Abstract:Purpose: The aim of the present study was to investigate the wear resistance and hardness of five kinds of composite resins. Materials and Methods: Sixty-five specimens were fabricated with one nano-hybrid (Charisma Diamond), two micro-hybrid (3MZ250, Clearfil AP-X) and two packable (3MP60, Surefil) composite resins, according to a randomized complete block design (n = 13, 8 for wear test; 5 for hardness test). The composites were filled in a rectangular mold, and light polymerization. After storage in 37 degrees C deionized water for 24h, all specimens were tested with a custom-made toothbrush machine with a stainless-steel ball as antagonist (3N loads, 1Hz, 6x10(5) cycles) immersed in calcium fluoride slurry. Wear volume, hardness and surface structure of each tested material was examined by a three-dimensional non-contact optical profilometer, Vickers indentation technique and scanning electron microscope. Results: The volume loss ranked from least to most as follows: Charisma Diamond, P60, Z250, Clearfil AP-X and Surefil. Regarding hardness, the rank from highest to lowest as follows: Clearfil AP-X, P60, Surefil, Z250, Charisma Diamond. The interactions between wear resistance and microhardness were not significant. Conclusions: The custom-made machine is considered suitable to simulate sliding of an antagonist cusp on an opposing occlusal composite restoration. Nanofilled composite may have superior wear compared to other composite resins.

Abstract:The purpose of this study was to evaluate the abrasive wear and surface roughness of 20 currently available commercial dental composite resins, including nanofilled, supra-nanofilled, nanohybrid and microhybrid composite resins. The volume loss, maximum vertical loss, surface roughness (R-alpha) and surface morphology [Scanning electron microscopy (SEM)] were determined after wear. The inorganic filler content was determined by thermogravimetric analysis. The result showed that the volume loss and vertical loss varied among the materials. The coefficients of determination (R-2) of wear volume loss and filler content (wt%) was 0.283. SEM micrographs revealed nanofilled composites displayed a relatively uniform wear surfaces with nanoclusters protrusion, while the performance of nanohybrid composites varied. The abrasive wear resistance of contemporary dental composite resins is material-dependent and cannot be deduced from its category, filler loading and composite matrix; The abrasive wear resistance of some fiowable composites is comparable to the universal/posterior composite resins.
Effect of Nanofiller on Wear Resistance and Surface Roughness of Resin Composites.

Abstract:OBJECTIVE:To compare the wear resistance and surface roughness of nanofiller-containing composites and microhybrid composites after simulated wear.METHODS:Five microhybrid composites and five nanofiller-containing resin composites were included in the study. Six cylindrical specimens with a diameter of 10 mm and a thickness of 6 mm for each material were prepared. The volume loss, vertical loss and the surface roughness (Ra) were determined after 800 cycles of simulated chewing motion. One specimen of each material was analysed by scanning electron microscopy (SEM) to compare the morphology of the wear surfaces. The microhybrid composites group and nanofiller-containing composites group were tested using the Mann-Whitney U test with a significance level of α = 0.05.RESULTS:For all microhybrid composites, the average wear volume loss and vertical loss were 56.44 mm3 and 730.6 µm, respectively, while the average wear losses of nanofiller-containing composites were 40.15 mm3 and 528.17 µm, respectively. The nanofiller containing composite GNH400N showed the least roughness (Ra = 0.346 ± 0.076 µm), while the conventional microhybrid composite Ceramage showed the highest roughness (Ra = 0.699 ± 0.214 µm). However, wear resistance and surface roughness for the two groups showed no statistical difference. SEM micrographs of the nanofiller-containing composites after wear testing showed smoother and more uniform wear surfaces than for the microhybrid composites.CONCLUSION:Nanofillers did not significantly influence the wear resistance of resin composites, but might improve the surface roughness of resin composites.
Three-body wear of resin denture teeth with and without nanofillers

Abstract:Statement of problem: The wear behavior of newly developed denture teeth with nanofillers may be different from teeth with other chemical formulations. Purpose: The purpose of this study was to examine the 3-body wear resistance of 11 different commercially available resin denture teeth. Material and methods: The materials tested were conventional (SR Orthotyp PE, Orthognath) and cross-linked acrylic resin teeth without inorganic fillers (Premium 8, SR Postaris DCL, Trubyte Portrait, Artiplus), composite resin teeth with inorganic fillers (SR Orthosit PE, Vitapan), and composite resin teeth (experimental materials) with inorganic nanofillers (NC Veracia Posterior, e-Ha, Mondial). Human enamel and a ceramic denture tooth (Lumin Vacuum) were used as reference materials. The 3-body wear test was performed in a wear machine developed by the Academic Center for Dentistry Amsterdam (ACTA), with millet suspension acting as an abrasive medium (n=10, test load: 15 N, slip rate: 20%, number of cycles: 100,000). Wear was determined with the aid of a profilometer. Data were analyzed with the Kruskal-Wallis test and Mann-Whitney U test using the closed testing approach (significance level for familywise error rate, alpha=.05). Results: None of the acrylic and composite resin materials tested in this study demonstrated the 3-body wear resistance of ceramic teeth or human enamel. Teeth with inorganic fillers demonstrated significantly lower wear values than conventional or cross-linked acrylic resin teeth without fillers. Composite resin teeth with traditional fillers showed significantly lower wear than composite resin teeth with nanofillers. Conclusions: Denture teeth with and without inorganic fillers differed significantly with regard to the degree of wear generated in the ACTA wear simulator. The incorporation of nanofillers did not improve the wear resistance compared to teeth with traditional fillers.

Abstract:Coat protein gp120 from the human immunodeficiency virus type-1 (HIV-1) increased intracellular free calcium and injured rodent retinal ganglion cells and hippocampal neurons in culture. Highly purified recombinant gp120 envelope protein produced these effects in a dose-dependent fashion at picomolar concentrations. Immunoprecipitation with antibody to gp120, but not with control immunoglobulin-containing serum, depleted solutions of the viral envelope protein and also prevented both the rise in intracellular calcium and neuronal toxicity. The gp120-induced increase in intracellular calcium was abrogated by transiently lowering extracellular calcium or by adding the dihydropyridine calcium channel antagonist nimodipine (100 nM). Calcium channel antagonists also prevented gp120-induced neuronal injury. In addition, intracellular stores appeared to contribute substantially to the increase in calcium elicited by gp120. Since increases in intracellular calcium have been associated with neurotoxicity, it is possible that an injurious effect of gp120 on neurons might be related to this mechanism and that treatment with calcium channel antagonists may prove useful in mitigating HIV-1-related neuronal injury.
Relative wear of flowable and highly filled composite

Abstract:Statement of problem: By decreasing the proportion of filler, flowable composites are easy to adapt to small cavity preparations. However, the wear resistance of flowable composites is a clinical concern, as improved wear has been related to increased filler percentage. Purpose: The purpose of this study was to compare wear for flowable composites and comparable heavily filled materials. Material and methods: Wear by attrition and abrasion (microm) of microfilled flowable (Heliomolar Flow) and heavily filled microfill (Heliomolar HB) was measured and compared to a microhybrid flowable (Tetric Flow) and a heavily filled microhybrid (Tetric HB). There were 5 specimens, 2 mm thick and 15 mm in diameter, in each group. The Oregon Health Sciences University oral wear simulator was used to produce abrasive wear and attrition for all specimens. Abrasion and attrition of the substrates were measured using a profilometer. Data were subjected to a 3-way analysis of variance for the 3 main factors: composite type, filler load, and wear type (alpha=.05) Results: Composite wear rates were significantly influenced (P <.01) by both the type of wear and the filler amount. Abrasive wear occurred less than wear by attrition, and the heavily filled composites experienced less wear than the flowable composites. Conclusions: Results of this in vitro study suggest that highly filled microhybrid composites may have greater wear resistance for contact-supporting posterior restorations. However, in small noncontact restorations where longevity may be dictated by abrasion, flowable microfilled resins may be adequate.
Simulated oral wear of packable composites

Abstract:Wear resistance has been a problem for the posterior application of resin composites. This study evaluated and compared the wear characteristics of two conventional and two packable composites. Opposing enamel wear was also measured. One traditional hybrid composite-Herculite XR (HXR), one micro-filled composite-Heliomolar (HM) and two packable composites-Filtek P60 (P60) and Surefil (SF) were formed into disks (n = 10) and used as substrates for the wear test. Enamel was harvested from extracted human third molars and machined into cusps with a 5-mm spherical radius (n = 40). The Oregon Health Sciences University oral wear simulator was used to evaluate abrasive wear and attrition of the composite materials and wear of the opposing enamel. The resulting enamel wear facets were measured and recorded in mm2 using optical scanning methods and a computer graphics program. Abrasion and attrition of the composite substrates were measured using a profilometer. Both sets of data were subjected to ANOVA and multiple comparison tests to determine significant differences. After wear testing, scanning electron micrographs were made using representative composite samples from each group. The packable composites showed significantly less attrition and abrasive wear (p < 0.001) than the conventional controls. The microfilled composite HM resulted in significantly lower enamel wear (p < 0.001) than the materials HXR and P60 but was not significantly different from the packable composite SF at the alpha = 0.05 level. The results of this in-vitro study suggest that packable composites may have improved wear resistance over some conventional composites. Clinical studies are needed to evaluate packable composites over time.

Abstract:The thiol-disulfide oxidoreductase DsbA is required for efficient formation of disulfide bonds in the Escherichia coli periplasm. The enzyme is the strongest oxidant of the family of thioredoxin-like proteins and three-dimensional structures of both oxidized and reduced forms are known. DsbA consists of a catalytic thioredoxin-like domain and a helical domain that is inserted into the thioredoxin motif. Here, the X-ray structure of a circularly permuted variant, cpDsbA(Q100T99), is reported in which the natural termini are joined by the pentapeptide linker GGGTG, leading to a continuous thioredoxin domain, and new termini that have been introduced in the helical domain by breaking the peptide bond Thr99-Gln100. cpDsbA(Q100T99) is catalytically active in vivo and in vitro. The crystal structure of oxidized cpDsbA(Q100T99), determined by molecular replacement at 2.4 A resolution, was found to be very similar to that of wild-type DsbA. The lower thermodynamic stability of cpDsbA(Q100T99) relative to DsbA is associated with small structural changes within the molecule, especially near the new termini and the circularizing linker. The active-site helices and adjacent loops display increased flexibility compared with oxidized DsbA.

Abstract:Aim: This study aimed to evaluate the clinical performance and occlusal wear resistance of nano- zirconia reinforced glass ionomer restorations in comparison to nano-hybrid resin composite restorations in proximal cavities. Material and Method: This randomized clinical trial was conducted on thirty-two patients who had proximal carious lesions in posterior teeth. Participants were randomly allocated into two groups (n=16 for each group) in which they received either; Nano zirconia reinforced glass ionomer restorations (Zirconomer Improved, Shofu, Japan) or Conventional nano hybrid resin composite (Filtek Z250 XT, 3M ESPE, USA), all materials were applied according to manufacturers’ instructions. Restorations were evaluated at baseline (immediately), after six months, and after 12 months by two blinded assessors using modified USPHS criteria measuring (postoperative hypersensitivity, secondary caries, gross fracture, color match, cavo-surface marginal discoloration, marginal integrity, proximal contact and wear resistance). Data was analyzed using Medcalc software, intergroup comparisons between interventions was performed using the Chi-Squared test with statistical significance level set at (P ≤ 0.05) while intragroup at (P ≤ 0.016) after Bonferroni correction, relative risk was used to assess the clinical significance. Results: Intergroup comparison between both materials regarding USPHS criteria and wear resistance have shown no statistically significant difference within different follow up periods; baseline, 6 and 12 months respectively except for the color match shown significant difference with highly translucent nano zirconia reinforced GI. Intragroup comparison within highly translucent nano zirconia reinforced glass ionomer and within conventional resin composite regarding USPHS criteria have shown no statistically significant difference between different followup periods except for the wear resistance shown significant difference within highly translucent nano zirconia reinforced GI. Conclusion: Nano zirconia reinforced glass ionomer and Nano hybrid resin composite have a similar clinical performance and wear resistance after one year follow up.

Abstract:Background/purpose: Few studies have focused on the influence of simulated toothbrush abrasion on the surface qualities of novel nanofilled and nanohybrid composites. The aim of the study was to evaluate the surface roughness and gloss values of resin-based composite (RBC) materials with various filler types before and after simulated toothbrush abra-sion.Materials and methods: One nanofilled (Filtek Z350 XT [FT3]), two nanohybrids (Harmonize [HM] and Clearfil Majesty [CM]) and one microhybrid (Filtek Z250 [FT2]) were evaluated. Twelve specimens of each material were made and polished with silicon carbide sandpapers. Initial surface roughness and gloss values were measured as negative controls. Then, all spec-imens were subjected to simulated toothbrush abrasion on a custom-made apparatus. After 2000, 4000 and 8000 cycles, the surface roughness and gloss values of all specimens were tested. One additional specimen from each group was selected for scanning electron micro-scope (SEM) analysis.Results: For FT3, Ra and GU values did not significantly change until after 8000 cycles during the process of toothbrushing (P > 0.05). For HM, CM and FT2, the Ra and GU values significantly decreased after 4000 and 8000 cycles of toothbrush abrasion (P < 0.05). After 8000 cycles of toothbrush abrasion, FT3 presented the lowest surface roughness and highest gloss values of all materials (P < 0.05). SEM images showed that surface textures and irregularities corresponded to the results of surface roughness and gloss.Conclusion: Surface roughness and gloss after simulated toothbrush abrasion were material dependent. Clinical evaluation of bulk-fill and universal nanocomposites in class II cavities: Five-year results of a randomized clinical split-mouth trial

Abstract:Aim: Evaluating the clinical survival and quality parameters of class-II restorations using 3M™ FiltekTM Bulk Fill Posterior Restorative compared to 3M™ FiltekTM Supreme XTE Universal Restorative over a period of five years. Materials and methods: A longitudinal, randomized, prospective split-mouth study with 60 patients (29 female, 31 male; mean age 44 y; range 20-77 y) and a total of 120 load-bearing class II restorations (TEST: n=60 Filtek Bulk Fill Posterior Restorative; CONTROL: n=60 Filtek Supreme XTE Universal Restorative) was conducted. Clinical evaluation was performed by blinded evaluators according to FDI criteria. Kaplan-Meier method was used for survival analysis and an intergroup comparison (Mann-Whitney-U-Test) was carried out. A basic significance level of 0.05 was corrected by the Bonferroni method to account for multiple testing (significance after correction: p<0.00067). Results: The mean overall survival of restorations was 92% after 56.98±1.51 months in the TEST group (95 CI= 54.02;59.94) and 92% after 57.25±1.46 months (95 CI= 54.39; 60.12) in the CONTROL group (log-rank p=0.995). In total, four failures occurred in both TEST and CONTROL group during the observation period (mean annual failure rate: 1.6%). The most common reasons for failure were chipping-fractures, debonding, cracked-tooth-syndrome and recurrent decay. With regard to the FDI criteria, no significant differences between TEST and CONTROL material occurred for any of the evaluated variables. In the TEST group two restorations had to be repaired and two had to be replaced, in the CONTROL group four restorations had to be replaced. Conclusion: Both materials showed acceptable clinical performance and survival during the 5-year observation period. Clinical significance: The use of a nanofilled bulk-fill composite proved to be an aesthetically, functionally and biologically satisfactory alternative in posterior dentition.
Attritional wear and abrasive surface alterations of composite resin materials in vitro

Abstract:Objectives: A laboratory study was performed with 232 specimens and 72 human enamel, 24 gold, 24 ceramic and 12 composite antagonists in 22 groups to test attritional and abrasive wear behavior of composite materials compared to wear behavior of human enamel. Methods: Belleglass HP, Concept Inlay/Onlay, Targis and Targis Upgrade 99 composite resin for lab-made restorations was tested as well as Tetric Ceram and FHC Merz light as resins for direct restorations. Natural human enamel specimens served as control. All specimens were subjected to long-term thermo-mechanical loading in a computer-controlled masticator, chemical degradation and toothbrush/toothpaste abrasion. Wear of specimen in occlusal contact area (OCA), contact-free occlusal area and wear of natural enamel cusps as well as antagonists made of gold, ceramic and composite in identical form was measured after 120,000, 240,000, 640,000 and 1200,000 load cycles. A qualitative SEM analysis was performed to support quantitative data. Results: Belleglass HP and Targis Upgrade 99 restorative materials showed wear resistance comparable to human enamel when loaded with enamel cusps. Wear of Targis versus composite and gold antagonists was significantly higher (p<0.0001). Analysis of surface alterations showed hygroscopic expansion in all composite resins during the test. Conclusions: As a consequence of this study, necessity to further improve physical properties of composites for long lasting restorations was obvious. Beside of attritional wear in OCA, attention must be given to stable filler-matrix interfaces and prevention of water sorption.
Comparison of Flexural Strength and Wear of Injectable, Flowable and Paste Composite Resins

Abstract:(1) Objectives: This study investigated and compared the wear and flexural strength of two highly filled (injectable), one flowable and one paste composite. (2) Methods: Two highly filled flowable composites (G-aenial Universal Injectable and Beautifil Plus F00), a paste composite (Empress Direct) and a conventional flowable (Tetric EvoFlow) were tested. A two-body wear test was carried out using 10 disc-shaped samples from each group, which were subjected to 200,000 wear machine cycles to simulate wear, followed by Scanning Electron Microscope analysis. Flexural strength was tested using a three-point bend test using 15 beam samples for each of the four groups. Values were statistically compared using one-way analysis of variance (ANOVA) for flexural strength and a Kruskal–Wallis test for wear. (3) Results: The median volume loss for G-aenial Universal Injectable and Beautifil Plus F00 was statistically lower than that of both Empress Direct and Tetric EvoFlow. For flexural strength the two highly filled flowable composites both exhibited statistically higher mean flexural strength values compared to Empress Direct (p < 0.004) and Tetric Evoflow (p < 0.001). There were no statistically significant differences in the values of wear and flexural strength between the two highly filled flowable composites. (4) Conclusions/significance: Highly filled flowable composite resins with nano filler particles outperformed a conventional flowable and a paste composite resin in terms of wear resistance and flexural strength, and may be suitable to use in occlusal, load-bearing areas.

Evaluating the Wear of Resin Teeth by Different Opposing Restorative Materials

Abstract:The aim of this study was to evaluate the wear properties of resin teeth with different opposing dental restorative materials. One type of resin tooth (Trubyte Biotone) was tested against six types of restorative materials including type III gold alloy (GO), monolithic zirconia (MZ), lithium disilicate glass ceramic (LD), nickel-chromium alloy (NC), feldspathic ceramic (FC), and steatite (ST). Two-body wear tests were performed under a vertical load of 5 kgf and thermo-cycling at 5/55 °C with a total of 120,000 cycles. The wear amount was quantified by measuring the volume loss of the resin teeth and the vertical substance loss of the opposing materials using three-dimensional images. The FC group showed a significantly greater amount of wear of the resin teeth, followed by the ST, NC, LD, MZ, and GO groups. The GO group showed significantly less wear of resin teeth than the other groups. There were no statistically significant differences in the wear of opposing restorative materials between groups. Within the limits of this study, it is recommended that zirconia, rather than feldspathic ceramic, should be used for restorations in the esthetic zone, and gold alloy should be used for areas with little or no esthetic demand.
Wear behavior of different materials used for pit and fissure sealing

Abstract:Purpose: To evaluate the wear of different materials used for pit and fissure sealing applied with non-invasive and invasive preparation techniques. Methods: A total of 170 molar teeth were divided into two main preparation groups (non-invasive and invasive), each consisting of eight subgroups after a control group was separated for wear standardization. Eight subgroups included: nano-filled flowable composite (Filtek Ultimate Flow), nanohybrid flowable composite (GrandioSo Flow), micro-hybrid flowable composite (Majesty Flow), resin-based unfilled fissure sealant (ClinPro Sealant), resin-based filled fissure sealant (Fissurit FX), resin-based highly filled fissure sealant (GrandioSeal), giomer-based fissure sealant (BeautiSealant), and glass-ionomer-based fissure sealant (Fuji Triage) (n= 10). The materials were applied according to the manufacturers' instructions. The initial data were obtained for wear analysis. The specimens were subjected to 2-year thermocycling and brushing simulations. Final data were obtained, and the wear characteristics were evaluated digitally. Data were statistically analyzed (P< 0.05). Results: There were no significant differences in wear between the non-invasive and invasive application groups (P< 0.05). In comparison of the materials, flowable composites presented the lowest wear (0.15 P± 0.13), and glass-ionomer-based fissure sealant presented the highest wear (0.66 P± 0.32). Clinical significance: The present study reported that the invasive preparation technique, which slightly abrades the enamel surfaces, did not adversely affect the wear of the sealant materials. Although the application of flowable composites as fissure sealants with a bonding agent is time-consuming and costly, it yielded better results in terms of wear.
An approach to understanding tribological behaviour of dental composites through volumetric wear loss and wear mechanism determination; beyond material ranking

Abstract:Objective: To investigate the fundamental wear mechanisms of six resin-based composite (RBC) formulations during short-term in vitro wear testing. Materials: RBC materials were condensed into rectangular bar-shaped specimens and light irradiated using the ISO 4049 specimen manufacture and irradiation protocol. Wear testing (n=10 specimens for each RBC) was performed on a modified pin-on-plate wear test apparatus and wear facets were analysed for wear volume loss using a white light profilometer. The wear tested RBC specimens and their corresponding antagonists were analysed using scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDS), respectively to determine the wear mechanism. Results: Data generated using the profilometer showed variations in the mean total wear volume (mm3) between the RBCs tested (p<0.05). Abrasive wear was evident in all RBCs investigated with varying degrees of damage. Material transfer/deposition of the filler particles on the corresponding antagonists was evident in two RBC materials (Filtek Supreme and Kalore) indicative of a further adhesive wear mechanism. Conclusion: It is proposed that the approach employed to use a combination of measurement and analytical techniques to quantify the wear facet volume (profilometry), wear trough (SEM) and material transfer (EDS) provides more useful information on the wear mechanism and the tribology of the system rather than relying on a simple wear ranking for the RBC materials as is routinely the case in dental research studies.
Wear resistance of CAD/CAM one-piece screw-retained hybrid-abutment-crowns made from different restorative materials

Abstract:Introduction: The aim of this study was to measure the wear progress of three high performance polymers (HPP) materials as well as that of zirconia after artificial aging (simulated 2.5- and 5-year of clinical service with thermo-mechanical loading) and compare it with the well-documented wear of lithium disilicate. Methods: Forty implants were used to restore a maxillary first premolar, where the abutment and the crown were manufactured as hybrid-abutment-crown and connected to the implant using a titanium insert. The implants were randomly divided, according to the restorative materials used, into five groups: 3Y-TZP zirconia (Z), lithium disilicate (L), ceramic-reinforced polyetheretherketon (P), nano-hybrid composite resin (C) and polymer-infiltrated ceramic-network (E). All hybrid-abutment-crowns were produced using CAD/CAM technology. A design of a maxillary first premolar was created with an angle of 120° between the buccal and palatal cusps, which were designed as planes. The restorations were adhesively luted onto the titanium inserts, according to the manufacturers' recommendations for each material individually, by means of dual-curing luting resin with the exception of group P, where the blocks were pre-fitted (heat-pressed) with an integrated titanium insert. The suprastructures were assembled onto the implants through titanium screws. The screw channels were sealed with Teflon tape and composite resin filling material that was polished to high-gloss. All specimens underwent 1 200 000 thermo-dynamic loading cycles with 49 N in a dual-axis chewing simulator. Elastomeric impressions were made for all specimens after 600 000 and after 1 200 000 cycles. The corresponding impressions were imaged using a laser scanning microscope and then 3D-analyzed using the software (Geomagic Wrap) to measure the volume loss of the wear area for all specimens. Statistical analysis was performed using Wilcoxon-Test regarding the two different time measurements for each material. For the analysis of the material variable, Kruskal-Wallis test was conducted followed by Mann-Whitney test. Results: Group Z showed statistically the lowest volume loss compared to the other test materials, both after 600 000 and 1 200 000 cycles of artificial aging, with a median value of 0.002 mm3 volume loss after 1 200 000 cycles. In contrast, group E showed the highest volume loss with median values of 0.18 and 0.3 mm3 after 600 000 and 1 200 000 cycles, respectively. Artificial aging had significantly negative effect on the volume loss for all test materials. In addition, the choice of material had statistical influence on the outcome. Conclusion: Monolithic zirconia ceramic demonstrated lower wear than that reported for enamel after simulated 5-year of clinical service, whereas all other test materials showed higher volume loss after artificial aging.

Abstract:HYPOTHESIS The aim of this study was to find suitable methods for basic anatomic evaluation of the supratubal recess and the anterior surface of the tensor fold. BACKGROUND The current method of superior microdissection via the middle fossa floor provides a good picture of the anatomy and pathology of the epitympanum, but the supratubal recess can be evaluated only after excision of the tensor fold. Postinflammation changes cannot be examined accurately because destruction of the tensor fold necessarily alters the anatomic details. METHODS Eight temporal bones were studied via a lateral and 14 via an anterior approach, both complemented by the superior microdissection. Data on 51 earlier superior dissections were reevaluated as to the state of the supratubal recess. Histology was documented from eight biopsy specimens and of four serially sectioned temporal bones, two normal and two infected. RESULTS The lateral route offered a good view to the tensor tendon and lower portion of the tensor fold, but the anterior malleal ligament obstructed the view to the fold's upper portion. The anterior route offered excellent visibility to the anterior pouch, mesotympanum, tensor fold, and the whole supratubal recess. The tensor fold was mostly fixed superiorly to the bony roof with a narrow or broad soft band of composite tissue, infrequently to the transverse crest directly. Inflammatory changes spread from the tympanic isthmus region to the supratubal space over the fold and, if extensive, formed broad inflammatory and scar tissue bands between the fold and the anterior bony wall. CONCLUSIONS The supratubal recess and the mesotympanum can best be evaluated via the anterior approach, which should be added to the temporal bone microdissection program. It serves well as the starting route, followed by the conventional superior dissection of the epitympanum. The knowledge gained is indispensable in surgery for chronic otitis media for creation of a large common middle ear air space and functioning aeration pathways.
Effect of Nanohydroxyapatite, Zirconia and Glass Filler Particles on the Wear and Microhardness of Experimental Dental Composite Resin

Abstract:Objectives: To study the influence of nanoparticles of hydroxyapatite, zirconia, and glass on the wear and the microhardness of the organic matrix of experimental dental composite resin.Materials and methods: The dental composite resin matrix was fabricated from bisphenol A-glycidyl methacrylate (Bis-GMA) (40 wt%), triethylene glycol dimethacrylate (TEGDMA) (36 wt%), and camphorquinone (0.4 wt%). Nanohydroxyapatite, glass, and zirconia fillers were silane treated. Nano-hydroxyapatite, glass, and zirconia were incorporated at three different concentrations. The polymerization of the dental composite resin was done using a light curing unit. Experimental dental composite resins were evaluated for wear and microhardness. The data were analyzed by one-way analysis of variance (ANOVA) test.Results: The experimental dental composite resin composed of 32% of nanohydroxyapatite, 27% of zirconia, and 19% of glass as filler showed the minimum amount of wear. The Vickers hardness (VHN) number was observed to be minimum for the experimental dental composite resin composed of 24.1% of nanohydroxyapatite, 22% of zirconia, and 14.5% of glass.Conclusion: The inclusion of 32% nanohydroxyapatite, 27% of zirconia, and 19% of glass as filler into the experimental dental composite resin decreased the wear and increased the hardness.How to cite this article: Mirajkar CK, Winnier J, Hambire U.
