Abstract:Objective To conduct a descriptive epidemiological study on thyroid cancer incidence in the urbanarea of Tianjin during 2002 - 2006 and to analyze the secular trend of incidences based on previous data of 26 years and those from Shanghai and the United States. Methods Annual percent change (APC%) of thyroid cancer incidence and the rank of all-site cancer profiles were estimated. The secular trend of different histological incidences and the proportion changes were analyzed. Results The incidence of female thyroid cancer was 6. 55/100000 in 2006, ranking the eighth in the female cancer profile. The incidence of thyroid cancer showed an increased secular trend in recent 26 years, especially a 5.7-fold increase in the papillary thyroid cancer (PTC). The incidence for female increased by 267% in 2006 compared to that in 1981, APC% was 3. 1%. The variation ranked second among all the female tumors. The incidences of thyroid cancer in Shanghai and the United States also elevated with time.Conclusions Thyroid cancer has been one of female common tumors from a once less common tumor,especially the proportion of PTC is expanding. It suggests that further study on the risk factors and preventive efforts should be made.
Analysis of Variation Trends of Thyroid Cancer Treated in Tianjin Cancer Hospital Form 1954 to 2009

Abstract:OBJECTIVE:To investigate and analyze the variation trends in the pathological composition of thyroid cancer patients treated in Tianjin Cancer Hospital from 1954 to 2009.METHODS:To retrospectively analyze the incidence and clinical features of different pathological types of thyroid cancers in 4342 patients between different time periods from 1954 to 2009.RESULTS:In the four main pathological types of thyroid cancers, the component ratio of papillary thyroid cancer in every period was 68.1%, 78.3%, 81.3%, 82.1%, 85.8%, respectively, while the morbidity of patients with papillary thyroid carcinoma concurrent with Hashimoto's thyroiditis was increased, so was the proportion of tumors in diameter < or = 2 cm. The proportion of follicular thyroid carcinoma and anaplastic thyroid carcinoma was decreasing accordingly; however, the proportion of medullary thyroid carcinoma did not change significantly.CONCLUSIONS:The pathological classification of the thyroid carcinoma patients has significant changes in the 4342 cases treated in our Hospital from 1954 to 2009. The proportion of papillary carcinoma is increased, while that of follicular carcinoma and anaplastic carcinoma is decreased. The reasons might attribute to the improved level of consultations and iodized diet or other factors.
Trend analysis of the incidence of prostate cancer in Tianjin between 1981 and 2004

Abstract:Objective To analyze the trend of the incidence of prostate cancer in urban Tianjin area between 1981 and 2004 so as to provide scientific rationales for the prevention and control of prostate cancer.Methods The data of prostate cancer cases were provided by the Tianjin Cancer Registry. The SAS system V9 was used to calculate the crude incidence, age-adjusted incidence and age specific incidence of prostate cancer in Tianjin between 1981 and 2004. And the Joinpoint 3.0 software was used to analyze secular trends. Results Between 1981 and 2004, a total of 1060 prostate cancer cases were diagnosed in Tianjin.And the age-adjusted incidence rate was 2.84/100 000 in 2004. The incidence of prostate cancer showed 3 incremental stages during the 24-year period at an annual average of 4.02%. An aging trend was observed for prostate cancer patients. The elder patients had a faster increment in incidence. Specifically, it increased annually at 4.63% for age group 65 - 74 year versus 5.18% for age group 75 years or more. Conclusion Despite a low incidence of prostate cancer in Tianjin, it is increasing at a fast rate. The prevention and control of prostate cancer should be strengthened.
Incidence trend of thyroid cancer in Beijing Dongcheng District,2001-2011

Abstract:Objective: To describe the incidence trends of thyroid cancer in Beijing Dongcheng District from 2001 to 2011. Methods: The data of thyroid cancer incidence in Being Dongcheng District from 2001 to 2011 was collected. The incidence rate was calculated by gender and age, and age-standardized rate (ASR) was estimated by world population. The annual percentage change (APC) of incidence was estimated by linear regression based on the logarithm of the incidence rates observed. Results: Total of 563 cases were diagnosed of thyroid cancer in 2001-2011 with the gender ratio of 1︰3.50 (125 males vs 438 females). Incidence rate of thyroid cancer increased dramatically from 2.69/100 000 in 2001 to 14.29/100 000 in 2011 with an increase percentage of 431.23%. ASR increased from 1.69/100 000 in 2001 to 9.33/100 000 in 2011 with an increase percentage of 452.07%. The age-specific rate increased with age rising. A rising trend of thyroid cancer could be seen in groups aged 25-64 years. The peak incidence was seen in males aged 70-74 years and females aged 45-49 and 75-79 years. APC of incidence rate of males and females were 16.88% and 15.60%, respecrively. Conclusion: The incidence of thyroid cancer increases rapidly from 2001 to 2011 in Beijing Dongcheng District, so the risk factors and related measurements should be taken into consideration in further study. Clinicopathologic Analysis of 464 Cases of Thyroid Carcinoma in Yanbian Area

Abstract:Objective To study the incidence trend of patients with thyroid carcinoma in Yanbian Area.Methods A retrospective analyze of 464 cases with thyroid carcinoma from 1970 to 2004 diagnosed by pathology.Results The incidence of thyroid carcinoma was increased in Yanbian area over the 1970~2004 periods. Papillary,follicular,medullary and anplastic carcinoma accounted for 70%,21%,4% and 3% of cases,respectively.The rate of papillary carcinoma increased from 58% in 1970 to 85% at present.The mean age at diagnosis was 43 years,and the peack age of incidence was 41~50 years old group.The female to male ratio was 5∶1.There was no significant difference in thyroid carcinoma incidence between two nationalities.Conclusion Thyroid carcinoma incidence has increased in Yanbian area and the rate of papillary carcinoma increased over the past 35 years.The incidence of thyroid carcinoma was no significant differences in two nationalities.

Abstract:OBJECTIVETo observe the trend in the change of incidence and mortality of female breast cancer in Tianjin and evaluate its effect of prevention.METHODSMethod of descriptive epidemiology was used to study the epidemic situation of female breast cancer in Tianjin.RESULTSThe incidence of breast cancer in Tianjin had increased by 39.7% from 1981 to 1997, as compared with the other cities in China. Especially, compared with the developed countries the world over, the incidence of breast cancer in Tianjin is much lower. The mortality rate of breast cancer had lowered by 31.0% from 1981 to 1997 and the 3-year and 5-year survival rates increased to various degrees.CONCLUSIONEarly detection and diagnosis of breast cancer are very important to both increase of survival and lowering of mortality of breast cancer. Preventive efforts should be made to the high risk people of breast cancer.

Abstract:Objective: To investigate the trend of thyroid cancer incidence in Pudong New Area in Shanghai from 2002 to 2009. Methods: The data of thyroid cancer incidence in Pudong New Area from 2002 to 2009 were collected. The incidence rate was calculated by gender and age, and the ASR (age-standardized rate) was estimated by world population. The APC (annual percent change) of incidence was estimated by linear regression based on the logarithm of the incidence rates observed. Results: During the period from 2002 to 2009, a total of 2 489 thyroid cancer cases had been diagnosed, and the ratio of male and female incidence was 1︰3.37. The incidence has increased from 6.71/100 000 in 2002 to 20.08/100 000 in 2009. The incidence of thyroid cancer from 2002 to 2009 was increasing in both male and female, substantially in the population with age of 35-39 years. Conclusion: The incidence of thyroid cancer was increased rapidly from 2002 to 2009, especially in young women. Abstract:Objective To study clinicopathological characteristics and tendency of incidence of lung tumors from 1980 to 2007 in Tianjin and to provide information for pathologists and clinicians.Methods The data of lung tumors are collected from General Hospital and the Second Hospital of Tianjin Medical University.The data are analyzed by SPSS11.5.Results Both of the ratios(the ratios of lung cancer to the total and lung cancer to the malignant tumors) have been increasing druing 28 years(P<0.05).Among 3 075 cases females are accounting for 29.5%,and the cases of females have been increasing further than males.The ratio to the total among the corresponding time period has the same trend(P=0.040).The ratio of adencarcinoma and large cell carcinoma is increasing while the ratio of squmous cell carcinoma and small cell carcinoma is decreasing.The cases of youngers have been also increasing and its proportions of histological types of lung cancers has a significant different compared with the middle-aged and olders(P=0.000).Adencarcinoma is the main type of youngers while squamous cell carcinoma is the main type in both the middle-aged and olderly group.Conclusion The detection rate of lung carcinoma especilly carcinoma in females is increasing,and the subtypes of lung carcinoma also changed.

Abstract:Objective:To analyze the incidence trend of patients with thyroid carcinoma (TC) in Yanbian area from 2005 to 2012.Methods:Total of 656 patients of TC were collected from 2005 to 2012 diagnosed by pathology.The incidence trend of TC was analyzed in different periods,different nationality,gender,age,and the variation trends in pathological types of TC.Results:The crude incidence of TC increased from 5.9/105 and 4.03/105 to 16.49/105 and 12.75/105 from 2005 to 2012 in Korean and Han female.The age peaking was at 50 years and 40 years in Korean and Han women,and it was 50 years group in Korean and Han men.The rates of PTC increased from 85.5% to 93.9% during the period 2005 to 2012.The rates of MPTC were increased 4.2 times.Conclusion:In recent years,the crude incidence of TC remarkably increased in Yanbian area,and it was significantly higher in Korean-women than that the Han-women in 2012.The age of TC patients was different between two nationalities woman.

Abstract:Objective: To examine the secular trends of thyroid cancer incidence and mortality and to estimate the proportion of thyroid cancer cases potentially attributable to overdiagnosis. Methods: Data on thyroid cancer cases from 1973 to 2015 were obtained from the Shanghai Cancer Registry. The average annual percent change (AAPC) was evaluated using the joinpoint regression analysis. The age, period, and birth cohort effects were assessed using an age-period-cohort model. The overdiagnosis of thyroid cancer cases was estimated based on the difference between observed and expected incidences using the rates of Nordic countries as reference. Results: From 1973 to 2015, the number of thyroid cancer cases was 23117, and 75% of the patients were women. The age-standardized rates were seven-to eightfold higher from 2013 to 2015 than from 1973 to 1977. Compared with relatively stable mortality, thyroid cancer incidence was dramatically increased from 2002 to 2015 in both sexes, with significant trends (men: AAPC = 21.84%, 95% CI: 18.77%-24.98%, P < .001; women: AAPC = 18.55%, 95% CI: 16.49%-20.64%, P < .001). The proportion of overdiagnosis has gradually increased over time, rising from 68% between 2003 and 2007 to more than 90% between 2013 and 2015. This increasing trend appeared to be similar between men and women. Conclusion: An increasing gap between thyroid cancer incidence and mortality was observed in Shanghai, and overdiagnosis has contributed substantially to the rise of incidence, which calls for an urgent update on the practice of thyroid examination. Trends in incidence and mortality of esophageal cancer from 1981 to 2002 in Tianjin

Abstract:Objective To investigate the trends in incidence and mortality of esophageal cancer from 1981 to 2002 in Tianjin,China and to predict future trends and facilitate future prevention.Methods All new cancer cases registered in the Tianjin Cancer Registry between 1981 and 2002 were reviewed.Yearly age-specific,crude and age-adjusted incidence rates were calculated for males and females.Secular trends of incidence and mortality were indicated by the estimated annual percentage change(EAPC).Results A total of 8206 cases of esophageal cancer were reported in Tianjin form 1981 to 2002,with 5533 males and before 45 years,but increased rapidly after 45 years.The median age of esophageal cancer cases was 69 years,with quartile range from 62 to 75 years.The peak for age specific incidence in females was 75-80 years,which was earlier than that in males(aged 80-85).But recent data indicated that the peak age in females had increased and became similar to that of the males.The crude and age-standardized incidence rates(ASIR)of esophageal cancer decreased in last 22 years.For male and female,crude incidence decreased by 2.22% and 3.56% per year and ASIR decreased by 5.18% and 6.56% per year.The mortality of esophageal cancer decreased in the last 22 years.Conclusion The incidence of esophageal cancer decreased in the last 22 years in Tianjin,but there is little overall improvement in survival.
Investigation on the Factors That Influence the Prevalence of Thyroid Nodules in Adults in Tianjin, China.

Abstract:Studies have shown that prevalence of thyroid nodules (TNs) has been increasing recently. However, the factors that may influence TN prevalence is not fully understood. In this study, we aimed to understand the prevalence of TNs and identify possible factors that are associated with the prevalence of TNs in Tianjin, China. Subjects aged 18 years or older were randomly collected and all subjects received thyroid ultrasonography, physical examination and questionnaires. Subjects (n = 2647) were divided into the case group in which the subjects had TNs and the control group in which the subjects did not have TNs. Potential influencing factors on TNs including sex, age, iodine status, thyroid volume, thyroid hormone (TSH), thyroid autoantibody TPOAb, TGAb and living habits were analyzed. Our results showed that the overall TN prevalence was 26.7%. The prevalence of TNs in women was higher than that in men (P < 0.05). TN prevalence increased with age (P < 0.001), and a U-shaped curve relationship between urine iodine concentrations (UICs) and prevalence of TNs was observed. The positive rate of TPOAb and goiter rate in case group was higher than that in control group (P < 0.05). The thyroid volume in case group was larger than that in control group (P < 0.001). Other factors that may influence TNs included high blood pressure, iodized salt, menopause, seafood intake, and education levels. None of UIC, TSH, TPOAb and TGAb were associated with TN prevalence in regression models. In conclusion, our results showed that TNs prevalence in Tianjin was high. TNs prevalence was higher in women than in men, and it increases with age. The older, female, high education level physical labor and goiter are independent risk factors for TNs.
Clinicopathological and Prognostic Analysis of Follicular Thyroid Carcinoma in a Single Institute over a 15-Year Period

Abstract:BACKGROUND:This study was to evaluate the clinicopathological and prognostic features of follicular thyroid carcinoma (FTC) in our institute over a 15-year period.METHODS:The clinical features, management and outcome of 134 consecutive patients were analyzed according to the time of diagnosis: Group I (1997-2001), Group II (2002-2006), and Group III (2007-2011).RESULTS:As time advanced, the ratio of FTC to papillary thyroid carcinoma decreased from 8.7% in group I to 4.3% in group III (p = 0.000). The percentage of patients undergoing total thyroidectomy seemed to be more commonly used in the later periods - from 10.5% in group I to 21.8% in group II and 18.9% in group III. The median diameter of tumors in group I was 4.2 cm and it showed a sharp decrease to 2.8 cm in group II and 2.9 cm in group III respectively. There was a trend towards a higher stage in patients from Group I vs. patients from Groups II and III (stage IV, 15.8% vs. 2.2% and 4.3%, p = 0.072). The outcome was improved in terms of disease-free survival (DFS). The 3-year DFS rate improved from 77.8% in group I to 93.7% in group II and 100% in group III (p = 0.008).CONCLUSIONS:The clinical features, management and outcome of FTC patients changed over 15-year period. Patients diagnosed after 2001 had a better prognosis. This improvement was probably related to earlier diagnosis with smaller tumor size and presentation at earlier tumor stage.
[Time Trends and Pathological Characteristics of Thyroid Cancer in Urban Beijing, 1995 - 2010].

Abstract:OBJECTIVE:To describe the incidence trends and to access the histological changing patterns of thyroid cancer in urban areas of Beijing.METHODS:Total of 4883 new cases diagnosed as thyroid cancer in 1995-2010 in urban areas of Beijing were extracted from the population-based data base of Beijing cancer registry, among which 1185 were males and 3698 were females with a coverage population of 56 143 454 person-years and 54 430 655 person-years, respectively. Incidence, age adjusted incidence, annual percentage change (APC) and gender specific incidence change of different histology patterns during last 16 years were calculate.RESULTS:There were 4883 cases diagnosed as thyroid cancer during 1995 - 2010 with the sex ratio of 1:3 (1185 males and 3698 females). Incidence rate of thyroid cancer in urban areas of Beijing increased dramatically from 1.55/100 000 (97/6 245 016) in 1995 to 9.90/100 000 (768/7 758 759) in 2010 with an increase rate of 538.71%. After adjusted by world population, the standardized incidence rate increased from 1.27/100 000 in 1995 to 6.96/100 000 in 2010 with a 12.12% annual percentage change (APC = 12.12%, P < 0.05). Among males, incidence rate increased from 0.85/100 000 (27/3 185 669) to 4.58/100 000 (179/3 912 458) with an increase rate of 438.82%. After adjusted by world population, the standardized incidence rate of males increased from 0.68/100 000 in 1995 to 3.19/100 000 in 2010 with an 11.09% annual percentage change (APC = 11.09%, P < 0.05). Among females, the incidence rate increased from 2.29/100 000 (70/3 059 347) to 15.31/100 000 (589/3 846 301) with an increase rate of 568.56%. After adjusted by world population, the standardized incidence rate of females increased from 1.89/100 000 in 1995 to 10.82/100 000 in 2010 with a 12.48% annual percentage change (APC = 12.48%, P < 0.05). Incidence rate ranked 18th and 12th in 1995 among males and females, respectively; while it ranked 14th and 5th in 2010 respectively. Among 4883 cases, 4594 cases were confirmed histologically as primary tumor accounting for 94.08% of total cases. Papillary carcinoma was the most common subtype accounting for 77.31% (3775/4883) of total cases. Among all the pathological types of thyroid cancer, the proportion of papillary carcinoma has risen from 51.55% (50/97) in 1995 to 87.63% (673/768) in 2010. However, the proportion of the follicular carcinoma decreased from 5.15% (5/97) to 1.69% (13/768).CONCLUSION:Incidence of thyroid cancer in urban areas of Beijing keeps increasing which is mainly due to the rising of papillary carcinoma. Risk factors of papillary thyroid cancer should be focused on when strategies for thyroid cancer prevention and control are conducted.

Abstract:Verification of the cases of lung cancer, from Tianjin Cancer Registration System in 1981 – 1987, was conducted. It was suggested that lung cancer incidence and mortality had increased and ranked first among various malignant tumors in that period. In urban area, the standardized incidence of male lung cancer was 44. 27/100,000, and was 24. 19% in malignant events; in female it was 32. 00/100, 000, and 26. 33% respectively. The ratio of male to female was 1.38. In comparing with the standardized incidence of lung cancer in some other countries and regions, the epidemiology features of the disease in Tianjin were lower in sex ratio and higher in female incidence, approximately similar to the levels in Shanghai and Beijing.

Abstract:Objective To evaluate the change of clinicopathological features and prognosis of papillary thyroid cancer over a 15-year period.Methods The clinicopathological features and outcomes of papillary thyroid cancer patients were analyzed in three groups according to the time of diagnosis:group Ⅰ (1997-2001),group Ⅱ (2002-2006),and group Ⅲ (2007-2011).Results As time advanced,the average age of papillary thyroid cancer patients increased,tumor stage,like size,extrathyroid invasion and lymph node metastasis decreased dramatically (P ＜ 0.01).The percentage of multifocality and bilaterality increased.The long-term follow up data (median follow up time was 6.6 years),indicated that the 15-year over all survival was 97.8％ and the 15-year disease-free survival was 90.2％.Tumor ≥3 cm,bilaterality,extrathyroid invasion,lymph node metastasis and AJCC stage were correlated with tumor recurrence.By multivariate COX-regression analysis only lymph node metastasis and bilaterality were independent risk factors.Conclusion The clinicopathological features of papillary thyroid cancer changed over 15 years,with the percentage of early-staged patients increased.Lymph node metastasis and bilaterality are two risk factors for tumor recurrence.

Abstract:<h3 class="Heading">Purpose</h3><p class="Para">Understanding secular trends of thyroid cancer is critical to plan strategies for cancer prevention and control. Our aim was to estimate the incidence and mortality trends of thyroid cancer in China during 2005–2015.</p><h3 class="Heading">Methods</h3><p class="Para">A retrospective cohort evaluation of thyroid cancer cases and deaths during 2005–2015 was performed using population-based data from the <em class="EmphasisTypeItalic">Chinese Cancer Registry Annual Report</em>. The incidence and mortality rates of thyroid cancer were stratified by gender, age group (0, 1–4, 5–9, 10–14…80–84, 85–), and area (urban or rural). A Joinpoint regression model was used to examine secular trends.</p><h3 class="Heading">Results</h3><p class="Para">In China, the age-standardized incidence was 3.21/10<sup>5</sup> in 2005, and increased to 9.61/10<sup>5</sup> in 2015. Besides, a significant increase incidence rate was observed with the average annual percent change (AAPC) of 12.4% (95% CI: 10.5%–14.4%) in the period 2005–2015. The age-standardized mortality was 0.30/10<sup>5</sup> in 2005 and 0.35/10<sup>5</sup> in 2015, and the AAPC was 2.9% (95% CI: 1.3%–4.5%). For both incidence and mortality, the rates of thyroid cancer were much higher in females than in males, and in urban areas rather than rural areas; however, the rates of increasing trends showed no significant differences. With respect to the highest age-specific rates, it appeared in the age group of 50–54 years old for incidence and in the age group of 80–84 years old for mortality. Notably, the rate of increasing incidence trend was lower in older age groups, especially for people aged 70–79 years old.</p><h3 class="Heading">Conclusion</h3><p class="Para">A rapid increase in incidence and a moderate increase in mortality of thyroid cancer were observed from 2005 to 2015 in our study. Effective measures and tailored programs should be taken to curb the growth trend and reduce the disease burden.</p>

Abstract:Objective To analysis the incidence and mortality status of thyroid cancer in China,2010.Methods Incidence and mortality data of thyroid cancer were derived from 145 cancer registries in China of 2010.Incidences,mortality,age-specific rates of thyroid cancer in different areas (urban and rural) were calculated.The incident cases and deaths were estimated using age-specific rates and national population data.Chinese census in 2000 and Segi's population were used for age-standardized incidence and mortality rates.Results The registration areas covered a total of 158 403 248 population (92 433 739) in urban areas and 65 969 509 in rural areas,which accounted for 12.05％ (158 403 248/1 314 873 293) of the total population of the whole country in 2010.The estimates of new cancer incident cases was 54 175 (male 12 962,female 41 213),with the crude incidence rate of 4.12/100 000 (54 175/1 314 873 293).Age-standardized incidence rates by Chinese standard population (ASIRC,2000) and by world standard population (ASIRW) were 3.62/100 000 and 3.23/100 000,respectively,which accounted for 1.75％ (54 175/3 093 039) of all malignant tumors in 2010.Male to female ratio was 1∶ 3.2,the incidence rate in urban areas was 1.46 times higher than that of rural areas.The estimates number of cancer deaths was 4 504(male 1 529 cases and female 2 975 cases) in 2010 in China with the crude mortality rate of thyroid cancer 0.34/100 000 (4 504/1 314 873 293).Age-standardized mortality rates by Chinese standard population (ASIRC,2000) and by world standard population (ASIRW) were 0.27/100 000 and 0.26/100 000,respectively,which accounted for 0.23％ (4 504/1 956 662) of all malignant related deaths.The cumulative incidence and mortality rates (0-74 years old) were 0.32％ and 0.03％,respectively.According to the data from 145 cancer registries,papillary carcinoma was the main pathology type,which accounted for 86.0％ (6 237/7 253) of all malignant tumors.Conclusion The incidence and mortality rates of thyroid cancer in urban areas of China were higher than that in rural areas,and females had higher incidence rate of thyroid than males.The reasons related the higher incidence rate of thyroid cancer in urban China should be paid more attention to in the future.

Abstract:Objective:To investigate the incidence trends and clinicopathologic characteristics of papillary thyroid cancer (PTC). Methods:A retrospective analysis was conducting using the following data:3,766 cases with thyroid disease in the People's Hospital of Wuhan University between January 2001 and July 2013;and 977 cases with thyroid cancer in the Hubei Cancer Hospital between Janu-ary 2006 and July 2013. Results:The incidence of thyroid cancer increased significantly since 2008, ranging from 14.94%to 18.10%(P<0.05). In particular, the PTC cases ranged from 85.33%to 90.89%(P<0.05). A total of 1,416 cases were diagnosed as PTC with a male to female ratio of 1:3.75. The positive rate of neck lymph node metastasis (NLNM) was significantly different in terms of gender and age (P<0.05). Significant differences were also found between the unifocal group and the multifocal group;the positive rate of NL-NM was 77.94%in the latter group. The rate of NLNM in PTC was 72.29%, which had higher significance compared with Hashimoto's thyroiditis or with nodular goiter. Conclusion: The incidence of thyroid cancer is increasing. Cases involving males aged below 45 years old and with>1 cm tumor diameter and multifocal PTC are more likely to be complicated with NLNM.
