and offers a focus on identity, meaning and resilience.</p>

THE THEORY OF MIND AND PSYCHOTIC SYMPTOMS PHENOMENOLOGY IN SUBSTANCE-INDUCED PSYCHOTIC DISORDER AND SCHIZOPHRENIA

Abstract:Objective: The study aimed to determine clinical indicators that could be used to differentiate between patients diagnosed with substance-induced psychotic disorder (SIPD) and patients diagnosed with schizophrenia by comparing their psychotic symptoms and theory of mind (ToM). Methods: The study included 43 male patients diagnosed with schizophrenia according to DSM-5 criteria and 43 male patients diagnosed with SIPD. The patients were administered the Sociodemographic and Clinical Data Form, Scale for the Assessment of Positive Symptoms (SAPS), Scale for the Assessment of Negative Symptoms (SANS), Psychotic Symptom Evaluation Scale (PSYRATS), and Reading the Mind in the Eyes Test (RMET). Results: In patients diagnosed with schizophrenia, the scores on SAPS subscales for structural thought disorder and bizarre behavior, as well as SANS total scores, were significantly higher compared to patients diagnosed with SIPD (z=2.679, p=0.007; z=2.984, p=0.003; z=6.916, p<0.001). The scores for recognizing negative and neutral expressions on the RMET were significantly higher in patients with SIPD than in patients with schizophrenia (z=3.540; p<0.001; z=4.404, p<0.001). It was found that as the scores on the SANS total and Affect Blunting or Flattening subscale, as well as the scores on the SAPS Bizarre Behavior subscale decrease, the probability of having SIPD increases. Conclusion: In patients diagnosed with SIPD, there are fewer disorganized and negative symptoms compared to patients diagnosed with schizophrenia. Patients with SIPD can recognize negative and neutral expressions better than patients with schizophrenia. When making a differential diagnosis between SIPD and schizophrenia, as blunting in affect, total negative symptoms, and severity of bizarre behavior decrease, the probability of being diagnosed with SIPD increases relative to the probability of being diagnosed with schizophrenia.

Abstract:Since its beginnings psychiatry controversely debated about the appropriate concept of illness, about nosology and the diagnostic terms and procedures based upon it. This paper discusses the approaches introduced by Emil Kraepelin (1856–1926), Karl Jaspers (1883–1969) and Arthur Kronfeld (1886–1941). Whereas Kraepelin postulated the existence of «natural entities» with regard to mental illness, Jaspers acknowledged the heuristic value of the term «disease entity», but declared it a «regulative idea» in a Kantian sense, i.e. an appropriate and useful, albeit never fully achievable aim. Kronfeld, in a way situated «between» Kraepelin and Jaspers, defended the notion of disease entities. However, he spoke clearly against primarily anchoring these entities in neighbouring scientific fields like neurobiology or social sciences. Psychiatry should stay (or become) «autological», in the first place using the psychological perspective. In full respect for neighbouring approaches as scientific fields in their own right, he rejected their premature transfer to mental phenomena as «heterological», i.e. as unduly narrowing down the scope of psychiatry. These issues, up to debate 100 years ago, still are of utmost relevance for psychiatry in the 21st century, a field struggling with its rather fragile scientific identity.

Epistemic Trust: a Keyword for Contemporary Clinical Practice and Psychopathology

Abstract:The increasing integration of eclectic knowledge from fields not directly related to clinical psychology is coherent with a current tendency to employ alternative ideas to investigate psychopathology in the light of a more phenomenological perspective. The concept of epistemic trust may provide the gateway to alternative causal models for personality psychopathology that links poor mentalizing environment to a more general lack of social support. People who have been denied the ability to trust the information circulating in their more proximate environment may showcase a remarkable disadvantage in terms of social adjustment that relate to early insecure attachment experiences. Research assumes that patients who have been suffering an epistemic deficient environment in infancy could be trapped into a vicious cycle of suffering, loneliness and inability to seek or accept help. Although most contributes cited in this brief article deal with epistemic trust as potential tool for social learning from a theoretical point of view, the recent introduction of systematic measures of epistemic trust as a dimensional personality variable outlines a likely future increase in the use of new questionnaires and protocols for clinical assessment and treatment monitoring expressively focused on epistemic trust. The benefits of adopting such a holistic etiological paradigm in personality pathology are outlined, as proposed by the many works that advocate a more equitable clinical practice that assume individual development in the context of an unequal social world, that is believed to determine the child's evolutionary trajectory from the very first stages of life. In the same vein, the importance of fostering an authentic relationship of trust between patient and therapist as a fundamental element of the therapeutic alliance, as well as a driving force for salutogenesis at the community level, is vividly highlighted.
