Abstract:Objective To observe the fixity of 125I seed after implantation in parotid region. Methods Ten patients treated with 125I seed interstitial brachytherapy in parotid were randomly selected. Within one week after the treatment, two plane radiographs were taken by radiotherapy simulator and 125I seed was counted. All plane dosimetry analysis were performed by treatment planning system. The areas surrounded by 50%, 100% and 150% of prescription dose curve and dose equality index were calculated. Two months after the treatment, the same examination were repeated and were compared with t test. Results The number of seeds was equal in two examination. There were no signifieant differences in two examinations either for the areas surrounded by 50%, 100% and 150% of prescription dose curve or for the dose equality index. Conclusions The position of 125I seed after implantation in parotid region is quite fixable, which can ensure the curative effect.

Abstract:OBJECTIVE:To observe the effect of 125I seeds implants in advanced head and neck cancer.METHOD:Treatment fields were designed to encompass the tumor/edema volume with a 2-3 cm margin, based on assessment of diagnostic CT images, 125I seeds were inserted into there and permanent left there for brachytherapy in 5 patients with advanced head and neck cancer including recurrent oropharyngeal cancer, supraglottic cancer and occult cancer.RESULT:Local tumor were controlled evidently and the patients suffering were alleviated, but 1 case occurred skin necrosis and 1 case occurred skin necrosis combined with internal carotid rupture.CONCLUSION:Permanent 125I implants offer useful pallition for patients with advanced recurrent head and neck cancer but need prevention fatal complication.
Radioactive Seed Migration Following Parotid Gland Interstitial Brachytherapy

Abstract:PURPOSE: To evaluate the incidence and associated factors of pulmonary seed migration after parotid brachytherapy using a novel migrated seed detection technique. METHODS AND MATERIALS: Patients diagnosed with parotid cancer who underwent permanent parotid brachytherapy from January 2006 to December 2011 were reviewed retrospectively. Head and neck CT scans and chest X-rays were evaluated during routine follow-up. Mimics software and Geomagic Studio software were used for seed reconstruction and migrated seed detection from the original implanted region, respectively. Postimplant dosimetry analysis was performed after seeds migration if the seeds were still in their emitting count. Adverse clinical sequelae from seed embolization to the lung were documented. RESULTS: The radioactive seed implants were identified on chest X-rays in 6 patients. The incidence rate of seed migration in 321 parotid brachytherapy patients was 1.87% (6/321) and that of individual seed migration was 0.04% (6/15218 seeds). All migrated seeds were originally from the retromandibular region. No adverse dosimetric consequences were found in the target region. Pulmonary symptoms were not reported by any patient in this study. CONCLUSIONS: In our patient set, migration of radioactive seeds with an initial radioactivity of 0.6-0.7 mCi to the chest following parotid brachytherapy was rare. Late migration of a single seed from the central target region did not affect the dosimetry significantly, and patients did not have severe short-term complications. This study proposed a novel technique to localize the anatomical origin of the migrated seeds during brachytherapy. Our evidence suggested that placement of seeds adjacent to blood vessels was associated with an increased likelihood of seed migration to the lungs. (C) 2017 American Brachytherapy Society. Abstract:Purpose: The management of recurrent head and neck cancer pose a challenging problem. To assess the feasibility, efficacy, and morbidity of 125I seed implantation for recurrent head and neck carcinoma. Materials and Methods: Between September 2002 and June 2009, seventeen patients with head and neck carcinoma underwent 125I seed implantation under CT or ultrasound guidance. All patients underwent a detailed tumor volume study using CT scans with 5 mm sections 1 – 2 weeks before seed implantation. Following adequate local anesthesia, thirteen patients underwent seed implantation guided by color Doppler ultrasound and three patients guided by CT scan when needed. Interstitial needles (18 – gauge) were inserted into the tumor in parallel arrays approximately 1 cm apart in GTV with a margin of 0.5 – 1.0 cm, in order to avoid the puncture of large blood vessels or organs. 18 – Gauge needles were kept 1.0 cm apart from blood vessels. 125I seeds were implanted using a Mick applicator with a spacing of approximately 1.0 cm center-to-center. Implant dosimetry was routinely obtained up to 24 h following seed implantation using 5 mm CT sectioning scans to identify the three - dimensional seed placement. The actuarial D90 of the 125I seeds implanted was 90 – 160 Gy (median, 126 Gy). Median followup was 10 months (range, 3–48). Results: Median local control was 16 months, the 1- and 2-yr local control rates were 66.5% and 49.9%, respectively. The 1- and 2-yr survival rates were 51.3% and 38.5%, respectively, with a median survival of 16 months (95% CI, 7.7 – 24.3). None of the patients experienced greater than serious Grade 4 toxicity events. Conclusions: 125I seed implants were feasible and effectively salvage treatment for patients with recurrent head and neck cancers, local control was good, and complications were minimal. Abstract:To enhance the accuracy of radioactive seed implants in the head and neck, a digital model individual template, containing information simultaneously on needle pathway and facial features, was designed to guide implantation with CT imaging. Thirty-one patients with recurrent and local advanced malignant tumors of head and neck after prior surgery and radiotherapy were involved in this study. Before (125)I implants, patients received CT scans based on 0.75mm thickness. And the brachytherapy treatment planning system (BTPS) software was used to make the implantation plan based on the CT images. Mimics software and Geomagic software were used to read the data containing CT images and implantation plan, and to design the individual template. Then the individual template containing the information of needle pathway and face features simultaneously was made through rapid prototyping (RP) technique. All patients received (125)I seeds interstitial implantation under the guide of the individual template and CT. The individual templates were positioned easily and accurately, and were stable. After implants, treatment quality evaluation was made by CT and TPS. The seeds and dosages distribution (D(90),V(100),V(150)) were well meet the treatment requirement. Clinical practice confirms that this approach can facilitate easier and more accurate implantation.
Automated Segmentation of the Parotid Gland Based on Atlas Registration and Machine Learning: A Longitudinal MRI Study in Head-and-Neck Radiation Therapy

Abstract:To develop an automated magnetic resonance imaging (MRI) parotid segmentation method to monitor radiation-induced parotid gland changes in patients after head and neck radiation therapy (RT).The proposed method combines the atlas registration method, which captures the global variation of anatomy, with a machine learning technology, which captures the local statistical features, to automatically segment the parotid glands from the MRIs. The segmentation method consists of 3 major steps. First, an atlas (pre-RT MRI and manually contoured parotid gland mask) is built for each patient. A hybrid deformable image registration is used to map the pre-RT MRI to the post-RT MRI, and the transformation is applied to the pre-RT parotid volume. Second, the kernel support vector machine (SVM) is trained with the subject-specific atlas pair consisting of multiple features (intensity, gradient, and others) from the aligned pre-RT MRI and the transformed parotid volume. Third, the well-trained kernel SVM is used to differentiate the parotid from surrounding tissues in the post-RT MRIs by statistically matching multiple texture features. A longitudinal study of 15 patients undergoing head and neck RT was conducted: baseline MRI was acquired prior to RT, and the post-RT MRIs were acquired at 3-, 6-, and 12-month follow-up examinations. The resulting segmentations were compared with the physicians' manual contours.Successful parotid segmentation was achieved for all 15 patients (42 post-RT MRIs). The average percentage of volume differences between the automated segmentations and those of the physicians' manual contours were 7.98% for the left parotid and 8.12% for the right parotid. The average volume overlap was 91.1% ± 1.6% for the left parotid and 90.5% ± 2.4% for the right parotid. The parotid gland volume reduction at follow-up was 25% at 3 months, 27% at 6 months, and 16% at 12 months.We have validated our automated parotid segmentation algorithm in a longitudinal study. This segmentation method may be useful in future studies to address radiation-induced xerostomia in head and neck radiation therapy.

Abstract:Objective: To explore the clinical value of 3D-CT reconstruction of the pterygopalatine fossa. Methods: Five cadavers' skull were scanned with Philips M×8000 scanner in succession in axial plane, and the data were subjected to volume rendering (VR) and multiplanar reconstruction (MPR) in M×View CT working station. Results: Pterygopalatine fossa structures were clearly and directly indicated at various VR (P0. 05). Conclusion: 3D-CT stereoscopically and directly document the structures of the pterygopalatine fossa, providing a basis for the clinical treatment.

Abstract:Objective To explore the method of target volume determination of postoperative 125I seeds interstitial brachytherapy in parotid gland carcinoma.Methods A total of 31 cases( 14 males and 17 famales) with primary parotid carcinoma who were treated in Peking University Hospital of Stomatology from Oct 2002 to Nov 2006.The patients' average age was 38.2 years.All patients underwent tumor resection and postoperative 125I seeds interstitial brachytherapy with 60 Gy matched peripheral dose.The spiral CT was performed for treatment plan and quality verification before and after the brachytherapy.The bone and muscle landmarks surrounding parotid were selected as reference for target volume determination.D90 of target volume and dose of organs at risk were calculated,while the target volume and D90 of target volume of verification were compared with that of treatment plan through quality verification.Results The target volume or D90 of target volume before and after treatment was not statistically different.D90 of target volume was more than 60 Gy.During 3 -7 years of follow-up,all patients had no recurrence.ConclusionsAccording to the follow-up results,the method used for target volume determination in this paper might be satisfied.

Abstract:OBJECTIVE:To investigate the efficacy of radioactive seeds for the treatment of recurrence malignant parotid gland tumors. METHODS:Thirty-three patients with recurrence malignant parotid gland tumor treated by radioactive seeds were retrospectively reviewed. Three patients received implanted radioactive seeds alone. Thirty patients underwent both tumor resection and radioactive seeds implanting. The patients were followed up for 1041 months. RESULTS:All three patients receiving implanted radioactive seeds alone achieved CR Twenty-four patients treated by operation and radioactive seeds implanting had no local recurrence. The overall local control rate was 88%. The facial nerves were preserved in thirteen of fifteen patients without facial nerve palsy before treatment, and the facial nerve functioned well after operation. Four patients showed slight radiotherapeutic side effect. CONCLUSIONS:Radioactive seeds implanting combined with operation or implanting radioactive seeds alone for the treatment of recurrent malignant parotid gland tumors could get good local control and treatment effects.

Abstract:Interstitial permanent radioactive seed implantation delivers a high local dose to tumors and sharply drops off at surrounding normal tissues. Radioactive seeds implanted via ultrasound or computed tomography (CT) guidance are minimally invasive and facilitate quick recovery. Transrectal ultrasound-guided 125I seed implantation assisted by a transperineal plane template is standard for early-stage prostate carcinoma, with a highly consistent target volume dose distribution. The postplan dose evaluation is consistent with the preplan evaluation. Until now, there was no workflow for seed implantation elsewhere in the body, and it was difficult to effectively preplan for seed implantation because of patients' position changes, organ movement, and bone structure interference. Along with three-dimensional (3D) printing techniques and seed implantation planning systems for brachytherapy, coplanar and X Y axis coordinate templates were created, referred to as 3D-printed coplanar templates (3D-PCT). 125I seed implantation under CT guidance with 3D-PCT assistance has been very successful in some carcinomas. Preplanning was very consistent with postplanning of the gross tumor volume. All needles are kept parallel for 3D-PCT, with no coplanar needle rearrangement. No standard workflow for 3D-PCT-assisted seed implantation exists at present. The consensus topics for CT-assisted guidance compared to 3D-PCT-assisted guidance for seed implantation are as follows: Indications for seed implantation, preplanning, definition of radiation doses and dosimetry evaluation, 3D-PCT workflow, radiation protection, and quality of staff. Despite current data supporting 125I seed implantation for some solid carcinomas, there is a need for prospectively-randomized multicenter clinical trials to gather strong evidence for using 125I seed implantation in other solid carcinomas.
Preliminary Investigation of PET-CT Applied in Malignant Maxillary Cancer Patients Undergoing Radioactive Seeds Implantation

Abstract:Objective: To investigate the value of PET-CT guided target localization technique in radioactive iodine 125 seeds implantation for treating malignant maxillary cancer.Methods: Eleven patients diagnosed with maxillary malignant tumors underwent PET-CT scans and CT scans. The three-dimensional tumor imaging generated data was converted into DICOM format by the treatment planning system. Virtual implantation was also manipulated according to the three-dimensional position of the tumor. Gross tumor volume and matched peripheral dose were compared in different imaging methods.Results: The volumes of BTV and GTV by PET-CT were (35.21±7.8) cm3 and (42.17±9.32) cm3, respectively. Compared to CT imaging, the volumes and dose were significantly smaller (t=7.25,P<0.05). No tumors recurred and there were no severe complications. Conclusions: PET-CT is a feasible and reliable method for the diagnosis and treatment of maxillary malignant tumor, and it can signiifcantly improve the accuracy of iodine 125 seeds implantation.
Clinical Application of Computer-Aided Design and Rapid Prototyping Technique for Radioactive Seeds Contained Maxillary Device

Abstract:OBJECTIVE:To test a new method to manufacture a maxillary applicator containing radioactive seeds for brachytherapy of malignant neoplasms based on computer aided design (CAD) and rapid prototyping (RP).METHODS:Six patients with maxillary malignancy which had eroded the inferior wall of orbit and skull base were included in this study. After excision of the tumors, head CT data of these patients were transmitted into the computer. Three-dimensional digital image of the patient's defected region was then obtained with special software processing based on Mimics 8.11 and Geomagic 7.0 and the resin cast of the defected region was manufactured by rapid prototyping. The elastic obturator was then made on this resin cast which can duplicate the undercut tissue of the defected region. After the obturator was completed, the radiotherapy plan was made. (125)I radioactive seeds were implanted into the tissues, and they were also implanted into the target area of the obturator which was used as a maxillary applicator at the same time. The number of radioactive seeds was then counted and the stability of radioactive seeds was determined by CT examination. All these 6 patients were followed-up for 12 months.RESULTS:All the obturators had good retention and stability and fitted the designed target tissue very well. (125)I radioactive seeds in the form of the obturator applicator were stable. For all patients, the total number of radioactive seeds used was 189. Among them, 105 seeds, 55.6% of the total, were contained in obturator applicators. After the obturator applicators were used, the amount of radioactive seeds irradiating the target regions was significantly increased when compared with that of before. After follow-up of 12 months, there was no recurrence nor severe complications.CONCLUSION:(125)I radioactive seeds contained maxillary applicator made by computer aided design and rapid prototyping can effectively improve the brachytherapy of (125)I when it is used for the post-operation radiotherapy of maxillary malignant tumors.

Abstract:Objective: To compare dose distributions of postoperative plans with preoperative plans for radioactive seed implantation of paravertebral/retroperitoneal tumors assisted by 3-dimensional printing noncoplanar templates and computed tomography. Methods: Sixteen patients with paravertebral/retroperitoneal tumors (21 lesions) underwent radioactive seed implantation with 3-dimensional printing noncoplanar templates. Prescribed dose was 110 to 160 Gy. We compared the dose distribution of the postoperative plan with the preoperative plan. Dose parameters were D90, minimum peripheral dose, V100, V150, conformal index and external index of the target volume, and the dose received by 2 cm(3) of normal tissue of organs at risk (spinal cord, aorta, and kidney). Results: Sixteen 3-dimensional printing noncoplanar templates were produced for 21 treatment areas. Mean gross tumor volume (preoperative) of patients was 61.1 cm(3), mean needle number was 17, mean number of implanted I-125 seeds was 65, and mean D90 of postoperative target area (gross tumor volume) was 131.1 Gy. Actual number of seeds postbrachytherapy increased by 1 to 12 in 8 cases. For postoperative plans, the mean D90, minimum peripheral dose, V100, V150 was 131.1 Gy, 67.1 Gy, 90.2%, and 64.1%, respectively, and 135.0 Gy, 64.7 Gy, 90.9%, and 64.1%, respectively, in preoperative plans. Comparing with the preplanned cases, the dose of the target volume was slightly lower and the high-dose area of the target volume was larger in postoperative cases, but the difference was not statistically significant (P > .05). Actual dose conformity of the target volume was lower than preplanned, and the difference was statistically significant (P = .005). Conclusion: Three-dimensional printing noncoplanar templates can provide good accuracy for positioning and direction in radioactive seed implantation.
Dosimetry Evaluation for CT Guided 125I Seeds Implantation Assisted by Three-Dimensional-printing Coplanar Coordinate Template in Chest Malignant Tumor

Abstract:Objective To verify the accuracy of three-dimensional printing coplanar coordinate template (3D-PCCT) technology for 125I seeds implantations in chest tumor at dosimetry level.Methods From January 2016 to June 2017,a total of 22 patients (15 males,7 females;median age 62 years) with chest tumors who received 3D-PCCT assisted 125I seeds implantation in Peking University Third Hospital were enrolled in this retrospective study.There were 8 patients with primary lung cancer and 14 with metastatic carcinoma.The sites for implantation included lung (12 cases),mediastinum (2 cases) and chest wall (8 cases).The prescribed dose was 110-180 Gy.The preoperative plan design,puncture and seeds implantation guided by template were carried out and the dose distribution of postoperative plan was compared with that of preoperative plan.Dose parameters included dose delivered to 90％ gross tumor volume (D90),minimum peripheral dose,the percentage of GTV receiving 100％ prescription dose (V100),the percentage of GTV receiving 150％ prescription dose (V150),conformity index,external index of target volume,uniformity index,D2 cm3 of spinal cord and aorta,and V20 of affected side lung.Wilcoxon signed rank test was used to analyze the data.Results The median D90 was 150.4(125.6,187.0) Gy.Postoperative D90 was higher than the prescribed dose in 68％ (15/22) cases.For median value of most parameters,there were no significant differences between the postoperative plans and preoperative plans (all P＞0.05) except for the actual V100,which was lower than the preplanned (95.5％ vs 97.2％;P=O.040).Conclusion 3D-PCCT could provide good accuracy in 125I seeds implantation for chest tumor.

Methods Forty patients with head and neck cancer were treated with ~(125)I-seed intersitial brachytherapy guided by ultrasound or CT.Among them thirty-six patients were performed under local anesthesia and four were under general anesthesia.Thirty-six patients were treated with ~(125)I-seed implantation under ultrasound guidance and four cases under CT guidance.Seed needles were implanted in parallel with each other,1-1.5 cm apart.Mick applicator was used to each needle to implant seed.The radioactive activity ranged 0.40-0.70 mCi,and the matched peripheral doses were 90-145 Gy.The mean number of ~(125)I-seed was 3-84.Five cases received external beam irradiation one week after seed implantation,and the total doses were 45-50 Gy(200 cGy/f).All the cases were examined by X-ray or CT after implantation for quality evaluation and by routine chest X ray within twenty-four to forty-eight hours after seed implantation. Results These patients were followed-up for 3-33 months.In 10 tongue carcinoma patients,complete response was shown in 3 cases,partial response in three cases,no change in three cases and progressive disease in one case.The local control rate was 60%,the median survival time was 11 monthes,and the one-year and two year survival rates were 87.50% and 35%,respectively.In 14 patients with recurrent cancer of head and neck the local control rate was 76.47%,the one-year and two-year survival rates were 66.08% and 24%,respectively,and the median survival time was 9 months.In 16 patients with metastatic cancer of head and neck the local control rate was 95.23%,the one-year and two-year survival rates were 54.55% and 32.73%,and the median survival time was 9 months.One seed migrated to the maxilla and loss of seeds happened in two cases,but there was no clinically related mobidity. Conclusion ~(125)I-seed implantation for recurrent or metastatic head and neck cancer under guidance of ultrasound or CT is safe,minimal invasive,with low morbidity and high efficacy,especially for recurrent and matastatic cancer of head and neck after surgery or radiotherapy.
Percutaneous Permanent Iodine-125 Implantation As Salvage Therapy for Recurrent Salivary Glands Carcinomas

Abstract:To investigate the safety and feasibility of percutaneous (125)I seed implantation for patients with recurrent salivary glands carcinomas. Nineteen (19) patients with recurrent salivary glands carcinomas underwent (125)I seed permanent implantation under ultrasonography or CT guidance and the feasibility and efficacy of (125)I seed implantation were assessed. The postplan evaluation showed that the actuarial D90 of (125)I seeds ranged from 90 to 160 Gy (median, 130 Gy). The activity of each (125)I seed ranged from 0.4 to 0.8 mCi (median, 0.68 mCi). The total number of sources implanted ranged from 3 to 80 (median, 31). The follow-up ranged from 3 to 40 months (median, 8 months). The 1-, 2-year local control rates were 59.5% and 12.4%, respectively, with a median local control of 10 months. The 1-, 2-, 3-and 5 year survival rates were 71.4%, 35.7%, 23.8%, and 23.8%, respectively (median, 10 months). Of the 19 patients, 2 (10.5%) died of local recurrence, and 4 (21.1%) died of metastases; 1 patients died of pneumonia; 1 patient died of heart disease. One (1) developed ulceration with tumor progression. Blood vessel damage and neuropathy were not observed. Percutaneous (125)I seed implantation is a feasible, safe salvage for patients with recurrent salivary glands carcinomas.

Abstract:Xerostomia (dry mouth), resulting from radiation damage to the parotid glands, is one of the most common and distressing side effects of head-and-neck cancer radiotherapy. Recent MRI studies have demonstrated that the volume reduction of parotid glands is an important indicator for radiation damage and xerostomia. In the clinic, parotid-volume evaluation is exclusively based on physicians' manual contours. However, manual contouring is time-consuming and prone to inter-observer and intra-observer variability. Here, we report a fully automated multi-atlas-based registration method for parotid-gland delineation in 3D head-and-neck MR images. The multi-atlas segmentation utilizes a hybrid deformable image registration to map the target subject to multiple patients' images, applies the transformation to the corresponding segmented parotid glands, and subsequently uses the multiple patient-specific pairs (head-and-neck MR image and transformed parotid-gland mask) to train support vector machine (SVM) to reach consensus to segment the parotid gland of the target subject. This segmentation algorithm was tested with head-and-neck MRIs of 5 patients following radiotherapy for the nasopharyngeal cancer. The average parotid-gland volume overlapped 85% between the automatic segmentations and the physicians' manual contours. In conclusion, we have demonstrated the feasibility of an automatic multi-atlas based segmentation algorithm to segment parotid glands in head-and-neck MR images.
