Abstract:Purpose To compare the discriminative power of the index scores of EQ-5D-5L (5L) and EQ-5D-3L (3L) in diabetes patients in China. Methods A consecutive sample of type 2 diabetes mellitus (T2DM) patients in the clinics self-completed the two versions of EQ-5D. The 3L index score was obtained from the Chinese 3L value set; the 5L index score was predicted from the 3L index score using an interim scoring. Relative efficiency (RE) of the 5L and 3L index scores was calculated to compare their ability in differentiating between T2DM patients with and without one of ten clinical conditions. The efficiency of the 5L and 3L health state classification systems was assessed using the Shannon index ( H ′) and in terms of ceiling effects. Results A total of 289 T2DM patients participated in this study. The 5L score was systematically lower than the 3L score for T2DM patients with and without a condition (range −0.36 to −0.06). The 5L score exhibited higher discriminative power in nine of ten conditions, with the mean RE value being 1.92. 5L had higher H ′ values than 3L in all the five EQ-5D dimensions: mobility (1.14 vs. 0.70), self-care (0.44 vs. 0.33), usual activities (0.72 vs. 0.47), pain/discomfort (1.58 vs. 1.10), and anxiety/depression (1.03 vs. 0.67). The overall ceiling effects decreased from 56.7 % (3L) to 36.7 % (5L). Conclusion The 5L index score is more discriminative than the 3L index score in T2DM patients and therefore is preferable for use in this population.
How the EQ-5D Utilities Are Derived Matters in Chinese Diabetes Patients: a Comparison Based on Different EQ-5D Scoring Functions for China.

Abstract:OBJECTIVES:In China, multiple approaches to calculating EQ-5D utilities are available, including the two EQ-5D-3L (3L2014 and 3L2018) scoring functions, the EQ-5D-5L (5L) scoring function, and the crosswalk function linking the 3L utilities and 5L health states. The study compared utilities derived from them in terms of agreement and discriminative power; and assessed whether the use of different approaches may affect QALY estimation in Chinese type 2 diabetes (T2D) patients.METHODS:Cross-sectional data of 289 T2D patients who self-completed both the 5L and 3L questions were used. Agreement were examined using intraclass correlation coefficient (ICC) and Bland-Altman plots. The ability of the EQ-5D utilities in differentiating the patients with and without clinical conditions was evaluated using F-statistics. Their influence on QALY estimation was assessed adopting mean absolute difference (MAD) in utility values between the patients.RESULTS:The ICC values were 0.881 (3L2014-3L2018), 0.958 (5L-c5L2014), and 0.806 (5L-c5L2018). The two 3L utilities and the three 5L utilities had poor agreement at the lower end of utility scale according to Bland-Altman plots. The 3L2018 utilities had lower F-statistics compared to the 3L2014 utilities; the two c5L utilities had larger or similar F-statistics compared to the 5L utilities. The mean MADs were 0.138 (5L), 0.116 (3L2014), 0.115 (c5L2014), 0.055 (c5L2018), and 0.055 (3L2018).CONCLUSION:The 3L2014 utilities is more discriminative than the 3L2018 utilities; and the two c5L utilities have no worse discriminative power compared with the 5L utilities. The choice of the approach to calculating the EQ-5D utilities is likely to affect QALY estimates.
Interpretation and use of the 5-level EQ-5D response labels varied with survey language among Asians in Singapore

Abstract:Objectives: This study aimed to investigate the comparability of the English, Chinese, and Malay versions of the 5-level EQ-5D (EQ-5D-5L) response labels in Singapore. Study design and setting: Visitors to a primary care institution in Singapore (n = 743) were asked to complete two exercises: (1) rating the severity of the EQ-5D-5L response labels presented in English, Chinese, or Malay using a 0-100 numeric rating scale and (2) using the labels to describe various hypothetical health problems. Label ratings and choices between language versions were compared using regression analysis. Results: Perceived severity of the English and Chinese labels was similar. Compared with their English counterparts, the Malay label "slight(ly)" was rated as more severe (adjusted mean difference: 10.5 to 14.5) and "unable"/"extreme(ly)" as less severe (adjusted mean difference: -13.3 to -11.0) (P < 0.001 for all). The Malay labels "no(t)" and "unable"/"extreme(ly)" and the less severe Chinese labels were more frequently used to describe hypothetical health problems than their English counterparts. Conclusion: Interpretation and use of the EQ-5D-5L response labels vary among Singaporeans using different language versions of the instrument. Future studies need to investigate ways to reduce the variations and increase the cross-cultural measurement equivalence of the instrument.
Validation of the EQ-5D in a general population sample in urban China

Abstract:Purpose To evaluate the reliability and validity of the EQ-5D in a general population sample in urban China. Methods Thousand and eight hundred respondents in 18 communities of Hangzhou, China were recruited by multi-stage stratified random sampling. Respondents self-administered a questionnaire including the EQ-5D, the SF-36, and demographic questions. Test–retest reliability at 2-week intervals was evaluated using Kappa coefficient, the intraclass correlation coefficient. The standard error of measurement (SEM) was used to indicate the absolute measurement error. Construct validity was established using convergent, discriminant, and known groups analyses. Results Complete data for all EQ-5D dimensions were available for 1,747 respondents (97%). Kappa values were from 0.35 to 1.0. The ICCs of test–retest reliability were 0.53 for the EQ-5D index score and 0.87 for the EQ VAS score. The SEM values were 0.13 (9.22% range) and 4.20 (4.20% range) for the EQ-5D index and EQ VAS scores, respectively. The Pearson’s correlation coefficients between the EQ-5D and the SF-36 were stronger between comparable dimensions than those between less comparable dimensions, demonstrating convergent and discriminant evidence of construct validity. The Chinese EQ-5D distinguished well between known groups: respondents who reported poor general health and chronic diseases had worse HRQoL than those without. Older people, females, people widowed or divorced, and those with a lower socioeconomic status reported poorer HRQoL. Respondents reporting no problems on any EQ-5D dimension had better scores on the SF-36 summary scores than those reporting problems. Conclusions The Chinese version of the EQ-5D demonstrated acceptable construct validity and fair to moderate levels of test–retest reliability in an urban general population in China.

Abstract:To validate and compare the psychometric properties of two generic preference-based HRQoL instruments, EuroQol (EQ-5D) and Short-Form 6D (SF-6D) among psoriasis patients in China. Validity of the EQ-5D and SF-6D was examined with the patients in conjunction with Dermatology Life Quality Index (DLQI). Responsiveness was tested using the effect size (ES), relative efficiency (RE) and receiver operating characteristic (ROC) curves. Agreement between EQ-5D and SF-6D was tested using intra-class correlation coefficient (ICC) and Bland-Altman plot. A total of 150 moderate to severe patients were included with 50% female, mean age of 43.87 and mean disease duration of 5.19 years. The mean utility scores (SD) were 0.64 (0.32) for EQ-5D and 0.72(0.12) for SF-6D. There were no serious floor effects for EQ-5D and SF-6D but large ceiling effects for EQ-5D existed. Validity was demonstrated by the moderate to strong correlation coefficients (range: 0.50-0.71, P<0.001) for six of the ten hypotheses in both instruments. Both of EQ-5D and SF-6D showed a well discriminative capacity similarly (ES=0.98-1.27, ES= 0.99-1.26) between groups with different psoriasis specific health status. RE showed that SF-6D (6.00-39.00%) was more efficient than EQ-5D in three domains of DLQI ( recreation, relationships and treatment). Conversely, SF-6D (-1.00 - -13.00%) was less efficient than EQ-5D in three domains of DLQI ( symptoms and feelings, daily activities and work and studies). The areas under ROC of them all exceeded 0.5 (0.71- 0.84, P<0.001). Poor agreement between them was observed with ICC (0.38, P<0.001) and Bland-Altman plot analysis. This study provides evidence that EQ-5D and SF-6D are valid and sensitive preference-based HRQoL instruments in Chinese patients with moderate to severe psoriasis. SF-6D may be a more effective tool with lower ceiling effect. Further study is needed to compare other properties, such as reliability and longitudinal response.

Abstract:PurposeTo compare the measurement precision and related properties between the 5-level EuroQoL Group’s 5-dimension (EQ-5D-5L) questionnaire and the Functional Assessment of Cancer Therapy—Breast (FACT-B) questionnaire in assessing breast cancer patients.MethodsAn observational study of 269 Singaporean breast cancer patients. To compare discriminative ability and responsiveness, the effect sizes (in standard deviation) of the EQ-5D-5L and the FACT-B in relation to health indicators and the change in performance status or quality of life were estimated. Test–retest reliability was examined using the intraclass correlation (ICC).ResultsUsing performance status, evidence of disease, and treatment status as the criteria, the differences (FACT-B minus EQ-5D-5L) in the effect size for discriminative ability were negative or closed to zero, and the 90 % confidence intervals totally fell within the zone that indicated the non-inferiority of the EQ-5D-5L. For responsiveness and test–retest reliability, the confidence intervals of the differences in effect size and ICC overlapped the non-inferiority margin; thus, non-inferiority in these two aspects could neither be confirmed nor rejected.ConclusionsThe EQ-5D-5L was non-inferior to the FACT-B in discriminating breast cancer patients with different health conditions cross-sectionally. The EQ-5D-5L serves as a reasonable alternative or supplementary instrument to the FACT-B in assessing breast cancer patients’ health outcomes.

Abstract:Objective:Use EQ-5D to evaluate the health related quality of life of patients with type 2 diabetes,and analysis the main influence factors.Methods:EQ-5D and interviews were used to study the quality of life and disease states of 256 patients with type 2 diabetes at outpatient. Results:The mean score of all cases was 0.790±0.158.Mean EQ-5D score of patients with ideal blood glucose control group had is 0.836(0.810～0.861;95%Cl).Mean score of the other two groups were corresponding to 92.2%,84.8%of that of ideal control group,respectively.Score of patients with one or more diabetes complications was 0.692(0.667～0.717;95%Cl),while score of patients with no complications increased to 0.863(0.842～0.883;95%CI).The occurrence and progression of complications and the blood glucose control condition had great influence on the quality of life.Conclusion:EQ-5D is applied to evaluate the health states of type 2 diabetes patients,and will be useful in cost-effectiveness assessments.Unsatisfied blood glucose control and every complication are the main factors that lower the quality of life of patients.

Abstract:To compare the time trade-off (TTO) utility values of EQ-5D-5L health states to mainland Chinese, Singaporeans, and Thais, and to understand how valuation of EQ-5D-5L health states differ across these populations. We analyzed the TTO data from the Chinese, Singaporean, and Thai EQ-5D-5L valuation studies in which the utility values of 86 EQ-5D-5L health states were elicited from general population samples using the EQ-VT composite TTO (cTTO) method. In a computer-assisted personal interview, each participant valued 10 EQ-5D-5L health states. The TTO data was analyzed using random-effects linear regression models to examine its association with the main effect of country and the interactions between country and health-state severity as well as severity of the individual health dimensions. A total of 1,323 mainland Chinese, 1,000 Singaporeans, and 1,265 Thais provided data. All health states considered, the mean TTO value was 0.28 for mainland Chinese, 0.07 for Singaporeans, and 0.36 for Thais. The adjusted difference (95% confidence interval [95%CI]) was 0.18 (0.15, 0.22) between mainland Chinese and Singaporeans, -0.11 (-0.14, -0.09) between mainland Chinese and Thais, and 0.28 (0.25, 0.31) between Thais and Singaporeans. The difference in TTO values between Singaporeans and mainland Chinese or Thais increased with the increasing health-state severity, and the widening between-country differences were because Singaporeans were more sensitive to increasing severity of self-care problems, pain/discomfort, and anxiety/depression than mainland Chinese and more sensitive to pain/discomfort and anxiety/depression than Thais. The difference between mainland Chinese and Thais decreased with increasing health-state severity, and the diminishing difference between them was mainly because the effect of increasing anxiety/depression on valuation was smaller in mainland Chinese than in Thais. The TTO values of EQ-5D-5L health states differ among those countries, and the magnitude of the differences varies with severity and type of health problems.
Comparing EQ-5D-3L and EQ-5D-5L performance in common cancers: suggestions for instrument choosing

Abstract:Purpose: To compare the performance of three-level EuroQol five-dimensions (EQ-5D-3L) and five-level EuroQol five-dimensions (EQ-5D-5L) among common cancer patients in urban China. Methods: A hospital-based cross-sectional survey was conducted in three provinces from 2016 to 2018 in urban China. Patients with breast cancer, colorectal cancer, or lung cancer were recruited to complete the EQ-5D-3L and EQ-5D-5L questionnaires. Response distribution, discriminatory power (indicator: Shannon index [H'] and Shannon evenness index [J']), ceiling effect (the proportion of full health state), convergent validity, and health-related quality of life (HRQoL) were compared between the two instruments. Results: A total of 1802 cancer patients (breast cancer: 601, colorectal cancer: 601, lung cancer: 600) were included, with the mean age of 55.6 years. The average inconsistency rate was 4.4%. Compared with EQ-5D-3L (average: H' = 1.100, J' = 0.696), an improved discriminatory power was observed in EQ-5D-5L (H' = 1.473, J' = 0.932), especially contributing to anxiety/depression dimensions. The ceiling effect was diminished in EQ-5D-5L (26.5%) in comparison with EQ-5D-3L (34.5%) (p < 0.001), mainly reflected in the pain/discomfort and anxiety/depression dimensions. The overall utility score was 0.790 (95% CI 0.778-0.801) for EQ-5D-3L and 0.803 (0.790-0.816) for EQ-5D-5L (p < 0.001). A similar pattern was also observed in the detailed cancer-specific analysis. Conclusions: With greater discriminatory power, convergent validity and lower ceiling, EQ-5D-5L may be preferable to EQ-5D-3L for the assessment of HRQoL among cancer patients. However, higher utility scores derived form EQ-5D-5L may also lead to lower QALY gains than those of 3L potentially in cost-utility studies and underestimation in the burden of disease.
EQ-5D-3L DECREMENTS BY DIABETES COMPLICATIONS AND COMORBIDITIES IN CHINA

Abstract:To estimate the decrements on EQ-5D-3L scores by diabetes complications and comorbidities in type 2 diabetes patients with ongoing oral drug treatments in China. The secondary database study analyzed 9,570 type 2 diabetes patients with ongoing oral drugs treatment who were enrolled from 75 hospitals in 9 cities in China. Surveys were used to collect information including demographic data, health conditions, oral treatment history, and health insurance coverage situation. The EuroQol (EQ-5D-3L) scale was used to collect health utility and was scored by a Chinese version EQ-5D-3L conversion formula. Linear regression was used to analyze the associations between complications and health utility. Among 7,081 patients completed survey data, mean age was 59.6 (SD=12.64) years old, 3,479 (49.13%) patients were females, average duration of type 2 diabetes was 7.91 (SD=6.22) years. 5180 (73.28%) patients had at least one complication. The most common complication was hypertension (51.25%), followed by hyperlipidemia (28.90%). Other complications included cardiovascular disease, stroke, retinopathy, nephropathy, diabetic foot, and hypoglycemia. Average number of complications among participants was 1.54. Each complication had negative influence on the EQ-5D-3L score Among patients with complications, diabetic foot had the worst effect on EQ-5D-3L score (coefficient=-0.12, p=.000), followed by stroke (coefficient=-0.99, p=0.000) and nephropathy (coefficient=-0.06, p=0.000). The common complications and comorbidities among type 2 diabetes patients with ongoing oral drug treatments have significant decrements on health utility scores, especially for those with diabetic foot, stroke, and nephropathy.
Comparing measurement properties of EQ-5D and SF-6D in East and South-East Asian populations: a scoping review

Abstract:Introduction: Euro-Qol-5 Dimension (EQ-5D) and Short-Form 6-Dimension (SF-6D) are the most commonly used generic multi-attribute utility instruments (MAUI) to calculate quality-adjusted life-years (QALYs) in East and South-East Asia. This study aims to systematically review and summarize current evidence on comparing measurement properties of EQ-5D and SF-6D in East and South-East Asian populations. Areas covered: Guided by the PRISMA systematic review and meta-analysis guidelines, a systematic literature search was conducted in databases of PubMed, Web of Science, MEDLINE, EMBASE and CINAHL (until June 2022) to obtain studies which compared measurement properties (feasibility, reliability, validity, responsiveness, and sensitivity) and agreement of EQ-5D and SF-6D in the populations. Expert opinion: In general, both EQ-5D and SF-6D had good measurement properties in East and South-East Asian populations; but their utility scores cannot be used interchangeably. Compared to the 3-level EQ-5D, SF-6D had better sensitivity and lower ceiling effects, but the comparison results between the 5-level EQ-5D and SF-6D were inconsistent across populations. This scoping review found that most studies did not consider order effects, did not specify the versions of SF-6D, and ignored certain measurement properties (reliability, content validity, and responsiveness). These aspects need to be further explored in future studies.
Health-related Quality of Life in Older Chinese Patients with Diabetes

Abstract:Background Although older diabetes patients with unique characteristics should be cared carefully to improve their health-related quality of life (HRQOL), the association between diabetes and HRQOL remain unclear, especially in Asians. We aimed to compare the HRQOL between older Chinese patients with type 2 diabetes (T2D) and their age-gender-matched controls. Methods Older patients with T2D were recruited from a community hospital in Suzhou located in the east part of China while controls were selected from a community-based health survey of older adults aged 60 years or older. HRQOL of cases and controls was assessed by the EQ-5D-3L. The impact of T2D on HRQOL was investigated using a liner regression model and the relationship between T2D and EQ-5D health problems was evaluated using logistic regression models. Results A total of 220 cases and 440 controls were included. The mean age of the participants was 68.8 years and women accounted for 69.1% of the study sample. The EQ-5D-3L index score was lower for older people with T2D (0.886) than their controls (0.955). After multivariable adjustment, the difference in ED-5D-3L index score between older people with and without T2D was 0.072. In logistic regression analyses, T2D was positively associated with reporting of problems in mobility (odds ratio [OR] = 5.00); pain/discomfort (OR = 1.66), and anxiety/depression (OR = 3.2). Conclusions T2D has a detrimental effect on HRQOL of older Chinese people.

Abstract:Objectives: To compare the time trade-off (TTO) utility values of EQ-5D-5L health states elicited from different general populations in Asia. Methods: We analyzed the TTO data from seven Asian EQ-5D-5L valuation studies in which utility values of 86 EQ-5D-5L health states were elicited from general population samples. An eight-parameter multiplicative regression model including five dimension parameters (mobility [MO], self-care, usual activities [UA], pain/discomfort, anxiety/depression) and three level parameters (level 2 [L2], level 3 [L3], and level 4 [L4]) was used to model the data from each of the populations. The model coefficients were compared to understand how the valuations of EQ-5D-5L health states differ. Results: For dimension parameters, Korea and Indonesia generally had the highest and lowest values among the populations, respectively; UA and MO commonly had the highest and lowest values among the parameters, respectively. For level parameters, Singapore and Korea generally had the highest and lowest values, respectively; L2 showed less variance compared to L3 and L4. Koreans, Indonesians, and Singaporeans appeared to have different health preferences compared with other populations. Conclusion: Utility values of EQ-5D-5L health states differ among Asian populations, suggesting that each health system should establish and use its own value set.

Abstract:Disease burden of type 2 diabetes (T2DM) is escalating. Quite a few studies have evaluated the health-related quality of life (HRQOL) of overall T2DM patients, but few focused on that of inpatients with specific diabetes-related complications. Reliable estimates of the impact of complications on HRQOL are important to understand the event burden and serve for economic evaluation. This study aims to estimate the utility-based HRQOL of inpatients with diabetes-related complications in China, and explore the influencing factors. This is a cross-sectional study conducted in a tertiary hospital in Shandong, China. T2DM patients (≥18 years) hospitalized during January, April and May 2017 for their complications, including ischemic heart disease (IHD), myocardial infarction (MI), congestive heart failure (CHF), stroke, impaired vision (IV), end-stage renal disease (ESRD), ulcer or amputation were recruited. The EuroQOL 5-dimensions 3-level(EQ-5D-3L) instrument was administered to these inpatients through face-to-face interviews. Clinical and sociodemographic data were collected from the survey. One-way analysis of variance and multiple linear regression analysis were performed. 802 T2DM inpatients (female 43%) provided data. Mean age was 62.67(SE:0.4) years. The most prevalent problems reported were impairments in Usual Activities. Mean EQ-5D index score of the total sample was 0.562(SE:0.007), with EQ-VAS score of 61.72(SE:0.606). HRQOL varied between complications with significant differences. Mean EQ-5D index scores were: IHD 0.620(0.012), MI 0.434(0.020), CHF 0.471(0.019), stroke 0.472(0.018), IV 0.714(0.012), ESRD 0.693(0.012), ulcer 0.431(0.029), and amputation 0.395(0.027). Mean EQ-VAS scores were: IHD 65.26(1.120), MI 58.84(1.722), CHF 56.70(1.800), stroke 58.59(1.565), IV 66.18(1.418), ESRD 63.57(1.568), ulcer 60.86(2.589), and amputation 55.00(5.426). Complication, exercise, confidence in treatment, worry about the disease were reported to influence the HRQOL. HRQOL of T2DM inpatients was largely impaired. Our estimates demonstrate the magnitude of the impact of eight complications on utility-based measures of HRQOL, which may be used for public health and cost-effectiveness modeling.
Validity and measurement equivalence of EQ-5D-5L questionnaire among heart failure patients in Malaysia: a cohort study

Abstract:Aim: This study aimed to examine the validity of EQ-5D-5L among HFrEF patients in Malaysia, and to explore the measurement equivalence of three main language versions. Methods: We surveyed HFrEF patients from two hospitals in Malaysia, using Malay, English or Chinese versions of EQ-5D-5L. EQ-5D-5L dimensional scores were converted to utility scores using the Malaysian value set. A confirmatory factor analysis longitudinal model was constructed. The utility and visual analog scale (VAS) scores were evaluated for validity (convergent, known-group, responsiveness), and measurement equivalence of the three language versions. Results: 200 HFrEF patients (mean age = 61 years), predominantly male (74%) of Malay ethnicity (55%), completed the admission and discharge EQ-5D-5L questionnaire in Malay (49%), English (26%) or Chinese (25%) languages. 173 patients (86.5%) were followed up at 1-month post-discharge (1MPD). The standardized factor loadings and average variance extracted were ≥ 0.5 while composite reliability was ≥ 0.7, suggesting convergent validity. Patients with older age and higher New York Heart Association (NYHA) class reported significantly lower utility and VAS scores. The change in utility and VAS scores between admission and discharge was large, while the change between discharge and 1MPD was minimal. The minimal clinically important difference for utility and VAS scores was ±0.19 and ±11.01, respectively. Malay and English questionnaire were equivalent while the equivalence of Malay and Chinese questionnaire was inconclusive. Limitation: This study only sampled HFrEF patients from two teaching hospitals, thus limiting the generalizability of results to the entire heart failure population. Conclusion: EQ-5D-5L is a valid questionnaire to measure health-related quality of life and estimate utility values among HFrEF patients in Malaysia. The Malay and English versions of EQ-5D-5L appear equivalent for clinical and economic assessments.

Abstract:Abstract Background This study aimed to explore the association between diabetes mellitus and health-related quality of life (HRQoL) of older people in Shanghai, China, especially regarding the differences in each aspect of the EQ-5D and how large the score gaps are between older people with and without diabetes. Methods A total of 11,103 people of either sex older than 60 years were enrolled from 17 districts of Shanghai. The EQ-5D-3L was used to assess the HRQoL of older people. The Wilcoxon rank sum test and t-test were used to compare the difference in HRQoL between people with or without diabetes. After univariate regression, multiple linear regression and ordinal logistic regression were conducted to evaluate the influence of diabetes mellitus and other confounding variables on the EQ VAS scores and on the five dimensions of the EQ-5D. Results Twelve percent of all participants had diabetes mellitus, and the proportion was almost the same between men and women. The EQ VAS scores of people with diabetes mellitus were approximately 3.70 points lower than those of people without diabetes (95% CI = -4.40, -2.99, p < 0.001) after adjusting for confounding variables. People with diabetes mellitus had increased problems with mobility (OR = 1.57, 95% CI = 1.33, 1.85), self-care (OR = 1.65, 95% CI = 1.35, 2.01), usual activities (OR = 1.78, 95% CI = 1.51, 2.11), pain/discomfort (OR = 1.42, 95% CI = 1.24, 1.64), and anxiety/depression (OR = 1.33, 95% CI = 1.07, 1.64). Conclusions This study showed that diabetes mellitus was associated with the HRQoL of older people and that older people with diabetes had poorer performance in every aspect of EQ-5D measurements.

Abstract:BackgroundUsed the EuroQoL-5 dimension (EQ-5D) to evaluate the health status of 5310 residents who live in East China, and compared the health-related quality of life (HRQoL) with 311 patients with type 2 diabetes as well as to explore the main influence factors to HRQoL in East China.MethodsThe cohort includes 5310 participants aged 18-89 years old lived in East China. EuroQoL-5 dimension (EQ-5D) scale was used for the assessment of health-related quality of life.ResultsThe mean age of the cohort was 52.2 ± 13.4 years of which 43.7% were male. A moderate level of health-related quality of life was measured of that EQ-5D index and EQ-VAS scores were 0.939 ± 0.111 and 80.06 ± 11.58, respectively. There was a significant difference between diabetes patients and non-diabetes (p = 0.029, p < 0.001, respectively). The age had an inverse correlation with the EQ-5D scores both in general population and diabetes patients. The EQ-5D Vas was weakly adversely associated with the FPG, HbA1c and HOMA-IR.ConclusionThe overall health-related quality of life of population in East-China was moderate. Diabetes patients had lower score of health-related quality. The healthy-related quality was associated with the age, gender, economic development of region, level of education and marital status.

Abstract:Objective Two EQ-5D-3L (3L) value sets (developed in 2014 and 2018) co-exist in China. The study examined the level of agreement between index scores for all the 243 health states derived from them at both absolute and relative levels and compared the responsiveness of the two indices. Methods Intraclass correlations coefficient (ICC) and Bland-Altman plot were adopted to assess the degree of agreement between the two indices at the absolute level. Health gains for 29,403 possible transitions between pairs of 3L health states were calculated to assess the agreement at the relative level. Their responsiveness for the transitions was assessed using Cohen effect size. Results The mean (SD) value was 0.427 (0.206) and 0.649 (0.189) for the 3L(2014) and 3L(2018) index scores, respectively. Although the ICC value showed good agreement (i.e., 0.896), 88.9% (216/243) of the points were beyond the minimum important difference limit according to the Bland-Altman plot. The mean health gains for the 29,403 health transitions was 0.234 (3L(2014) index score) and 0.216 (3L(2018) index score). The two indices predicted consistent transitions in 23,720 (80.7%) of 29,403 pairs. For the consistent pairs, Cohen effective size value was 1.05 (3L(2014) index score) or 1.06 (3L(2018) index score); and the 3L(2014) index score only yielded 0.007 more utility gains. However, the results based on the two measures varied substantially according to the direction and magnitude of health change. Conclusion The 3L(2014) and 3L(2018) index scores are not interchangeable. The choice between them is likely to influence QALYs estimations.
