Abstract:Objective To investigate the efficacy of centrally fixed eyeball for assessment of the depth of anesthesia in premature infants undergoing outpatient fundus examination. Methods Fifty eight premature infants undergoing examination of fundus of eyes were enrolled in this study. Their gestational age (from the first day of last menstruation period to birth) + after birth age (from birth to the day when examination of fundus of eyes was performed) = 44-64 weeks. The patients were randomly divided into 2 groups: Ⅰ group body movement (group M, n = 27) and Ⅱ group centrally fixed eyeball (group E, n = 31). Anesthesia was induced and maintained with isoflurane inhalation. The patients were breathing spontaneously. The eyelids were kept open with speculum after induction of anesthesia. The EC50 of sevoflurane concentration which could inhibit body movement or make eyeballs centrally fixed was determined by up-and-down sequential experiment. The initial isoflurane concentration was 3% in both groups. Each time the isoflurane concentration was increased/decreased by 0.5 %. 95 % confidence interval (CI) was calculated. The lowest SpO2, respiratory rate and coughing during maintenance of anesthesia were recorded. Results The EC50 of sevoflurane (95% CI) was 2.9% (2.2%-3.6%) in group M and 3.4%(2.6%-4.6%) in group E. Examination was successfully completed in all patients. No respiratory depression and coughing occurred during examination and no vomiting and coughing were observed during feeding at 1 h after recovery from anesthesia. No body movement occurred in 15 patients whose eyeballs were centrally fixed in group E. Conclusion Centrally fixed eyeball can be used as sign of appropriate depth of anesthesia for fundus examination in premature infants.
The Depth of Anesthesia under Sevoflurane Can Be Monitored Based on Calling Names Language Paradigm and Ag/AgCl Electrooculogram Electrode

Abstract:In the depth of anesthesia monitoring, electrooculogram is usually treated as an artifact of EEG. However, the amplitude of the electrooculogram is much greater than that of electroencephalography. Its requirements for electrode materials and detection circuits are also far lower than electroencephalography. In this prospective cohort study, we use a new language paradigm that elicited rhythmic ocular muscle activity to explore whether the electrooculogram can be used for monitoring the depth of anesthesia. Thirty-three patients inhaled sevoflurane via a laryngeal mask for induction of anesthesia. According to the end-tidal sevoflurane concentration, patients were studied under three depth of sedation. Throughout all periods, the rhythmic calling of the patient’s own name script was sporadically played at a frequency of 0.5 Hz. We found that the electrooculogram was synchronized to the language paradigm during induction with sevoflurane, and the Ag/AgCl electrode has a significantly higher signal-to-noise ratio and 0.5 hz spectral peak value than other electrode. With the increase in the sevoflurane concentration, the synchronizing ability gradually weakened and eventually disappeared. Based on this evidence, we proposed a new method that can improve the performance of the anesthesia prediction monitoring model.
EOG Elicited by Rhythmic&nbsp;Auditory Predicts the Depth of Sedation During Sevoflurane Anesthesia

Abstract:Abstract Background: Several electroencephalographic (EEG) variables have been used to estimate changes in human brain electrical activity during changing states of consciousness as a clinical monitoring tool to anesthesia and sedation. However, the EEG signal is weak and extremely susceptible to suppression interference. The use of EEG variables as anesthesia monitors is still controversial. Recent studies have shown a new language paradigm that elicited rhythmic brain responses and ocular muscle activity tracking sentence rhythms in speech. Electrooculogram (EOG) driven by rhythmic auditory stimulation has a larger amplitude than EEG variables and a higher temporal resolution than event-related brain potentials, which may be a potential monitor of anesthetic depth.Methods: Twenty-five patients inhaled sevoflurane via laryngeal mask for induction of anesthesia. The initial concentration of sevoflurane was set to 0.4% and increased by 0.1% every 1 min until loss of consciousness. During the entire operation, rhythmic auditory stimulation was presented binaurally to elicit ocular muscle activity.Results: When the end-tidal sevoflurane concentration (Etsevo) was low, the EOG had a significant spectral peak at 0.5 Hz, which is the same frequency as the rhythmic auditory stimulation, and then the peak value of EOG decreased with the increase of Etsevo. Peak value of EOG spectrum under the rhythmic auditory stimulation (PERA) showed significant differences in different depth of sedation (wakefulness: 84.0 ± 22.8, light sedation: 47.0 ± 27.7, deep sedation: 10.9 ± 16.5, P<0.01). PERA is correlated well with Etsevo (-0.75, P<0.01) and was able to predict the sedation depth during the induction period of anesthesia well (PK = 0.92 ± 0.08).Conclusion: We found that the ocular muscle activity synchronizes to the rhythmic auditory stimulation structures during induction with sevoflurane. With the increase of Etsevo, the synchronizing ability of the ocular electricity to rhythmic auditory stimulation gradually weakens and eventually disappears. Based on this evidence, the EOG driven by a rhythmic auditory stimulation may be a new and reliable approach to characterize and predict anesthetic depth objectively. Abstract:In balanced anesthesia, sevoflurane and propofol are often used in combination to achieve a better anesthetic effect. However, methods for on-line monitoring of concentrations of the two anesthetics in patients are still rare in clinical. This study proposed a non-invasive method utilizing a fast gas chromatograph combined with a surface acoustic wave sensor (Fast GC-SAW) to simultaneously on-line monitor sevoflurane and propofol in patients' exhaled gas. By using the direct resistive heating capillary column, the single detection time of Fast GC-SAW system was significantly shortened to 90s, as well as the size reduced to (40cm x 30cm x 20 cm). Besides, in the calibration of sevoflurane, Fast GC-SAW system showed a good linear correlation (R-2 = 0.9925, P < 0.01) with gas chromatography-mass spectrometer (GC-MS), which ensured the reliability and accuracy of the Fast GC-SAW system. Finally, clinical exper. iments on patients under balanced anesthesia were conducted. The varied concentrations measured by Fast GC-SAW extraordinarily matched the clinical usages of these two anesthetics. (C) 2017 Elsevier B.V. All rights reserved.
Efficacy of Midazolam Premedication for Prevention of Emergence Agitation after Sevoflurane Anesthesia in Children Undergoing Minor Surgery

Abstract:Objective To evaluate the efficacy of midazolam premedication for prevention of emergence agitation (EA) after sevoflurane anesthesia in children undergoing minor surgery.Methods One hundred and twenty ASA physical status Ⅰ or Ⅱ paediatric patients,aged 3-9 yr,weighing 15-35 kg,scheduled for elective tonsillectomy and adenoidectomy,were randomly divided into 4 groups (n =30 each):control group (group C) and midazolam 0.25,0.50 and 0.75 mg/kg groups (groups M1-3).The 10 ml mixture of midazolam 0.25,0.50 and 0.75 mg/kg and 10％ glucose was taken orally at 30 min before anesthesia in M1-3 groups,respectively,while 10 ml of 10％ glucose was taken orally in group C.Anesthesia was induced with inhalation of sevoflurane and maintained with iv infusion of remifentanil and inhalation of sevoflurane.EA was assessed using the Pediatric Anesthesia Emergence Delirium (PAED) scale.Results Compared with group C,the PAED scores and incidence of EA were significantly decreased in groups M2 and M3 (P ＜ 0.05),and no significant change was found in group M1 (P ＞ 0.05).Compared with group M1,the PAED scores and incidence of EA were significantly decreased in groups M2 and M3 (P ＜ 0.05).There was no significant difference in the PAED scores and incidence of EA between groups M3 and M2 (P ＞ 0.05).Conclusion Premedication with oral midazolam can prevent EA following sevoflurane anesthesia in children undergoing minor surgery and the optimum dose is 0.50 mg/kg.

Abstract:P>Objective: Screening examination for retinopathy of prematurity (ROP) is stressful and painful to the neonate. Sevoflurane has been used successfully in anesthesia for full-term and premature neonates and has been recently used for pediatric outpatient procedures. In this study, we examined the effective inspired concentration of sevoflurane to prevent movement in response to stimulation of examination in 50% of patients (EIC50) of sevoflurane, as well as the response of sevoflurane to age in the preterm outpatients undergoing fundus examination. Methods: Preterm pediatric outpatients at different postconceptual ages (duration from the mother's last menstrual period to the date when the examination for ROP was performed) with < 44 weeks (group A) and 44-64 weeks (group B) undergoing fundus examination were included. SpO(2) and ECG were monitored in operation room. In the process of anesthetic induction, the oxygen flow rates were 3 l center dot min-1, and subjects spontaneously breathed 6% sevoflurane by mask. The time to loss of movement was recorded as induction time. Six percent sevoflurane was inhaled continuously for the same duration, and then the inspired concentration of sevoflurane was adjusted to maintenance concentration. When no movement or crying was observed, the speculums were used to keep the eyelids open, and then the eye examinations were performed by the same ophthalmologist. During induction time and maintenance time, the occurrence of coughing, clenching, gross purposeful movement, breath holding or desaturation to SpO(2) < 95% was recorded. After completion of the procedure, patients were observed in the same room until spontaneous eye opening or verbalization occurred. Up and down method was used to determine subsequent maintenance concentration in each group. The initial maintenance concentration was 3%. The gradient of increase or decrease was 0.5%. If the preceding subject had not moved, the sevoflurane concentration was decreased by 0.5%; if the preceding subject had moved, the concentration was increased by 0.5%. When at least six independent alternations from no-movement to movement were observed, test was terminated. Results: Twenty-four pediatric outpatients completed the investigation in group A. The effective inspired concentration prevented movement in response to stimulation of examination in 50% of patients (EIC50) of sevoflurane was 2.5% in group A. Twenty-seven patients completed the investigation and EIC50 values of sevoflurane were 3.0% in group B. The induction time in group A was significantly lower than that in group B. The gestational ages, anesthesia time, and awake time were similar in two groups. Conclusion: Anesthesia with inhaled sevoflurane by a face mask can be accomplished in preterm outpatients undergoing fundus examination without intubation and i.v. accession. The EIC50 is lower, and the induction time is shorter in smaller aged patients compared with those in older ones.
Clinical study of sublingual atropine as premedication of sevoflurane anesthesia in premature infants undergoing fundus examination of eyes

Abstract:Objective To investigate the effect of sublingual atropine as premedication of sevoflurane anesthesia in premature infants undergoing fundus examination of eyes.Methods Forty premature infants undergoing examination of fundus of eyes were enrolled in this study. Their gestational age (from the first day of last menstruation period to birth)+after birth age (from birth to the day when examination was performed)=(46±5) weeks. The patients were randomly divided into two groups:group A and group C. Infants in group A were given atropine solution 0.02 mg/kg sublingually 15 min before anesthesia, while those in group C received the same dose of normal saline.Anesthesia was induced and maintained with 6% and 3% sevoflurane.At before atropine administration(T0),5 min(T1),10 min(T2),15 min(T3)after atropine administration HR, SpO2, T, RR, PETCO2 SEV-e and the incidence of oculocardiac reflex (OCR), the lowest HR during OCR period and the salivary secretion were recorded, and adverse events were evaluated.Results HR and T were significantly increased at T1-T3 in group A than that at T0. Compared with those in group C, HR was significantly increased at T4-T6, the lowest HR during OCR period was significantly increased, and the salivary secretion significantly decreased in group A. Conclusion Sublingual atropine as premedication can decreas the secretion of salivary and stabalize HR during OCR. It is an effective, safe, and easy premedication method in premature infants undergoing fundus examination.
Accuracy of BIS value for monitoring depth of sevoflurane-nitrous oxide anesthesia in children

Abstract:Objective To evaluate the accuracy of BIS value for monitoring the depth of sevoflurane-nitrous oxide in children. Methods Seventy-two ASA Ⅰ or Ⅱ children aged 1-14 yr undergoing abdominal surgery were randomly assigned into 3 groups ( n = 24 each) :sevoflurane group (group S), sevoflurane + 30% N2O group (group SN1 )and sevoflurane + 60% N2O (group SN2 ). Anesthesia was induced with atropine, lidocaine, propofol, rocuronium and remifentanil. After tracheal intubation, the patients were mechanically ventilated. PETCO2 was maintained at 35-45 mm Hg. Anesthesia was maintained with 2.5% sevoflurane combined with 0, 30% and 60% nitrous oxide in group S, SN1 and SN2 respectively. The end-tidal sevoflurane concentration (CETSev) was maintained at 2.5%, 2.0% and 1.5%. Each CETSev was maintained for at least 10 min after the begining of the surgery. Then the CETSev was modified to maintain BIS value at 40-60. BIS value was recorded before anesthesia induction and each stable CETSev. CETSev was recorded at maintaining BIS value of 40-60 for at least 20 min ( C50 ).Results BIS value and C50 were significantly lower in group SN2 than in group S and SN1 ( P ＜ 0.05), while no significant difference was found between group S and SN1 ( P ＞ 0.05). BIS value was negatively correlated with CETSev in all 3 groups (r = -0.736, -0.817, -0.729, P ＜ 0.01).There was no significant difference in the correlation coefficients among the 3 groups ( P ＞ 0.05 ). Conclusion BIS value can accurately reflect the depth of sevoflurane-nitrous oxide anesthesia in children.
Quantitative Pupillometry As a Predictor of Pediatric Postoperative Opioid-Induced Respiratory Depression.

Abstract:BACKGROUND: Safe postoperative pain relief with opioids is an unmet critical medical need in children. There is a lack of objective, noninvasive bedside tool to assess central nervous system (CNS) effects of intraoperative opioids. Proactive identification of children at risk for postoperative respiratory depression (RD) will help tailor analgesic therapy and significantly improve the safety of opioids in children. Quantitative pupillometry (QP) is a noninvasive, objective, and real-time tool for monitoring CNS effect-time relationship of opioids. This exploratory study aimed to determine the association of QP measures with postoperative RD, as well as to identify the best intraoperative QP measures predictive of postoperative RD in children. METHODS: After approval from the institutional review board and informed parental consent, in this prospective, observational study of 220 children undergoing tonsillectomy, QP measures were collected at 5 time points: awake preoperative baseline before anesthesia induction (at the time of enrollment [T1]), immediately after anesthesia induction before morphine administration (T2), 3 minutes after intraoperative morphine administration (T3), at the end of surgery (T4), and postoperatively when awake in postanesthesia recovery unit (PACU) (T5). Intraoperative use of opioid and incidence of postoperative RD were collected. Analyses were aimed at exploring correlations of QP measures with the incidence of RD and, if found significant, to develop a predictive model for postoperative RD. RESULTS: Perioperative QP measures of percentage pupil constriction (CONQ, P = .027), minimum pupillary diameter (MIN, P = .027), and maximum pupillary diameter (MAX, P = .034) differed significantly among children with and without postoperative RD. A predictive model including the minimum pupillary diameter 3 minutes after morphine administration (MIN3), minimum pupillary diameter normalized to baseline (MIN31), and percentage pupillary constriction after surgery (T4) standardized to baseline (T1) (CONQ41), along with the weight-based morphine dose performed the best to predict postoperative RD in children (area under the curve [AUC], 0.76). CONCLUSIONS: A model based on pre- and intraoperative pupillometry measures including CONQ, MIN, along with weight-based morphine dose-predicted postoperative RD in our cohort of children undergoing tonsillectomy. More studies with a larger sample size are required to validate this finding.
Minimum Alveolar Concentration of Sevoflurane for Endotracheal Intubation Without Body Movement in Premature Infants

Abstract:Objective To determine the minimum alveolar concentration for endotracheal intu-bation (MACEI )of sevoflurane for curbing the responses to endotracheal intubation in 50% premature infants less than 37 weeks of corrected age.Methods Twenty-seven ASA Ⅰ or Ⅱ premature infants less than 37 weeks of corrected age were enrolled in this study.At first,the anesthesia induction was started by inhaling 6% sevoflurane.After the patient lost consciousness,the end tidal sevoflurane concentration (CET Sev)was adjusted to the target concentration and maintained stable for 1 5 min.En-dotracheal tube was then intubated.The up-and-down sequential method was used to calculate the MAC.The initial CET Sev was 3.0% and it was increased or decreased by 0.2% in the next patient ac-cording to the endotracheal intubation response.If the intubation response was positive,the CET Sev was increased;if the intubation response was negative,the CET Sev was decreased.The midpoint from negative response to positive response was set as a balance point and the mean value of the concentra-tions of sevoflurane at all the balance points were calculated as MACEI .Results The end tidal sevoflurane concentration for blunting the responses to endotracheal intubation in 50% premature in-fants was 2.55%±0.20%,and the MAC9 5 was 2.81% (95% confidence interval 2.67%-3.58%). Conclusion The MACEI of sevoflurane for curbing the responses to endotracheal intubation in 50%premature infants less than 37 weeks of corrected age is 2.55%,which is lower than that in the full-term children.
[Efficacy of Pressure Support Ventilation General Anesthesia in Infants and Premature Patients Undergoing Ophthalmologic Surgeries].

Abstract:OBJECTIVE:To investigate the efficacy of pressure support ventilation (PSV) general anesthesia in infants and premature patients undergoing ophthalmologic surgeries. METHODS:A total of forty-two infants undergoing vitreoretinal surgeries in Peking university people's hospital from Dec. 2014 to Jul. 2015 were enrolled in the study, whose post-conceptual age (PCA, duration from the mother's last menstrual period to the date surgery was performed) were 38-92 weeks. They were divided into 3 groups according to PCA, which were group A (PCA ≤ 44 weeks), group B (44 weeks < PCA ≤ 64 weeks) and group C (64 weeks < PCA ≤ 92 weeks). Anesthesia was induced with 6% sevoflurane inhalation by mask. After placement of laryngeal mask airway, patients were breathing spontaneously during the whole procedure with the inhalational maintenance concentration of sevoflurane and ventilation parameters adjusted according to clinical set. The surgical procedure begun when satisfying retrobulbar nerve block was achieved. The end-tidal concentration- of sevoflurane, hemodynamic and respirational data were recorded at the time of before surgery (T0), 15 minutes after surgery (T1) and 30 minutes after surgery (T2). The time interval from cease of sevoflurane inhalation to the laryngeal mask removal and adverse events during and after surgery were also recorded. RESULTS:A total of 42 patients were enrolled in the study, with 10 cases in group A, 17 cases in group B and 15 cases in group C. The hemodynamics of all patients in three groups were stable. There was no significant difference in removal time interval among groups (all P > 0.05). No significant difference was found in end-tidal concentrations of sevoflurane, minute ventilation, pressure of end-tidal carbon dioxide, lung compliance among three time points(all P > 0.05). The respiration rate (per minute) of group B and group C were (33.1 ± 5.7) and (29.4 ± 6.4) at T1, (35.8 ± 7.4) and (32.1 ± 5.6) at T0, respectively, with statistical difference (F = 5.087, 5.628; P < 0.05) at both time points between groups. The peak airway pressure (cmH2O) of group B and group C were (14.5 ± 3.1) and (14.3 ± 2.2) at T1, (14.1 ± 2.9) and (13.1 ± 1.5) at T0, respectively, with statistical difference (F = 4.923, 12.393; P < 0.05) at both time points between groups. There was no significant difference in end-tidal concentrations of sevoflurane, pressure of end-tidal carbon dioxide, expiratory tidal volume per kg, minute ventilation per kg, respiration rate and peak airway pressure among groups (all P > 0.05). There was no severe adverse events during and after surgery in any group. CONCLUSION:The application of PSV during laryngeal mask airway general anesthesia with spontaneous breathing maintained can be a choice for infants and premature patients undergoing ophthalmologic surgeries.

Abstract:Objective To observe the systemic inhalation anesthetic effects of preterm children with different gestational ages under ocular fundus examination,and to assess its safety.Methods Fifty-one preterm children with retinopathy of prematurity (ROP) were included in the study.These kids were divided into 2 groups,group Ⅰ included 24 kids with a corrected gestational age of 33 to ＜44 weeks,and group Ⅱ included 27 kids with a corrected gestational age of 44 to 64 weeks.The preterm months were same (t=-1.3,P＞0.05),but the body weights were different (t=-10.5,P＜0.05) between these two groups.Anesthesia was induced by inhalation of 6％ sevoflurane,and the period from the beginning of inhalation to disappearance of body movement was the induction time.6％ sevoflurane was inhaled continuously for another period of the induction time,and then the concentration of sevoflurane was adjusted to a maintenance concentration.The initial maintenance concentration was 3％,and was adjusted by 0.5％ each time.Sequential method was used to determine the subsequent maintenance concentration.If the preceding patient had not moved during the maintenance period,the sevoflurane concentration was decreased by 0.5％ for the next patient.If the preceding patient had moved during the maintenance period,the sevoflurane concentration was increased by 0.5％ for the next patient.Respiratory depression and cough during the induction and maintenance period,duration of anesthesia and recovery time were recorded.Choking and vomiting during drinking or milk-feeding in one hour after the ocular fundus examination were also recorded.Results The effective inhale concentration in 50％ patient of sevoflurane was 2.5％ in group Ⅰ,2.9％ in group Ⅱ.The average maintenance concentration was (2.5 ± 0.5) ％ in group Ⅰ,(3.0 ±0.5)％ in group lⅡ.The difference was statistically significant (t =-3.3,P＜0.05).The average duration of anesthesia and the average awake time were the same (t =0.04 and 1.0 respectively,P＞ 0.05)between these two groups.The average induction time was significantly shorter in group Ⅰ than in group Ⅱ,the difference was statistically significant (t =-4.9,P ＜ 0.05).All patients were successfully completed the ocular examination.No respiratory depression or cough occurred during and after the examination.No choking and vomiting during drinking or milk-feeding in one hour after the ocular fundus examination.Conclusion Anesthesia with inhaled sevoflurane by a face mask is safe for preterm outpatients undergoing fundus examination.

Abstract:Objective To determine the minimum alveolar concentration(MAC) of sevoflurane without body movement during laryngeal mask airway(LMA)intubation in premature infants less than 37 weeks of corrected gestational age undergoing total inhalation general anesthesia induction.Methods Twenty-one ASA Ⅰ or Ⅱ premature infants less than 37 weeks of corrected gestational age undergoing elective inhalation general anesthesia were enrolled in this study.At first,the general anesthesia induction was started by inhaling 6 ％ sevoflurane.After the premature infant lost consciousness,the end tidal sevoflurane concentration(ET-sev)was adjusted to the predetermined concentration and maintained stable for 15 min.After that,LMA was inserted.The up-anddown sequential allocation was used to determine MAC.The initial ETsev was 2 ％,which was increased or decreased by 1 gradient concentration in the next case according to the LMA insertion body movement response.The adjacent concentration gradient was 0.2％.The midpoint from th body movement response to non-body movement response was set as the balance point and the mean value of the concentrations of sevoflurane at all the balance points were calculated as MAC.Results The end tidal sevoflurane con centration without the body movement responses to LMA insertion was 1.71％.Conclusion The MAC of sevoflurane without the body movement responses to LMA insertion in premature infants less than 37 weeks of corrected gestational age is 1.71％,which is lower than that in the normal children and probably because imperfect central nervous system development in premature infants.

Abstract:Application of propofol in preventing emergence agitation after sevoflurane anesthesia in children was evaluated. Clinical data of 200 children who received sevoflurane anesthesia in Children's Hospital of Xuzhou Medical University were retrospectively analyzed. Among them, 120 patients who received inhaled sevoflurane for pediatric anesthesia and intravenous infusion of propofol (2 mg/kg) were included in observation group. The remaining 80 cases who were directly anesthetized by sevoflurane alone were the control group. T PAED scores, modified Aldrete scores, extubation time, PACU time and adverse reactions (gastrointestinal tract and respiratory response) were analyzed and compared between the control and observation group. PAED scores, extubation time, PACU time and incidence of adverse reactions were significantly lower in observation than in control group, and the modified Aldrete scores were higher in observation than in control group (P<0.05). Spearman's correlation analysis showed that the PAED scores were negatively correlated with modified Aldrete scores and positively correlated with extubation time. There was positive correlation between the PACU time and incidence of adverse reactions and between the PAED scores and extubation time. There was negative correlation between PACU time and incidence of adverse reactions and between Aldrete scores and extubation time (P<0.05). Therefore, we conclude that propofol can be used to prevent agitation after sevoflurane anesthesia in children.

Abstract:Background: The aim of this study was to compare the effects of sevoflurane on the neurologic pupil index (NPi), obtained by means of automated pupillometry, between intensive care unit (ICU) and surgical patients. Methods: This was a prospective single-center study conducted between December 2021 and February 2023. The eligible population comprised all patients undergoing general anesthesia (GA) for visceral surgery (VS) or neurosurgery (NS) and ICU patients receiving inhaled sevoflurane, according to the decision of the treating physician. The NPi measurements were conducted before GA (T0), after induction (T1), after the initiation of sevoflurane (T2), and at the point of discontinuation of sevoflurane (T3). Results: A total of 41 VS, 16 NS, and 22 ICU patients (out of which, 12 had a brain injury) were included. In the VS and NS groups, there was a significant decrease in the NPi over time, which remained within normal ranges. The NPi values decreased over time in the ICU group after sevoflurane administration. At T2, the NPi values were lower in the ICU group compared to the other groups. Lower NPi values were observed in the ICU patients with a brain injury compared to other patients. Conclusions: The administration of inhaled sevoflurane was associated with a significant reduction in the NPi values of the ICU patients with a concomitant brain injury.

Abstract:Accurate monitoring of the depth of anesthesia (DOA) is essential to ensure the safety of the operation. In this study, a new index using near-infrared spectroscopy (NIRS) signal was proposed to assess the relationship between the DOA and cerebral hemodynamic variables. Methods: Four cerebral hemodynamic variables of 15 patients were collected, including left, right, proximal, distal, oxygenated (HbO and deoxygenated (Hb) hemoglobin concentration changes. The Phase-Amplitude coupling (PAC), an adaptation of cross-frequency coupling to reflect the modulation of the amplitude of high-frequency signals by the phase of low-frequency signals, was measured and the modulation index (MI) was obtained to monitor the DOA afterwards. Meanwhile, the BIS value based on electroencephalogram is also measured and compared. Results: Compared with awake period, in anesthesia maintenance period, the PAC was strengthened. The analysis of receiver operating characteristic (ROC) curve showed that the MI, especially the MI of rp-HbO 2 , could effectively discriminate these two periods. Additionally, during the whole anesthesia process, the BIS value was statistically consistent with the MI of cerebral hemodynamic variables, and cerebral hemodynamic variables were immune from interference by clinical electric devices. Conclusion: The MI of cerebral hemodynamic variables was appropriate to be used as a new index to monitor the DOA. Significance: This study is of great significance to the development of new modes of anesthesia monitoring and new decoding methods, and is expected to develop a high-performance anesthesia monitoring system.

Abstract:BACKGROUND:To explore the effect of sevoflurane inhalation anesthesia on evoked electromyography monitoring of spinal nerve root in children associated with cerebral palsy. METHODOLOGY:Children with cerebral palsy (n=40) were selected and further divided into 1MAC (minimum alveolar concentration) sevoflurane group and 2MAC sevoflurane group. Following the induction of anesthesia, Nicolet Endeavor-CR16 channel electrophysiological monitor was used to implement three times of successive electrical stimulation with interval of 5 sec at 3.50 mA. RESULTS:Our results suggested a statistical significance of amplitude retention ratio and latency in the sevoflurane inhalation time (P<0.01), with an interaction effect between the sevoflurane inhalation time and concentration for amplitude retention ratio (P<0.01), while there is no interaction effect between the sevoflurane inhalation time and concentration for latency (P>0.05). Compared to 1MAC sevoflurane group, the amplitude retention ratio of 2MAC sevoflurane group decreased remarkably (P<0.01) and the latency of 2MAC sevoflurane group extended at T3 and T4 (P<0.05 or P<0.01). CONCLUSIONS:In evoked electromyography monitoring of spinal nerve root in children with cerebral palsy, with the increasing of concentration and duration of sevoflurane inhalation, evoked electromyogram retention ratio reduces gradually, latency extends and the retention ratio has more changes than the latency.

Abstract:Background High-density electroencephalographic (EEG) monitoring remains underutilized in clinical anesthesia, despite its obvious utility in unraveling the profound physiological impact of these agents on central nervous system functioning. In school-aged children, the routine practice of rapid induction with high concentrations of inspiratory sevoflurane is commonplace, given its favorable efficacy and tolerance profile. However, few studies investigate topographic EEG during the critical timepoint coinciding with loss of responsiveness - a key moment for anesthesiologists in their everyday practice. We hypothesised that high initial sevoflurane inhalation would better precipitate changes in brain regions due to inhomogeneities in maturation between three different age groups compared with gradual stepwise paradigms utilised by other investigators. Knowledge of these changes may inform strategies for agent titration in everyday clinical settings. Methods 37 healthy children aged five to ten years underwent induction with >=4% sevoflurane in high flow oxygen. Perturbations in anesthetic state was investigated in 23 of these children using 64-channel EEG with Hjorth Laplacian referencing scheme. Topographical maps illustrated absolute, relative and total band power across three age groups: 5-6 years (N = 7), 7-8 years (N = 8) and 9-10 years (N = 8). Results Spectral analysis revealed a large shift in total power driven by increased delta oscillations. Well-described topographic patterns of anesthesia, e.g.: frontal predominance, paradoxical beta excitation and increased slow activity, were evident in the topographic maps. However, there were no statistically significant age-related changes in spectral power observed in a midline electrode subset between the groups when responsiveness was lost compared to the resting state. Conclusions High initial concentration sevoflurane induction causes large scale topographic effects on the pediatric EEG. Within the minute following unresponsiveness, this dosage may perturb EEG activity in children to an extent where age-related differences are not discernible.

Abstract:Abstract Background: Cortical electroencephalography (EEG)-based devices are used to monitor the depth of anesthesia. In this study, we compared the values of bispectral index (BIS) and patient state index (PSI) during sevoflurane anesthesia in children. The ability/accuracy of BIS and PSI to predict the steady and recovery state of anesthesia was evaluated based on prediction probability (Pk) values and the secondary outcomes were agreement and correlation of two monitors. Methods: Fifty children (3-12 years old) were enrolled and the patients received sevoflurane anesthesia with remifentanil. Before the induction of anesthesia, BIS and PSI sensors were simultaneously placed on the forehead, and data were collected until the end of anesthesia. Steady state was defined as the period following intubation until the cessation of sevoflurane, while recovery state was defined as the period following the cessation of sevoflurane until awake. The prediction probability (Pk), agreement or correlation of BIS and PSI in different anesthesia state were calculated. Results: Anesthesia reduced mean BIS and PSI values. Pk of BIS (95% confidential interval (CI) [0.78-0.91])) and PSI (95% CI [0.82-0.91]) for anesthesia were 0.85 and 0.87, respectively. Agreement was 0.79 for recovery state and 0.73 for steady state. Conclusions: Pk values were comparable for BIS and PSI. Agreement between BIS and PSI measurements in the same state was relatively good. Therefore, these monitors are appropriate for monitoring for different state of anesthesia in pediatric population. Trial Registry Number: Clinical Trials.gov NCT03792334.

Abstract:Sevoflurane is one of the most widely used inhaled anesthetics; however, the mechanisms underlying its anesthetic action are still not completely understood. This work identifies a cluster of sleep‐related urocortin 1 (UCN1+)/cocaine‐ and amphetamine‐regulated transcript (CART+) neurons in the midbrain with high enrichment of the growth hormone secretagogue receptor (GHSR). Sevoflurane activates the UCN1+/CART+ neurons through GHSR to promote anesthesia. Sevoflurane has been the most widely used inhaled anesthetics with a favorable recovery profile; however, the precise mechanisms underlying its anesthetic action are still not completely understood. Here the authors show that sevoflurane activates a cluster of urocortin 1 (UCN1+)/cocaine‐ and amphetamine‐regulated transcript (CART+) neurons in the midbrain involved in its anesthesia. Furthermore, growth hormone secretagogue receptor (GHSR) is highly enriched in sevoflurane‐activated UCN1+/CART+ cells and is necessary for sleep induction. Blockade of GHSR abolishes the excitatory effect of sevoflurane on UCN1+/CART+ neurons and attenuates its anesthetic effect. Collectively, their data suggest that anesthetic action of sevoflurane necessitates the GHSR activation in midbrain UCN1+/CART+ neurons, which provides a novel target including the nucleus and receptor in the field of anesthesia.
