Abstract:Objectives/Hypothesis: A piezoelectric sensor, floating piezoelectric microphone, driven by acoustic vibration of the ossicles, is one possible design for a microphone for a totally implantable cochlear implant. The purpose of the article was to study the frequency response of the floating piezoelectric microphone and to identify the ideal feasible position in the ossicular chain.Study Design: Basic Research.Methods: The frequency response of the floating piezoelectric microphone was analyzed by finite-element modeling and in vitro testing of fresh cadaveric heads. The floating piezoelectric microphone, 5.0 mm in length and 1.5 mm by 1.2 mm in rectangular cross section, as a piezoelectric microphone, was placed at various locations on the ossicular chain and stimulated by pure tones of different frequencies.Results: The floating piezoelectric microphone can pick up the vibration of the ossicular chain and effectively convert it into the electronic signals effectively both in the long process of incus and in the malleus. The average sensitivity of the FPM is -44.22 dB rms ref 1V at 1000Hz in the long process of incus, -53.33 dB rms ref 1V at 1000Hz in the malleus, and -108.59 dB rms ref 1V at 1000Hz in the tympanic cavity.Conclusions: The floating piezoelectric microphone is expected to be used as an implantable middle ear microphone for the totally implantable cochlear implant.

Abstract:ObjectiveOur team designed a long‐lasting, well‐sealed microphone, which uses laser welding and vacuum packaging technology. This study examined the sensitivity and effectiveness of this new floating piezoelectric microphone (NFPM) designed for totally implantable cochlear implants (TICIs) in animal experiments and intraoperative testing.MethodsDifferent NFPM frequency responses from 0.25 to 10 kHz at 90 dB SPL were analyzed using in vivo testing of cats and human patients. The NFPM was tested in different positions that were clamped to the ossicular chains or placed in the tympanic cavity of cats and human patients. Two volunteers' long incus foot and four cats' malleus neck of the ossicular chain were clamped with the NSFM. The output electrical signals from different locations were recorded, analyzed, and compared. The NFPM was removed after the test without causing any damage to the middle‐ear structure of the cats. Intraoperative tests of the NFPM were performed during the cochlear implant surgery and the cochlear implant surgery was completed after all tests.ResultsCompared with the results in the tympanic cavity, the NFPM could detect the vibration from the ossicular chain more sensitively in cat experiments and intraoperative testing. We also found that the signal output level of the NFPM decreased as the acoustic stimulation strength decreased in the intraoperative testing.ConclusionThe NFPM is effective in the intraoperative testing, making it feasible as an implantable middle‐ear microphone for TICIs.Level of Evidence4 Laryngoscope, 134:937–944, 2024

Abstract:Objectives Many microphones have been developed to meet with the implantable requirement of totally implantable cochlear implant (TICI). However, a biocompatible one without destroying the intactness of the ossicular chain still remains under investigation. Such an implantable floating piezoelectric microphone (FPM) has been manufactured and shows an efficient electroacoustic performance in vitro test at our lab. We examined whether it pick up sensitively from the intact ossicular chain and postulated whether it be an optimal implantable one. Methods Animal controlled experiment: five adult cats (eight ears) were sacrificed as the model to test the electroacoustic performance of the FPM. Three groups were studied: (1) the experiment group (on malleus): the FPM glued onto the handle of the malleus of the intact ossicular chains; (2) negative control group (in vivo): the FPM only hung into the tympanic cavity; (3) positive control group (Hy-M30): a HiFi commercial microphone placed close to the site of the experiment ear. The testing speaker played pure tones orderly ranged from 0.25 to 8.0 kHz. The FPM inside the ear and the HiFi microphone simultaneously picked up acoustic vibration which recorded as .wav files to analyze. Results The FPM transformed acoustic vibration sensitively and flatly as did the in vitro test across the frequencies above 2.0 kHz, whereas inefficiently below 1.0 kHz for its overloading mass. Although the HiFi microphone presented more efficiently than the FPM did, there was no significant difference at 3.0 kHz and 8.0 kHz. Conclusions It is feasible to develop such an implantable FPM for future TICIs and TIHAs system on condition that the improvement of Micro Electromechanical System and piezoelectric ceramic material technology would be applied to reduce its weight and minimize its size.

Abstract:BACKGROUND:A new floating piezoelectric microphone (NFPM), driven by the acoustic vibration of the ossicles, has been manufactured in the lab.OBJECTIVES:This study aimed at exploring the stability and bio-compatibility of this NFPM.MATERIALS AND METHODS:The NFPM was implanted into the cat tympanic cavity by clamping it to the handle of the malleus, and then a temporal bone computed tomography (CT) plain scan and three dimensional reconstruction were performed to observe its coupling with the handle of the malleus. After six months of implantation, cats' blood samples were taken for pro-inflammatory factor analysis. Then, the operative cavity was opened to check the NFPM, the auditory ossicular chain and some granulation tissue surrounding the NFPM.RESULTS:The NFPM was firmly clamped to the handle of the malleus of the cats during the six months of implantation. Besides, there was no obvious systemic inflammatory response in the experimental animals. In addition, local proliferation of granulation tissue occurred in the tympanic cavity without hampering the movement of the auditory ossicle,or causing ischemia of the auditory ossicle.CONCLUSION:The NFPM could be implanted in our experimental cats for a long period of time and had good bio-compatibility.

Abstract:Design of a floating mass type piezoelectric transducer characterized by small size, low power consumption and broad frequency bandwidth for an implantable middle ear hearing aid was presented in this paper. This transducer consisted of a piezoelectric stack, a metal case and a clamp is used to convert electrical energy, which is obtained from sound signals generated externally, into mechanical vibration energy, which is a means of amplifying acoustic sound for patients with sensory neural hearing loss. To aid the design process, a coupling biomechanics model of the human middle ear and the piezoelectric actuator was constructed. This model was build based on a complete set of computerized tomography section images of a healthy volunteer's left ear by reverse engineering technology. The validity of this model was confirmed by comparing the motion of the tympanic membrane and stapes footplate obtained by this model with published experimental measurements on human temporal bones. It is shown that the designed actuator can be implanted on the incus long process by a simple surgical operation, and the stapes footplate displacement by its excitation at 10.5 V rms was equivalent to that from acoustic stimulation at 100 dB SPL, which is adequate stimulation to the ossicular chain. The corresponding power consumption is 0.04 mW per volt of excitation at 1 kHz, which is low enough for the transducer to be used in a implant-able middle ear hearing aid.

Abstract:A dynamic micropumping device has been developed, which is based on acoustic streaming generated by a piezoelectric microtransducer actuated in d(31) mode without any check valve. Piezoelectric microtransducers immersed in liquid (water) are used as actuators due to some of their unique properties such as lower noise generation and ease of miniaturization. Quantitative results of maximum local velocity are presented for different tip configuration of the same actuator for comparison. In addition, this work also demonstrates the quantitative measures of the pumping performance such as the pressure head generated as a function of different relevant parameters such as applied electrical field, AC frequency and length of the actuator.

An implantable microphone has the potential to provide a range of benefits over external microphones for cochlear implant users including the filtering ability of the outer ear, cosmetics, and usability in all situations. This paper presents results from experiments in human cadaveric ears of a piezofilm microphone concept under development as a possible component of a future implantable microphone system for use with cochlear implants. This microphone is referred to here as a drum microphone (DrumMic) that senses the robust and predictable motion of the umbo, the tip of the malleus.

Abstract:Implantable middle ear hearing devices (IMEHDs) have been developed as a new technology to overcome the limitations of conventional hearing aids. The piezoelectric cantilever transducers currently used in the IMEHDs have the advantages of low power consumption and ease of fabrication, but generate less high-frequency output. To address this problem, we proposed and designed a new piezoelectric transducer based on a piezoelectric stack for the IMEHD. This new transducer, attached to the incus body with a coupling rod, stimulates the ossicular chain in response to the expansion-and-contraction of its piezoelectric stack. To test its feasibility for hearing loss compensation, a bench testing of the transducer prototype and a temporal bone experiment were conducted, respectively. Bench testing results showed that the new transducer did have a broad frequency bandwidth. Besides, the transducer was found to have a low total harmonic distortion (<0.75%) in all frequencies, and small release time (1 ms). The temporal bone experiment further proved that the transducer has the capability to produce sufficient vibrations to compensate for severe sensorineural hearing loss, especially at high frequencies. This property benefits the treatment of the most common sloping high-frequency sensorineural hearing loss. To produce a 100 dB SPL equivalent sound pressure at 1 kHz, its power consumption is 0.49 mW, which is low enough for the transducer to be utilized in the IMEHD.

Abstract:Objective: A novel implantable piezoelectric transducer has been developed in this laboratory. The transfer functions of the transducer were assessed in anesthetized acutely implanted cats, and were compared with the microphone of a cochlear implant's speech processor. Methods: The piezoelectric transducer was fixed to the head of the malleus of the cat. Pure tone signals of 97 dB SPL ranging from 250 to 8,000 Hz delivered from a loudspeaker placed beside the auricle of the cat were used to vibrate the tympanic membrane. The frequency response of the transducer was measured by monitoring the output signal of the transducer with an oscilloscope. The transfer functions of the transducer were then compared with the standard external microphone receiving the same tonal stimulus. Results: The average sensitivity of the implantable piezoelectric transducer was -38.7 dB re 1 V/Pa at 1,000 Hz. The frequency-response curve of the transducer mirrored that of the external microphone, even with high-frequency stimuli. Abstract:A new incus-body driving type transducer relying on piezoelectric stack, with broad frequency bandwidth, is proposed for use in a middle ear implant. To aid the design process of this transducer, a coupling biomechanical model of the human middle ear and the piezoelectric transducer was established by reverse engineering technology. The validity of this model was confirmed by comparing model predicted motions with experimental measurements. Based on this verified biomechanical model, the main parameters of the transducer were determined. And its power consumption was calculated. Finally, to verify the capability of the designed piezoelectric transducer, a human temporal bone experimental platform was built. And the dynamic characteristics and the stimulated performance of the piezoelectric transducer were tested. The result showed that stapes displacement stimulated by the transducer excitation at 10.5 V RMS was equivalent to that from acoustic stimulation at 100 dB SPL, which is an adequate stimulation to the ossicular chain. The corresponding power consumption is 0.31 mW per volt of excitation at 1 kHz, which is low enough for the transducer to be used in a middle ear implant. Besides, this transducer demonstrates high performance at high frequencies.
The UmboMic: A PVDF Cantilever Microphone

Abstract:Objective: We present the "UmboMic," a prototype piezoelectric cantilever microphone designed for future use with totally-implantable cochlear implants. Methods: The UmboMic sensor is made from polyvinylidene difluoride (PVDF) because of its low Young's modulus and biocompatibility. The sensor is designed to fit in the middle ear and measure the motion of the underside of the eardrum at the umbo. To maximize its performance, we developed a low noise charge amplifier in tandem with the UmboMic sensor. This paper presents the performance of the UmboMic sensor and amplifier in fresh cadaveric human temporal bones. Results: When tested in human temporal bones, the UmboMic apparatus achieves an equivalent input noise of 32.3 dB SPL over the frequency range 100 Hz to 7 kHz, good linearity, and a flat frequency response to within 10 dB from about 100 Hz to 6 kHz. Conclusion: These results demonstrate the feasibility of a PVDF-based microphone when paired with a low-noise amplifier. The reported UmboMic apparatus is comparable in performance to a conventional hearing aid microphone. Significance: The proof-of-concept UmboMic apparatus is a promising step towards creating a totally-implantable cochlear implant. A completely internal system would enhance the quality of life of cochlear implant users.

Abstract:To optimize the implant performance of a floating mass type piezoelectric actuator for implantable hearing devices,a displacement amplifier was designed to improve the output characteristics of the actuator.A finite element model of human ear consisting of the external ear canal,middle ear and simplified cochlea was constructed via micro-computer tomography imaging and the technique of reverse engineering.The validity of the model was verified by comparing the model-derived results with experimental data from reference.Then an ear-actuator coupled mechanical model was developed,and the multi-field coupling was considered to study the implant performance of the actuator before and after the displacement amplifier was added.The results showed that the adoption of displacement amplifier can increase the equivalent sound pressure level of the actuator in the middle and high frequency ranges,and the power consumption can effectively be reduced at the same time.

Abstract:The human ear receives the sound signals from the ambient and converts those signals to the electrical domain and transmits them to the brain. According to the prevalence of human hearing impairments and necessity of using the hearing aid, this paper has been concentrated on the design of microphones to be implanted inside the ear. The intended design for this application is using microelectromechanical technology and piezoelectric material. Based on the important role of the diaphragm in the microphones structure and in order to achieve maximum displacement in diaphragm, variety of accurate studies in terms of shape and dimension have been done and will be presented in this paper. The examined shapes for selecting the optimum diaphragm’s form are circle, square and hexagonal. In the performed simulations and analysis, four dimensions in micrometer have been considered for each selected shape. According to the contemplated the efficiency parameters (diaphragm’s center displacement, first resonant frequency) and results of the simulations, the circular diaphragm’s performance with dimension of 350 µm compared to the other cases is desirable for medical application of the microphone. The analysis and simulations have been performed in COMSOL software. The fabrication process of the proposed diaphragm has been done using bulk and surface micromachining. In addition, AlN was utilized as the piezoelectric material in this structure.

Abstract:This study presents a middle ear implantable transducer for fully implantable cochlear implants. The transducer`s dimensions are compatible with routine cochlear implant procedures. The multi‐channel system covers the daily acoustic band with a high signal‐to‐noise ratio. Objective audiological evaluations of speech signals underscore the encouraging results, affirming the feasibility of the middle ear implantable multi‐channel transducer for the first time. This paper presents an acoustic transducer for fully implantable cochlear implants (FICIs), which can be implanted on the hearing chain to detect and filter the ambient sound in eight frequency bands between 250 and 6000 Hz. The transducer dimensions are conventional surgery compatible. The structure is formed with 3 × 3 × 0.36 mm active space for each layer and 5.2 mg total active mass excluding packaging. Characterization of the transducer is carried on an artificial membrane whose vibration characteristic is similar to the umbo vibration. On the artificial membrane, piezoelectric transducer generates up to 320.3 mVpp under 100 dB sound pressure level (SPL) excitation and covers the audible acoustic frequency. The measured signal‐to‐noise‐ratio (SNR) of the channels is up to 84.2 dB. Sound quality of the transducer for fully implantable cochlear implant application is graded with an objective qualification method (PESQ) for the first time in the literature to the best of the knowledge, and scored 3.42/4.5.

Abstract:A piezoelectric stack actuator for incus driving type middle ear implant was proposed and designed.To aid the design,a coupled mechanical model between a human middle ear and a piezoelectric stack actuator was constructed.This model was built based on a complete set of computerized tomography section images of a healthy volunteer's left ear with reverse engineering technology.The result showed that the stapes footplate displacement stimulated with the designed piezoelectric stack actuator's excitation at 10.5 V rms is equivalent to that from acoustic stimulation at 90 dB SPL,adequate to the ossicular chain;the corresponding power consumption is 0.03 mW per volt of excitation at 1 kHz,low enough to meet the requirements of a middle ear implant.
Experiment of Audition Compensation Performance of an Incus Driving Piezoelectric Vibrator in Human Temporal Bone

Abstract:To overcome the shortcomings of small output gain and narrow frequency bandwidth in traditional piezoelectric middle-ear implants, a new incus-body driving vibrator attached to a piezoelectric stack is proposed. To verify the feasibility of this proposal, a human temporal bone experimental platform with earphone, probe microphone, piezoelectric vibrator and laser vibrometer is built. Based on this platform, the dynamic characteristics of the piezoelectric vibrator and the ear-bone chain are tested under the sound excitation and the piezoelectric vibrator excitation. Through the comparison and analysis of the dynamic performances of the stirrup-bone under the two excitations, the audition compensation performance of the piezoelectric vibrator is studied. The result shows that the incus driving piezoelectric vibrator can compensate hearing loss efficiently under low-power and low-voltage conditions. This vibrator has very good audition compensation effect at high frequencies. Under the same driving voltage, it can actuate the vibration as high as 130 dB corresponding to the eardrum sound excitation, and has high output gain at high frequencies with small harmonic distortion.

Abstract:The middle ear implantable hearing aid, as a new treatment for sensorineural hearing loss, has attracted more attentions of researchers. A new kind of incus driving type middle ear implant, which contains a stack of piezoelectric crystals in a compact structure and a position adjustment apparatus, is proposed in this paper. In the design of the hearing aid apparatus, the embedded location, space, and the implant surgery process, etc. are considered. Finite element method (FEM) is used to assess resonant frequency and output displacement of the device. It is shown that the designed apparatus can be implanted in the middle ear cavity easily, has a good dynamic response, and the total harmonic distortion is below 1 %. Temporal bone experiment reveals that stapes displacement under actuator stimulation of 6.9 Vrms is higher than that under acoustic stimulation of 90 dB SPL over the operating frequency range, which can be applied to compensate the hearing loss.

Abstract:Fully implantable, self-powered hearing aids with no external unit could significantly increase the life quality of patients suffering severe hearing loss. This highly demanding concept, however, requires a strongly miniaturized device which is fully implantable in the middle/inner ear and includes the following components: frequency selective microphone or accelerometer, energy harvesting device, speech processor, and cochlear multielectrode. Here we demonstrate a low volume, piezoelectric micro-electromechanical system (MEMS) cantilever array which is sensitive, even in the lower part of the voice frequency range (300⁻700 Hz). The test array consisting of 16 cantilevers has been fabricated by standard bulk micromachining using a Si-on-Insulator (SOI) wafer and aluminum nitride (AlN) as a complementary metal-oxide-semiconductor (CMOS) and biocompatible piezoelectric material. The low frequency and low device footprint are ensured by Archimedean spiral geometry and Si seismic mass. Experimentally detected resonance frequencies were validated by an analytical model. The generated open circuit voltage (3⁻10 mV) is sufficient for the direct analog conversion of the signals for cochlear multielectrode implants.

Abstract:To design a simplefloating mass type piezoelectric transducer,consisting of a piezoelectric stack,a metal case and a clamp,for middle ear implant,a biomechanics model coupled by human middle ear and piezoelectric transducer is constructed.This model is built based on a complete set of computerized tomography section images of a healthy volunteer's left ear by reverse forming technology.The validity of this model is confirmed by comparing the result of motion of the stapes of this model with the published experimental data on human temporal bones.It is shown that the designed transducer can be implanted on the incus long process by a simple surgical operation,and the stapes footplate displacement by its excitation at 10.5 Vrms is equivalent to that from acoustic stimulation at 100 dB SPL,which is adequate stimulation to the ossicular chain.
