Abstract:OBJECTIVETo observe the trend in the change of incidence and mortality of female breast cancer in Tianjin and evaluate its effect of prevention.METHODSMethod of descriptive epidemiology was used to study the epidemic situation of female breast cancer in Tianjin.RESULTSThe incidence of breast cancer in Tianjin had increased by 39.7% from 1981 to 1997, as compared with the other cities in China. Especially, compared with the developed countries the world over, the incidence of breast cancer in Tianjin is much lower. The mortality rate of breast cancer had lowered by 31.0% from 1981 to 1997 and the 3-year and 5-year survival rates increased to various degrees.CONCLUSIONEarly detection and diagnosis of breast cancer are very important to both increase of survival and lowering of mortality of breast cancer. Preventive efforts should be made to the high risk people of breast cancer.
Analysis on Mortality Distribution of Female Breast Cancer in China, 1991-2010

Abstract:Objective To analyze the mortality distribution of female breast cancer in China from 1991 to 2010,and to provide evidences for prevention and treatment of female breast cancer.Methods The death data of female breast cancer in China from 1991 to 2010 was used to describe its epidemiological characteristics,like the dynamic change of the standardized mortality rate,urban-rural differences and age difference.Besides,the trend-surface analysis was used to study the geographical distribution of the mortality.Results In China,the mortality rate of female breast cancer increased yearly,and that in city was higher than that in countryside.Over the past decade,all peak ages of death had been delayed where the first of it was showed on 55-age group;and the geographical analysis showed that the mortality rate increased from southwest to northeast,and from west to east.Conclusions The standardized mortality rate of female breast cancer is rising.The regional disease control is focused on the northeast,east and southeast part of China for female breast cancer,and the key objects of prevention and treatment is urban women aged from 55.

Abstract:Introduction: This study reported the trends in female breast cancer incidence and mortality rates in China, and analyzed the corresponding age-period-cohort effects.Methods: Data from 22 population-based cancer registries in China between 2003 and 2017 were analyzed. Age-standardized incidence rates (ASIR) and mortality rates (ASMR) were calculated using Segi's world standard population. Joinpoint regression was employed to evaluate trends, and age-period-cohort effects were examined using the intrinsic estimator method.Results: The ASIR for female breast cancer exhibited a more rapid increase in rural areas compared to urban areas across all age groups. The most substantial increase was observed in the 20-34 age group in rural areas [annual percent change (APC)=9.0%, 95% confidence interval (CI): 7.0%-11.0%, P<0.001]. The ASMR for females under 50 years old remained stable from 2003 to 2017 in both urban and rural areas. However, the ASMR for females over 50 in rural areas and those over 65 in urban areas demonstrated a significant increase, with the most pronounced increase observed among females over 65 in rural areas (APC=4.9%, 95% CI: 2.8%-7.0%, P<0.001). Age-period-cohort analysis revealed increasing period effects and decreasing cohort effects for female breast cancer incidence and mortality rates in both urban and rural settings. Notably, the cohort effect for incidence displayed a slight upward trend for females born between 1983 and 1992 in rural areas.Conclusions: Our study revealed a rapid increase in breast cancer incidence among younger generations and an accelerated mortality rate in older populations residing in rural areas. To effectively address the growing burden of female breast cancer in China, it is essential to develop and implement targeted intervention strategies.

Abstract:Breast cancer is the most common neoplasm diagnosed amongst women worldwide and is the leading cause of female cancer death. However, breast cancer in China is not comprehensively understood compared with Westernised countries, although the 5-year prevalence statistics indicate that approximately 11 % of worldwide breast cancer occurs in China and that the incidence has increased rapidly in recent decades. This paper reviews the descriptive epidemiology of Chinese breast cancer in terms of incidence, mortality, survival and prevalence, and explores relevant factors such as age of manifestation and geographic locations. The statistics are compared with data from the Westernised world with particular emphasis on the United States and Australia. Potential causal agents responsible for differences in breast cancer epidemiology between Chinese and other populations are also explored. The need to minimise variability and discrepancies in methods of data acquisition, analysis and presentation is highlighted.

Abstract:Objective To learn about the incidence and epidemiological characteristics of breast cancer in parts of China by a survey of women's breast health. Methods Multi-stage stratified randomized cluster sampling of the cross-sectional study was applied in collecting data of women aged 25 to 70 in Shandong, Hebei, Jiangsu and Tianjin province. All subjects included in the investigation attended face-toface interviews and clinical breast examinations. Results 122 058 females aged 25-70 years old in three provinces and Tianjin metropolitan were included. 320 (58 incident cases) breast cancer cases were documented in this investigation, that is 262.2/105 in prevalence. There was a higher breast cancer prevalence in subjects aged 45 to 54 and aged 55 to 64 within one year. The standardized detection rate of 54.2/105 in rural areas was higher than urban areas' 45.3/105. Conclusion Carring out the screening of female breast cancer and popularizing the knowledge of breast cancer prevention consistently, which could improve the detected rate of breast cancer.

Abstract:OBJECTIVE Using comprehensive available data on women breast cancer in China, to describe the mortality trends from late 1970s, estimate and project the profile in 2000 and 2005, and to aim to provide a reference for clinic, basic research and prevention and control strategy making for breast cancer in China. METHODS Using Joinpoint model, the mortality trends were analyzed on the basis of routine surveillance data. Combining with the data from the second national mortality survey and several cancer registries, using the log-linear model (based on Poisson distribution), the breast cancer profile in 2000 and 2005 were estimated and projected. RESULTS Although there was a slight decline in mortality between early 1970s and 1990s, the age-specific mortality rates among young and middle age women increased dramatically which followed a continuing increase trend on both rates and absolute numbers, in both urban and rural areas in recent 15 years. Compared with 2000, there are 470 thousands more new breast cancer cases and 130 thousands more deaths from breast cancer in 2005. CONCLUSION Due to the double effects of both increasing risk factors and population growth and ageing, breast cancer will be one of the most extensively increasing cancers in Chinese women. The prevention and control of breast cancer will be of great emphasis for future cancer control strategy in China.

Abstract:Objective To study clinicopathological characteristics and tendency of incidence of lung tumors from 1980 to 2007 in Tianjin and to provide information for pathologists and clinicians.Methods The data of lung tumors are collected from General Hospital and the Second Hospital of Tianjin Medical University.The data are analyzed by SPSS11.5.Results Both of the ratios(the ratios of lung cancer to the total and lung cancer to the malignant tumors) have been increasing druing 28 years(P<0.05).Among 3 075 cases females are accounting for 29.5%,and the cases of females have been increasing further than males.The ratio to the total among the corresponding time period has the same trend(P=0.040).The ratio of adencarcinoma and large cell carcinoma is increasing while the ratio of squmous cell carcinoma and small cell carcinoma is decreasing.The cases of youngers have been also increasing and its proportions of histological types of lung cancers has a significant different compared with the middle-aged and olders(P=0.000).Adencarcinoma is the main type of youngers while squamous cell carcinoma is the main type in both the middle-aged and olderly group.Conclusion The detection rate of lung carcinoma especilly carcinoma in females is increasing,and the subtypes of lung carcinoma also changed.

Abstract:Background: Breast cancer is ranked among the most prevalent malignancies in the Chinese female population. However, comprehensive reports detailing the latest epidemiological data and attributable disease burden have not been extensively documented. Methods: In 2018, high-quality cancer surveillance data were recorded in 700 population-based cancer registries in China. We extracted data on female breast cancers (International Classification of Diseases, Tenth Revision [ICD-10]: C50) and estimated the incidence and mortality in 2022 according to the baseline data and corresponding trends from 2010 to 2018. Pathological types were classified according to the ICD for Oncology, 3rd Edition codes. Disability-adjusted life years (DALYs) were calculated as the sum of the years of life lost (YLLs) and years lived with disability (YLDs). Results: In 2022, approximately 357,200 new female breast cancer cases and 75,000 deaths occurred in China, accounting for 15.59% and 7.94% of total new cancer cases and deaths, respectively. The age-standardized incidence rate (ASIR) was 33.04 per 100,000. When analyzed by pathological type, the ASIRs for papillary neoplasms, invasive breast carcinoma, rare and salivary gland-type tumors, and other types were 1.13, 29.79, 0.24, and 1.88 per 100,000, respectively. The age-standardized mortality rate (ASMR) was 6.10 per 100,000. A total of 2,628,000 DALYs were found to be attributable to female breast cancer in China, comprising 2,278,300 YLLs and 349,700 YLDs. The ASIR, ASMR, and age-standardized rate (ASR) for DALYs in urban areas were consistently higher than those in rural areas. We observed a four-fold increase in the ASIR and ASR for DALYs and an eight-fold increase in the ASMR among females over 55 years compared with those aged under 55 years. Conclusion: These data provide invaluable insights into the latest epidemiology of female breast cancer in China and highlight the urgency for disease prevention and control strategy formulation.

Abstract:Abstract Objective Compare the urban-rural disparity in cancer mortality and changing trend during the past 18 years in Tianjin, China. Methods Cancer death data were obtained from Tianjin All Cause of Death Registration System (CDRS), which covers the whole population of Tianjin. We calculated and compared the constituent ratio of cancer deaths, age-standardized mortality rate(ASR)and changing trends between urban and rural areas. Results From 1999 to 2016, a total of 245,744 cancer deaths were reported, accounting 21.7% of all deaths in Tianjin. The ASR of total cancer mortality was higher in urban areas than in rural areas. A total of 33,739 persons were avoided dying of cancers in rural area compared to the urban death level from 1999 to 2016, which was 40.1% compare to the current level of rural areas. But the gap between urban and rural areas became narrowed gradually. The urban-rural ratios (urban/rural) of total cancer mortality changed from 1.76 (125.7/71.5)[95%CI,1.67,1.84] in 1999 to 1.11 (99.6/90.0)[95%CI,1.06,1.15] in 2016. The ASR of lung, liver and esophagus cancer became higher in rural areas than in urban areas in 2016. Conclusion Cancer transition was obviously occurred in Tianjin and showed different speeds and big gap between urban and rural areas. Much more attention was needed to pay in rural areas which still have increasing trends in most cancers mortality recently.

Abstract:Objective To conduct a descriptive epidemiological study on thyroid cancer incidence in the urbanarea of Tianjin during 2002 - 2006 and to analyze the secular trend of incidences based on previous data of 26 years and those from Shanghai and the United States. Methods Annual percent change (APC%) of thyroid cancer incidence and the rank of all-site cancer profiles were estimated. The secular trend of different histological incidences and the proportion changes were analyzed. Results The incidence of female thyroid cancer was 6. 55/100000 in 2006, ranking the eighth in the female cancer profile. The incidence of thyroid cancer showed an increased secular trend in recent 26 years, especially a 5.7-fold increase in the papillary thyroid cancer (PTC). The incidence for female increased by 267% in 2006 compared to that in 1981, APC% was 3. 1%. The variation ranked second among all the female tumors. The incidences of thyroid cancer in Shanghai and the United States also elevated with time.Conclusions Thyroid cancer has been one of female common tumors from a once less common tumor,especially the proportion of PTC is expanding. It suggests that further study on the risk factors and preventive efforts should be made.

Abstract:OBJECTIVE: Breast cancer was the most common cancer and the fifth cause of cancer deaths among women in China in 2015. The evaluation of the long-term incidence and mortality trends and the prediction of the future burden of breast cancer could provide valuable information for developing prevention and control strategies.METHODS: The burden of breast cancer in China in 2015 was estimated by using qualified data from 368 cancer registries from the National Central Cancer Registry. Incident cases and deaths in 22 cancer registries were used to assess the time trends from 2000 to 2015. A Bayesian age-period-cohort model was used to project the burden of breast cancer to 2030.RESULTS: Approximately 303,600 new cases of breast cancer (205,100 from urban areas and 98,500 from rural areas) and 70,400 breast cancer deaths (45,100 from urban areas and 24,500 from rural areas) occurred in China in 2015. Urban regions of China had the highest incidence and mortality rates. The most common histological subtype of breast cancer was invasive ductal carcinoma, followed by invasive lobular carcinoma. The age-standardized incidence and mortality rates increased by 3.3% and 1.0% per year during 2000-2015, and were projected to increase by more than 11% until 2030. Changes in risk and demographic factors between 2015 and 2030 in cases are predicted to increase by approximately 13.3% and 22.9%, whereas deaths are predicted to increase by 13.1% and 40.9%, respectively.CONCLUSIONS: The incidence and mortality of breast cancer continue to increase in China. There are no signs that this trend will stop by 2030, particularly in rural areas. Effective breast cancer prevention strategies are therefore urgently needed in China.

Abstract:Verification of the cases of lung cancer, from Tianjin Cancer Registration System in 1981 – 1987, was conducted. It was suggested that lung cancer incidence and mortality had increased and ranked first among various malignant tumors in that period. In urban area, the standardized incidence of male lung cancer was 44. 27/100,000, and was 24. 19% in malignant events; in female it was 32. 00/100, 000, and 26. 33% respectively. The ratio of male to female was 1.38. In comparing with the standardized incidence of lung cancer in some other countries and regions, the epidemiology features of the disease in Tianjin were lower in sex ratio and higher in female incidence, approximately similar to the levels in Shanghai and Beijing.

Abstract:BackgroundBreast cancer incidence rates are increasing among Asian women, likely due to the changes in risk factors caused by globalization. Trends in breast cancer rates among Chinese women may differ from other Asian regions due to the implementation of a nationwide family planning program and resulting changes in women's reproductive practices. Appraisal of cancer trends can direct cancer control and public health planning, but relevant studies in China are scarce due to a lack of long-term data. We sought to evaluate secular time trends in breast cancer incidence and mortality using 40 years of cancer registry data for women in urban Shanghai.Materials and methodsData on invasive breast cancer incidence and mortality were collected by the Shanghai Cancer Registry. Age-standardized rates (ASRs) for incidence and mortality were calculated using the Segi/Doll 1960 world standard population. Age, period, and birth cohort effects were evaluated using age–period–cohort (APC) Poisson regression models. Overall linear trends, interpreted as the estimated annual percentage change (EAPC), were derived from the net drift in age–drift models.ResultsA total of 53 885 breast cancer cases and 17 235 breast cancer-specific deaths were documented among women in urban Shanghai between 1 January 1973 and 31 December 2012. Breast cancer incidence and mortality ASRs increased by 141.2% and 26.6%, respectively. Significant age, cohort, and period effects were identified in both incidence and mortality APC models; cohort effects were pronounced. Overall, a substantial increase in breast cancer incidence (EAPC = 2.96%/year) and a moderate increase in breast cancer mortality (EAPC = 0.87%/year) was observed. A notable downward trend in mortality was identified among younger women born after 1960.ConclusionsForty years of cancer registry data document a tremendous increase in incidence and a slight increase in mortality for breast cancer among women in Shanghai. Effective, appropriate, and affordable breast cancer prevention and control strategies are urgently needed in China.

Abstract:Objective:To understand the mortality trend of female breast cancer in last ten years,explore the high risk population,provide a scentific basis for formulating strategies for reducing mortality of breast cancer.Methods:The female mortality data based on Chinese Health Statistics Annual(2002~2005) and National Health Statistical Yearbooks(1996~2001) were analysed by description statistical analysis.Results:①The mortality of breast cancer ranged from 8/100 000 to 10/100 000 in urban areas,4/100 000 to 6/100 000 in rural areas from 1996 to 2005;②The women more than 84 in urban areas and more than 74 in rural areas had the highest mortality rates of breast cancer;③In 2005,of all female cancer deaths,the proportion of breast cancer deaths aged 25~59 years old was higher than that in the other age groups;④In 2001,the mortalities of female breast cancer in age group 30～34 years from high to low were listed as follows:country with low economy level,country with middle economy level,country with high economy level,small and medium-sized cities,large cities;in 2005,the mortalities of female breast cancer in age group 25～29 years from high to low were listed as follows: small and medium-sized cities,country,large cities.Conclusion:The mortality of female breast cancer remains stable for the last 10 years in China,but regional gap exists;more attention should be paid to young women.

Abstract:Abstract Introduction: As a metropolis with rapid social and economic development over the past three decades, Shanghai has a breast cancer incidence that surpasses all other cancer registries in China.Methods: In order to estimate the regular changing patterns of female breast cancer in urban Shanghai, population-based incidence data from 1975 to 2004 were studied. In addition, a one-hospital-based in-patient database of 9709 female breast cancer patients treated surgically between Jan-1990 and Dec-2008 were reviewed, retrospectively.Results: We observed that breast cancer incidence increased dramatically over the past 30 years and documented a peak incidence represented by the middle-age group (45-59 yrs), which emerged in the last 20 years. The incidence peak moved from the 40-44 year group in the previous two decades to the 50-54 year group in the most recent decade. Median age at diagnosis was earlier in Shanghai than in the western countries, although it increased from 47.5-yr in 1990 to 51-yr in 2008. Considerably higher exposure to reproductive risk factors and relatively fewer hormone-dependent cases were observed- only 49.2% of breast cancers were estrogen receptor(ER) positive, 46.1% were progesterone receptor positive. The proportion of asymptomatic cases detected by screening gradually increased, as well as that of early-stage cases (from 78.6% in 1990 to 87.9% in 2008 and carcinoma in situ (14.9% in 2008). Analysis of surgical treatment patterns suggested a trend of less-invasive options.Conclusions: Rapid socioeconomic growth and birth-control policies have revealed some unique risk factors, which pushed the pattern of breast cancer in Shanghai towards that of the western countries. Age of peak incidence as well as median age at diagnosis increase with time, which suggests that increased incidence trending along with increasing age will be observed in the future. The middle-age group born after 1950s would be involved in current birth-control policies and they are also the cohorts related to the incidence boom. Thus specific screening protocols should be refined for the Chinese population, considering not only increasing age but also birth cohorts. In addition, a one-hospital-based in-patient database of 7,443 female breast cancer patients treated surgically between January-1990 and July-2007 were reviewed, retrospectively. We observed that breast cancer incidence increased dramatically over the past 30 years and documented a peak incidence represented by the middle-age group (45-59 years), which emerged in the last 20 years. The incidence peak moved from the 40-44 year group in the previous two decades to the 50-54 year group in the most recent decade. Median age at diagnosis was earlier in Shanghai than in the western countries, although it increased from 47.5-year in 1990 to 50-year in 2007. Considerably higher exposure to reproductive risk factors and relatively fewer hormone-dependent cases were observed. The proportion of asymptomatic cases detected by screening gradually increased, as well as that of early-stage cases (from 78.6% in 1990 to 93.3% in 2007) and carcinoma in situ (14.7% in 2007). Analysis of surgical treatment patterns suggested a trend of less-invasive options. Both age of peak incidence and median age at diagnosis increase with time, which suggests that increased incidence trending along with increasing age, will be observed in the future. Consequently, specific screening protocol should be refined to consider birth cohorts.

Abstract:Abstract Background Women's cancers, represented by breast and gynecologic cancers, are emerging as a significant threat to women's health, while previous studies paid little attention to the spatial distribution of women's cancers. This study aims to conduct a spatio-temporal epidemiology analysis on breast, cervical and ovarian cancers in China, thus visualizing and comparing their epidemiologic trends and spatio-temporal changing patterns. Methods Data on the incidence and mortality of women’s cancers between January 2010 and December 2015 were obtained from the National Cancer Registry Annual Report. Linear tests and bar charts were used to visualize and compare the epidemiologic trends. Two complementary spatial statistics (Moran’s I statistics and Kulldorff’s space–time scan statistics) were adopted to identify the spatial–temporal clusters. Results The results showed that the incidence and mortality of breast cancer displayed slow upward trends, while that of cervical cancer increase dramatically, and the mortality of ovarian cancer also showed a fast increasing trend. Significant differences were detected in incidence and mortality of breast, cervical and ovarian cancer across east, central and west China. The average incidence of breast cancer displayed a high-high cluster feature in part of north and east China, and the opposite traits occurred in southwest China. In the meantime, the average incidence and mortality of cervical cancer in central China revealed a high-high cluster feature, and that of ovarian cancer in northern China displayed a high-high cluster feature. Besides, the anomalous clusters were also detected based on the space–time scan statistics. Conclusion Regional differences were detected in the distribution of women’s cancers in China. An effective response requires a package of coordinated actions that vary across localities regarding the spatio-temporal epidemics and local conditions.
Trends in incidence and mortality of esophageal cancer from 1981 to 2002 in Tianjin

Abstract:Objective To investigate the trends in incidence and mortality of esophageal cancer from 1981 to 2002 in Tianjin,China and to predict future trends and facilitate future prevention.Methods All new cancer cases registered in the Tianjin Cancer Registry between 1981 and 2002 were reviewed.Yearly age-specific,crude and age-adjusted incidence rates were calculated for males and females.Secular trends of incidence and mortality were indicated by the estimated annual percentage change(EAPC).Results A total of 8206 cases of esophageal cancer were reported in Tianjin form 1981 to 2002,with 5533 males and before 45 years,but increased rapidly after 45 years.The median age of esophageal cancer cases was 69 years,with quartile range from 62 to 75 years.The peak for age specific incidence in females was 75-80 years,which was earlier than that in males(aged 80-85).But recent data indicated that the peak age in females had increased and became similar to that of the males.The crude and age-standardized incidence rates(ASIR)of esophageal cancer decreased in last 22 years.For male and female,crude incidence decreased by 2.22% and 3.56% per year and ASIR decreased by 5.18% and 6.56% per year.The mortality of esophageal cancer decreased in the last 22 years.Conclusion The incidence of esophageal cancer decreased in the last 22 years in Tianjin,but there is little overall improvement in survival.
