The Value of ~(18)F-FDG Coincidence SPECT for Detecting Bone Metastases and Compared with ~(99)Tc~m-Mdp Bone Scan

Abstract:Objective To evaluate diagnostic accuracy of 18 F-fluorodeoxyglucose (F DG) coincidence SPECT in detecting bone metastases and compared with 99 Tc m-methylen e diphosphonic acid (MDP) bone scan. Methods A retrospective review of 55 patients with known or suspected malignancy su ggested by 18 F-FDG coincidence SPECT imaging was conducted. Forty -one of the 55 patients underwent 18 F-FDG coincidence SPECT and 99 Tc m-MDP whole body scan and the two modalities were perfo rmed apart within 4 weeks. Thirty patients (9 lung cancer, 3 br east cancer, 3 colon and rectal cancer, 9 unknown primary cancer, 6 others) were confirmed with bone metastases by plain film radiography (6), CT (11), MRI (9), or bone biopsy (4). Fifteen patients were excluded from being with bone metasta ses by MRI (3), bone biopsy (1) and clinical follow-up (11). Ten patients (7 lu ng cancer, 3 others) suspected by 18 F-FDG coincidence SPECT with bone involvement by tumor direct invasion were evaluated separately. Resu lts The sensitivity, specificity, positive and negative predicting rate, and diagnostic accuracy for detecting bone metastases was 100% (30/30), 93% (14 /15), 97% (30/31), 100% (14/14), and 98% (44/45), respectively with 18 F-FDG coincidence SPECT, and was 95% (18/19), 50% (6/12), 75% (18/24), 86 % (6/7), 77% (24/31), respectively with bone scan. 18 F-FDG coin cidence SPECT imaging was superior to bone scan in specificity ( P =0.024), po sitive predicting rate ( P =0.035), and diagnostic accuracy ( P =0 0 07). 18 F-FDG also identified primary or metastatic lesions outside the b one for 20 of the 30 patients with bone metastases. Four of 10 patien ts suspected with tumor bone invasion by 18 F-FDG coincidence SPEC T were confirmed with bone involvement by surgery, while 6 patients were confirm ed without bone involvement. Conclusions 18 F-FDG coi ncidence SPECT is an accurate method for bone metastasis detection, which can identify no t only bone metastases, but also lesions outside the bone. Bone scan is also a s ensitive method for bone metastasis detection, but with lower specificity.
