Abstract:OBJECTIVES:To test the effectiveness of sealant and flowable composite coating on eroded enamel, dentin and cementum under erosive/abrasive challenges in vitro.METHODS:A total of 108 tissue sections (36 each for enamel, dentin and cementum) from third molars were assigned to three groups: Seal & Protect sealant (S&P), Tetric EvoFlow composite (TEF) and control. Erosive/abrasive lesions were created on each specimen by citric acid and brushing with toothpaste. S&P and TEF were applied to the lesions and subjected to erosive/abrasive cycling included 24 cycles of immersion in citric acid (pH 3.6) for 60min, followed by remineralization for 120min and brushing with toothpastes for 600 strokes at 150g. Erosive wear of materials or dental tissues were measured with 3D scanning microscopy and data were analyzed using ANOVA.RESULTS:Treatments with S&P and TEF created a protective material coating of 42.7±17.8μm and 150.8±9.9μm in thickness, respectively. After 24 cycles of erosive/abrasive challenges, tissue losses were -346.9±37.3μm for enamel, -166.5±26.3μm for dentin and -164.7±18.2μm for cementum in untreated controls, as compared to material losses of -24.4±3.3μm for S&P, and -10.8±4.4μm for TEF, respectively. Both S&P and TEF were effective in protecting enamel, dentin and cementum against erosive tooth wear (p<0.01). S&P exhibited faster wear than TEF (p<0.01) and showed spotted peeling in a third of the specimens. TEF remained intact on all three types of dental tissues at the end of the 24 cycles of erosive/abrasive challenges.CONCLUSIONS:A thin coating of flowable composite resin 150μm in thickness may provide long-term protection against erosive/abrasive tooth wear. Resin sealant may provide adequate protection for dental hard tissues in short-term and may require repeated applications if long-term protection is desired.
Effect of Two Kinds of Materials on the Prevention of Early Erosive Enamel Wear

Abstract:Objective To investigate the preventive effect of resin infiltration and adhesive on early erosive enamel wear. Methods Orthodontic reduction premolars collected from Central Laboratory of Nanjing Stomatological Hospital, Medical School of Nanjing University were used to prepare 70 specimens. Forty samples were divided into eight groups (n=5) and treated with different conditions (pH=1.6, 2.4, 3.2 or 4.0 hydrochloric acid solution, etching time was 30 or 60 s), and the conditions for obtaining early erosive enamel samples were selected. Based on this procedure, thirty early erosive enamel samples were made and divided into three groups: control group, resin infiltration group, and adhesive group. And the treatment of 30 days acid abrasion cycle was carried out. Confocal microscopy was used to measure the thickness changes of enamel or material before and after cycle. Results Early erosive enamel samples was obtained when pH was 4.0 and etching time was 60 s. After 30 days cycle, the wear of enamel was (29.71±6.72) μm in control group, (5.60±2.24) μm in resin infiltration group and (2.89±1.03) μm in adhesive group. In infiltration group and adhesive group, lower enamel was not affected by the cycle, and the material loss ratios of the infiltration resin group and adhesive group were 0.41 ± 0.14 and 0.29 ± 0.13, respectively. The ratio of material loss was not significantly different (P>0.05). But infiltration group lost (12.95±2.22) μm of enamel during the application of the material. Conclusions Resin infiltration and adhesive have the same short?term protective effect against early erosive enamel wear. Adhesive has less damage to enamel and better effect.
The Enamel Softening and Loss During Early Erosion Studied by Afm, Sem and Nanoindentation

Abstract:Enamel dissolution occurs when it contacts with acids produced by plaque bacteria, foods or drinks. There have been numerous and varied studies quantifying and characterizing the rate, extent and chemical aspects of enamel erosion; however, there is still hot debate about the amounts of enamel softening and loss. The objective of this study was to measure the enamel erosion process with high accuracy. Native third molars were partly eroded in citric acid ( pH = 3.8) with some domains protected by a Ti coating layer. The surface morphology and structures before and after exposure to citric acid for different time periods were studied by AFM, SEM and nanoindentation, respectively. Based on this, the functions between the amounts of enamel softening, loss and erosion time were established for the first time. It was demonstrated that AFM, SEM and nanoindentation were suitable for measuring the early stages of enamel demineralization qualitatively and quantitatively.
Erosion of CAD/CAM restorative materials and human enamel: An in situ/in vivo study

Abstract:This in situ/in vivo study aimed to evaluate the effects of erosion on the surface microhardness, substance loss, and surface roughness of CAD/CAM restorative materials and human enamel. This study used a 2-treatment (14 days each) crossover design with 8 healthy volunteers. Each volunteer wore an intraoral appliance containing 3 CAD/CAM restorative material specimens [IPS e.max CAD, Lava Ultimate CAD/CAM Restorative, and a poly (methyl methacrylate) (PMMA) block for CAD/CAM] and 1 human enamel specimen. The specimens were subjected to in vivo erosion cycles by rinsing with 150 ml of cola drink (4 <math>×</math> 5 min/day) for 14 days. The surface microhardness, substance loss, and surface roughness of the specimens were measured at baseline (T1), day 7 (T2) and day 14 (T3). The data were statistically analyzed using repeated measures ANOVA and Tukey's test (α = 0.05). After erosion, significant increases in substance loss and surface roughness were observed for enamel, though the surface roughness and substance profile of the tested restorative materials remained unchanged. Erosion significantly decreased the surface microhardness of all materials. For IPS e.max and Lava Ultimate, a significantly higher percentage of surface microhardness loss (%SMHl) was found at T3 than at T2, while no significant difference was found between T2 and T3 for enamel and the PMMA block. In conclusion, CAD/CAM restorative materials showed smaller changes in surface roughness and the surface profile than human enamel after in situ/in vivo erosion. However, CAD/CAM restorative materials and human enamel showed similar changes in surface microhardness after in situ/in vivo erosion.

Methodology One hundred and forty bovine incisors were collected and divided into two groups: 72 enamel and 72 dentin blocks (4×4 mm). Half of the specimens were restored with CR (Filtek Z350 XT) and the other half with RMGIC (Fuji II LC). Then, samples were submitted to a demineralization cycle (5 days, 4×2 min/day, 1% citric acid, pH 3.2) and exposed to three different toothpastes (2×15 s/day): without fluoride (WF, n=12), sodium fluoride-based (NaF, n=12), and stannous fluoride-based (SnF2, n=12). Surface wear, as well as restoration interfaces wear, were investigated by profilometry of the dental substrates and restorative materials. All representative surfaces underwent AFM analysis. Data were analyzed by two-way analysis of variance and Tukey's tests (α=0.05). Results NaF-based toothpaste caused the greater dentin surface wear (p<0.05). Toothpastes affected only enamel-restoration interfaces. AFM analysis showed precipitate formation in dentinal tubules caused by the use of fluoride toothpastes. Conclusions NaF-based toothpastes had no protective effect on enamel adjacent to CR and RMGIC against erosion-abrasion challenges, nor on dentin adjacent to RMGIC material. SnF2-based toothpastes caused more damage to interfaces between enamel and RMGIC.

The Effects of Three Different Food Acids on the Attrition-Corrosion Wear of Human Dental Enamel

Abstract:With increased consumption of acidic drinks and foods, the wear of human teeth due to attrition in acidic environments is an increasingly important issue. Accordingly, the present paper investigates in vitro the wear of human enamel in three different acidic environments.Reciprocating wear tests in which an enamel cusp slides on an enamel flat surface were carried out using acetic, citric and lactic acid lubricants (at pH 3-3.5). Distilled water was also included as a lubricant for comparison. Focused ion beam milling and scanning electron microscopy imaging were then used to investigate the enamel subsurfaces following wear tests. Nanoindentation was used to ascertain the changes in enamel mechanical properties.The study reveals crack generation along the rod boundaries due to the exposure of enamel to the acidic environments. The wear mechanism changes from brittle fracture in distilled water to ploughing or shaving of the softened layer in acidic environments, generating a smooth surface with the progression of wear. Moreover, nanoindentation results of enamel samples which were exposed to the above acids up to a duration of the wear tests show decreasing hardness and Young's modulus with exposure time.
Comparative study on the impact-sliding wear behaviour of CAD/CAM resin-ceramic materials and tooth enamel

Abstract:Objectives: To compare the impact-sliding wear of different CAD/CAM resin-ceramic materials and tooth enamel, and explore the corresponding wear damage mechanism. Methods: Human tooth enamel (EN), Vita ENAMIC (Vita, VE), Lava Ultimate (3 M, LU), and GC CERASMART (GC, CS) were used in this study. The hardness, elastic modulus, and roughness values of the samples were measured. Further, impact-sliding wear tests were performed in a ball-on-flat configuration with spherical zirconia antagonists and the coefficients of friction (CoF) were recorded simultaneously. Additionally, a white light interferometer was used to determine the volume losses and scanning electron microscopy was used to observe the wear morphology of the wear scars and the damage feature in the vertical sections to clarify the damage mechanism during the impact-sliding wear test. Results: EN exhibited the highest elastic modulus and CoF, followed by VE, LU, and CS. The hardness and roughness of EN and VE were similar and were higher than those of LU and CS. Throughout the wear tests, VE exhibited the highest volume loss, whereas CS exhibited the lowest. The wear damage characteristics of VE were similar to those of EN, displaying brittle fractures of inorganic substances and plastic deformation of organic substances in the impact part, exhibiting plough marks in the sliding parts. In the case of LU and CS, the entire wear areas displayed plastic deformation of the resin matrix, exfoliation of the filler particles, and plough marks. Significance: Enamel and polymer-infiltrated ceramic network materials exhibit similar wear damage modes. Additionally, the high-density nanocomposite resin material is the most resistant to impact-sliding wear from a tribological perspective.
Enamel erosion and prevention efficacy characterized by confocal laser scanning microscope

Abstract:The aim of this study was to evaluate the erosion-inhibiting effect of two toothpastes on the development of erosion-like lesions, by a confocal laser scanning microscope (CLSM). Forty human enamel blocks were divided into five groups (n = 8), in accordance to evaluate the GC MI Paste Plus and Oral B with stannous fluoride, applied as slurries and associated with toothbrush. Specimens were submitted to an erosion challenge from citric acid (0.5%, pH = 2.8), for 5 min, six times a day, alternating in artificial saliva immersions. Reference group was not exposed to treatment. Part of specimens (Groups 02 and 03) was exposed twice daily just to slurries, for 2 min, therefore specimens from Groups 04 and 05 were also abraded, for 30 s. The enamel surfaces were morphological characterized using CLSM images, with mineral loss being measured using the resulting 3D images referenced to an un-challenged portion of the sample. Step values were compared using the one-way ANOVA test. CLSM was shown to be a viable, noncontact, and simple technique to characterize eroded surfaces. The statistical difference in the step size was significant between the groups (P = 0.001) and using multiple comparisons a statistically significant protective effect of toothpastes was shown when these were applied as slurries. Although groups submitted to tooth brush showed mineral loss similar to reference control group, due to the damages of abrasion associated.

Abstract:This in vitro study used the same frequency and duration of acid contact as a previous in situ/in vivo study to evaluate the effect of erosion on CAD/CAM restorative materials and human enamel and to compare the effects of in vitro and in situ/in vivo acid challenges on CAD/CAM restorative materials and human enamel. The CAD/CAM restorative materials (IPS e.max CAD, Lava Ultimate, and PMMA block) and human enamel were eroded by immersion in 150 ml of cola drink for 14 days (4 <math>×</math> 5 min/day). The surface microhardness and surface roughness of the specimens were measured at baseline (T1), day 7 (T2), and day 14 (T3). The substance losses were measured at T2 and T3. The data were statistically analyzed using repeated measures ANOVA and Bonferroni's test (α = 0.05). Erosion significantly decreased the surface microhardness of the CAD/CAM restorative materials and human enamel (all P < 0.001). The overall percentage of surface microhardness loss (%SMHl) of the PMMA block and enamel due to in vitro erosion was significantly higher than that due to in situ/in vivo erosion (P = 0.02 and P < 0.001, respectively). Consistent with in situ/in vivo erosion, the surface roughness and profile of the tested restorative materials remained unchanged after in vitro erosion. A significant increase in the surface roughness and substance loss was observed for enamel after in vitro erosion (all P < 0.001). The overall substance loss of enamel due to in vitro erosion was significantly higher than that due to in situ/in vivo erosion (P < 0.001). In conclusion, erosion decreased the surface microhardness of the CAD/CAM restorative materials and human enamel. Moreover, erosion negatively influenced the substance loss and surface roughness of human enamel. For the substance loss of enamel and %SMHl of PMMA block and enamel, the in vitro erosive effects were approximately 1–2 times greater than the in situ/in vivo effects. However, for the surface roughness and profile of the CAD/CAM restorative materials, no significant difference was found between in vitro and in situ/in vivo erosion.

[The Effect of Air Abrasion on Enamel Composite Resin Adhesive Strength].

Abstract:OBJECTIVE:In this study we observed the effect of air abrasion on enamel-composite resin adhesive strength in vitro.METHODS:Thirty teeth were prepared and divided into three groups. One group was air abraded and was acid-etched, another one was air abraded, and the other one was acid-etched. Next the bond and resin were used. Then we observed and compared the shear bond strength. The data were analysed by Chi square(chi(2)) test.RESULTS:Enamel-composite resin adhesive strength were not affected by acid etching and air abrading, but can be affected by air abrading alone.CONCLUSION:In clinic the traditional acidetch technique that used in composite resin restoration can be replaced with air abrasion surface preparation technique.
Wear resistance and abrasiveness of CAD-CAM monolithic materials

Abstract:Statement of problem: Computer-aided design and computer-aided manufacturing (CAD-CAM) restorations are in contact with the antagonist tooth, either a natural tooth or a restoration. Therefore, clinicians should be aware of the wear resistance of CAD-CAM materials and the wear behavior of the antagonist. Purpose: The purpose of this in vitro study was to evaluate the wear resistance and abrasiveness of CAD-CAM materials. Material and methods: In a 2-body wear test, the materials IPS e.max CAD (Ivoclar Vivadent AG), Vita Suprinity (Vita Zahnfabrik), Lava Ultimate (3M ESPE), Vita Enamic (Vita Zahnfabrik), and Lava Plus (3M ESPE) acted as abraders and, together with bovine enamel, also as antagonists. Each antagonist wheel ran against each abrader wheel for 200000 cycles, with a spring force of 15 N, and at a rotational speed of 1 Hz in distilled water. The wear rate was determined with a surface profilometer. The surfaces were observed with scanning electron microscopy, and their hardness, coefficient of friction, and roughness were evaluated. Results: Lava Plus and IPS e.max CAD exhibited the highest potential for wear of Lava Ultimate. These 2 materials, together with Vita Suprinity, provided the highest wear of enamel and Vita Enamic. Vita Suprinity and IPS e.max CAD had higher wear than Lava Plus, and the inverse was also true. Vita Enamic and Lava Ultimate were among the materials that caused the lowest wear of enamel and all other evaluated materials. Scanning electron microscopy images revealed that except for Lava Ultimate, all other materials damaged enamel, in which Vita Suprinity and IPS e.max CAD were more aggressive when sliding against the materials. Lava Plus had the greatest hardness, followed by Vita Suprinity and IPS e.max CAD, Vita Enamic, and then Lava Ultimate. The coefficient of friction varied from 0.42 to 0.53. The Vita Enamic and Lava Ultimate showed the highest surface roughness. Conclusions: The nanofilled composite resin and polymer-infiltrated ceramic were more antagonist-friendly (whether enamel or CAD-CAM material) than glass-ceramics and zirconia. Care should be taken when selecting the material that will contact mainly with glass-ceramics. Hardness should also be considered when selecting a material.
Enamel wear against monolithic zirconia restorations: A meta-analysis and systematic review of in vitro studies

Abstract:Objective: An assessment was performed to identify and evaluate dental enamel wear caused by monolithic zirconia restoration. Literature searches were conducted in PubMed, Science Direct, Cochrane Evidence, and the Cochrane Library up to May 2020. Material and methods: Studies were selected for systematic review according to the inclusion (articles conducted on the wear of enamel samples opposing monolithic zirconia) and exclusion (case reports, non-English articles, and monolithic zirconia samples facing other materials rather than human enamel) criteria. Of those, articles on polished and glazed monolithic zirconia subjected to a 50 N vertical load with a range of 240,000-250,000 cycles, equivalent to 1 year of in vivo mastication, were included in the meta-analysis. Results: In total, 3968 articles were pooled. Twenty-five articles met the inclusion criteria for the systematic review. Three studies were included in the meta-analysis. The results showed that the enamel wear against monolithic zirconia was within the statistically accepted level. Moreover, the polished monolithic zirconia surface caused less enamel wear than the glazed surface. Conclusion: This review indicates that monolithic zirconia restorations cause acceptable antagonist enamel wear. Moreover, the meta-analysis results agreed that the final restoration's surface texture plays an essential role in the wear process. Clinical significance: Monolithic zirconia restorations have been widely used in dental practice because they eliminate the chipping problems resulting from using veneered restorations. With recent technology development, monolithic zirconia has obtained more esthetic features and a more natural look. However, due to the high strength and surface roughness of monolithic zirconia, wear on the antagonist's teeth was detected. The results showed that this wear amount was statistically acceptable and lower than other ceramics such as feldspathic porcelain and enamel. Furthermore, surface treatment methods must be applied to minimize tooth wear, as polished or glazed surfaces interfere with enamel loss.
Friction and wear of dental CAD/CAM composite resin

Abstract:Objective To evaluate the friction and wear of CAD/CAM composite resin comparing with natural enamel. Methods The surface roughness ( Sa) of three kinds of CAD/CAM composite resins ( Upcera composite resin,Lava Ultimate,High-Class) and the natural enamel were determined by laser scanning confocal microscope. The specimens were tested on a micro friction and wear test apparatus in artificial oral environments, while the natural enamel served as antagonist. The wear morphology of the CAD/CAM composite resins was observed and the wear areas were measured by laser confocal microscope. The enamel wear scar width was measured by laser confocal microscope. Scanning electron microscopy was used to observe the wear surface micro-morphology of CAD/CAM composite resins and enamel. Statistical analysis was performed using one-way analysis of variance ( ANOVA ) . Results The wear areas and wear scar width of enamel were greater than that of the CAD/CAM composite resin materials (P <0. 05). Conclusion The three kinds of CAD/CAM composite resin materials had superior wear resistance and lower antagonistic wear comparing with natural enamel.
Wear of human enamel opposing monolithic zirconia, glass ceramic, and composite resin: an in vitro study

Abstract:Statement of problem: Demand is increasing for ceramic and composite resin posterior restorations. However, ceramics are recognized for their high abrasiveness to opposing dental structure. Purpose: The purpose of this study was to investigate the wear of enamel as opposed to dental ceramics and composite resin. Material and methods: Twenty-four test specimens (antagonists), 6 each of monolithic zirconia, glass ceramic, composite resin, and enamel, were prepared into cylindrical rods. Enamel specimens were prepared from 24 extracted human permanent molar teeth. Enamel specimens were abraded against each type of antagonist with a pin-on-disk wear tester under a constant load of 25 N at 20 rpm for 4800 cycles. The maximum depth of wear (Dmax), mean depth of wear (Da), and mean surface roughness (Ra) of the enamel specimens were measured with a profilometer. All data were statistically analyzed by 1-way ANOVA, followed by the Tukey test (α=.05). A paired t test was used to compare the Ra of enamel at baseline and after testing. The wear of both the enamel and antagonists was evaluated qualitatively with scanning electron microscopic images. Results: No significant differences were found in enamel wear depth (Dmax, Da) between monolithic zirconia (2.17 ±0.80, 1.83 ±0.75 μm) and composite resin (1.70 ±0.92, 1.37 ±0.81 μm) or between glass ceramic (8.54 ±2.31, 7.32 ±2.06 μm) and enamel (10.72 ±6.31, 8.81 ±5.16 μm). Significant differences were found when the enamel wear depth caused by monolithic zirconia and composite resin was compared with that of glass ceramic and enamel (P<.001). The Ra of enamel specimens increased significantly after wear tests with monolithic zirconia, glass ceramic, and enamel (P<.05); however, no difference was found among these materials. Conclusions: Within the limitations of this in vitro study, monolithic zirconia and composite resin resulted in less wear depth to human enamel compared with glass ceramic and enamel. All test materials except composite resin similarly increased the enamel surface roughness after wear testing.
Wear evaluation of the human enamel opposing different Y-TZP dental ceramics and other porcelains

Abstract:PURPOSE: This study examined the wear resistance of human enamel and feldspathic porcelain after simulated mastication against 3 zirconia ceramics, heat-pressed ceramic and conventional feldspathic porcelain.MATERIALS AND METHODS: Human teeth and feldspathic porcelain cusp were tested against ceramic discs. 5 brands were tested - 3 monolithic zirconia, Prettau, Lava, and Rainbow, one lithium disilicate, IPS e.max Press, and one feldspathic porcelain, Vita-Omega 900. The surface was polished using a 600 grit and 1200 grit SiC paper. Each group was loaded for 300,000 cycles in a chewing simulator. The wear resistance was analyzed by measuring the volume of substance lost. The wear surfaces were observed by scanning electron microscopy to determine the wear characteristics.RESULTS: Vita-Omega 900 led to the greatest amount of enamel wears followed by IPS e.max Press, Prettau, Lava and Rainbow. There was a significant difference between Vita-Omega 900 and IPS e.max Press (p<0.05). The wear values for human enamel were significantly greater than those for feldspathic porcelain, regardless of the surface roughness of the ceramic specimens (p<0.05).CONCLUSION: The wear behaviour of human enamel and feldspathic porcelain varies according to the type of substrate materials. On the other hand, 3 zirconia ceramics caused less wear in the abrader than the conventional ceramic.CLINICAL SIGNIFICANCE: Dental professionals should be aware of the wear effect of dental restorations on the opposing teeth or restorations. The amount of enamel wear was highest in feldspathic porcelains whereas zirconia ceramics caused less wear on the opposing teeth.

Effect of Acidic Solutions on the Surface Roughness and Microhardness of Indirect Restorative Materials: A Systematic Review and Meta-analysis

Abstract:Purpose: To evaluate the effects of acidic solutions on the surface roughness (SR) and surface microhardness (SMH) of indirect restorative materials by analyzing in vivo and in vitro studies. Materials and Methods: Studies comparing the SR and SMH of indirect restorative materials after erosion were included in the meta -analysis. Meta-analyses were conducted with a random-effects model at a significance level of P < .05 using Review Manager software (Cochrane Collaboration). The literature search was conducted using the Web of Science, MEDLINE (PubMed), and Cochrane Library databases, with no limitations on publication year. Results: Of the 1,097 potentially relevant studies, 32 full-text articles were assessed for eligibility. A total of 16 articles were included in the systematic review, and 15 were included in the meta-analyses. The kappa score between the two reviewers was 0.80. The effects of erosion on ceramics, indirect composites, and hybrid ceramics were evaluated by immersion in or rinsing with acidic solutions (eg, hydrochloric acid [HCI], cola drinks, and juices). An increase in the SR (Ra values) (P < .00001; mean difference [MD] = -0.04; 95% CI =-0.06 to-0.03) and a decrease in the SMH (Vickers microhardness [VHN]) (P < .00001; MD = 13.94; 95% CI = 12.33 to 15.55) were found after erosion. Subgroup analyses revealed that in vitro erosion led to significantly higher Ra values and lower VHN values than in vivo erosion; cola drinks and juices led to more surface degradation than HCI; and the SR and SMH of polymer-infiltrated ceramic network (PICN) materials remained unchanged after erosion. Conclusion: After acidic challenges, significantly increased surface roughness and decreased surface microhardness were found on the surfaces of indirect restorative materials. Among all the indirect restorative materials, PICN showed the best acid resistance. Nevertheless, the changes in the SR and SMH of indirect restorative materials after erosion detected in the present study were minor and may not have any clinical significance. Abstract:Objectives This study evaluated the resistance of S-PRG (Surface Pre-Reacted Glass-ionomer) composites and other restorative materials against erosive and abrasive challenges and their protective effect on enamel adjacent to the restorations. Materials and methods Bovine enamel blocks were prepared and randomized into 12 groups, including 6 types of material, each of them subjected to erosion_e or erosion+abrasion_ea: Beautifil II (S-PRG); Beautifil Bulk Restorative (S-PRG); Filtek Z250 XT; Filtek Bulk Fill; EQUIA Forte; Riva Light Cure. Cavities were prepared in the middle of enamel blocks and restored with the materials. Initial profiling measurement was performed on the material and on adjacent enamel (100, 200, 300, 600 and 700 μm from the restoration margin). Palatal intraoral appliances with the restored enamel blocks were used by the volunteers (n=10). During 5 days appliances were immersed in 2.5% citric acid for 2 min; 6 × /day (erosion_e). For ea condition, blocks were brushed for 1 min after each acid immersion. Final profile assessment was performed. Data were analysed by two and three way ANOVA followed by Tukey's test (p<0.05). Results Material wear: Riva Light Cure showed the highest wear followed by EQUIA Forte and then all resin composites, including the ones with S-PRG (p=0.000). Enamel wear: there was significant interaction among type of restorative material, wear condition and distance (p=0.014), enamel around materials showed similar wear (p=0.983) and the enamel subjected to ea exhibited highest wear (p=0.000). Conclusion SPRG based composites showed resistance against erosive and abrasive challenges but were not able to protect enamel adjacent to the restorations. Clinical significance S-PRG composites exhibit resistance to material wear comparable to resin composites. However, they have shown an inability to effectively protect the adjacent enamel under in situ erosive-abrasive conditions, despite the presence of mineral-loss-preventing ions surrounding materials.

The Efficiency of Fluoride Bioactive Glasses in Protecting Enamel Surrounding Orthodontic Bracket

Abstract:Objectives: The aim of this study was to evaluate the protective effect of using four different fluoride bioactive enamel sealers against an acidic erosion challenge. Materials and methods: A sample of 50 freshly extracted sound upper premolars had their buccal surface bonded to 50 orthodontic brackets using Transbond PLUS color change adhesive; the first four groups had four compositions of fluoride bioactive glasses based on 37 mol% SiO2, 43.9-53.9 mol% CaO, 6.1 mol% P2O5 and CaF2, and 0-10 mol% of Na2O applied to their surfaces and the fifth group served as control (which was not treated by any bioactive sealer). All specimens were challenged by 1% citric acid for 18 minutes which was stirred by a magnetic stirrer. The enamel surfaces next to the orthodontic brackets were examined by SEM. The Wilcoxon signed-rank test was used to compare the area covered by the fluoride bioactive pastes before/after erosion (p < 0.05). Samples from the layer formed on top of the examined teeth were tested before/after erosion to be examined by the attenuated total reflectance Fourier-transform infrared spectroscopy (FTIR/ATR). Results: The FTIR/ATR test showed that fluoride bioactive pastes' applications resulted in the formation of a hydroxyapatite-rich layer; the SEM analysis showed that the aforementioned layer significantly resisted erosion challenge when compared to the control group (p < 0.05). Conclusions: Fluoride bioactive pastes can efficiently protect the enamel surfaces next to orthodontic brackets from acidic erosion challenges.

Abstract:The study aimed to compare various toothpastes and mouthwashes on permanent tooth dentin after erosive and abrasive challenges. 130 sound premolars dentin were randomly submitted to an initial erosive challenge and a cycle of erosive and abrasive challenges for five days. The five experimental groups (n = 26) were: (1) Control group (artificial saliva), (2) Elmex erosion protection toothpaste and mouthwash, (3) Vitis anticaries biorepair toothpaste and mouthwash, (4) Oral B Pro-expert toothpaste and Oral B Fluorinse mouthwash, and (5) MI Paste ONE toothpaste and Caphosol mouthwash. Microhardness, surface roughness values, and the topographical characteristics of the dentin surface were assessed. The highest percentage of recovered dentin microhardness (%RDMH) value was observed in groups 2 and 4, followed by groups 5 and 3, respectively. The %RDMH values in groups 2 and 4 did not demonstrate a significant difference (p = 0.855). The highest percentage of improvement in surface roughness was recorded in groups 2 and 4, with no significant differences (p = 0.989). The atomic force microscopy (AFM) findings were consistent with the surface roughness data. The best recovery of dentin microhardness and roughness were measured with the Elmex and Oral B toothpaste and mouthwash, followed by MI Paste ONE toothpaste and Caphosol mouthwash and Vitis anticaries biorepair toothpaste and mouthwash.

Abstract:The primary objective of this in vitro study was to investigate the erosive potential of enamel under the use of clear aligners (CA), by simulating in vivo conditions experienced by patients who do not remove their CA during the consumption of acidic beverages. In addition, the difference in erosion protection conferred by artificial and human saliva was also evaluated. Sound-extracted human premolars ( n = 20) had half of their surfaces protected with acid-resistant nail polish and were randomly distributed into two experimental groups ( n = 10): teeth immersed in human saliva or artificial saliva. All teeth had half of their lingual surfaces enclosed by a CA device. The erosive challenges consisted of individual immersion of each sample in citrus acid three times a day, intermediated by immersion in human saliva or artificial saliva for 2 h, during ten days of the erosive protocol. The enamel mineral content was analyzed by high-resolution microtomography. The differential mineral concentration profiles were obtained by subtracting the profile of the mineral concentration of the exposed area and enamel under the CA area from the respective sound area (control). In addition, enamel wear and enamel volume loss were measured. Scanning electron microscopy (SEM) was also performed to analyze the enamel surface. Data were analyzed by two-way ANOVA, followed by the Student-Newman-Keuls test. The enamel wear was higher in teeth immersed in artificial saliva, when compared to human saliva ( p < 0.001). The volume loss of the exposed enamel area was lower for tooth immersed in human saliva than in artificial saliva ( p < 0.001), during the acid challenge protocol. The use of CA during acid challenges promoted wear and mineral loss of dental enamel, being these changes more pronounced on the enamel surface under the CA. These results open a new path for the development of further studies adopting clinical protocols that promote more accurate responses in the clinical practice during orthodontic treatment.
