Abstract:Objective To investigate the proportions and features of tumors,cysts and tumor-like lesions in the oral-maxillofacial region.Methods 9 056 cases of primary tumors,cysts and tumor-like lesions in the oral-maxillofacial region which were confirmed by pathological diagnosis(from January 1955 to December 2006 at the Department of Oral and Maxillofacial Surgery,Second Affiliated Hospital,School of Medicine,Zhejiang University) were retrospectively analyzed about their clinical pathological features,histological types,gender proportions and primary locations.Results There were 6 337 cases of tumors,2 363 cysts and 356 tumor-like lesions in the oral-maxillofacial region.Among the tumors,the benign tumor accounted 56.7%(3 596/6 337) while the malignant tumor took up 43.3%(2 747/6 337).Pleomorphic adenoma,hemangioma and ameloblastoma were the most common benign tumors.While the principle malignant tumor was carcinoma(86.2%,2 367/2 747),most of which were squamous cell carcinoma(50.0%,1 184/2 367),majorly in tongue,gingiva and cheek,with a gender ratio of 2.34∶1.Meanwhile,the most common benign and malignant salivary gland tumors were pleomorphic adenoma and adenocarcinoma,respectively,commonly sited in parotid gland,submandibular gland,palatine gland and parotid gland,palatine gland,submandibular gland,respectively.The dentigerous cyst(33.9%,408/1 202) and the sublingual gland cyst(41.1%,477/1 161) were the most common types of the odontogenic cysts and non-odontogenic cysts in the oral-maxillofacial region separately.The most frequent tumor-like lesion was epulis (54.8%,195/356).Conclusion Within this research,tumor is the most common lesion,with a number of begnin tumor more than malignant tumor,and the amount of male patients is larger than that of female patients with tumors,cysts or tumor-like lesions in the oral-maxillofacial region(261 words).

Abstract:Objective To investigate the clinicopathologic characters of primary minor salivary gland tumor in the pharynx,larynx and cervical trachea. Methods 13 cases with primary minor salivary gland tumor in the pharynx,larynx and cervical trachea were analyzed retrospectively.The exact diagnosis of the tumors was determined by frozen pathological examination.Two cases of pleomorphic adenoma of the parapharyngeal space and two cases of laryngeal pleomorhic adenoma were removed by transcervical approaches.Two cases of pharyngeal myoepithelioma and two cases of small pleomorhic adenoma in the parapharyngeal space were excised by transoral approaches.A case of supraglottic adenoid cystic carcinoma(T1N0M0) was removed by supraglottic horizontal hemilaryngectomy.Two cases of subglottic adenoid cystic carcinoma were excised by hemilaryngectomy.Two cases of malignant cervical trachea neoplasm were removed with trachea segmental resection and end-to-end anastomosis and postoperative radiotherapy was given. Results The ratio of malignant and benign neoplasm among 13 tumors was 2:1 and male-female ratio was 1:2.The lesion sites were as follows: 6 cases in the pharynx,4 cases in the larynx,2 cases in the cervical trachea.Of 8 cases of benign tumors whose disease course was from 10 days to 2 months,6 cases were pleomorphic adenoma and 2 cases were myoepithelioma.Of 5(4) malignant tumors,4 cases were adenoid cystic carcinoma and one case was mucoepithelioma.All patients were uneventful.After 2~12 years follow-up survey,2 patients of 8 cases with benign tumors lost follow-up survey after operation,one case died of other diseases and the other five cases did no relapse;one patients of 5 cases with malignant tumor lost follow-up survey and other four cases were free from their primary malignant tumors. Conclusion Primary minor salivary gland tumors in the pharynx,larynx and cervical trachea have their special features that early symptoms are not typical or not distinct and local function can be affected when tumors develop gradually.These tumors can be treated by operation.

Abstract:Salivary gland tumors are a group of the most common tumors in the oral and maxillofacial region. They have features with complicated histopathologic classification and wide distribution. There is little knowledge on the clinicopathologic characterics and clinical behaviors of salivary gland tumors of various locations and subtype. The management of salivary gland tumors is also contraversial. Biopsy is not suggested because of seeding of tumor cells. Therefore adjunctive methods are important for preoperative diagnosis. Since 1985, our research group has performed a series of basic and clinical researches on salivary gland tumors including clinicopathologic characteristis and behavior, preoperative adjunctive diagnosis, mechanism, prevention and treatment of distant metastasis of salivary adenoid cystic carcinoma, and histogenesis of salivary gland tumor. These studies have improved our knowledge on the clinical, histopathological, cytological, imaging features, and biobehavior of different types of slivary gland tumors. We have also suggested related principles of management, modified the operative methods of benign tumor of parotid gland. Our efforts have raised the standards of diagnosis and treatment of salivary gland tumors.

Abstract:Salivary gland tumors are a group of the most common tumors in the oral and maxillofacial region with obvious characteristics of clinical, histopathologic, and therapeutic aspects. During the past 50 years, our research group performed a systemic study on salivary gland tumors including imaging diagnosis, fine needle aspiration cytology, FT-mid-IR fiber optics spectroscopy, histopathologic features and biological behaviour of the tumors, functional parotid surgery, and (125)I seed implant branchytherapy. More reasonable principles of management for various types of the tumors were put forward based on the histopathologic features of the tumors. The surgical techniques were modified and the postoperative complications were reduced obviously. Thus, the quality of life of the patients was improved greatly.

Abstract:Chinese patients have a higher rate of lymphoepithelial carcinoma (LEC) and salivary duct carcinoma (SDC). Comprehensive use of diagnostic modalities, neck dissection, and postoperative radiation will improve the treatment results for salivary gland tumors (SGTs).To study the clinicopathological characteristics of SGTs in a Chinese population.The records of SGT patients operated in a tertiary cancer hospital of China were retrieved.From December 1997 to December 2007, 289 malignant and 887 benign SGTs were operated at Cancer Hospital, Shanghai, China. Pleomorphic adenoma and Warthin's tumor were the most common types of SGT. Mucoepidermoid carcinoma (24.6% of malignant cases) and adenoid cystic carcinoma (18.0%) were the most frequent malignant cases, followed by acinic cell carcinoma (12.1%), LEC (9.7%), and SDC (9.3%). The sensitivity and specificity of ultrasound scan, fine needle aspiration biopsy, and frozen section were 58.3 and 88.6%, 87.2 and 96.7%, 86.9 and 99.6%, respectively. Neck dissections and postoperative radiation were carried out for 48.6 and 48.0% of carcinomas, respectively. The percentage of tumors by pathologic TNM stage were 23.7% for stage I, 32.9% for stage II, 17.3% for stage III, and 26.1% for stage IV. The 5-year overall survival rate was 88.0%.

Abstract:The aims of this study were to investigate the epidemiological and clinical characteristics of epithelial salivary gland tumours in a northern Chinese population and to evaluate the current TNM classification system. A demographic and descriptive analysis of 7190 epithelial salivary gland tumours was performed. There were 4654 benign tumours and 2536 malignant tumours. The percentage of tumours located in the parotid, submandibular, sublingual, and minor salivary glands was 62.66%, 9.92%, 2.57%, and 24.85%, respectively; 22.26%, 35.76%, 92.97%, and 61.89% of the tumours, respectively, were malignant. Over 90% in the tongue and maxillary sinus were malignant. Warthin tumour, salivary duct carcinoma, and squamous cell carcinoma were predominant in males, while basal cell adenoma, myoepithelioma, and pleomorphic adenoma were predominant in females. Further, 2.55% of the tumours were in children and adolescents: 44.81% of the tumours were malignant, as opposed to 35.02% in adults. According to the 7th TNM classification, the percentages of T3 and stage III tumours were approximately 10%. Salivary gland tumours show distribution patterns according to histological type, location, and patient age and sex. The limitations of the current TNM classification of salivary gland carcinoma should be considered and revisions made.

Abstract:The majority of the salivary gland tumours are of epithelial origin. The parotid gland is the most common location of these tumours. Surgery is the main modality for the management of salivary gland tumours. The dissection and preservation of the facial nerve are very important in most cases of parotidectomy. Partial parotidectomy has potential advantages in suitable cases. Sacrifice of the facial nerve is necessary for some patients with parotid carcinoma. The defects of the facial nerve should be reconstructed as soon as possible. Functional training of facial muscles is helpful for the recovery of facial nerve function. The indications for elective neck dissection, radiotherapy and chemotherapy are discussed.

Abstract:Four hundred five cases with carcinoma of salivary glands were analyzed clinicopathologically; 181 tumors were located in parotid, 52 were in submandibular, 21 were in sublingual, and 151 were in minor salivary glands. Mucoepidermoid carcinoma and adenoid cystic carcinoma were the most common histologic types. The overall 3-, 5-, 10-, and 15-year survival rates were 77.8%, 69.6%, 55.8%, and 36.7%, respectively. The difference between survival and recurrence-free survival rate was 9%. The prognosis of acinic cell carcinoma and mucoepidermoid carcinoma was much better than that of squamous cell carcinoma, adenocarcinoma, and undifferentiated carcinoma. The survival rates of minor salivary gland tumors was the highest, and those of submandibular gland tumors were the lowest. Postoperative radiotherapy improved the prognosis of adenoid cystic carcinoma. The overall recurrence rate was 40.2%; the cervical lymph node metastases rate was 15.3%, and the distant metastases rate was 11.1%.
Myoepithelial Carcinoma of the Salivary Glands：behavior and Management

Abstract:OBJECTIVE To investigate the biological behavior and proper management of myoepithelial carcinomas of salivary glands. METHODS Twenty-seven cases of myoepithelial carcinoma of salivary glands were retrospectively studied and their detailed clinical and follow-up data were presented. RESULTS The subjects consisted of 17 men and 10 women aged 16 to 73 years (mean age: 51 years). The parotid gland was the most common site (n = 14) of cancer. Clinical features included extensive local growth, invasion of the surrounding tissues, infrequent cervical lymph node metastasis but high rates of distant metastasis, frequent/multiple recurrences and poor prognosis. CONCLUSIONS Myoepithelial carcinomas of the salivary gland should be classified as high-grade malignancies. Early and radical surgery with close follow-up are essential for achieving favorable outcomes. Radiotherapy appears to be non-sensitive and elective neck dissection is generally unnecessary.

Abstract:In a series of 2871 epithelial salivary gland neoplasms managed in the Peking University School of Stomatology between 1974 and 1999, 86 arose in children <16 years of age (52 parotid, 12 submandibular gland, 2 sublingual gland, and 20 minor salivary gland). Considerable delay was encountered in diagnosis (benign 24 months and malignant 16 months). In this group of children, 46 tumours (53%) proved to be malignant, with an incidence in the parotid, submandibular, sublingual, and minor salivary glands of 31/52 (60%), 2/12, 0/2, and 13/20 (65%), respectively. Sixty-six of 86 neoplasms (77%) occurred in children between 10 and 16 years of age. Only six neoplasms were encountered in children of 5 years or younger, four of which were high-grade malignant tumours. Benign tumours were successfully treated by local excision with only one recurrence. Of 46 malignant neoplasms, 8 were treated palliatively; of the remainder 8 were lost to follow-up and 2 patients died of their disease.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objective: Salivary gland tumors are a heterogenous group of lesions with variable pathology and clinical outcomes. Most published data are derived from studies conducted at tertiary care centers. Our study analyzed the experience from a community setting to determine significant differences, if any, in pathological distribution and clinical outcomes compared to the existing literature. Methods: We performed a retrospective analysis of all major salivary gland tumors that presented to a large community practice over a 20 year period. Retrospective chart analysis was performed for demographics, clinical presentation, imaging, cytology, histopathology, and clinical outcome data. Results: Of 806 patients, the parotid gland was the most common site in 683 patients (84.7%), followed by submandibular in 78 (9.7%) and sublingual in 45 (5.6%). A total of 203 patients were managed conservatively with observation without definitive diagnosis or lost to follow-up. A total of 495 patients underwent surgical intervention within the community practice. Twenty-six patients underwent surgical excision at an outside hospital. Eighty-two patients were determined to have a benign diagnosis based on ultrasound-guided fine needle aspiration or excisional biopsy alone. Final histopathology was benign in 505 cases (83.7%), while 98 tumors (16.3%) received a diagnosis of primary or secondary malignancy. For the parotid gland, pleomorphic adenoma (155) and Warthin's tumor (155) were the most common benign diagnoses, while mucoepidermoid carcinoma (13), adenocarcinoma (8), and acinic cell carcinoma (8) were the most common primary malignancies. Conclusions: We found a higher rate of benign tumor pathology compared to the existing literature. While the outcome data on surgical treatment of benign tumors are comparable to the existing literature, the same conclusion cannot be drawn for malignant tumors, given relatively small numbers in our series and likely disparity in the complexity of the surgical cases in tertiary care centers.

Abstract:Objective: To investigate the clinical features, the operative type and therapeutic-efficacy of adenocarcinoma salivary glands. Methods: Thirteen cases of salivary glands adenocarcinoma were reviewed and pathologic diagnosis was based on the WHO Classification. The clinical features were retrospectively analyzed and followed-up for prognosis. Results: Parotid gland was the most common location of salivary adenocarcinoma. In the 13 cases, the average age of onset was 49.6, with a male preponderance. The median survival time, 3-year and 5-year survival rate was 8.5years, 83.3%, and 66.7%, respectively. Conclusions: The male patients predominate in the incidence of adenocarcinoma salivary glands, with parotid as the predilection site, surgical operation as the main treatment, complete initial-operation as the key of the treatment. Individualization is needed in the regimens. The mortality occurs mainly within 6 years after operation, after which the survival time significantly prolongs.

Abstract:Objective: To evaluate the clinical features, treatments and prognoses of malignant salivary gland tumors of the larynx. Methods: A total of 14 patients with malignant salivary gland tumors of the larynx diagnosed and treated in our hospital between 2003 and 2014 were analyzed retrospectively, and were compared with 62 patients from the Surveillance, Epidemiology, and End Results: (SEER) database of American for survival rates. Results Among 14 patients from our hospital, 9 males and 5 females, with a median of 66.5 years old; 8 patients with adenoid cystic carcinoma, 4 with mucoepidermoid carcinoma and 2 with adenocarcinoma; supraglottic, glottic and 4 subglottic malignancies respectively accounting for 5, 5 and 4 cases; 8 cases treated with surgery, 5 with surgery and postoperative radiotherapy and/or chemotherapy, and 1 with radiotherapy alone. The 14 patients showed a three-year overall survival rate of 50.0% and a five-year overall survival rate of 42.9%, respectively in contrast with the rates of 67.1% and 59.8% in SEER database. Conclusions: The malignant salivary gland tumor of the larynx is rare and it is difficult to be diagnosed early. It is more likely to occour local recurrence and distant metastasis. Extensive surgical resection with postoperative radiotherapy is advocated.
Managing malignant sublingual gland tumors: a single institution experience with 23 patients

Abstract:Background: Due to the relative rarity of malignant sublingual gland tumors, diagnosing and treating them clinically pose challenges. Hence, there's a need to explore the pathological types, characteristics, treatment methods, and prognosis of primary malignant tumors of the sublingual gland to improve our understanding and management of these rare yet highly malignant conditions. Methods: This study reviewed cases of primary malignant sublingual gland tumors, analyzing their characteristics. The treatment methods included surgical excision, with additional radiotherapy, or brachytherapy for advanced stages or positive surgical margins. The study also summarized different treatment approaches, including lymph node dissection and soft tissue reconstruction using free flaps such as the anterolateral thigh flap and forearm flap. Results: We have gathered 23 cases of sublingual gland malignancies treated at the Department of Oral and Maxillofacial Surgery, School and Hospital of Stomatology, Wuhan University, from January 2013 to May 2024. The most common pathological types were adenoid cystic carcinoma and mucoepidermoid carcinoma, with rare cases of mucosa-associated lymphoid tissue (MALT) lymphoma and nonspecific salivary gland clear cell carcinoma. Early diagnosis and surgical intervention were crucial for a favorable prognosis. Marginal mandibulectomy was necessary for cases involving the mandible. Patients with positive preoperative lymph node detection required cervical lymph node dissection. Extensive tissue defects in the floor of the mouth were effectively reconstructed with free flaps to prevent oral-mandibular fistula. Conclusion: Surgical excision remains the preferred treatment for malignant sublingual gland tumors. Early diagnosis and comprehensive surgical management are essential for improving prognosis. The study's limitations include a small sample size and short follow-up duration, necessitating further research with larger clinical samples to confirm these findings.

Abstract:Salivary glands tumors are uncommon neoplasms with variable incidence, heterogenous histologies and unpredictable biological behaviour. Most tumors are located in the parotid gland. Benign salivary tumors represent 54–79% of cases and pleomorphic adenoma is frequently diagnosed in this group. Salivary glands malignant tumors that are more commonly diagnosed are adenoid cystic carcinomas and mucoepidermoid carcinomas. Because of their diversity and overlapping features, these tumors require complex methods of evaluation. Diagnostic procedures include imaging techniques combined with clinical examination, fine needle aspiration and histopathological investigation of the excised specimens. This narrative review describes the advances in the diagnosis methods of these unusual tumors—from histomorphology to artificial intelligence algorithms.

Abstract:PURPOSE: To analyse the clinical epidemiologic features of epithelial tumors of the salivary glands from 1990 to 2005. METHODS: 1209 cases of epithelial tumors of salivary glands who were treated at the Affiliated Stomatological Hospital, Medical College of Nanjing University from 1990 to 2005 were retrospectively studied. The pathological types and distributions of location, age and sex of them were analysed. RESULTS: In 1209 cases, the ratio of benign to malignant tumors was 1.96∶1, and that of male to female was nearly 1∶1. The most common type of benign tumors was pleomorphic adenoma, followed by Warthin's tumor. Accordingly, the most common malignant tumor was mucoepidermoid carcinoma, followed by adenoid cystic carcinoma. As to the location, benign and malignant tumors were alike. They developed mostly in the parotid gland, followed by the palatal minor salivary gland and the submandibular gland. The age of the patients was mostly concentrated between 20 and 70 years old. CONCLUSIONS: Epithelial tumors of the salivary glands can occur in any ages. The morbility of benign tumor is more than that of malignant one. The most common type of epithelial tumors of salivary glands is still pleomorphic adenoma.

Abstract:Abstract Salivary gland-type tumors of the lung are thought to originate from the submucosal exocrine glands of the large airways. Due to their rare occurrence, reports of their study are limited to small-scale or case reports. Therefore, daily clinical practices often require a search for previous reports. In the last 20 years, several genetic rearrangements have been identified, such as MYB::NF1B rearrangements in adenoid cystic carcinoma, CRTC1::MAML2 rearrangements in mucoepidermoid carcinoma, EWSR1::ATF1 rearrangements in hyalinizing clear cell carcinoma and rearrangements of the EWSR1 locus or FUS (TLS) locus in myoepithelioma and myoepithelial carcinoma. These molecular alterations have been useful in diagnosing these tumors, although they have not yet been linked to molecularly targeted therapies. The morphologic, immunophenotypic, and molecular characteristics of these tumors are similar to those of their counterparts of extrapulmonary origin, so clinical and radiologic differential diagnosis is required to distinguish between primary and metastatic disease of other primary sites. However, these molecular alterations can be useful in differentiating them from other primary lung cancer histologic types. The management of these tumors requires broad knowledge of the latest diagnostics, surgery, radiotherapy, bronchoscopic interventions, chemotherapy, immunotherapy as well as therapeutic agents in development, including molecularly targeted agents. This review provides a comprehensive overview of the current diagnosis and treatment of pulmonary salivary gland tumors, with a focus on adenoid cystic carcinoma and mucoepidermoid carcinoma, which are the two most common subtypes.

Abstract:INTRODUCTION Tumors of minor salivary gland origin are uncommon lesions, representing 2–3% of all malignant neoplasms of the upper aerodigestive tract and 9-23% of all salivary gland tumors. The aim of this study is to report the demographic features, sites, histological types and the management and outcomes of oral and oropharyngeal minor salivary gland tumors diagnosed and treated in a University Hospital with a multidisciplinary head and neck team. MATERIALS AND METHODS A single-center retrospective observational study was conducted in a cohort of patients who received diagnosis of minor salivary gland carcinoma of oropharynx and oral cavity between July 30th 2000 and 30th September 2021. The following data of the included patients were collected: age, gender, smoke history, alcohol consumption, past medical history, comorbidities, anatomic location of the tumor, histopathology, staging, management, and outcomes. RESULTS A total of 30 cases (16 females, 14 males) of oral and oropharyngeal minor salivary gland tumors were reviewed. The most frequent histotype was polymorphous adenocarcinoma (12 patients). The majority of patients presented with low stage at diagnosis, with 66,66% of the population classified as stage I or II at diagnosis. On the whole, 29 patients out of 30 benefitted from a surgical approach as first treatment. In 11 patients, adjuvant radiotherapy was performed and in 6 cases it was associated with chemotherapy. Brachytherapy with different timing was performed in 5 cases. The recurrence rate was 26.66%. Overall disease specific survival at five and ten years was found to be approximately 81% and 33% respectively. CONCLUSIONS Surgery is still considered to be the gold standard of the treatment of minor salivary gland tumors. Radiation therapy, in spite of not being considered as a curative primary treatment for salivary malignancies, is extensively used as an adjuvant treatment.

Abstract:Nineteen cases of myoepithelial carcinoma (malignant myoepithelioma) in salivary glands were studied clinicopathologically. The clinical features are as follows: arise most frequently within the parotid glands, followed by the submandibular glands; rapidly enlarging mass with extensive invasion of the surrounding tissues in some cases; low rate of cervical lymph node metastasis but high rate of distant metastasis; very frequent recurrence after surgical excision; and poor prognosis. The carcinoma belongs to the high grade malignant tumor. It must be treated radically. The elective neck dissection is generally unnecessary. It is resistant to radiotherapy. It is possible to get rather good result in limited recurrent tumors by prompt surgery.
